Avaliação da performance da inspeção visual, sondagem, radiografia interproximal, separação dental e laser de fluorescência no diagnóstico de lesões cariosas proximais /

Orientador : Pedro Felício Estrada Bernabé / Resumo: As lesões cariosas de superfícies proximais se destacam por sua grande incidência, tanto pelo fato da região propiciar o acúmulo de biofilme bacteriano, quanto pelas dificuldades de higienização e de diagnóstico precoce. O objetivo deste estudo foi avaliar a performance da inspeção visual + sondagem (IV+S), laser de fluorescência (L), radiografia interproximal (RXI), separação dental + inspeção visual + sondagem (SD+IV+S) e separação dental + laser de fluorescência (SD+L) no diagnóstico de lesões cariosas proximais sem e com cavitação. Foram examinadas, por 2 profissionais, 167 superfícies de 30 pacientes de faixa etária entre 4 e 12 anos de idade, utilizando-se os 5 tipos de exames. Como método de validação para lesões cariosas com cavitação foi utilizada a dupla impressão das áreas interproximais com silicona de condensação. Em relação às lesões cariosas sem cavitação, os valores médios de sensibilidade encontrados para os exames de IV+S, L, RXI, SD+IV+S e SD+L foram de 44%, 33%, 49%, 100% e 48%, respectivamente. Em relação às lesões cariosas com cavitação, a sensibilidade foi de 15%, 40%, 59%, 59% e 41%, respectivamente. A correlação dos métodos de diagnóstico, em relação às lesões cariosas com cavitação, sugeriram uma maior precisão para a SD+IV+L. Os resultados sugeriram que a utilização da separação dental aumenta o desempenho no diagnóstico de lesões cariosas proximais, e que o método de impressão com silicona pode ser indicado como um método auxiliar definitivo de diagnóstico de lesões com cavitação. / Abstract: The approximals carious lesions stand out for its great incidence, so much for the fact of the area to propitiate the accumulation of bacterial plaque, as for the difficulties of toothbrushing and of precocious diagnosis. Laser fluorescence in detecting and monitoring the progression of occlusal dental caries lesions and for screening persons with unfavourable dietary habits

Abstract This study focuses on the clinical use of laser fluorescence compared to visual inspection (VI) for detecting and monitoring the progress of caries lesions during a one-year follow-up period and for screening subjects with unfavourable dietary habits causing demineralization of teeth. The effect of professional cleaning on laser fluorescence was also studied. The study groups were comprised of schoolchildren (n = 259), and altogether 3 651 occlusal tooth surfaces were examined visually and by using laser fluorescence. Laser fluorescence was found to be useful as an adjunct to visual inspection in detecting dental caries lesions during routine dental check-ups. The variation of laser fluorescence values in each visual category excludes its use as a primary or only method for caries detection. It rather functions as an alarm for a closer or more thorough examination. In addition, it can be a useful tool when deciding on the intervention method and the length of the recall-interval. The best cut-off point for considering operative intervention was found to be 30/99. Laser fluorescence was also found to be useful in monitoring lesion progression. Monitoring can be done through clear sealants. High laser fluorescence values (> 20) of sound tooth surfaces may predict decaying within a period of one year. Professional cleaning increased laser fluorescence values of molars, especially second molars. The increase was significant in molars with a visual score of > 0 or when visually detected initial and dentinal caries lesions were included. Professional cleaning was most efficient when using only a rubber cup and water spray without paste. A computer-based questionnaire on dietary habits was used to evaluate the cariogenity of children's dietary habits. The laser fluorescence values of tooth surfaces of children with harmful dietary habits were found to be higher than among children with favourable dietary habits. Laser fluorescence can be used for screening children whose current dietary habits may harm their teeth.

