Estudo comparativo de duas técnicas de enxerto autógeno utilizando piezocirurgia para levantamento de seio maxilar / Comparative study of two autogenous graft techniques using piezosurgery for sinus lifting

A técnica de levantamento de seio maxilar apresenta como uma possível complicação a perfuração da membrana sinusal. A utilização de dispositivos ultrassônicos apresenta-se como uma interessante ferramenta, particularmente segura e eficiente para realização das ostectomias de parede sinusal e divulsão de membrana sinusal. O presente trabalho teve por objetivo comparar duas técnicas que utilizam a cirurgia ultrassônica para realização de enxerto autógeno para levantamento de seio maxilar. Dez coelhos foram utilizados no estudo, sendo que um deles não foi submetido a procedimento cirúrgico. Os nove coelhos operados tiveram os seios maxilares preenchidos com enxertos autógenos coletados de díploe externa de calota craniana, nas formas particulado do lado direito e raspado do lado esquerdo, ambos com aparelho ultrassônico. Os dados de densidade óssea nos seios maxilares esquerdo e direito, obtidos por meio de tomografia computadorizada nos sentidos transversal e longitudinal, registrados 90 dias após a realização dos enxertos foram comparados estatisticamente. Cortes histológicos das áreas enxertadas foram realizados com finalidade descritiva. Não houve diferenças estatisticamente significantes entre as técnicas de enxerto que utilizaram osso raspado e particulado coletado por meio de dispositivo ultrassônico da calota craniana de coelhos. O aspecto histológico observado após 90 dias foi semelhante nos dois grupos, mostrando tecido ósseo maduro em meio a tecido conjuntivo frouxo. Os procedimentos transoperatórios nos levaram a concluir que o ultrassom piezoelétrico mostrou-se um instrumento seguro na abordagem cirúrgica do seio maxilar de coelhos, permitindo a manutenção da integridade da membrana sinusal durante as manobras cirúrgicas. / Maxillary sinus lifting is a technique which presents as a possible complication the sinus membrane perforation. The use of ultrasonic devices is presented as an interesting tool, particularly safe and efficient to perform ostectomies of sinus wall and dissection of sinus membrane. This study aimed to compare two techniques that used ultrasound surgery to perform autogenous graft for maxillary sinus lifting. Ten rabbits were used in the study, one of which did not undergo surgery. The other nine rabbits had their maxillary sinuses filled with autogenous bone grafts collected from the external skull diploe in particulate form on the right side and shaved on the left side, both with ultrasonic device. Data on bone density in left and right maxillary sinus, obtained by computed tomography in transverse and longitudinal sections, recorded 90 days after the grafts, were compared statistically. Histological sections of the grafted areas were performed with descriptive purposes. There were no statistically significant differences between the two techniques that used shaved and particulate bone collected by means of ultrasonic device of the skull of rabbits. Histological aspects after 90 days were similar in both groups, showing mature bone through a fibrous connective tissue. Operatory assessment procedures leads us to conclude that the piezoelectric ultrasound showed as a safe tool in the surgical approach of the maxillary sinus of rabbits, allowing the maintenance of sinus membrane integrity during surgical procedures.

Análise tomográfica e histomorfométrica comparativa entre enxerto ósseo autógeno e enxerto ósseo xenogênico em cirurgia de levantamento de seio maxilar em coelhos

