Optimal feeding systems for small scale dairy herds in the North-West Province of South Africa

The North West Province (NWP) identified dairy farming as a priority as it has the potential, not only for job creation, but also as a sustainable source of high quality protein for rural communities. With the correct type of management systems, small-scale dairy farms have the potential to be economically feasible. For the purposes of this study, a small-scale dairy farm was defined as a farm which produced less than 500 litres of milk a day irrespective of the number of cows or size of the farm. The study area was Central North West Province and the study was a longitudinal observational study conducted with 15 small-scale dairy farmers from 2002-2006. Nutrition was found to be a major constraint to the production capacity of dairy cows studied. It was found that farmers were deficient in the knowledge, skills and experience required to develop an affordable and balanced feeding system based on locally available ingredients. Dairy rations were given to prevent malnutrition or starvation, rather than to increase production. It was also shown that feeding of the cattle on the farms investigated, was influenced more by availability and affordability of locally obtained feed ingredients than by planning nutrition to increase milk production. Available statistics show that there are approximately 257 000 dairy cattle in NWP, with the greatest numbers in the Central Region (175 235) and smaller numbers in the Western (59 852) and Eastern (21 873) Regions. These cattle produced approximately 230.4 million litres of milk annually (12.5% of national production) with an estimated value of R304.1 million at R1.32/l, excluding value-added products in the form of cheese, yoghurt, milk powder, and others in 2002. The method used was a longitudinal study conducted from 2002 to 2006 in three phases. In the first phase, situational analysis using participatory rural appraisal (PRA) and observation was used to outline the extent of the constraints and start to design appropriate interventions. Feeds used by the farmers for feeding dairy cows – both supplements and roughage - were tested and evaluated. In the second phase, three different feeding systems were designed from the data obtained from PRA, in consultation with small-scale dairy farmers, established commercial dairy farmers, state veterinary and agriculture staff, feed manufacturers and distributors and the commodity organization (MPO) to optimize the nutrition of the dairy cows. The third phase was field testing of interventions and observations of the implementation by farmers. It was found during the PRA phase that the majority (n=9) of farmers had been in dairy farming for not more than five years, five farmers had six to ten years in dairy farming and only one farmer had 11 to 15 years in dairy farming. Dairy farming is a very highly skilled operation and farmers need to have experience and knowledge to succeed. Five years is insufficient. Therefore capacity building and training were instituted over the period 2002 to 2005. Also, 60% (n=9) of the farmers were not affiliated to any agricultural organisation, so membership of the Milk Producers Organisation was facilitated for all farmers in the study. It was also found that the cattle were not identified and neither production nor financial records were kept. Testing and evaluation of feed used showed that it was of poor quality, deficient in protein, energy and minerals and no effort was made to balance the ration. In the second phase, three feeding systems were developed form data obtained and observations during phase one. These were A: a semi-intensive farm based ration using available crops, pastures and crop residues with minimal rations purchased; B: an intensive, zero-grazing dairy system using a total mixed ration (TMR) for farmers with smallholdings of less than 5 hectares per cow and C: Traditional, extensive or dual purpose system where the calf drank from the cow until weaning and milking was done only once a day, for farmers with more than 5 ha grazing available per cow. The last was a low-input/low output system and was implemented by a majority (n=8) of the farmers. System B was chosen by two farmers and not adopted by any of the two farmers in the long run. System A was adopted by three farmers. Four farmers left dairy farming for various reasons during the study. By July 2006, the farmers had changed to commercially formulated rations or licks and the body condition score of the cows had improved. Milk production per cow did not increase, but this may have been due to the increased price of meat and the fact that a majority of the farmers were using a dual purpose system and selling calves at weaning for a very good price. It was concluded that extension officers should get extra training in dairy if there are dairy farmers in their areas as this is a very specialist type of extension. They should also work closely with veterinary services including veterinarians, animal health technicians and the health inspectors. Further research should be done to optimise the traditional model as this is relatively profitable, has a lower risk and is less labour intensive. It is probably a good way to increase food security, particularly in families when only one or two members have an income from a pension or part-time employment. The prices realised from informal sales of milk and calves can give a stable income. The “community farms” should be economically evaluated in terms of each beneficiary being able to get a “living wage” out of the projected profits of the farm. The MPO and other stakeholders should give very specific training to new dairy farmers, based on the models that were used in this study. It is essential that framers be taught to “look forward” and get a pro-active attitude. They must also understand that quality, balanced rations are the key to success – poor rations are expensive rations, because they result in unhealthy cows and poor production. Development of teenage pregnancy prevention model for learners in schools in North West Province : implications for policy and practice considerations

