Who cared for the carers? : a study of the occupational health of general and mental health nurses 1890 to 1948

This thesis set out to explore the neglected field of nurses’ occupational health. Evidence from the three case study hospitals confirms that attitudes toward nurses’ health changed between 1888 and 1948. The health of nurses was an issue that was always taken seriously but each institution approached the problem differently and responses showed much variation over time. There were good reasons for this but the failure to adopt a coherent and consistent policy worked to the detriment of nurse health. This difficulty helps explain the ambiguous treatment of occupational health within wider histories of nursing. This can lead to the erroneous conclusion that occupational health was somehow neglected by contemporary actors, thereby facilitating the omission of the subject from historical studies concentrating on professional projects and the wider politics of nursing. This study takes a different approach showing that occupational health issues were inexorably connected to these nursing debates. Occupational health cannot be understood without reference to professional projects. This is as true in debates where occupational health was obscured as it was in cases of overt concern. The history of the occupational health of nurses is also important because it offers a new perspective on two other themes central to nursing history, particularly class and gender. This focus helps understand why attitudes towards the care of sick nurses changed over time and varied between different types of institutions. By concentrating on individual nurses’ experiences we reveal something new about the way national conversations affected ordinary nurses’ lives. Recognition that nursing presents a serious occupational health risk is a relatively recent phenomenon; it was not until the 1990s that most nurses had access to occupational health units. This study not only sheds light on why nurses’ health attracted little attention before the Second World War but also explains why this situation began to change from the 1940s.

This is a cross-sectional analytical study with a quantitative approach, using the Nordic Osteomuscular Symptoms Questionnaire (QNSO) and an occupational sociodemographic questionnaire. A total of 119 nurses and nursing technicians from the various sectors of the Hospital Escola were surveyed. After the application of the instruments, the data were tabulated in a spreadsheet using the Excel software (Microsoft 2013) and later analyzed with the statistical package Statistical Package of Social Sciences (SPSS 24.0). Through this, descriptive analyzes were performed using frequencies, mean and standard deviation. The characterization of the sociodemographic profile and organizational environment were performed by means of the absolute (n) and relative (%) frequency. The normality of the data was verified using the Shapiro- Wilk test. The comparison of the musculoskeletal symptoms scores with the sociodemographic profile and organizational environment was performed using the Mann-Whitney or Kruskal-Wallis tests. Of the 119 professionals interviewed, 97.5% were female, 103 reported some type of symptom and 63% reported having children. Of these workers, 60.5% reported pain in the lower back in the last 12 months and 43.7% in the last seven days, with pain in this region being the major cause of withdrawal from work. People who had children moved away from work more than those who did not have children (p = 0.03). Those who reported some symptom in the last 12 months also considered that their work environment posed some health risk (p = 0.02). In addition, those interviewed who reported that " (p <0.01) and in the last seven days (p = 0.02). The present study demonstrated the need to seek improvements in working conditions in order to reduce the risk of nursing professionals developing musculoskeletal disorders related to the work environment. / O objetivo desse estudo foi avaliar os sintomas de distúrbios osteomusculares relacionados ao trabalho na equipe de enfermagem do Hospital Escola Santa Casa de Misericórdia de Goiânia/GO. Trata-se de um estudo transversal analítico com abordagem quantitativa, utilizando-se do Questionário Nórdico de Sintomas Osteomusculares (QNSO)e um questionário sociodemográfico ocupacional. Foram pesquisados 119 enfermeiros e técnicos de enfermagem dos diversos setores do referido Hospital Escola. Após a aplicação dos instrumentos, os dados foram tabulados em planilha com a utilização do software Excel (Microsoft 2013) e posteriormente analisados com o auxílio do pacote estatístico Statistical Package of Social Sciences (SPSS 24.0). Por meio deste, foram realizadas análises descritivas utilizando-se frequências, média e desvio padrão. A caracterização do perfil sociodemográfico e ambiente organizacional foram realizadas por meio da frequência absoluta (n) e relativa (%). A normalidade dos dados foi verificada utilizando o teste de Shapiro-Wilk.A comparação dos escores dos sintomas osteomuscularescom o perfil sociodemográfico e ambiente organizacional foi realizado utilizando os testes de Mann-Whitney ou Kruskal-Wallis. Dos 119 profissionais entrevistados, 97,5% eram do sexo feminino, 103 referiram algum tipo de sintoma e 63% relataram ter filhos. Desses trabalhadores, 60,5% relataram dor na região lombar nos últimos 12 meses e 43,7% nos últimos sete dias, sendo a dor nessa região a maior causa de afastamento do trabalho.Pessoas que possuíam filhos se afastaram mais do trabalho do que aquelas que não possuíam filhos (p=0,03).Aqueles que relataram algum sintoma nos últimos 12 meses também consideraram que o seu ambiente trabalho trazia algum risco à saúde (p=0,02).Além disso, os entrevistados que informaram que “costumavam se cansar” também apresentaram algum sintoma osteomuscular nos últimos 12 meses (p <0,01) e nos últimos sete dias (p=0,02). O presente estudo demonstrou necessidade de se buscar melhorias nas condições de trabalho, com vistas a diminuir o risco de os profissionais de enfermagem desenvolverem distúrbios osteomusculares relacionados ao ambiente de trabalho.