O objetivo deste trabalho foi avaliar a neoformacão óssea induzida por um xenoenxerto bovino composto comparativamente ao ósseo autógeno, em cirurgias de levantamento de seio maxilar em coelhos. Nesse estudo utilizouse o seio maxilar de 18 coelhos machos, sendo que no seio maxilar esquerdo foi colocado 200mg de osso autógeno córtico-medular triturado e no seio maxilar direito, o biomaterial, de origem bovina, composto por matriz orgânica cortical e inorgânica medular, \'pool\' de BMPs e colágeno (Gen- Tech®). Realizou-se análise tomográfica por tomógrafo computadorizado (TC) para avaliar a densidade óptica comparativamente entre os grupos e períodos experimentais, a histológica descritiva para verificar a resposta tecidual frente aos materiais implantados e a histomorfométrica para obter os dados de densidade de volume de tecido ósseo neoformado, tecido conjuntivo e/ou granulação, material residual e tecido mielóide. Os resultados obtidos pela TC demonstraram densidade óptica cerca de 2 vezes maior no grupo teste. Na análise histológica observou-se infiltrado inflamatório apenas no período de 2 semanas do grupo teste e exclusivamente ao redor do componente orgânico do biomaterial. Quanto a neoformação óssea, não houve diferença estatisticamente significativa entre os dois grupos experimentais ou ao longo dos períodos, no entanto a área de tecido mielóide foi cerca de 2 vezes maior no grupo controle. Considerando-se os limites do modelo experimental e da técnica empregada podemos concluir que o biomaterial Gen-Tech® é potencialmente indicado como substituto ósseo em cirurgias de levantamento de seio maxilar, uma vez que promove uma neoformação óssea similar à do osso autógeno. / The purpose of this study was to evaluate new bone formation induced by a bovine xenograft compared to autogenous bone in maxillary sinus augmentation in rabbits. The maxillary sinus of 18 male rabbits were used, whereas the left sinus was filled with 200mg of a mixture of cortical and medullar autogenous bone and the right sinus was filled with a mix of organic and inorganic bovine matrix, plus \'pool\' of BMPs and collagen (Gen-Tech®). Computer tomography (CT) analysis was realized to evaluate and compare the optical density among the groups and experimental periods; descriptive histologic analysis was used to verify the tissue response to the materials implanted. In addition, histomorphometric analysis was realized to evaluate volume density of the newly formed bone, connective tissue and/or granulation tissue, residual material and myeloid tissue. The results obtained in the CT demonstrated an optical density approximately 2 times greater in the test groups. Histological analysis revealed an inflammatory infiltrate at 2 weeks in the test group and exclusively around the organic portion of the material. Regarding new bone formation, no statistical differences were observed among the two experimental groups or throughout the experimental periods. However the area of myeloid tissue was 2 times greater in the control group. Considering the limits of the experimental model and technique used in this study, it is possible to conclude that Gen-Tech® is a biomaterial with potential applications as a bone substitute in maxillary sinus augmentation once it is able to promote the formation of new bone similarly to autogenous bone.

Resolution of Maxillary Sinus Mucositis after Endodontic Treatment of Maxillary Teeth with Apical Periodontitis: A Cone-beam Computerized Tomography Pilot Study

This study characterized maxillary sinus mucositis (SIMS) adjacent to teeth with apical periodontitis (AP), and assessed its resolution three months after endodontic treatment. 29 subjects who maxillary posterior teeth with AP were imaged with cone-beam computed tomography (CBCT). Resolution of SIMS was assessed with CBCT three months after treatment, and periapical healing was assessed using the PAI after six months. Four non-compliant subjects were discontinued and SIMS was identified in 14/25 subjects (56%). Three months post-treatment, SIMS was resolved fully in 3/10 subjects (30%), and partially in 3/10 subjects (30%). Six months post-treatment, 6/10 subjects (60%) were classified as healed or healing. CBCT revealed a lower-than-expected prevalence of SIMS adjacent to teeth with AP. In specific cases SIMS might linger beyond three months after the elimination of the endodontic infection. Due to the low statistical power, association between the resolution of SIMS and periapical healing could not be explored.

Ecogeographic Patterns of Maxillary Sinus Variation Among Homo sapiens: Environmental Adaptation or Architectural By-product?

Ecogeographic patterns of modern human craniofacial diversity suggest external nasal structures reflect climatic adaptations for respiratory and thermoregulatory functions. Regarding internal structures, the maxillary sinus supposedly varies as a function of the nasal cavity while not contributing in respiratory function. Owing to conflicting results, this study reinvestigates that claim by evaluating maxillary sinus variation in a larger sample (n=200) spanning 11 ecogeographic regions. The surface-area-to-volume (SA:V) ratio (i.e., relative mucosal area) was collected in addition to sinus volume and linear dimensions. Pearson correlations show nasal cavity breadth and maxillary sinus volume are not significantly correlated, and individuals from cold, versus hot, climates exhibit larger volumes with lower surface-area-to-volume (SA:V) ratios. Individuals from high altitudes display a unique configuration with high SA:V ratios and large maxillary sinus volumes. Analyses of variance largely fail to find significant differences among the 11 samples. However, a canonical variate analysis of nasal and sinus dimensions indicates clear separations between the heat- and cold-adapted populations, as well as among the cold-adapted populations. Specifically, Arctic populations display smaller sinus volumes and lower SA:V ratios. Mantel tests indicate certain sinus dimensions depart from isolation-by-distance models. Results indicate that maxillary sinus form does not simply follow isolation-by-distance models and cannot simply be explained in terms of nasal cavity breadth or craniofacial architecture— suggesting that environmental pressures are directly acting on the sinus. Functional possibilities for the sinus include thermoregulatory functions among Arctic populations and/or nitric oxide production for high-altitude populations. Additional considerations and future lines of research are presented.

