A process evaluation of the Breakfast For Kids (BFK) student nutrition programs: perspectives of program coordinators

Ensuring students are well fed can have positive social, behavioural, and academic benefits. Schools reach almost all children and the food they consume there can significantly contribute to their overall dietary intake. Universal access to school nutrition programs (SNPs) can ensure that children at-risk for poor nutrient intake have access to safe, healthy foods, thereby promoting growth and development and enhancing academic performance. The purpose of this research study was to evaluate the processes and structures of an Ontario Region’s student nutrition programs and to determine how the public health departments’ staff can support the program. This mixed method evaluation included a quantitative survey (n=62; 76% response rate) and qualitative interviews involving 22 program coordinators. The survey elicited a description of programs and variations in components being offered. Interviews with coordinators provided information regarding strengths, weaknesses, opportunities, and threats. Current and potential partnerships between programs and health units were also evaluated. All school levels were well represented in both quantitative and qualitative methods, with community-based programs being under-represented. Survey results showed that teachers and volunteer program coordinators were the most involved in planning and delivering programs. Also, more programs had public health inspectors involved (22.4%), compared to public health nurses (14.0%) or nutritionist/dietitians (9.1%). Although only 17.3% of coordinators reported wanting more general public health involvement, 27.8% wanted menu planning and nutrition support, and 25.5% wanted food safety training. Overall, qualitative results showed that SNPs in the Region varied enormously. The major strengths reported by coordinators included universality, the ability to reach needy students, and the ability to provide social opportunities for students. Major weaknesses included forming partnerships, lack of volunteers, scheduling/timing issues, and coordinator work-load. Common threats included lack of sustainable funding, complexity in tracking program use, unreliable help, and school administration conflicts. Finally, opportunities described by coordinators included assistance with menu planning, expansion of program offerings, and assistance with finding community partners. This research has identified strategies to help support SNPs. Because not one program in the Region is run the same way, multiple strategies need to be in place to support programs at individual levels. Therefore, health units can have a major role, whether it is through menu planning, food safety training, helping coordinators find healthy food options, or helping them build partnerships to enhance program support and operations.

Association between food assistance program participation and overweight / Associação entre participação em programa de assistência alimentar e sobrepeso

OBJETIVO Investigar associação entre participação no programa de assistência alimentar e sobrepeso/obesidade, segundo nível de pobreza. MÉTODOS Estudo transversal com dados sobre 46.217 mulheres não gestantes e não lactantes, de Lima, Peru, obtidos de pesquisas, com representatividade nacional, nos anos de 2003, 2004, 2006 e 2008-2010. A variável dependente foi o sobrepeso/obesidade e a independente foi a participação no programa de assistência alimentar. Utilizou-se regressão de Poisson; os modelos foram estratificados por nível socioeconômico familiar para todo o país, por área de residência (Lima versus o resto do país; urbano versus residência rural) e anos de estudo (2003-2006 versus 2008-2010). Os modelos foram ajustados por idade, nível acadêmico, urbanização e ano de estudo. RESULTADOS Participar do programa de assistência alimentar associou-se com risco aumentado de sobrepeso/obesidade para as mulheres que viviam em domicílios sem indicadores de pobreza (PR = 1,29; IC95% 1,06;1,57). Quando estratificados por área de residência, foram observadas associações similares para as mulheres que vivem em Lima e em áreas urbanas; não foram encontradas associações entre a participação no programa de assistência alimentar e sobrepeso/obesidade entre as mulheres que vivem fora de Lima ou em áreas rurais, independentemente de sua condição de pobreza. CONCLUSÕES Participar do programa de assistência alimentar associou-se com sobrepeso/obesidade para mulheres não pobres. Estudos adicionais serão necessários em países que enfrentam ambas as faces da má nutrição. / pia.chaparro@chess.su.se / OBJECTIVE The objective of this study was to investigate the association between food assistance program participation and overweight/obesity according to poverty level. METHODS A cross-sectional analysis of data from 46,217 non-pregnant and non-lactating women in Lima, Peru was conducted; these data were obtained from nationally representative surveys from the years 2003, 2004, 2006, and 2008-2010. The dependent variable was overweight/obesity, and the independent variable was food assistance program participation. Poisson regression was used to stratify the data by family socioeconomic level, area of residence (Lima versus the rest of the country; urban versus rural), and survey year (2003-2006 versus 2008-2010). The models were adjusted for age, education level, urbanization, and survey year. RESULTS Food assistance program participation was associated with an increased risk of overweight/obesity in women living in homes without poverty indicators [prevalence ratio (PR) = 1.29; 95% confidence interval (CI) 1.06;1.57]. When stratified by area of residence, similar associations were observed for women living in Lima and urban areas; no associations were found between food assistance program participation and overweight/obesity among women living outside of Lima or in rural areas, regardless of the poverty status. CONCLUSIONS Food assistance program participation was associated with overweight/obesity in non-poor women. Assessing the Sustainability of Manitoba School Breakfast Programs: A Qualitative Approach

