LATE ADULT LIFE PLANNING: ADDRESSING SELF CAPACITY AND NEGLECT

This study explores the relationship between Adult Protective Service (APS) referrals and the link between dementia type behaviors. The question is does self-neglect, due to dementia precipitate multiple APS referrals? A quantitative-study was conducted based upon a random selection of fifty client files from the years 2014 and 2015. The information collected was based upon data provided to the APS hotline and what was determined by the APS investigators. SPSS software was used to determine the values of the data. Table 1 displays the results of demographics with 37% males and 63% females. Results of this study are inconsistent with the existing literature.

Primary Prevention of Child Abuse and Neglect: Identification of High Risk Adolescents Prior to Parenthood

The purpose of the study was to develop a valid and reliable instrument designed to assess the parenting and child rearing attitudes of adolescents, Responses to the instrument would allow for the identification of "high risk" adolescents, i.e., those adolescents whose attitudes towards parenting and child rearing indicated a high potential for child abuse. A review of the literature identified the following four parenting constructs most commonly associated with abusive parents: Construct A: Inappropriate parental expectations of the child; Construct B: Inability of the parent to be empathically aware of the child's needs; Construct C: Strong parental belief in the value of punishment; Construct D: Role reversal. These constructs served as the basis for the development of the item domain. The sample used in this study consisted of 2,628 adolescents attending schools located in Utah and Idaho. Employing a Likert format, three Prototypes of the instrument were developed and field tested. Substantial content validity was acquired through the judgments made by a panel of experts in child abuse, test construction, and attitudinal measurement. Construct validity of the instrument was established through the results obtained from factor analysis, interitem correlations, and item-construct correlations. The data generated from the factor analysis indicated 32 items had the highest positive factor loadings (>= .20) in each of the four identified constructs. The range of item-construct correlations for the 32 items (.53 to .75) indicated adequate to high degrees of relationship between the item scores and total construct scores. The data obtained relative to the internal consistency of the items indicated adequate level s of reliability for each construct (Construct A, .70; Construct B, .75; Construct C, .81; Construct D, .82). The test-retest reliability coefficient of the instrument showed an adequate level of stability over a one week period of time (.76). Raw scores, converted into factor scores, indicated approximately 3% of the adolescents who responded to the instrument scored -2 and -3 standard deviations away from the mean. Syftet med min studie är att bidra med kunskap kring hur man kan samverka mellan förskola och socialtjänst gällande barn som far illa. Min studie är utförd med kvalitativa intervjuer med verksam personal inom förskoleverksamheten och socialtjänsten. Jag intervjuade en förskollärare, en förskolechef och en specialpedagog inom förskoleverksamheten samt två socialsekreterare på barn - och familjeenheten på socialtjänsten. Mina intervjufrågor handlade om vilka erfarenheter de har av att samverka tillsammans samt tillvägagångssätten från att upptäcka oro för ett barn till att det görs en anmälan. Resultatet visar på att det håller på att det utarbetas en större grad av samverkan mellan de olika professionerna. Samt att de olika yrkesprofessionerna uttrycker att de vill ha en mer kontinuerlig kontakt och samverkan med varandra för att de anser att detta gynnar barnets och familjen bästa.  Mitt resultat visar även på att förskolepersonalen anser att det är en jobbig del av arbetet att göra en anmälan. De känner sig ofta osäkra på om de tecken som de ser är befogade nog till att göra en anmälan eller inte. Socialsekreterarna lyfter vikten av att göra en anmälan, de menar på att minsta misstanke på att ett barn far illa ska anmälas. De menar även att det inte är förskolans ansvar att avgöra om det är befogat eller inte, utan alla misstankar är befogade. De lyfter även att det inte är förskolans uppdrag att utreda, utan att det är socialtjänstens ansvar. / The purpose of my study is to provide knowledge on how to collaborate between preschool and social services regarding child abuse. My study is designed with qualitative interviews with staff working in early childhood education and social service. My interview questions were about their experiences of collaborating together regarding concern for a child. The results in my study shows that a bigger development between preschool and social service is being developed and the staff within the various professions want a more continuous contact and interaction with each other. They believe that this will benefit the child. My results also show that the preschool staff believes that it is a difficult part of the work to make a report to the social service. They often feel uncertain about the things they see are legitimate enough to make a report or not. Social workers highlight the importance of making a report, they argue that the slightest suspicion that a child is mistreated should be notified. They also believe that it is not the preschool staff responsibility to determine whether it is justified or not. They think that all suspicions are justified. They also highlight that there are the social workers responsibilities to investigate not the preschool workers.

