Profesní psychohygiena učitele v mateřské škole - případová studie / Professional psychohygiene of teacher in a nursery school - case study

The theoretical part of this diploma thesis focuses on defining psychohygiene. The principles of psychohygiene are presented, such as efficient use of time, healthy diet, sufficient exercise, etc. Furthermore, the term stress is defined and explained in more detail by stressors that can burden teachers in a kindergarten. The position of a teacher in a kindergarten, his personality and competences are scrutinized here. The practical part is focused on a qualitative research. Using the case study defined here, the workplace of a private kindergarten is examined in detail using an observation method. Another method in the research is a questionnaire for pedagogical staff in two branches of kindergartens and nurseries. Based on the findings, a conclusion is drawn regarding the psychohygiene of teachers. The aim of the work is to reveal the positives and negatives that affect teachers in this kindergarten and to find out how aware they are about psychohygiene. The research shows that the kindergarten supports employees in healthy eating, exercise, effectively planned working hours and in constantly possible communication with parents. Introduktion: WHO betonar vikten av att integrera traditionell- och komplementärmedicin i nationella hälsovårdssystem. I Sverige förekommer integrativ medicin med antroposofisk inriktning på Vidarkliniken i Järna. Syfte: Att studera förändringar av självskattat hälsotillstånd hos patienter som erhållit antroposofisk vård på Vidarklinikens öppenvårdsmottagning i Norrköping. Metod: En kvantitativ metod med enkäter till 26 patienter på Vidarklinikens öppenvårdsmottagning i Norrköping, konsekutiv datainsamling. Hälsorelaterad livskvalitet skattades med hjälp av EQ-5D (rörlighet, hygien, aktivitet, smärta och oro) och EQ-VAS (totalt hälsotillstånd) samt två enkäter, konstruerade för denna studie, med VAS-skalor för sömnkvalitet, fysiskt tillstånd, psykiskt tillstånd och förutsättningar att hantera sin livssituation, samt patienternas beskrivning av måluppfyllelse. Därutöver undersöktes patienternas sjukskrivningsgrad och läkemedelsförbrukning. Resultat: Patienterna hade mycket varierande diagnoser och ofta sammansatt problematik. Vanligast var utmattning, fibromyalgi och smärta. Resultaten varierade mellan diagnosgrupperna. Huvuddelen av patienterna rapporterade stor tillfredsställelse med vård, behandling och bemötandet från personal, till hög grad uppnådda förväntningar och mål, samt att överlag ha fått bättre förutsättningar att hantera sin livssituation. Det fanns ingen signifikant skillnad i medelvärdet för EQ-5D index eller EQ-VAS, men trend till signifikant förbättring vad gäller sömnkvalitet och psykiskt tillstånd, samt signifikant förbättring av självskattat fysiskt tillstånd (p=0.021). Vid delanalys av de enskilda dimensionerna (EQ-5D) fann man förbättrat tillstånd i någon/några av dimensionerna hos 36 % av patienterna, oftast vad gäller rörlighet och minst vanligt vad gäller smärta. Slutsats: I denna grupp med omfattande och varierande och ofta mycket långvarig problematik påvisades stor uppskattning av vården, trender till förbättrade resultat vid sömnkvalitet och psykiskt tillstånd, samt signifikant förbättrade resultat vid självskattat fysiskt tillstånd. / Introduction: WHO stresses the importance of integrating traditional and complementary medicine into national health systems. Method: A quantitative method using questionnaires to 26 patients at the Vidar Clinic outpatient facility in Norrköping, [using] consecutive data collection. Health-related quality of life was estimated using EQ-5D (mobility, hygiene, activity, pain and anxiety) and EQ-VAS (overall health status) along with two questionnaires specially designed for this study, with VAS scales for sleep quality, physical condition, mental condition and the ability to manage their lives, as well as the patients’ description of whether or not they felt fulfilled in their lives. In addition, the patients’ sickness absence rates and drug consumption were analysed. Results: The patients had extremely varying diagnoses and often complex problems. The most common were fatigue, fibromyalgia and pain. The results varied between the diagnostic groups. The majority of the patients reported great satisfaction with their care and treatment and how they were being cared for by the staff; to a large extent they felt that their expectations and goals had been achieved, and that in general they had been given better capabilities to manage their lives. There was no significant difference in the mean value for the EQ-5D index or EQ-VAS, but a tendency to significant improvement in sleep quality and mental condition, and a significant improvement in self-estimated physical condition (p=0.021). When a partial analysis was conducted of the individual dimensions (EQ-5D), improved conditions were found in one/some of the dimensions in 36 per cent of the patients, usually in terms of mobility and least common with regard to pain. Conclusion: In this group, with extensive and varied and often very long-term problems, great appreciation of the care was shown, there were tendencies to improved results in sleep quality and mental condition, and significantly improved results in self-estimated physical condition.

Použití omezovacích prostředů u pacientů s násilným a agresivnímchováním / Use of restraints in patients with violent and aggressive behavior.

