Implementace paliativní péče do nemocnic / Implementation of palliative care in hospital setting

This thesis deals with the implementation of palliative care in hospitals and perceives the topic in context of change management in health facilities. In the Czech Republic, this type of care is still at its beginning in hospitals and its implementation is a big challenge. The aim of the thesis is to find out what factors influence the implementation of palliative care in hospitals. In the first part of the text, the theoretical bases concerning palliative care and change management are outlined. The empirical part introduces a qualitative survey identifying barriers that had to be overcome during the implementation of palliative care in hospitals and factors that were instrumental in the implementation. Within the survey, 66 interviews with members of multidisciplinary teams from 17 hospitals were made. All the hospitals participated in the Together until the end programme of the Avast Foundation. The assessment of the survey results focuses on recommendations for hospitals that decide to extend or launch the palliative attitude in their premises and on challenges for the future of palliative care in the Czech Republic. The diploma thesis follows up the treatment of marketing plan in area of concrete home care service development. The purpose of the thesis enlarges field home care service by the development of appropriately chosen marketing plan. The first, theoretical, part of thesis explains marketing including related terms, and it analyzes strategic management and the concept of strategic analysis. It describes a law about the social services and the principles of senior care in real surroundings in case of the demographics. The practical part has been articulated for the research chapter, including marketing research between home care service consumers/clients, and the analytical chapter. The result of marketing strategic analysis. On the base of detected documents was developed the marketing plan, which is covering appropriate marketing mix for development concrete field home care services. In conclusion there are valorized objectives of marketing plan realization, and there are another recommendations, whose contribute for enlarge of services and the success of the organization at the market. The objective of my diploma thesis called "Project Planning and Evaluation of Establishment and Operation of a Retirement home" is a feasibility study of a social care investment project. The theoretical part covers methodical foundations based on available resources. In the practical part, I described specific steps of the feasibility study which are relevant to the social care project. The initial part is dedicated to the environment analysis. Based on the market analysis, I defined the target group of the business plan, analyzed the competitive factors and designed a marketing strategy including a marketing mix strategy. Then I defined the project schedule, project management and human resources management. The thesis contains financial analysis determining the need for securing long-term and short-term capital. The thesis is concluded with chapters on project effectiveness and sustainability assessment and on risk management.

The dissertation analyzes the personnel management system in healthcare facilities with after-care and long-term care in the Czech Republic, focusing on the implementation of certain personnel activities. The main focus is on the characteristics of health facilities and the subsequent long-term care in the Czech Republic, staffing and elements that influence the work of personnel in such organizations. The individual personnel activities in healthcare facilities with after-care and long-term care in the Czech Republic are put into a structure. The thesis includes a qualitative research survey of the level of personnel management in healthcare facilities with after-care and long-term care in the Czech Republic established by both public and private sector, having different legal structures. The research survey focuses on learning about the form of personnel management in healthcare facilities with after-care and long-term care in the Czech Republic, it tracks personnel management as a complex system of partial personnel activities.

Patient empowerment in long-term conditions : development and validation of a new measure

Background: Patient empowerment is viewed as a priority by policy makers, patients and practitioners worldwide. Although there are a number of measures available, none have been developed specifically for patients in the UK with long-term conditions. It is the aim of this study to report the development and preliminary validation of an empowerment instrument for patients with long-term conditions in primary care.Methods: The study involved three methods. Firstly, a systematic review was conducted to identify existing empowerment instruments, and to describe, compare and appraise their content and quality. The results supported the need for a new instrument. Item content of existing instruments helped support development of the new instrument. Secondly, empowerment was explored in patients with long-term conditions and primary care practitioners using qualitative methods, to explore its meaning and the factors that support or hinder empowerment. This led to the development of a conceptual model to support instrument development. Thirdly, a new instrument for measuring empowerment in patients with long-term conditions in primary care was developed. A cross-sectional survey of patients was conducted to collect preliminary data on acceptability, reliability and validity, using pre-specified hypotheses based on existing theoretical and empirical work. Results: Nine instruments meeting review inclusion criteria were identified. Only one instrument was developed to measure empowerment in long-term conditions in the context of primary care, and that was judged to be insufficient in terms of content and purpose. Five dimensions (‘identity’, ‘knowledge and understanding’, ‘personal control’, personal decision-making’, and ‘enabling other patients’) of empowerment were identified through published literature and the qualitative work and incorporated into a preliminary version of the new instrument. A postal survey achieved 197 responses (response rate 33%). Almost half of the sample reported circulatory, diabetic or musculoskeletal conditions. Exploratory factor analysis suggested a three factor solution (‘identity’, ‘knowledge and understanding’ and ‘enabling’). Two dimensions of empowerment (‘identity’ and ‘enabling’) and total empowerment showed acceptable levels of internal consistency. The measure showed relationships with external measures (including quality of chronic illness care, self-efficacy and educational qualifications) that were generally supportive of its construct validity.Conclusion: Initial analyses suggest that the new measure meets basic psychometric criteria and has potential for the measurement of patient empowerment in long-term conditions in primary care. The scale may have a role in research on quality of care for long-term conditions, and could function as a patient-reported outcome measure. However, further validation is required before more extensive use of the measure.

A drug development from risk management perspective / Vývoj léků z pohledu risk managementu

The purpose of this diploma thesis is to understand financing of drug development from an enterprise risk management perspective as well as to critically assess the efficiency of the ISO framework and risk management techniques used for determining whether to fund drug development or not. The diploma thesis is divided into theoretical and practical part. The first part starts with perception and assessment of uncertainty and risk in the past. It describes how risk-averse individuals attempted to deal with uncertainty and different risk. This is followed by the evolution of traditional risk management into the fast developing enterprise risk management. The text further analyses commonly used risk management standards COSO ERM and ISO 31000:2009. However, the main focus is on the critical assessment of analytical tools which are frequently used for evaluating and assessing risks, especially financial ones, during drug development. The theoretical part is finished by a drug development process, whose phases are briefly described. The practical part was written in co-operation with AstraZeneca, a top-notch pharmaceutical company. The overview of its business is preceded by an explanation of current issues in the pharmaceutical industry. Furthermore, the risk analysis is conducted with respect to the ISO framework. Subsequently, selected risk assessment techniques are applied on the simplified financial model of two different drugs, which was created based on AstraZeneca's real data. These risk assessment tools are used in different phases of drug development so it could be seen clearly how the results are changing during a project. The outcomes of this risk analysis are compared with original plans used by AstraZeneca which were used for deciding whether to fund drug development or not.
