Lors de ce travail, nous nous sommes intéressés à létude des protéases et de langiogenèse au niveau de deux aspects distincts de la reproduction : 1) Une situation physiologique quest la placentation par le biais dun modèle murin ; 2) Une situation pathologique chez les femmes souffrant de métrorragies dysfonctionnelles suite à lutilisation dun système intra-utérin à libération de progestatif comme moyen de contraception

O objetivo deste estudo foi avaliar a efetividade do Sistema Intra-uterino liberador de Levonorgestrel no tratamento das lesões adenomióticas. Realizou-se um ensaio clínico não aleatorizado, em que o sujeito foi o seu próprio controle, pré e pós-intervenção, ensaio do tipo N=1. Vinte e nove mulheres com idade entre 24 a 46 (média de 37,5 ± 5,1) anos, com quadro de dismenorréia e menorragia, atendidas no ambulatório de Endometriose do CAISM-Unicamp, entre 2004 e 2006, foram selecionadas após confirmação do diagnóstico da adenomiose através da ressonância nuclear magnética. Foi inserido um Sistema Intra-uterino liberador de Levonorgestrel e repetida a ressonância nuclear magnética, após seis meses da inserção, para controle das lesões. As mulheres foram reavaliadas em três e seis meses para coleta de dados. Foram avaliados a dismenorréia, através da Escala Visual Analógica de Dor, o padrão menstrual e os efeitos colaterais. As variáveis estudadas foram volume uterino, zona juncional e espessura endometrial, avaliados pela ressonância nuclear magnética, antes e após seis meses da inserção do Sistema Intra-uterino liberador de Levonorgestrel. Para a comparação pré-tratamento e seis meses após, foram realizados os testes T student pareado, Wilcoxon e teste Friedman para variáveis contínuas, e teste de McNemar para as variáveis categóricas. O nível de significância foi assumido em 5%. Os resultados obtidos foram redução do quadro menorrágico em todas as mulheres, redução significativa do quadro doloroso através da Escala Visual Analógica 80-0 (p<0,0001), da espessura do endométrio 10,2-5,8 (p<0,0001) e do percentual da zona juncional em 24,17% (p<0,0001). Houve discreta redução do volume uterino 142,6-136,4 (p=0,2077). Os efeitos colaterais mais comuns foram ganho de peso superior a 2kg (27,5%), cefaléia (20,6%), mastalgia (20,6%), seborréia e acne (6,8%) e quadro depressivo transitório resolvido sem medicação (6,8%). Nenhum Sistema Intra-uterino liberador de Levonorgestrel foi removido e não ocorreu sequer uma expulsão. Concluímos que o Sistema Intra-uterino liberador de Levonorgestrel é um tratamento clínico eficaz para tratamento dos sintomas com poucos efeitos colaterais, baixo custo e que reduz as lesões causadas pela adenomiose / Abstract: The levonorgestrel-releasing intrauterine system (MIRENA ®) has been recently used in the symptomatic treatment of adenomyosis, although its effect on the lesions is unknown. The objective of this study was to evaluate the effectiveness of the levonorgestrel-releasing intrauterine system in the treatment of adenomyotic lesion. The type of study was a clinical trial nonrandomized, where the subject is its own control, before and after the intervention, as N=1. Twenty-nine women age between 24 and 46 years old (average 37 years old ) presenting dysmenorrhea and menorrhagia were selected after the confirmed diagnosis of adenomyosis through magnetic resonance imaging in the Endometriosis Clinic of CAISM - UNICAMP between the years of 2004 and 2006. It was inserted an levonorgestrel-releasing intrauterine system and then repeated the magnetic resonance imaging 6 months later, to control the lesions. The women underwent clinic examination at the insertion and then again, three and six months later for data coletion. Dysmenorrhea was evaluated through the Visual Analogue Scale of Pain, their menstrual standard and side effects. The parameters studied were uterine volume, junctional zone, endometrial thickness evaluated through magnetic resonance imaging before and after insertion of levonorgestrel-releasing intrauterine system. To compair