A mutual aid group programme for emergency personnel

Submitted in partial fulfilment of the requirements for the degree of Master of Arts in the Department of Psychology University of Zululand, 2004. / This report presents the findings of a study conducted in 2003-2004, involving eight members of emergency services in Stanger, KwaZulu Natal, South Africa. The emergency personnel included five firefighters and three paramedics. The aim of the study was to develop and evaluate a mutual aid group programme in order to prevent symptoms of trauma, with special reference to anxiety and depression, and promote psychological well-being in emergency personnel. Emergency personnel are exposed to traumatic events during the line of duty. The study investigated stressors, which were identified as organizational, management style, ineffective communication, stressors relating to patient care (personal loss, traumatic stimuli, high expectations) and low job and high workloads. The study also investigated psychological, physical and social effects of emergency work. Psychological effects that were identified were mental illnesses such as depression and posttraumatic stress disorder. Symptoms included irritability, anger, frustration, hopelessness, helplessness, fear and anxiety. Physical effects included fatigue, difficulty breathing, startle response, nausea, trembling and racing heart. Social problems such as conflicts with family, friends and colleagues were also identified. The study provided group members with the ability to identify symptoms of trauma and accept vulnerability, which served as important preventative measures for mental illness. The group created a safe atmosphere where members were able to share their feelings without the fear of being judged. It also provided members with new coping strategies for dealing with their feelings. Regarding further support systems the group members identified an ongoing need for help via psychologists, psychological debriefing and support from management, family as well as the community. / National Research Foundation

The effects on staff of working in an eating disorders unit

1 online resource (vii, 138 leaves : ill.) / Following an awareness of an increased turn over of staff in the eating disorder unit as compared with other specialised units, in a psychiatric hospital in Johannesburg, South Africa, Tara Hospital, the researcher was motivated to investigate the issue. An exploratory, descriptive based research was chosen to explore and gain information about the topic and its implications. A qualitative research approach was used to gain insight into the perceptions and needs of the team working on the unit. The case study method was used. A pilot study was undertaken to test the validity of the interview schedules. The sampling category was a non- probability one. Individuals were selected from the population of staff working on the unit. Participants were selected from two groups, those who were presently working on the unit and those who had previously worked on the unit and now working in other units. Interview procedure involved personal semi-structured interviews conducted by the researcher and analysed qualitatively and a structured interview questionnaire analysed quantitatively. The researchers assumption that many staff members move from working in an eating disorders unit was confirmed and is due to the following: Staff turnover is due to constant exposure to occupational stress and burnout. Feelings of helplessness, a sense of being unappreciated and excessive exposure to conflict from the patients. In addition, staff experience minimum rewards leading to lowered job satisfaction due to the patients slow recovery rates and a poor prognosis of the illness. Staff also experience a change in their eating patterns and an increased awareness around food and food issues. Recommendations to the staff include: * Psycho-education on eating disorders. * Implementation of strategies to provide supportive care for all staff members. * Education on stress management and strategies to prevent staff burnout and lowered job satisfaction. * A multidisciplinary teamwork approach by the staff, when working in the unit. / Social Work / M. An investigation into the effects of vicarious trauma experienced by health care workers

This study features an examination of the effects of working with traumatised individuals, namely vicarious traumatisation. Predictor variables such as age, career longevity, personal trauma history and workload were correlated with vicarious trauma. Support systems used by nurses were explored. The questionnaire consisted of items of the TSI Belief Scale, demographic characteristics of persons with a personal trauma history, work related aspects and support systems. Participants were 37 nurses (30 female and 7 male) who render a community mental health service in the Cape Metropole. Results of the study indicate that the variables age and career longevity were statistically significantly related to vicarious trauma. There was no relationship between personal trauma history, workload and vicarious trauma. The nurses identified the psychiatrist and psychologist as their main support systems in dealing with vicarious trauma. Long-term implications of critical incident stress among emergency responders

Critical Incident Stress has the potential to affect emergency services personnel to the degree that it can change the way the responder acts and reacts in all facets of his or her life, including the job and his or her family. Research into these potential effects has produced a greater understanding of the responders experiences within a short period of time after the perceived critical incident. This study investigates the long-term effects of critical incident stress among emergency responders from two cities in the three emergency services professions were interviewed to determine what their experiences were at least six months post critical incident. A structured incident had in three areas of teh emergency responders lives: impact on job, impact on the individual responder, and perceived impact on emergency responders families. For particpants, symptoms of Critical Incident Stress lasted between 6 months and 2 years after the perceived critical incident. Analysis of the data indicatees that single responder critical incidents have the potential to negatively affect emergency responders resulting in the loss of enthusiasm and passion for their work, debilitating psychological distress, and isolation from valued support systems. Long-term effects of Critical Incident Stress change the perceptions that responders have about the job, about themselves, and the relationships with their families. The culture of emergency services, changing identities, and the lack of support from both within the system and outside of the system were seen as variables that contribute to the long-term effects of Critical Incident Stress. / xi, 181 leaves ; 29 cm.

A case study describing factors perceived to be impacting staff satisfaction amongst health care professionals at the East London Hospital complex

This thesis was born from a concern the researcher had with regard to negative reports in the media emanating from 4 babies that died at Cecilia Makiwane Hospital Peadiatric ICU unit due to power supply failure. The most significant of these negative reports was in 2007 when the Daily Dispatch ran a series of articles regarding what they termed avoidable deaths over the last 14 years. The ease with which staff communicated with the media together with the high absenteeism rate and high turnover was a cause for concern. When the researcher analysed the history of the problem, it immerged from the respondents’ responses that the rationalistion process undertaken by the Eastern Cape Department of Health (ECDoH) was a significant root cause to the problem. The literature review focused on three areas viz.: Organisational Culture, Organisational Change, Foundations of Satisfaction. This focus was used to confine the problem to a manageable project but secondly each of the aspects are interwoven. Routledge (2010) notes that culture is the reflection of the values advocated by a founder or leader by way of his/her day to day actions. This is done by the leader creating a perception or viewpoint that assists the employees to achieve the organisation’s mission, vision and goals. In any organisation change is a constant and it needs to be effectively managed. With government institutions like the East London Hospital Complex (ELHC) directives come from the top and are implemented by an unprepared and untrained leadership and management cadre and clear communication of vision and objective of the desired outcomes never happens. The aim of the research was to: describe the existing Organisational Culture present at ELHC (Perform an organisational diagnosis); describe the impact of change (rationalisation) and to analyze why there was such a high staff turnover. It is clear from the results of the survey conducted that significant dissatisfaction prevailed relating to how the institution was managed. Dissatisfaction amongst the health professionals was general but also specific to the following: leadership and management issues, fairness, remuneration and lack of resources. The recommendations therefore focused on developing management and leadership within the proposal of Dubrin’s model (2001).

Literature on the Emergency Medical Services (EMS) has paid much attention to stressors, stress and symptoms, coping, and support of Emergency Medical Workers (EMWs). However, it has paid little attention to the needs of EMWs, and the satisfaction thereof, which should result in their improved well-being and motivation. In an attempt to rectify this imbalance, this study investigated the needs of EMWs as suggested by descriptions of their experiences within the EMS. Interviews were conducted with EMWs employed by a private EMS company. The interviews were analysed for emerging needs by means of interpretive analysis. These needs comprised the Need for Freedom, the Need for Competence, the Need for Recognition, the Need for a Challenge, and the Need to be Understood. The findings were linked with theory and it is proposed that the relevant EMS company incorporates participative management into its management approach.