IN VITRO VALIDATION OF LASER FLUORESCENCE-BASED SUBGINGIVAL CALCULUS DETECTION INSTRUMENT

Objectives: Because subgingival dental calculus in periodontal pockets is associated in the etiopathogenesis of progressive human periodontitis, and is difficult to accurately detect with conventional manual explorers and probing instruments, there is an urgent clinical need for more reliable diagnostic methods for the detection and localization of subgingival dental calculus. A low-power ( 40). Results: A total of 50 root surfaces exhibited a modified SCI score = 0 (no root surface dental calculus detected), whereas 19 root surfaces revealed modified SCI scores = 1 (root surface dental calculus detected in thin deposits, but not in a markedly-raised ledge), and 31 root surfaces had modified SCI scores = 2 (root surface dental calculus detected in a markedly-raised ledge). A high level of both intra- and inter-examiner reproducibility of visible red laser fluorescence intensity readings was found with both tooth root evaluation protocols, despite the marked differences between the two dentist examiners in their educational backgrounds and length of clinical dental care experience, with correlation coefficient values ranging from r = 0.948 to r = 0.999 for duplicate assessments made by the two independent examiners themselves and between them. Mean visible red laser fluorescence intensity values recorded by the two independent examiners with the instrument perpendicularly directed along tooth root surfaces (first evaluation protocol) were 98.9 (standard deviation ± 0.4) and 99.0 (standard deviation ± 0.0), respectively, on dental calculus-positive root surfaces, which were significantly greater than mean values of 10.9 (standard deviation ± 6.0) and 12.3 (standard deviation ± 8.1), respectively, recorded on dental calculus-negative root surfaces (P 40 for visible red laser fluorescence intensity values offered 90% sensitivity, 100% specificity, a 100% positive predictive value, a 90.9% negative predictive value, and an odds ratio relationship of 36.6 [95% confidence interval = 16.7, 80.2] for the presence of dental calculus on tooth root surfaces. Conclusions: These in vitro findings document, for the first time, a high level of intra- and inter-examiner reproducibility of visible red laser fluorescence intensity measurements on human tooth root surfaces, regardless of the whether the instrument is directed either perpendicular or parallel to extracted tooth root surfaces. Dental calculus- positive root surfaces on extracted teeth exhibited significantly higher visible red laser fluorescence intensity scores than dental calculus-negative root surfaces, particularly when dental calculus deposits were present in markedly-raised ledges. In addition, a threshold level of > 40 for visible red laser fluorescence intensity readings offered greater diagnostic accuracy than a threshold level of ≥ 5 for identification of dental calculus on root surfaces of extracted teeth. These findings provide further in vitro validation for use of the visible red laser fluorescence-emitting instrument for detection of dental calculus on root surfaces of human teeth. Additional validation studies, conducted clinically in vivo, on the visible red laser fluorescence-emitting instrument are warranted. / Oral Biology

O presente trabalho objetivou testar o uso da secagem prolongada (30s) na tentativa de melhorar a habilidade do método de fluorescência a laser em avaliar a atividade de lesões de cárie oclusais em dentes decíduos. Noventa e três crianças, com idade entre 4 e 14 anos, concluíram todas as fases deste estudo. Um examinador, previamente treinado, realizou as medições, após secagem prévia por 3 segundos (controle) e 30 segundos, com o aparelho de fluorescência a laser (Ddpen) nas superfícies oclusais dos molares decíduos selecionados (n=515). Para a validação concorrente, os dentes previamente selecionados foram classificados por dois examinadores quanto à presença e atividade das lesões de cárie através do critério de inspeção visual ICDAS e a avaliação da atividade das lesões foi realizada através da ponderação mental das características clinicas da lesões. Para a validação preditiva, após 1 ano do exame inicial, 19 crianças foram reexaminados pelos métodos descritos acima. Com o intuito de verificar a habilidade do método em diferenciar lesões ativas e inativas, foram consideradas as medições após 3 s e 30 s de secagem, além da diferença entre essas medidas (DIF 30s-3s). Para isso, foram utilizadas análises de regressão linear multinível. Para a validação preditiva, modelos de regressão logística foram usados e o risco relativo com intervalo de confiança de 95% foi calculado. Houve discreta diferença entre os dois tempos de secagem (3s e 30s). Para as lesões clinicamente em esmalte, não houve diferença na magnitude das leituras do DDpen, independentemente do tempo de secagem utilizado. Entretanto, quando foi considerado somente lesões não cavitadas, observaram-se, em média, maiores leituras para as lesões de cárie inativas do que para as ativas, efeito que desapareceu quando a análise foi ajustada pela pigmentação das lesões não cavitadas. A DIF 30s-3s não contribuiu na diferenciação de lesões inativas e ativas, nem considerando lesões clinicamente em esmalte, nem apenas as lesões não cavitadas. As lesões cavitadas ativas tiveram maior leitura de fluorescência a laser que as lesões inativas. Na validação preditiva, as medidas de fluorescência a laser também não foram associadas à progressão das lesões, nem à alteração do status de atividade. A slight difference between values was found, for both drying periods (3s and 30s). Concerning the lesions that were clinically classified in enamel, no significant statistical difference was observed on laser fluorescence readings, independently of air-drying time. However, when considering only non-cavitaded lesions, higher mean readings were observed for the inactive lesions than for the active ones. However, this effect disappeared when adjusting the analysis for the stained non-cavitaded lesions. The DIF 30s-3s did not contribute to distinguishing active and inactive lesions. In the predictive validation, the measurements of laser fluorescence were not also associated to the greatest risk of progression or changes in the status of the activity. It was concluded that the Ddpen is suitable to differentiate cavitated active and inactive lesions independently of the drying period, and that the difference measurement between the obtained readings after 3s and 30s of air-drying does not help in caries activity assessment of noncavitated caries lesions.