Its impact is not only felt by the teenage girls, but also by their families, schools, and the country in general. This study sought to develop an educational social group work model on teenage pregnancy for learners in schools in the North West province. To this effect, this study was anchored on five objectives, namely: to establish the extent of teenage pregnancy amongst learners in schools in the North West province; to assess factors that predispose learners to indulge in sexual activities despite the risk of pregnancy and sexually transmitted diseases; to describe the roles of parents in the prevention of teenage pregnancy amongst learners; to identify prevention measures used by the schools to prevent teenage pregnancy; and to develop, implement, and evaluate an educational social group work model on teenage pregnancy for learners. The study adopted a mixed methods approach by combining the qualitative and quantitative paradigms. The population of the study consisted of learners, while school principals, deputy principals, Life Orientation teachers, and parents/guardians were the key informants. The findings of this study confirm that teenage pregnancy is a serious problem that continues to affect schools. All grades from the secondary schools that participated in this study reported cases of learner pregnancies. The grades with high reported cases include Grades 10, 11, and 12, while Grades 8 and 9 appeared to be the least affected. Furthermore, the findings indicate that male learners are most likely to father children with female learners. The development of an educational social group work model on teenage pregnancy for learners was achieved via a literature review and the empirical findings from learners and the key informants. Group work as a method of the social work profession was chosen in this study because it is intensive in nature, and the follow-up sessions provided a platform for the evaluation of social worker interventions and the performance of individual members and the entire group. Background: The global burden of cardiovascular diseases (CVDs) is escalating as part of the rapid health transition that developing countries are experiencing. This increase is associated with shifts in demographics and economics, two of the major factors that affect diet and activity. The term social drift phenomenon (SDP) is used to describe the observations that: in the early stages of the epidemiological and nutrition transitions, it is usually the more affluent, higher socio–economic groups that are affected; in the later stages, it is the poor, lower socio–economic groups that display the consequences of these transitions. Therefore, in developing countries at the beginning of the transition, affluent people have higher prevalence of obesity and increased CVD risk. In developed countries, at much later stages of the transition, obesity and increased CVD risk is more prevalent in the lower socio–economic groups. In South Africa, the Transition and Health during Urbanisation of South Africans (THUSA) study which was done in 1996/1998 indicated that at that time, most of the risk factors for CVD were observed in the more urbanised (richer) subjects. It is not known if this pattern changed in any way due to the present rapid urbanisation of South African blacks. Therefore, in this study we explored the associations between socio–economic status (SES) (measured by level of urbanisation, education and employment) and CVD risk factors in an African population undergoing transition in the North–West Province of South Africa, that were prevalent in 2005 when the baseline data for in the Prospective Urban and Rural Epidemiology (PURE) study were collected. Objectives: The main objective of this dissertation was to examine the SDP in an African population in a nutrition and health transition, by: (i) Reviewing the literature on associations between socio–economic variables and biological health outcomes focusing on CVD risk factors in developed and developing countries; (ii) Analysing the baseline data from the 2005 PURE study to examine the relationships between components of SES, namely level of iii urbanisation, education and occupation, and nutrition–related CVD risk factors in men and women participating in the PURE study; and (iii) Comparing results on these associations between CVD risk factors and SES from the PURE study with those found in the THUSA study, which was conducted almost 10 years earlier, to examine if social drift in these associations has taken place. Study design: The dissertation is based on a comparison of the CVD risk factors and socio–economic status of the THUSA and PURE studies. Secondary analysis of the baseline cross–sectional epidemiological data from the PURE study was executed. The South African PURE study is part of a 12–year Prospective Urban and Rural Epidemiology study which investigates the health transition in urban and rural subjects in 22 different countries. The main selection criterion was that there should be migration stability within the chosen rural and urban communities. The rural community (A) was identified 450 km west of Potchefstroom on the highway to Botswana. A deep rural community (B), 35 km east from A and only accessible by gravel road, was also included. Both communities are still under tribal law. The urban communities (C and D) were chosen near the University in Potchefstroom. Community C was selected from Ikageng, the established part of the township next to Potchefstroom, and D from the informal settlements surrounding community C. The baseline data for PURE were collected from October to December 2005. A total of 2010 apparently healthy African volunteers (35 years and older), with no reported chronic diseases of lifestyle, tuberculosis (TB) or known human immunodeficiency virus (HIV) were recruited from a sample of 6000 randomly selected households. Methods: A variety of quantitative and qualitative research techniques was used by multidisciplinary teams to collect, measure