results before the insertion and 6 months after it, we used macthed t-student test, Wilcoxon and Friedman Test for continuous variable and Mc. Nemar for category variable. Significance level was assumed at 5%. Results were: reduction of the menstrual flow in every woman , a great reduction of pain through the Visual Analogue Scale 80 - 0 (p< 0,0001) , the endometrial thickness 10, 2 - 5, 8 (p <0,0001) and junctional zone percentage in 24,17% (p <0,0001), but with discreet uterine volume reduction 142,6 - 136,4 (p = 0,2077). The most common side effects were: weight gain superior to 2kg (27,5%), headaches (20,6%), breast tenderness (20,6%), seborrhea and acne (6,8%) and reported a transitory depressive episode (6,8%), that was solved without pharmacologic treatment. None of the levonorgestrel-releasing intrauterine system was removed or expelled. We concluded that the intra uterine system that releases levonorgestrel is an efficient symptomatic treatment, with very few side effects, low cost and that reduces lesions caused by adenomyosis / Mestrado / Tocoginecologia / Mestre em Tocoginecologia

SUJEITOS E MÉTODOS: Estudo de coorte prospectivo com acompanhamento de um ano, composto por dois grupos: 38 usuárias de SIU-LNG, como grupo estudo e 38 usuárias de DIU TCu 380A, como grupo-controle, pareadas por índice de massa corpórea (kg/m2 )(± 2) e idade (± 2 anos), selecionadas no Ambulatório de Planejamento Familiar do Centro de Atenção Integral à Saúde da Mulher (CAISM) da Universidade Estadual de Campinas (Unicamp). Para avaliação da mudança de composição corporal utilizaram-se variações percentuais de massa gorda e de massa magra e do peso (kg). O estudo da massa óssea incluiu a mudança total e percentual da densitometria mineral óssea (DMO) de coluna (L1-L4), fêmur total e colo femoral pela técnica de absorciometria de duplo feixe de raio-X (DEXA). A primeira avaliação foi realizada antes de se iniciar o uso do método contraceptivo e a segunda após um ano de uso do método. A evolução e a diferença entre os grupos quanto à densidade mineral óssea foram avaliadas através do teste t-Student pareado e Wilcoxon pareado. RESULTADOS: A idade média (±EP) foi 34,4 (±1.2) e 34 (±1.3) anos, IMC foi 25.1 (± 0,7) e 25,9 (± 0,7), e o número de gestações foi 1,9 (±0,2) e 1,7(±0,2) em usuárias de SIU-LNG e DIU TCu380A, respectivamente. Houve uma perda de massa óssea significante de 0,5% em coluna lombar das usuárias de SIU-LNG e um aumento de massa óssea de 1,4% e 1,5% em colo femoral e coluna lombar em usuárias de DIU TCu380A. Os dois grupos tiveram aumento no peso corporal, mas não houve diferença significativa entre eles aos 12 meses. O percentual de gordura aumentou nas usuárias do SIU-LNG, enquanto o grupo DIU TCu380A teve aumento de massa magra aos 12 meses. CONCLUSÕES: O grupo SIU-LNG apresentou pequena perda de massa óssea no primeiro ano de uso do método, provavelmente sem significância clínica. O grupo do DIU TCu 380A, que ganhou massa magra, apresentou também ganho na massa óssea em colo femoral e coluna lombar / Abstract: OBJECTIVE: This study evaluated body composition and bone mineral density in users of levonorgestrel-releasing intrauterine system (LNG-IUS) and compared to TCu380A intrauterine device (IUD) users. SUBJECTS AND METHODS: A prospective group study conducted for 12 months with 38 LNG-IUS users and 38 TCu 380A IUD users, were paired by age (± 2 years) and the body mass index (kg/m²) (±2), recruited at Woman Hospital of University of Campinas (Unicamp). Two evaluations were realized about the body composition and bone mineral density of the Lumbar Spine (L1-L4), femoral neck and total femur through the technique of dual x-ray absorciometry (DEXA). The first evaluation is going to be at the beginning of the use of the contraceptive method and the second will be after the method has been used for one year. The data was evaluated trough t- Student test for paired samples, Wilcoxon, and Chi-square. RESULTS: The mean (±SEM) age was 34.4 (±1.2) years old and 34 (±1.3) years old, BMI