Våga inte blunda : en studie om förskollärares syn på anmälningsplikt enligt socialtjänstlagen

The aim of this study was to capture preschool-teachers thoughts and experience basing on the obligation. Are there any policies and procedures for preschool-teachers to use when suspicion that a child is being abused occur. Are there some factors that may affect preschool-teachers to report to the social service? How does the preschool-teacher find the contact with the Social Service? I conducted four interviews with four preschool-teachers, two of these preschool teachers are employed at the same municipal preschool and the other two are employed at two other preschools. The interviews were semi-structured and were conducted at each individual’s workplace. To analyze my result, I used symbolic interactionism as a theory. The result shows that pree-schoolteachers find it difficult to use the mandatory notification law and lack of routines and not functional collaboration between the preschool and the social service are also some reasons why preschool teachers finds it difficult to report.

ATTENTION AND THE PARIETAL CORTEX: INVESTIGATIONS OF SPATIAL NEGLECT, OPTIC ATAXIA, AND THE INFLUENCE OF PRISM ADAPTATION ON ATTENTION

Some authors have argued that the primary function of the posterior parietal cortex is to control visual attention and awareness, whereas others have argued that the posterior parietal cortex is specialized for controlling actions. The purpose of the present thesis was to examine the influence of prism adaptation – a visuomotor adaptation technique – on visual attention deficits in patients with lesions of parietal cortex. Lesions to dorsal regions of the posterior parietal cortex lead to optic ataxia – a disorder in which visually guided reaching is disrupted. In contrast lesions to ventral (i.e. inferior) regions of the posterior parietal cortex of the right hemisphere lead to spatial neglect – a disorder in which patients are unaware of people or objects in contralesional (left) space. Chapter 1 presents an overview of the organization of the posterior parietal cortex, as well as an introduction to the disorders of spatial neglect and optic ataxia and the use of prism adaptation as a treatment for spatial neglect. Chapter 2 examined the influence of prism adaptation on attentional deficits in patients with right brain damage. Results demonstrated that prism adaptation reduced both the disengage deficit and the rightward attentional bias, two of the classic attentional deficits in neglect. Chapter 3 investigated the role of the dorsal posterior parietal cortex in controlling both reflexive and voluntary attention in two patients with optic ataxia. Lesions to the dorsal posterior parietal cortex led to both a disengage deficit and a rightward attentional bias, similar to patients with neglect, even though neither of the patients had any clinical symptoms of neglect. Contrary to previous work these results indicated that dorsal portions of the posterior parietal cortex – a region not commonly damaged in neglect – are important for controlling the orienting and reorienting of both reflexive and voluntary attention. Furthermore, these results indicated that optic ataxia is not purely a visuomotor disorder that is independent of any perceptual or attentional deficits as was previously assumed. Based on the results of Chapters 2 and 3 it was hypothesized that the beneficial effects of prism adaptation on attention may operate via the superior parietal lobe, a region which is typically undamaged in neglect, and is known to be important for controlling attention and action. Chapter 4 provided support for this hypothesis by demonstrating that a patient with lesions to the superior parietal lobe, who had the same attentional deficits as the right brain damaged patients tested in Chapter 2, failed to demonstrate any beneficial effects of prism adaptation on his attentional performance. Specifically, prism adaptation had no influence on his disengage deficit or his rightward attentional bias. Therefore, these data provide direct evidence that the beneficial effects of prisms on attention rely, at least in part, on the superior parietal lobe. Finally, Chapter 5 concludes with a summary of the research findings from the present thesis, and puts forward a new theory to conceptualize the mechanisms underlying the beneficial effects of prisms in patients with neglect.