and interpret data generated from biological samples and validated questionnaires. For this study, the statistical package for social sciences (SPSS) package (version 17.0, SPSS Inc) was used to analyze the data. Means and 95% confidence intervals (CI) of CVD risk and dietary factors were calculated. Participants of both genders were divided into different groups (according to urbanisation, education and employment levels) and compared. Estimated significant differences between rural and urban participants were determined with analysis of variance using the general linear model (GLM), multivariate procedure. Univariate analysis was used to explore further the influence of education on CVD risk factors and dietary intakes. Employment was used as a proxy for income, and pairwise comparisons using GLM, multivariate procedure were done for comparing the three groups (Not answered, employed and not employed). Tests were considered significant at P<0.05. Results: Comparison of urban with rural subjects participating in the PURE study showed that urban men had significantly higher systolic and diastolic blood pressures and lower fibrinogen levels than rural men. In women, systolic and diastolic blood pressure, fasting blood glucose and serum triglycerides were significantly higher in urban subjects whereas fibrinogen levels were significantly lower among urban subjects. After examining the relationship between the level of education and CVD risk factors, we observed that men with higher education levels had significantly higher BMI. In women, serum triglycerides and blood pressure were lower and BMI was significantly higher in the educated subjects. Because it was difficult to distinguish between reported household and individual income levels, we compared CVD risk factors of employed and unemployed subjects. Employed men had significantly higher BMI whereas the unemployed men had significantly higher fasting glucose and fibrinogen levels. Although mean blood pressure of employed men was higher than that of unemployed men, the difference did not reach significance. In women, the only significant difference seen was that employed women had lower high density lipoprotein (HDL) cholesterol, fasting glucose, triglycerides and fibrinogen levels, but they had a significantly higher BMI. Employed women had significantly higher BMI than unemployed women (27.9 [26.3–29.4] versus 26.5 [26.0–27.0] kg/m2). It seems that most of the nutrition related CVD risk factors were still higher in the higher socio–economic group, a situation similar to that reported in the THUSA study. v Conclusion: The results of this study showed little evidence of a major social drift in CVD risk factors from subjects participating in the 1996/1998 THUSA study to those in the 2005 PURE study. Most cardiovascular disease risk factors are still higher in the higher SES groups. However, there were some indications (increased fibrinogen in both men and women living in rural areas; higher triglyceride and fasting glucose levels in unemployed women; no significant differences in blood pressure and total cholesterol across different SES groups which existed in the THUSA study) that a social drift in CVD risk factors in our African population is on the way. Supervision and trust in community health worker programmes at scale: developing a district level supportive supervision framework for ward-based outreach teams in North West Province, South Africa

Philosophiae Doctor - PhD / Introduction: National community health worker (CHW) programmes are to an increasing extent being implemented in health systems globally, mirrored in South Africa in the ward-based outreach team (WBOT) strategy. In many countries, including South Africa, a major challenge impacting the performance and sustainability of scaled-up CHW programmes is ensuring adequate support from and supervision by the local health system. Supervisory systems, where they exist, are usually corrective and hierarchical in nature, and implementation remains poor. In the South African context, the absence of any guidance on CHW supportive supervision has led to varied practices across the country. Improved approaches to supportive supervision are considered critical for CHW programme performance. However, there is relatively little understanding of how this can be done sustainably at scale, and effective CHW supervisory models remain elusive. Research to date has mostly positioned supervision as a technical process rather than a set of relationships, with the former testing specific interventions rather than developing holistic approaches attuned to local contexts. This doctoral study was exploratory in nature, seeking to generate an in-depth and contextualised understanding of the supervision phenomenon in one specific district in the North West Province (NWP) in South Africa. Using co-production methodology in an iterative approach, the study culminated in the formulation of a supportive supervision framework with CHWs and other frontline actors. Methods: The study was based on a holistic conceptual framework of supportive supervision, which was viewed as comprising three core functions ‒ accountability, development and support ‒ embedded in a complex and multi-level system of resources, people and relationships. To address the study objectives, the research used a mix of qualitative and quantitative methods. Three studies were conducted in a phased process: study 1 comprised a qualitative description of policy and practices in two districts related to the supervision of WBOTs; study 2 identified the main actors and mapped the supervisory system of WBOTs in the district, using social network analysis (SNA); and study 3 involved a qualitative exploration of workplace and interpersonal trust factors in the district and the supervisory system of WBOTs in the district. These three studies provided inputs for a workshop aimed at developing recommendations for a district-level, WBOT supportive supervisory framework. Four published papers reporting on the research conducted are presented in this thesis. It should be noted that