was 25.1 (± 0.7) and 25.9 (± 0.7) and with 1.9 (±0.2) and 1.7 (±0.2) pregnancies in LNG-IUS and TCu380A IUD users, respectively. There was a significant decrease in BMD of 0.5% in LNG-IUS at LS and an increase of 1.4% and 1.5% in TCu380A IUD users at FN and LS, respectively when compared values at baseline and at one year of use. The two groups had an increase in body weight; however, without significance between groups at 12 months. LNG-IUS users showed a fat percentage increased; notwithstanding, the TCu380A IUD users had an increase of lean mass, both at 12 months. CONCLUSIONS: It was observed a small loss of BMD up to the 1st year of LNG-IUS use; probably without clinical significance. Com o objetivo de adquirir a capacidade de fecundação precisam passar por três processos seqüenciais, maturação epididimária, capacitação e reação acrossômica (RA). A capacitação é um processo que acontece no trato reprodutor feminino e define-se como uma mudança bioquímica na membrana plasmática do espermatozóide, sendo a presença de progesterona e o aumento na concentração intracelular do cálcio ('Ca POT +2¿) eventos característicos e obrigatórios para a posterior RA. O levonogestrel (LNG) é um progestágeno sintético, semelhante a progesterona, o qual tem sido usado como contraceptivo feminino, seja na contracepção de emergência (CE), nas pílulas contraceptivas combinadas ou com o dispositivo intra-uterino (DIU). Entretanto, na literatura não há dados que relacionem a eficácia contraceptiva do lenovogestrel com possíveis efeitos deste hormônio sobre os espermatozóides humanos. Assim, com o objetivo de adicionar dados a respeito do mecanismo de ação contraceptiva do LNG, este estudo teve como objetivo avaliar o efeito de 5 concentrações de LNG, comparáveis as concentrações encontradas no plasma sanguíneo de mulheres, após a ingestão de LNG sob a forma de (CE) com a concentração intracelular do cálcio em espermatozóides capacitados de homens férteis. Um total de 24 amostras de sêmen, provenientes de 4 homens férteis foram avaliadas. Os espermatozóides foram selecionados através de um gradiente de Percoll, e foram submetidas a um período de capacitação. Às amostras foi adicionado o corante Fura2/AM e a seguir estas foram expostas às concentrações de 8, 10, 14, 1.000 e 10.000 ng/mL de LNG e a dois controles, um positivo e outro negativo, sendo respectivamente a progesterona (32 'mu' M = 11.000 ng/mL) e a meio de cultivo human tubal fluid (HTF). A seguir as amostras foram avaliadas através da espectrofotometria de fluorescência, com exitação de 340 nm e emissão de 510 nm. A taxa de fluorescência foi convertida em medidas de concentração intracelular de cálcio ([('Ca POT 2+¿]i), utilizando a equação de Grynkiewicz et al. (1985), a qual demonstrou que as concentrações de 8, 10, 14 e 1.000 ng/mL de LNG foram capazes de induzir um aumento na [('Ca POT 2+¿]i significativamente maior quando comparados ao obtido com HTF, e significativamente menor que ao obtido com progesterona (o indutor natural), excetuando-se a concentração de 10.000 ng/ml (32 'mu' M) de LNG, a qual foi estatisticamente similar que a progesterona 11.000 ng/ml (32 'mu' M). Assim, é possível sugerir que o LNG in vitro, em baixas concentrações, aumente a [('Ca POT 2+¿]i nos espermatozóides. No entanto, este mecanismo, de maneira isolada, não deva ser o responsável pela indução da RA. Entretanto a [('Ca POT 2+¿]i obtida através de altas concentrações de LNG (10.000 ng/ml) pode ser sim, um possível mecanismo de ação do LNG como droga contraceptiva / Abstract: The spermatozoa is a highly specialized cell because it contains morfophysiological modifications that guarantees fecundation success. Spermatozoa must undergo three sequential changes before they acquire their fertilizing capacity: maturation in epididymis, capacitation and acrosome reaction (RA). Capacitation is a process that takes place in the female reproductive tract and is defined as a biochemical modification on the spermatozoa plasmatic membrane. The presence of progesterone (P) and