This Diploma Work is focussed on application of restrictive agents with patients who are afflicted with violent and aggressive behaviour. The up-to-date civilization has brought, expect for further changes, also a very principal change that presents a substantial increase of psychic and psychiatric disorders. That is why it is necessary to think about the question how to prevent aggression and violence in medical facilities. It is evidenced that more cases of patients showing aggressive and violent behaviour occur nowadays. What are preventive news in occurrence of aggression while hospitalization? What are legal news and standards applied to limiting and restrictive means? It is not a rule any more that we meet a patient with aggressive or violent behaviour only in psychiatric departments. However, we can meet those patients in any departments and facilities of healthcare. The Diploma Work is split into a theoretical and a practical section. In the theoretic part we state the human psyche evolution, aggression and violence origination and evolution, kinds of aggression, violent behaviour and demographic occurrence thereof within existing population. Further on, we mention kinds of restrictive measures, their indications and the statutory framework dealing with this problem area. A status of nurse and a nurse competence in that area represent an important element of the theoretic work. Four objectives were set in the practical part of the Work. The first goal was aimed to find out work fastidiousness of nurse providing care to patients with aggressive and violent behaviour. The second goal had to recognise what was taken for the greatest problem area within nursing care at treatment of patients with aggressive and violent behaviour. The third goal was to find out any benefits and shortages in the application of restrictive agents at treatment of patients with aggressive and violent behaviour. Finally, the fourth goal was set to find out and compare nursing staff?s opinions on the application of restrictive agents and main problems in that field. The above objectives were met. Based on those objectives research questions were defined. What is the work fastidiousness of nurses who take care of patients with violent and aggressive behaviour? What is considered by the nurses as the most problem area within providing healthcare to aggressive patients? What is the positive application of restrictive agents with patients showing aggressive or violent behaviour? What special reform should be performed in the application of restrictive - limiting means? The asked questions were answered. A qualitative investigation was used to work out the Diploma Work. Collection of needed data was carried out by help of procedure of a non-standardized, structured dialogue. Our investigation took place in the psychiatric department of Klatovy Hospital a.s. , the psychiatric department of University Hospital in Plzeň , Psychiatric Medical Institution in Dobřany and other more departments. The nurses? answers were compiled in casuistries from which though maps were made up. The described investigation was running from January to July 2011. The results of my Diploma Work might serve as a feedback for the hospitals and medical institutions in which the above investigation was performed. An educative programme for nurses was created.

In this article are presented the results of research of first-year students of the Institute of Physical Culture, Sports and Youth Policy of the Ural Federal University, studying on courses "Hotel business", "Service" and "Physical Culture" and several points of view of scientists studying interrelation between the physical and mental condition of students and the effect on them of a physical break and independent physical exercise. As a result we proposed the conditions for improving the physical and mental condition of first-year students, which make it possible to solve some problems associated with the process of adaptation to the educational process at the university. / В работе представлены результаты исследования первокурсников Института физической культуры, спорта и молодежной политики (ИФКСиМП) Уральского Федерального университета (УрФУ), обучающихся на направлениях «Гостиничное дело», «Сервис» и «Физическая культура», а также несколько точек зрения ученых, изучающих связь физического и психического состояния студентов и влияние на них физкультурной паузы и самостоятельных занятий физическими упражнениями. Предложены условия улучшения физического и психического состояния первокурсников, позволяющие решить, в том числе проблемы, возникающие в процессе адаптации студентов к учебному процессу в вузе.

The diploma thesis deals with problems of depression in older people. Mainly the work is focused on identifying and analyzing the role of nurses in screening for depression in older people in primary care, acute care, long-term care and home care. This thesis was focused on theoretical direction and was used the method of design and demonstration. In this thesis was set one main goals with five research questions. The main goal was to identify and analyze the role of nurses in screening for depression in the elderly. RQ 1: What is the role of the nurse in screening for depression in the elderly? RQ 2: What is the role of the nurse in the primary care in screening for depression in the elderly? RQ 3: What is the role of the nurse in screening for depression in hospitalized patients in acute care? RQ 4: What is the role of the nurse in screening for depression in seniors in long-term and home care? RQ 5: What rating scales and methods are used in screening for depression in the elderly? The thesis introduce the concept of depression. The following are specified the causes of and the important factors that affect depression in the elderly. It also deals the differences in the clinical symptomatology of depression in old age. It explains possibilities and various barriers in the diagnosis of depression. Another chapter introduces complete geriatric examination, diagnostic classification systems, possible screening methods and scales for detection of depression in the elderly population. It also deals methods of pharmacological and non-pharmacological treatment and its possible complications associated with older age. By reason of increased suicide rate caused by depressive disorder the issue of suicidal behavior in the elderly is introduced. The next chapter deals with the nursing process, which is used by nurses in practice. It consists of the evaluation of the patient's health condition, making nursing diagnosis, creating nursing plan and subsequent implementation and evaluation. The nursing process is also needy for providing quality care. The nursing process in the stage of nursing diagnosis, introduces possible nursing diagnosis for a patient suffering from depression, which are based on the latest classification. Finally is described the role of nurses in screening for depression in the elderly in different health facilities and their contribution to the timely evaluation of depression in the elderly. This chapter introduces the role of nurses, nursing screening and collaboration with a physician. The role of nurses in screening for depression in different medical facilities is based on the first phase of the nursing process of assessment. On the basis of objective and subjective information, the nurse will assess the overall health and mental condition of the patient. Primarily, it was investigated what is the role of the nurse in screening for depression. On the basis of content analysis and synthesis it was necessary to used and processed domestic and foreign literature. A number of relevant sources are the results of various studies and Meta-analyzes, mostly from abroad, but also from the Czech Republic. The thesis can serve as a basis for nurses. The result of this thesis is to create e-learning material available for students in the Faculty of Health and Social Sciences of South Bohemia in Ceske Budejovice in the tutorial called Moodle.