the research was conducted during a turbulent political and administrative period in the NWP, when the WBOT programme changed from being a flagship programme for the country to one in crisis. This shifting context needs to be borne in mind when the findings are viewed and interpreted. Results: The study identified weaknesses in both the design and implementation of the supervisory system of WBOTs, with the absence of clear guidance resulting in WBOTs and PHC facilities performing their roles in an ad hoc manner, defined within local contexts. The study documented evidence of high internal cohesion within WBOTs and (where present) with their immediate outreach team leaders (OTLs). However, the relationships between WBOTs and the rest of the primary health care (PHC) and district health system were characterised by considerable mistrust – both towards other workers and the system as a whole. This occurred against a backdrop of increasing OTL vacancies, and the perceived abandonment of WBOT training and development systems and career opportunities. These findings are not dissimilar to those reported previously on the WBOT programme in South Africa and in programmes in other low-resource settings. Nevertheless, through its in-depth, exploratory and participatory approaches, this study provides additional insights into the phenomenon of supportive supervision. Firstly, in conceptualising supportive supervision as a set of ‘bundled’ practices within complex local health systems, the findings reflected the complexity of everyday realities and lived experiences. Secondly, through the embedded nature of the research and the phased data-collection process, the study was able to observe the impact of wider health system contexts and crises on the coalface functioning of the WBOT programme. Thirdly, the study emphasised how supportive supervision depends on healthy relational dynamics and trust relationships, and, finally, how a co-production approach can translate broad guidance, experience and theoretical understanding into meaningful, local practice owned by all the actors involved. Ultimately, the process of engagement, building relationships and forging consensus proved to be more significant than the supportive supervision framework itself. Conclusion: The lack of explicit, coherent and holistic guidance in developing CHW supportive supervision guidance and the failure to address supervision constraints at a local level undermine the performance and sustainability of CHW programmes. Effective supportive supervisory systems require bottom-up collaborative platforms characterised by active participation, sharing of local tacit knowledge and mutual learning. Supervisory systems also need to be designed in ways that promote relationships and generate trust between CHW programmes, other actors and the health system.

The implementation of professional development in the foundation phase in the North West Province with reference to Curriculum and assessment policy statement

The purpose of this study was to investigate the impact of the training given to Foundation Phase (FP) teachers in the implementation of Curriculum and Assessment Policy Statement (CAPS), with the intention to develop an alternative professional development (PD) approach for this Phase. Literature indicates that the academic success of learners can be significantly affected by Foundation Phase teachers’ access and participation in quality PD activities. It also indicates that PD is much more than training. It includes on-going workshops, follow-up, study, reflections, observations and assessment which accommodates FP teachers as learners, recognises the long-term nature of learning, and utilises methods that are likely to lead teachers to improve their practice as professionals. In this study, mixed methods research approach was employed to collect empirical data. It includes questionnaires responded to by 84 FP teachers, three sets of interview questions, that is, one for 9 primary school principals, one for 3 subject advisors, and the last one for the Focus group made up of 10 FP teachers who were not included in questionnaires. Observation at training centre was also conducted. The study revealed that the responses from the five sets of data collection instruments seemed to agree on the following: - FP teachers were not involved in the design of their PD activities; - An appropriate time for FP teachers to engage in PD activities is during school holidays; - The length of the training was too short; - Training was not based on the teachers identified needs; and - It was not designed by teachers in cooperation with experts in the field. This study therefore recommends the use of mixed-models approach because the Cascade model that was used, is not addressing the needs of all FP teachers. / Educational Leadership and Management / D. As a result, the impact and outcome of climate change on rural livelihood capitals, including economic development cannot be overemphasized in Ngaka Modiri Molema District Municipality of the North West Province of South Africa, where the study took place. It is one of the largest maize production regions in South Africa, where a preponderance of the people in the province obtain their livelihood from agriculture which contributes enormously to the promotion of household’s food security. The study, therefore, investigated the adaptation strategies, awareness of climate change, factors that influenced climate change adaptation in North West Province of South Africa, with the aim of ascertaining the effects of climate change on livelihood capitals among small and emerging maize farmers. Stratified random sampling technique was used to select three hundred and forty-six (346) farmers who were interviewed from the study area, while a pre-tested questionnaire was administered to the maize farmers, aiming at matters related to climate change impact on livelihood and adaptation. Data were analyzed using descriptive statistics while inferential statistical tools employed were Principal Component Analysis, Two-Stage Least Square regression model, Binary