This is a cross-sectional analytical study with a quantitative approach. An interview was conducted with 56 nurses from the Family Health Strategy of the northern and northwestern regions of the city of Goiânia/GO. Two questionnaires were used, being an occupational sociodemographic and the other evaluating stress at work. A descriptive analysis of the variables was performed using mean, frequency and standard deviation. The scores for each question were calculated as well as the overall stress score. The test used to evaluate the existence or not of a statistically significant difference (p≤0.05) between independent and multiple variables was analysis of variance (ANOVA). The results showed that the higher the age, the greater the level of irritation about this organizational situation (p = 0.046). Regarding the gender issue, the study showed that women comprised 94.6% of the sample, and the stressor is directly related to the supervisor, who is not responsible for significant responsibilities (p = 0.011) and those who affirm that they do not have a religion constitute a group Which tends to feel isolated in the organization (p = 0.003). It is concluded that the nurses were highly stressed with the management mode in which they are submitted, the lack of appreciation, discredit their work and empower third parties of their deeds. Such stressors can trigger moral distress, various signs and symptoms, associating stress with different pathologies, such as Burnout Syndrome and chemical dependence. / Este estudo teve o objetivo de avaliar o nível do estresse ocupacional dos enfermeiros da Estratégia Saúde da Família. Trata-se de um estudo transversal analítico com abordagem quantitativa. Foi realizado uma entrevista com 56 enfermeiros da Estratégia Saúde da Família das regiões norte e noroeste da cidade de Goiânia/GO. Foram utilizados dois questionários, sendo um sociodemográfico ocupacional e outro de avaliação do Estresse no Trabalho. Foi realizada uma análise descritiva das variáveis utilizando-se média, frequência e desvio padrão. Após foram calculados os escores de cada questão da escala, bem como o escore de estresse global. O teste utilizado para avaliar a existência ou não de diferença estatisticamente significativa (p≤0,05) entre amostras independentes e múltiplas variáveis foi a análise de variância (ANOVA). Os resultados identificaram que quanto mais elevada a idade, maior também é o nível de irritação acerca dessa situação organizacional (p=0,046). Na questão de gênero a pesquisa demonstrou que as mulheres compuseram 94,6% da amostra, sendo que o fator estressor está diretamente relacionado ao supervisor de não as incumbirem de responsabilidades importantes (p=0,011) e aqueles que afirmaram não ter religião constituem um grupo que tende a se sentir isolado na organização (p=0,003). Conclui-se que os enfermeiros encontravam-se altamente estressados com o modo de gestão em que estavam submetidos, pela falta de valorização, desacreditados de seu trabalho e empoderamento de terceiros de seus feitos. Tais fatores estressores podem desencadear sofrimento moral, sinais e sintomas diversos, associando o estresse a diferentes patologias, como a Síndrome de Burnout e a dependência química.