the intracellular calcium concentration increase inside the spermatozoa are typical and obligatory events in the capacitation stage for further RA. Levonogestrel (LNG) is a synthetic progestin, similar to P, which has being used as a female contraceptive in several manners, among them they are: emergency contraception (EC), contraceptive pills combine and with the intrauterine system (IUS). Nevertheless, there are no available data or the scientific literature that correlate LNG contraceptive efficacy with possible effects of the hormones over human spermatozoa. Therefore, to add data over the contraceptive action mechanism of LNG, the aim of this study was to evaluate the effect of 5 concentrations of LNG somes comparable to the levels found in serum following ingestion of LNG as EC on the calcium intracellular concentration (['Ca POT 2+¿]i) rates of capacitated spermatozoa of fertile men. A total of 24 semen samples from 4 fertile men were evaluated. The spermatozoa were selected by Percoll gradient, following incubation with human tubal fluid medium supplemented with bovine serum albumin (HTF/BSA) under capacitating conditions. The sperm suspensions were incubated with Fura2/AM Subsequently the cells were exposed to the following concentration of LNG: 8, 10, 14, 1,000 and 10,000 ng/mL, and two controls (positive and negative). The positive control was P (32 'mu' M=11,000 ng/ml) and the negative one was medium (HTF). Cells were transferred to a quartz cuvette to make the reading in the spectrofluorometer at 340 nm excitation with emission at 510nm. Fluorescence measurements were converted to ['Ca POT 2+¿]i by the equation of Grynkiewicz et al. (1985). The Grynkiewicz equation shown that LNG concentration of 8, 10, 14, and 1,000 ng/mL were able to induce increase in the ['Ca POT 2+¿]i with significantly higher values, when compared with the rate obtained with HTF medium ( as a negative control), and significantly lower than the rate obtained with natural P (as a positive control). However, the exception was the LNG concentration of 10,000 ng/mL (32 'mu' M) in which values of ['Ca POT 2+¿]i were similar to those observed with P at a concentration of 11,000 ng/mL (32 'mu' M). In conclusion, it is possible to suggest that although LNG in vitro, in low concentration, increase the ['Ca POT 2+¿]i in the spermatozoids. Despite the fact that this mechanism is not the only responsible for the RA induction the calcium obtained through high concentration of LNG (10,000 ng/ml) might have a contraceptive action in the spermatozoa / Mestrado / Histologia / Mestre em Biologia Celular e Estrutural

MATERIAIS E MÉTODOS: Quarenta e oito experimentos foram realizados em 14 mulheres menstruando regularmente. Quatro grupos foram formados diferindo entre eles os intervalos coito?tratamento e tratamento?recuperação dos espermatozóides e obtenção da biópsia. RESULTADOS: A concentração dos espermatozóides recuperados a partir da cavidade uterina 24 ou 48 horas após o tratamento foi de 14,5 ± 3,9 x 106 e 17,3 ± 6,8 x 106 células/ml, respectivamente. Não houve diferenças entre a taxa de RA e a intensidade da expressão da glicodelina-A endometrial nos ciclos tratados com LNG ou placebo. Após 24 horas da administração do LNG o valor no fluido de lavagem da cavidade uterina representou 1,38% dos valores observados no soro. CONCLUSÕES: A RA ou a expressão da glicodelina-A endometrial não foram influenciadas pelo LNG após 24 ou 48 horas da sua administração para AE. O LNG não prejudicou o muco Resumo x cervical uma vez que espermatozóides viáveis foram encontrados no trato genital feminino 36-60 horas após o coito e 24-48 horas após a ingestão do LNG. O mecanismo de ação do LNG para AE permanece enigmático / Abstract: The objectives were to assess acrosomal reaction (AR) of spermatozoa following flushing the uterus, to measure levonorgestrel (LNG) in serum and in the flushing fluid of the uterus and the endometrial glycodelin-A expression after administration of LNG as emergency contraception (EC). MATERIALS AND