Logistic regression model, and Tobit regression model. The results of the study showed that climate change was linked to rural livelihood capitals as climate change awareness, low profit and co-operative finance were statistically significant (p<0.05). The study also established that majority of the rural farmers in the study area were aware of climate change, in which farm size, education, ownership of the farm, information received on climate change, source of climate change information, climate change information through extension services, channel of information received on climate change and support received on climate change were statistically significant (p<0.05). Factors such as farm size, household gender, type of farms, who owns the farm, land acquisition, source of climate change information, support received on climate change, and adaptation barrier were statistically significant (p<0.05) and influenced climate change adaptation in the study area. Conclusively, climate change is entwined with rural livelihood, and the variables that are significant to the study were identified. It was therefore recommended that government intervention, access to information, extension service and support, farmers’ networking, adoption of drought and heat stress tolerant seeds, indigenous knowledge should be improved, practiced and promoted among the rural farmers and the stakeholders involved in the study area. / Agriculture, Animal Health and Human Ecology / D. The implementation of professional development in the foundation phase in the North West Province with reference to Curriculum and assessment policy statement

The purpose of this study was to investigate the impact of the training given to Foundation Phase (FP) teachers in the implementation of Curriculum and Assessment Policy Statement (CAPS), with the intention to develop an alternative professional development (PD) approach for this Phase. Literature indicates that the academic success of learners can be significantly affected by Foundation Phase teachers’ access and participation in quality PD activities. It also indicates that PD is much more than training. It includes on-going workshops, follow-up, study, reflections, observations and assessment which accommodates FP teachers as learners, recognises the long-term nature of learning, and utilises methods that are likely to lead teachers to improve their practice as professionals. In this study, mixed methods research approach was employed to collect empirical data. It includes questionnaires responded to by 84 FP teachers, three sets of interview questions, that is, one for 9 primary school principals, one for 3 subject advisors, and the last one for the Focus group made up of 10 FP teachers who were not included in questionnaires. Observation at training centre was also conducted. The study revealed that the responses from the five sets of data collection instruments seemed to agree on the following: - FP teachers were not involved in the design of their PD activities; - An appropriate time for FP teachers to engage in PD activities is during school holidays; - The length of the training was too short; - Training was not based on the teachers identified needs; and - It was not designed by teachers in cooperation with experts in the field. This study therefore recommends the use of mixed-models approach because the Cascade model that was used, is not addressing the needs of all FP teachers. / Educational Leadership and Management / D. Management of discipline in a post corporal punishment environment : case study of primary schools in the informal settlements in the North West Province

The study deals with the management of discipline in a post corporal punishment environment in South Africa through a case study of selected primary schools in informal settlements in the North West Province. The following research questions were formulated:  What is the current thinking and practice of discipline in the primary schools in the informal settlements in the North West Province?  Which disciplinary measures and procedures are currently used in the selected primary schools?  How effective are the current management strategies of discipline used in the selected primary schools?  How can recommendations assist educators in selected primary schools to deal more effectively with disciplinary challenges? A literature review on the management of school discipline provided a conceptual framework for the empirical inquiry and indicated a lack of empirical studies on the management of discipline in primary schools in informal settlement in the North West Province. This matter is dealt with mostly frequently at high school level. An empirical study using qualitative research methods explored the management of discipline in twenty sampled primary schools in informal settlements in the North West Province, selected through purposive sampling. Data were gathered by in-depth interviews and focus groups with educators, principals and School Governing Body chairpersons. Data were categorized into themes. Findings showed that some educators still view corporal punishment as the most appropriate strategy to deal with ill-disciplined behaviour in schools; hence corporal punishment is still rife in some of the sampled schools. However, other participants agree that corporal punishment should not be accommodated as stipulated by the South African Constitution and other legal frameworks, instead effective alternative strategies should be deployed to handle ill-disciplined behaviour. Inhuman and outdated approaches should be avoided. Diverse ways of dealing with discipline which are goal oriented and foster good relations between the learner and the educator should be implemented. Sound relations in turn lead to the production of learners with the potential to become good citizens. Based on the literature and empirical inquiry, recommendations were made which advocate educator training on handling discipline in primary schools and the design of effective strategies to maintain sound discipline. / Educational Leadership and Management / D.
