No. of bitstreams: 1 Ferreira_ConsueloGoncalves_M.pdf: 3166152 bytes, checksum: 5db9effacb53834e7a5276d34d5ada71 (MD5) Previous issue date: 2002 / Resumo: O objetivo do presente estudo foi avaliar o desempenho dos laboratórios que compõem a rede ligada ao SUS e que estão sob a gestão da Direção Regional de Saúde de Ribeirão Preto (DIR XVIII) do Estado de São Paulo, nos anos de 1998 e 1999. A avaliação foi realizada a partir de um evento traçador, utilizado para iluminar aspectos particulares das atividades laboratoriais. Para cumprir este papel foi selecionada a meningite, através de critérios que a qualificaram como boa condição traçadora e a partir desta definição, foram construídos indicadores que expressam, no conjunto, os resultados das ações laboratoriais na vigilância e controle das meningites e as atividades laboratoriais realizadas. Os dados analisados foram coletados de duas fontes: Sistema de Informação de Agravos de Notificação (SINAN) e registro de exames do Instituto Adolfo Lutz, Laboratório Regional de Ribeirão Preto, ambos para o mesmo período (1998 e 1999). No geral, a avaliação revelou um desempenho não satisfatório desse sistema de laboratórios, se tomarmos os resultados finais do número de casos registrados no SINAN de Meningite Bacteriana Não Especificada (MBNE), de Doença Meningocócica (DM) sem sorogrupo, naquelas com sorogrupo mas não soro e subtipadas e na existência de resultados registrados no IAL de Ribeirão Preto e não registrados no Sistema de Notificação. Os resultados foram satisfatórios para o grupo de meningites não especificadas e aceitáveis para as meningites diagnosticadas por cultura. Por outro lado, ao analisar a utilização dos laboratórios componentes do sistema na região, as possibilidades diagnósticas para o agente etiológico foram aceitáveis ou satisfatórias. O estudo revelou, ainda, que duas microrregiões são mais deficitárias e apontou, também, os fatores que podem estar determinando aqueles resultados, apesar das atividades diagnósticas estarem sendo realizadas. Por fim, foi discutido que essa situação pode estar se repetindo em outras áreas do sistema pois reflete, sobretudo, a necessidade de definição do papel de um sistema de laboratórios de saúde pública dentro das ações implementadas pelo Sistema de Saúde. Aponta, também, que a real situação no Estado de São Paulo será melhor conhecida com o desenvolvimento de avaliações em outros pontos do Sistema / Abstract: This study is a performance evaluation of the public health laboratories which are part of the Health Region of Ribeirão Preto (Health Direction XVIII), State of São Paulo, in the period of 1998 and 1999, in the context of The National Health System (SUS). It was accomplished an outcomes evaluation using a tracer event to light up particular aspects of the laboratories activities. Meningitis was the tracer event selected, after considering all the criteria that qualified this disease as a good tracer. Indicators have been defined to express, as a whole, the results from laboratorial actions in surveillance and meningitis control, and laboratorial activities that have been carried out. The analyzed data has been taken from two sources: the Information System for Diseases of Notification (SINAN) and the registry of the Adolfo Lutz Institute, Regional Laboratory of Ribeirão Preto, both for the same period (1998 and 1999). This evaluation showed, as a whole, a non satisfactory performance of the Public Health Laboratories System. It is much clearer when considering the the registered number of Non Specified Bacterial Meningitis, Meningococcal Disease (DM) without sorogroup, those with sorogroup but not soro and subtype, and the existence of results registered at the Adolfo Lutz Institute of Ribeirão Preto and not registered at the State Notification System. There were satisfactory results for the non specified meningitis group and acceptable for the diagnosis of meningitis by culture. On the other hand, when analyzing the utilization of laboratories, which are part of the system in this region, the possibilities of diagnosing the ethiological agent were either acceptable or satisfactory. The study has revealed two deficient micro-regions. It has also pointed out some factors that are possibly determining those results, although the diagnostic activities has being carried out. At last, it has been discussed that this situation can be similar in other areas of the system, as it reflects, most of all, the need for defining the role of public health laboratory system inside the actions implemented by The National Health System. It also shows that the real situation in São Paulo State will be better known with the development of other evaluations in different parts of the system / Mestrado / Mestre em Saude Coletiva