Investigating Spatial Working Memory and Saccadic Remapping Processes in Healthy Young and Elderly Participants

Additional cognitive deficits, including impairments in spatial working memory and/or saccadic remapping processes, have recently been implicated in unilateral neglect – a neurological condition classically characterized as a disorder of attention. The interactions between saccadic remapping and three memory processes (position memory, object memory and object-location binding) were investigated in healthy young (n=27) and elderly (n=20) participants to establish a baseline of comparison for future use with neglect patients and to study the effects of aging on these processes. In a computerized task, participants were instructed to first detect a target, and then hold in memory either its position, identity or both over a delay period. Subsequently, participants were tested on their memory for that particular task. The saccadic remapping component was introduced at the onset of the delay period with the fixation cross shifting either to the left, or right, requiring participants to remap the visual array into either right or left space, or remaining in the centre of the screen (i.e., no remapping condition). In the position memory and object-location binding task, a consistent cost to memory performance was found when remapping right only for the young participants. Overall the elderly did not perform any of the tasks involving a position memory component as well as the young participants and showed spatial asymmetries in the target detection task. The lack of an effect of remapping in the elderly group may be due to a general decrement in performance. These results are discussed in terms of hemispheric asymmetries and cognitive theories of aging.

RELPH: A Computational Model for Human Decision Making

The updating process, which consists of building mental models and adapting them to the changes occurring in the environment, is impaired in neglect patients. A simple rock-paper-scissors experiment was conducted in our lab to examine updating impairments in neglect patients. The results of this experiment demonstrate a significant difference between the performance of healthy and brain damaged participants. While healthy controls did not show any difficulty learning the computer’s strategy, right brain damaged patients failed to learn the computer’s strategy. A computational modeling approach is employed to help us better understand the reason behind this difference and thus learn more about the updating process in healthy people and its impairment in right brain damaged patients. Broadly, we hope to learn more about the nature of the updating process, in general. Also the hope is that knowing what must be changed in the model to “brain-damage” it can shed light on the updating deficit in right brain damaged patients. To do so I adapted a pattern detection method named “ELPH” to a reinforcement-learning human decision making model called “RELPH”. This model is capable of capturing the behavior of both healthy and right brain damaged participants in our task according to our defined measures. Indeed, this thesis is an effort to discuss the possible differences among these groups employing this computational model.

Using iPhones to Enhance and Reduce Face-to-Face Home Safety Sessions

Innovative handheld technologies are changing the possibilities for delivering public health interventions. The present research describes a preliminary examination of the effects of iPhone™ both as an assessment tool for data collection and as an enhancement to an in-home child safety intervention. Three families with children under age seven were trained to use an iPhone to video targeted rooms in their homes following SafeCare® safety module intervention implementation during which rooms were secured for accessible safety and health hazards. The iPhone was used to communicate feedback, logistical information, and clarification of safety content. The effectiveness of iPhone and iPhone video was examined using a multiple baseline design across settings replicated across families. All rooms across subjects demonstrated significant decreases in home hazards. Face-to-face (F2F), in-home time of the home visitor was progressively reduced and replaced by video data collection over the course of the intervention. These data suggest that handheld technology tools are a promising means of data collection for in situ safety interventions and for augmenting interaction during intervention. Implications of these findings for reducing costs of F2F intervention as high-quality handheld video capabilities become increasingly ubiquitous and for engaging and retaining participants are discussed.