A inspeção visual das superfícies é um dos métodos diagnósticos mais utilizados e os critérios: cor, brilho, textura e distância das lesões em relação à margem gengival precisam ser considerados para a determinação da atividade de cárie no esmalte não-cavitado. Um aparelho laser fluorescente, KaVo DIAGNOdent@, foi proposto para quantificar a perda mineral de lesões de cárie e, a validade e reprodutibilidade de seus resultados têm sido investigadas in vitro. Devido a dificuldade em se determinar clinicamente a atividade ou inatividade de lesões de cárie incipientes e, frente à possibilidade de utilização do laser fluorescente como um auxiliar diagnóstico na quantificação da perda mineral, o recente trabalho teve como objetivos: 1. promover a difusão de conhecimentos e de critérios clínicos para o diagnóstico de lesões de cárie ativas e paralisadas; 2. determinar, in vivo, a reprodutibilidade de duas escalas de classificação dos resultados do laser DIAGNOdent@, propondo uma escala para o diagnóstico de cárie em superficies lisas livres; 3. pesquisar a reprodutibilidade e a validade do laser DIAGNOdent@, em lesões de cárie de superflcies lisas livres, quando comparado com o critério clínico de validação, in vivo. Dentro das condições experimentais deste trabalho, observou-se que: 1. através da divulgação de casos clínicos, os critérios de atividade e inatividade das lesões de cárie foram ilustrados, incentivando o diagnóstico diferencial e precoce; 2. a escala de classificação dos valores do laser DIAGNOdent@ que mostrou melhor desempenho no diagnóstico de cárie, em superfícies lisas livres, foi a escala de LUSSI et al., 1999 adaptada por PINELLI et al., 2001 (unpublished data), sendo proposta para o diagnóstico de lesões incipientes em superfícies lisas livres; 3. o laser DIAGNOdent@ mostrou-se reprodutível e válido no diagnóstico de cárie in vivo, e a associação dos métodos clínico e laser pode aprimorar a eficácia do diagnóstico / Abstract: Caries diagnosis has assumed a particular importance, since the ability to detect earlier the reversible lesions offers several advantages for the patients, in terms of treatment and preventive measures. The visual inspection of the surfaces remains the method of first choice and clinical criteria such as color, shininess, roughness, and distance from the lesion to the gingival margin must be considered when determining caries activity on non-cavitated enamel. A laser fluorescent system, Ka V o DIAGNOdent@, was proposed to quantify mineral loss of caries lesions, and the validity and the reproducibility of the laser results has been investigated in vitro. Due to difficulty at determining clinical caries activity or inactivity, and due to possibilities of applying the laser fluorescence device as an auxiliary method to quantify mineral loss, the present work was aimed to: 1. disclose some clinical criteria for detecting active or arrested caries lesions; 2. verify, in vivo, the reproducibility of two scales of classification for DIAGNOdent@, and to propose a scale for the diagnosis of caries on free smooth surfaces; 3. test the reproducibility and validity of DIAGNOdent@, on caries lesions of free smooth surfaces, when the validation criterion was the visual examination, in vivo. Under the experimental conditions, it was verified: 1. afier the publication of clinical cases, the clinical criteria related to caries activity or inactivity were illustrated, aiming at differential and earlier diagnoses; 2. the scale which showed a better performance in caries diagnosis of free smooth surfaces was the scale proposed by LUSSI et al., 1999, modified by PINELLI et al., 2001 (unpublished data), and _it was suggested to diagnose incipient caries on free smooth surfaces; 3. the laser DIAGNOdent@ was reproducible and valid for caries diagnosis in vivo, and the utilization of both methods can improve caries diagnostic efficacy / Doutorado / Dentística / Doutor em Clínica Odontológica