METHODS: Forty-eight experiments were conducted on 14 regularly menstruating women. Four groups were formed based on different intercourse - treatment interval and treatment ? recovery of spermatozoa and the biopsies. RESULTS: Spermatozoa recovered from uterus 24 or 48 hours after treatment were 14,5 ± 3,9 x 106 and 17,3 ± 6,8 x 106 cells/ml, respectively. There were no differences between the AR rate and the endometrial glycodelin-A staining intensity on LNG or placebo treated cycles. LNG at uterine flushing medium represents 1.38% of the values observed in serum at 24 hours after LNG intake. CONCLUSIONS: AR status or glycodelin-A were not influenced after 24 or 48 hours of administration of 1.5 mg of LNG as EC indicating no significant effect. LNG did not impair the cervical mucus either because viable spermatozoa were found in the genital tract 36-60 hours after coitus and 24-48 hours after LNG intake. Este estudo foi desenvolvido com o intuito de se avaliar as quimiocinas Interleucina 8 e 6 CKine no endométrio de usuárias do sistema intra-uterino liberador de levonorgestrel e correlacioná-las com as populações de leucócitos, células natural killer uterinas e mastócitos. Amostras de endométrio foram obtidas de dois grupos de usuárias do sistema intra-uterino liberador de levonorgestrel: 11 mulheres em amenorréia e 15 mulheres com algum tipo de sangramento. Os leucócitos foram determinados através de anticorpos específicos para neutrófilos, células natural killer uterinas e mastócitos. Foi realizada imuno-histoquimica para identificação das quimiocinas. Os neutrófilos encontravam-se em pequeno número, não havendo diferença entre os dois grupos. As células natural killer uterinas encontravam-se em número maior no grupo com sangramento (p<0,0001). Não houve diferença entre o número total de mastócitos e mastócitos ativados, porém houve um aumento da área extracelular positiva para triptase (p<0,05) nas mulheres com sangramento. As quimiocinas Interleucina-8 e 6CKine mostravam-se abundantes no estroma e no epitélio, não havendo diferença entre os dois grupos. Houve maior número de células natural killer uterinas e maior área extracelular positiva para triptase em pacientes que apresentavam sangramento. Apesar de não haver diferença entre o número total de mastócitos e mastócitos ativados e das quimiocinas Interleucina-8 e 6CKine, as células natural killer uterinas e os produtos dos mastócitos podem ter exercido papel importante na perda da integridade endometrial em usuárias do sistema intra-uterino de levonorgestrel / Abstract: Changes in the bleeding pattern are almost universal in women using progestin-only methods of contraception, and are a frequent cause of discontinuation of these methods. The present study was designed to assess endometrial chemokines in users of the Levonorgestrel intrauterine system and correlate them with leukocyte populations, uterine natural killer cells, and mast cells. Endometrium was obtained from two groups of Levonorgestrel intrauterine users: 11 amenorrhoeic women and 15 women with some form of bleeding pattern. Leukocytes were assessed using specific antibodies for neutrophils, uterine natural killer cells and mast cells. Immunohistochemistry was performed to locate chemokines 6Ckine and Interleukin-8. Neutrophils were few and without differences between the two groups. Uterine natural killer cells were significantly higher in the bleeding group (p< 0.0001). There was no difference between the total number of mast cells and activated mast cells but there was a greater extracellular area stained for mast cell tryptase (p<0.05) in women with bleeding. Chemokines 6CKine and Interleukin-8 were abundant in the stroma and in the epithelium and there was no difference between the groups. We observed more uterine natural killer cells in users with bleeding, and a greater extracellular area stained for mast cell tryptase, although there were no differences between the number of mast cells and activated mast cells or the chemokines 6CKine and Interleukin-8.
