An analysis of the role of state, economy and civil society in the development, management and reform of the NHS, 1948-1997

Lockett, Anthony E. January 1999 (has links)
The NHS is the centrepiece of the UK welfare state. For fifty years it has provided the majority of health-care in the UK. However the running of the service has not been marked by a smooth operation. Repeated reforms have occurred since 1948 in attempts to increase the efficiency and effectiveness of the service. These reforms have been credited with varying degrees of success. Even the most radical reforms, initiated in 1990, have been marked by some failures - particularly in respect to the provision of services to 'at risk' groups such as the elderly, leading to criticisms of a lack of coherent policy making. The reasons that underlie the success of the NHS in the midst of failure are complicated, but one hypothesis is that the structure of the NHS does not reflect its basic functions. Those functions can be broken down into 2. First is the relief of suffering from illness; second is the support of the industrial and economic base of the UK. The existence of this pluralistic purpose implies that the management of the service requires balancing the forces of economic, state and civil society requirements for the NHS. This management is embodied in a complicated institutionalisation of care, covered in chapter 1. The empirical evidence gathered in the thesis, in chapters 2 and 3 both from literature and case studies, would indicate that at least part of the problems seen in the NHS result from a failure to balance this institutionalisation. However, the situation is made more complex as the result of this imbalance creates further increased demands from some of the elements in the management of the service. Therefore the failure to balance the interactions that surround the NHS increases the pressures on it which in turn increases the imbalance leading to a feedback loop magnifying the problem. The source and problems of this feedback are best exemplified by a case study of the most recent reforms -covered in chapters 4-11 of this thesis. This case study demonstrates that the way in which the 1990 reforms were formulated and implemented took little notice of the impact of the changes on the street level NHS managers - with the results that the reforms did not represent a coherent policy. The result of the lack of coherence is that the changes have not generated efficiency gains, and in some cases have diverted resources away from those most in need. The underlying cause of this is the predominance of non- market forces in the decision making process - i.e. the values of the purchasers and the power of the providers to influence decision making. The linkages between these features of the post reform NHS are described in chapter 12. It is likely that the only way in which the circle of problems in the NHS can be addressed is re-establishing the corporate relationship that surrounds health care. However unlike previous relationships the evidence suggests that the relationship should be established at a policy level, rather than the current trends for a local level relationship. The NHS is not unique in this aspect, as this is the pattern of change seen in many European Countries.

Inovação na atenção primária à saúde : um estudo de caso no município de Marau - RS

Silva, José Pretto da 31 August 2012 (has links)
Submitted by Ana Guimarães Pereira (agpereir@ucs.br) on 2014-11-12T11:28:30Z No. of bitstreams: 1 Dissertacao Jose Pretto da Silva.pdf: 1665682 bytes, checksum: b2736ff728060341ddcec157b0b48ed0 (MD5) / Made available in DSpace on 2014-11-12T11:28:30Z (GMT). This way, this mastering research searched to identify innovations on Health Primary Attention in the city of Marau-RS. To do so, a qualitative, exploratory, case study type survey was performed, applying as data collect instrument a semi structured interview with open and closed questions based on Oslo Manual (2004), reference of innovation of products and services, processes, marketing and management. A survey sample was formed by eight unit managers of Family Health Strategy (ESF) of the city cited above, as well as representatives of every department of central management of such structures. So, as final results it is possible to say the city has innovation on its public health system, but they are, however, just local innovations and it has reactive attitude, passive, regarding innovation. Among the innovations met on the city public health it is pointed innovations on marketing, such as Hepatitis B Vaccination Campaign; innovations on products and services, such as technological adaptation of municipal pharmacy to dispense basic, special and exceptional medicines and the promotion and adherence to programs such as Health at the Countryside and Health at the road; innovation on processes, such as the creation of Humanization Work Group and Family Health Support Center and Organizational or Management Innovations, pointing out city mapping due to elevated population growth and creation of ESFs coordinator post.

Building an E-health system for health awareness campaigns in poor areas

Gremu, Chikumbutso David January 2015 (has links)
Appropriate e-services as well as revenue generation capabilities are key to the deployment and the sustainability for ICT installations in poor areas, particularly common in developing country. The area of e-Health is a promising area for e-services that are both important to the population in those areas and potentially of direct interest to National Health Organizations, which already spend money for Health campaigns there. This thesis focuses on the design, implementation, and full functional testing of HealthAware, an application that allows health organization to set up targeted awareness campaigns for poor areas. Requirements for such application are very specific, starting from the fact that the preparation of the campaign and its execution/consumption happen in two different environments from a technological and social point of view. Part of the research work done for this thesis was to make the above requirements explicit and then use them in the design. This phase of the research was facilitated by the fact that the thesis' work was executed within the context of the Siyakhula Living Lab (SLL; www.siyakhulaLL.org), which has accumulated multi-year experience of ICT deployment in such areas. As a result of the found requirements, HealthAware comprises two components, which are web-based, Java applications that run in a peer-to-peer fashion. The first component, the Dashboard, is used to create, manage, and publish information for conducting awareness campaigns or surveys. The second component, HealthMessenger, facilitates users' access to the campaigns or surveys that were created using the Dashboard. The HealthMessenger was designed to be hosted on TeleWeaver while the Dashboard is hosted independently of TeleWeaver and simply communicates with the HealthMessenger through webservices. TeleWeaver is an application integration platform developed within the SLL to host software applications for poor areas. Using a core service of TeleWeaver, the profile service, where all the users' defining elements are contained, campaigns and surveys can be easily and effectively targeted, for example to match specific demographics or geographic locations. Revenue generation is attained via the logging of the interactions of the target users in the communities with the applications in TeleWeaver, from which billing data is generated according to the specific contractual agreements with the National Health Organization. From a general point of view, HealthAware contributes to the concrete realizations of a bidirectional access channel between Health Organizations and users in poor communities, which not only allows the communication of appropriate content in both directions, but get 'monetized' and in so doing becomes a revenue generator.

New Public Management and Governance Collide: Federal-Level Performance Measurement in Networked Public Management Networks

DeGroff, Amy S. 12 December 2009 (has links)
The purpose of this research was to investigate the implications of networked public management on the design, implementation, and utilization of federal performance measurement systems. A multiple, instrumental case study of four public health programs funded by CDC and implemented nationally through vertical and horizontal network structures was conducted. Cross-case findings suggest that the networked implementation structures for the four federal-level, public health programs have important implications for the design of the performance measurement systems. Specifically, the performance measurement systems were affected by four consequences of the implementation networks: the political influence of collaborative stakeholders; network variability; dependencies on voluntary, horizontal network partners to achieve outputs and outcomes; and jointly produced outcomes that compromise assigning agency-specific attribution and accountability. While these four factors did not deter the use of performance measurement by any of the programs, all had important consequences for the development and subsequent design of the performance measurement systems, including limiting the choice and types of measures, level of measurement, potential uses of the measures, and resources needed to implement and support the systems.