Foram selecionados 25 pacientes que foram submetidos a cirurgia para elevação do seio maxilar, através da técnica aberta sendo divididos em 3 grupos: A. Osso autógeno particulado; AB. Osso autógeno e heterógeno (Bio-oss®) e B. Apenas osso heterógeno (Bio-oss®). Passados seis meses desta intervenção, os pacientes foram submetidos a cirurgia para instalação dos implantes, concomitante a remoção de amostra do osso enxertado nesse local, previamente. Na avaliação histológica foi observada neoformação óssea nos três grupos, com presença de trabéculas de osso maduro. Nos grupos B e AB, foi observada a presença de grânulos do biomaterial com tecido ósseo circundante. A análise estatística apontou diferença significante (ANOVA p=0.002), sugerindo uma maior neoformação óssea no grupo de osso autógeno. Na avaliação imunoistoquímica não foram observadas diferenças estatisticamente significativas na comparação entre os grupos experimentais (A, B e AB), bem como as proteínas analisadas (OC: p=0,657; VEGF: p=0,133; TRAP: p=0,163). Conclui-se que a utilização de Bio-Oss®, associado ou não ao osso autógeno, para levantamento de seio maxilar através da técnica da janela lateral resulta em reparo ósseo. Uma previsível formação óssea é possível quando se utiliza esse material como osteocondutor / The objective of this study was to evaluate, through histological and immunohistochemistry analysis, the neoformation and bone remodeling after surgery of maxillary sinus lift, through immunostaining of the osteocalcin, VEGF and TRAP proteins. Were selected 25 patients who underwent surgery for maxillary sinus lift, through the open technique, and these were divided into 3 groups: A -particulate autogenous bone; AB - autogenous and heterogeneous bone (Bio-oss®) and B - only heterogeneous bone (Bio-oss®). After six months of this intervention, the patients were submitted to surgery for installation of implants and concomitant removal of grafted bone sample from the surgical site. The histological evaluation showed bone neoformation in three groups, with presence of trabeculae of mature bone. In groups B and BA, it was observed the presence of granules of the biomaterial with surrounding bone tissue. A statistical analysis showed significant difference (ANOVA p=0,002), suggesting a greater bone neoformation in group of autogenous bone. In immunohistochemical evaluation no statistically significant differences were observed in the comparison between the experimental groups (A, B and AB), as well as the proteins analyzed (OC: p= 0.657; VEGF: p= 0,133; TRAP: p= 0.163). It is concluded that the use of Bio-Oss ®, associated or not to autogenous bone, for maxillary sinus lift through the side window technique results in bone repair. O β-TCP vem sendo utilizado em cirurgias de enxertia óssea, principalmente em regiões superiores de maxila no intuito de favorecer a reabilitação protética com implantes. O objetivo desse trabalho foi avaliar a reparação tecidual de um biomaterial a base de beta fosfato tricálcio o β-TC em levantamento de membrana de seios maxilares de coelhos, através de análises histológica, histomorfométrica e imunoistoquimica. Vinte coelhos Albinus foram submetidos à cirurgia de elevação da membrana do seio maxilar bilateral, onde o seio maxilar do lado direito foi preenchido por coágulo e do lado esquerdo por β-TCP. Após períodos de 3, 7, 15 e 45 dias os animais sofreram eutanásia e tiveram sua maxila removida para processamento e análises histológica, histométrica e imunoistoquímica. Houve formação óssea para ambos os grupos somente a partir do período de 15 dias. Foi aplicado o teste estatístico de Tukey, e houve diferença estatística (p<0,01) entre o grupo controle e o grupo β-TCP. O melhor índice de formação óssea foi para o grupo β-TCP no perído 45 dias (0,0294 ± 33,011 %) em comparação ao grupo controle (1,824 ± 30,502%). Houve também diferença significativa dos grupos entre si, em relação aos períodos estudados. Sendo assim, comprovamos que o βTCP foi um substituto ósseo competente em relação à neoformação óssea e manutenção de área de defeitos clínicos, desejados para uma correta reabilitação oral / The beta tri calcium phosphate (β-TCP) is a biomaterial with excellent osteoconductive properties, biocompatibility by being chemically and crystallographically similar to the human bone tissue as well as having rapid reabsorption which reduces biological reactions. The β-TCP has been used in