This work deals with the study of fluorescence lifetime and spectral changes of nanoparticles in cell biology. It describes the principle of fluorescence, fluorescence microscopy and laser confocal microscope Leica TCS SP8. The classic FLIM method, the Lambda Square mapping and the division of nanoparticles are introduced there. In the practical part, the created program for the evaluation of fluorescence lifetime and spectral changes is described. The program can show two-dimensional lambda maps, the fluorescence lifetime and spectral shift in the space area. In the final part of the thesis, an experiment with rhodamine nanoparticles is carried out and it is evaluated using the created program and then discussed.

The theoretical part of this master's thesis is focused on research of confocal microscopy and FLIM method. There are a principles and types of confocal microscopy and the use of broad-spectrum laser as a basic light source of these microscopes. It gives what the FLIM method and its use not only in cell biology. The practical part thesis includes the acquisition of three sets of fluorescence intensity images with use of applications tunable pulsed laser, function TimeGate and detection of hybrid detectors. For practical elaboration of this thesis is in the software Fiji created a plugin, which is the source code in the Java programming language. The types of plugins and their uses are described in the third chapter of the thesis. This plugin including the graphical user interface in the form of the dialog box, proceses the fluorescence intensity images and creates a graphical representation of data showing the fluorescence lifetime, so called pseudocolor map.

Des diagnostics optiques non-intrusifs sont développés afin d’améliorer la compréhension des phénomènes d’évaporation de gouttes de combustible multicomposant. La configuration simplifiée retenue est un jet linéaire de gouttes monodispersées. L’évaporation est réalisée soit à la température ambiante, soit à des températures plus élevées de l’ordre de 500°C. Le combustible utilisé est composé d’éthanol et d’acétone, dont les volatilités sont très différentes. Deux nouvelles techniques de mesures fondées sur la Fluorescence Induite par Laser (LIF) ont été développées au cours des ces travaux. La première, la LIF à trois couleurs permet de mesurer la température moyenne de gouttes binaires. La deuxième exploite la fluorescence émise par l’acétone excitée par un rayonnement laser à 266 nm afin de déterminer la composition instantanée du mélange. Les résultats expérimentaux ont été comparés à un modèle à composants discrets utilisant une approche 1D quasi-stationnaire / Non intrusive optical diagnostics are developed to investigate the mechanisms governing multicomponent droplets evaporation. The case of periodically and linearly arranged monodisperse droplets is considered in these experiments. Droplets are injected either at ambient temperature or into a hot air plume at about 500°C. The fuel mixture is made of ethanol and acetone, which exhibit very different volatilities. Two techniques based on Laser Induced Fluorescence (LIF) have been developed to investigate heat and mass transfers. The first one, the three colours LIF allows obtaining the average temperature of binary droplets. The second one used the fluorescence of acetone excited by an UV laser (266 nm) in order to quantify the acetone molar fraction. Finally, measurements are compared with a numerical model that is derived from a discreet component approach and the assumption of 1D quasi-steady evolution of the gas phase