bone grafting surgery, especially in the upper regions of the maxilla in order to facilitate the prosthetic rehabilitation with implants. The aim of this study is to evaluate the healing of a biomaterial-based beta tri calcium phosphate, β-TC in sinus lifting of rabbits, using histological, histomorphometric and immunohistochemistry. Twenty Albinus rabbits underwent surgery to the bilateral maxillary sinus lift, where the right maxillary sinus was filled with blood clot and the left by β-TCP. After periods of 3, 7, 15 and 45 days the animals were euthanized and their jaw removed for processing and histological, immunohistochemistry and histometric analysis. There was bone formation for both groups only from the period of 15 days. The Tukey statistical test was applied, and there was statistical difference (p <0.01) between the control group and the β-TCP group. The best index of bone formation was for the β-TCP group in period prescribed 45 days (33,011 ± 0.0294%) compared to the control group (30.502 ± 1.824%). There were also significant differences among groups in relation to the studied periods. A elevação da membrana do seio maxilar com abordagem pela parede lateral é a técnica cirúrgica melhor documentada para a reconstrução desta região, com utilização de enxertos de origem autógena, xenógena, homógena ou aloplástica. O objetivo deste estudo foi avaliar a neoformação óssea e caracterizar os eventos que marcam a remodelação óssea após a realização de cirurgias de elevação de seio maxilar, utilizando o Bio-Oss® associado a osso autógeno ou isolado, através de imunomarcação das proteínas dos novos membros do fator de necrose tumoral, osteoprotegerina e o ligante do receptor ativador para o fator nuclear kβ (RANKL). 25 pacientes com bom estado de saúde geral foram selecionados para realizar procedimentos reconstrutivos da região posterior da maxila e posteriormente realizar a instalação de implantes osseointegrado em um segundo tempo cirurgico. Os seios maxilares foram divididos em 3 grupos, dependendo do material de enxerto: Grupo B: Bio-Oss®; Grupo BA: 1:1 Bio-Oss® + osso autógeno; Grupo A: Osso autógeno. Biopsias das áreas de instalação de implantes foram coletadas para avaliação histomorfométrica e imunoistoquímica. Os dados da avaliação histométrica foram submetidos à análise estatística paramétrica pelo analise de variância (ANOVA) e o teste de Tukey para determinar diferenças entre os grupos. A avaliação imunoistoquímica foi realizada por análise qualitativa ordinal através da atribuição de diferentes scores. Na avaliação histológica foi observada neoformação óssea nos três grupos, com presença de trabéculas... / The prosthetic rehabilitation with dental implants has become a common treatment, to replace missing teeth, after that the alveolar process go through the natural process of resorption, resulting ridges with poor bone volume for a good surgical and prosthetic planning. Sinus lifting technique, by the lateral window technique is well documented, using autogenous bone grafts, xenogenous, homogenous or alloplastic materials. The aim of this research was to assess new bone formation and the events of bone remodeling after maxillary sinus lifting surgery using Bio-Oss® associated with autogenous bone or isolated by immunolabeling of the new members of necrosis tumoral factor Osteoprotegerin (OPG) and the Receptor Activator for Nuclear Factor kβ (RANKL). Twenty-five patients with good general health status were selected to perform reconstructive surgeries of the posterior maxilla and later performed the installation of osseointegrated dental implants in a second surgical time. Maxillary sinus were divided in 3 groups, depending on the graft as follow: Group B: Bio-Oss®; Group BA: 1;1 Bio-Oss® + bone graft; Group A: Autogen bone graft. Biopsies of implant placement areas were collected for histometric and immunohistochemical assesses. Histometric data were statistically analyzed by ANOVA and Tukey parametric test, to establish differences among groups. The immunohistochemical assess was qualitative ordinal, giving scores to the protein stained. Histologic findings showed new bone formation in all groups, particles of Bio-Oss were observed on groups Bio-Oss and Bio-Oss-autogenous bone graft 1;1, surrounded by new bone. There were significant differences among groups related to new bone formation (ANOVA p=0.002), pointed out more new bone formation on the autogenous group.