Purpose: To access long-term sustainability of Manitoba SBP by examining the beliefs, attitudes and values of the diverse stakeholders involved in ensuring that SBP run. Sample/ Setting: Eighteen participants from the public school system, government departments and private funders in Manitoba. Method: Semi structured interviews to explore the perceptions, attitudes, and experiences of participants. Results: The perceived impact of SBP varied between each group of stakeholders though all shared a common belief that SBPs were beneficial to students. Major barriers identified were related to funding and manpower. Conclusion: All stakeholders support SBP because they understand and value the positive health, educational and social benefits SBP have for students and the wider school community. Long-term sustainability of SBP depends on creating and maintaining a positive policy environment in which SBP are seen as a valuable tool in supporting child nutrition so that funding and resources are allocated appropriately. / October 2015

Teenage attitudes/practices toward the School Breakfast Program and the National School Lunch Program at James Monroe High School, Fredericksburg, Virginia

The National School Lunch Program (NSLP) and the School Breakfast Program (SSP) operate daily in the nation's schools providing nutritious meals to children. These federally sponsored programs operate with the goal of providing nutritious meals for children so as to safeguard their health and well-being. Although participation rates in the school nutrition programs vary according to locality it has been shown that as students progressed in grade level and age participation in the school nutrition programs declined. This study identified the attitudes and practices of teenage students toward the school nutrition programs and evaluated if these attitudes had an affect on participation. Teenagers from a Fredericksburg, Virginia school were questioned using a survey format as the research tool. Students believed (77%) that the SBP and the NSLP should continue and they responded that these programs insured that they ate meals. Teenagers receiving free or reduced priced meals showed attitudes more favorable toward the programs while students paying a full price were more inclined to express dissatisfaction. Respondents did not view the school menu as "healthy" (58%). Eating at school provided an opportunity to socialize with friends which was an important aspect of eating at school for these students. Teenagers expressed an avid interest in learning about nutrition (70%) yet, they did not choose food according to nutritional content (70%). Teens found eating breakfast and lunch meals made them feel better physically and helped them concentrate on their school work (55%). / Master of Science

Storytelling as a food safety training tool in school foodservice

Sauer / Understanding what motivates food handlers is important for developing effective interventions to increase compliance with food safety guidelines. Traditional food safety training techniques improve knowledge, but do not always result in improved performance in the workplace. Studies have consistently demonstrated the persuasive power of stories to influence beliefs and behavior. Transportation into a narrative world is a state of immersion into a story. When individuals are transported into the world of a story, they integrate story information into their real-world beliefs and behavior. In this study, foodservice employees were shown a brief video dramatizing a foodborne illness outbreak. Behavioral intent to comply with food safety guidelines was measured both before and after watching the video. Transportation into the story and story-specific beliefs were measured as well. Study results confirmed previous findings involving narrative transportation. Participants who were highly transported into the story of Glenda’s Horrible Day reported stronger food safety behavioral intent, specifically in areas highlighted by the story, after viewing the video. Highly transported participants also reported stronger agreement with food safety messages after viewing the video. For participants who experienced low transportation into the story, there were no significant increases in behavioral intent or story-specific beliefs after viewing the video. Highly transported participants were those who (a) were familiar with the story topic, (b) were mentally engaged with the story, (c) responded emotionally to the story, and (d) identified with and felt empathy for the story characters.

Introdução: O baixo consumo de frutas e hortaliças é um importante fator de risco para diversas doenças crônicas e obesidade. Na população brasileira, o consumo destes alimentos é tradicionalmente abaixo das recomendações internacionais. Dentro deste cenário, os locais de trabalho vêm atraindo cada vez mais interesse como ambientes propícios para o desenvolvimento de ações de promoção de alimentação saudável direcionadas à população adulta. Objetivo: Avaliar o impacto de uma intervenção para aumentar a oferta e o consumo de frutas e hortaliças em empresas cadastradas no Programa de Alimentação do Trabalhador. Métodos: Trata-se de um estudo de intervenção com amostra de 29 empresas divididas em dois grupos: intervenção (n=15) e controle (n=14). Avaliou-se uma amostra de trabalhadores em todas as empresas participantes do estudo para verificar os efeitos da intervenção sobre o consumo de frutas e hortaliças. A intervenção foi baseada no modelo ecológico de promoção de saúde, teve quatro etapas com duração total de seis meses. A coleta de dados ocorreu em dois momentos: antes e após a intervenção. O efeito da intervenção foi investigado por meio de dois indicadores: oferta em gramas de frutas e hortaliças nas refeições servidas nas empresas e o consumo em gramas destes alimentos pelos trabalhadores. De forma secundária, avaliou-se o impacto da intervenção sobre a oferta de energia, macronutrientes e fibras nas refeições. Para avaliar o efeito da intervenção sobre a oferta de alimentos e nutrientes nas refeições, realizou-se comparação entre os grupos intervenção e controle e a comparação intra-grupos antes e após a intervenção. Para o consumo dos trabalhadores, utilizou-se análise de regressão linear para avaliar a mudança no consumo no grupo intervenção após a intervenção. Resultados: A oferta de frutas e hortaliças aumentou de forma significativa após a intervenção nos cardápios oferecidos pelas empresas no grupo intervenção, cerca de + 49g, enquanto não houve variação no grupo controle. Também se observaram efeitos positivos na oferta de fibras e redução na oferta de gorduras. A intervenção também aumentou o consumo de frutas e hortaliças entre os trabalhadores nas refeições oferecidas pelas empresas, o aumento médio foi de 11g, após ajustes pelas variáveis sociodemográficas dos trabalhadores. A sample of workers in all companies participating in the study was evaluated in order to assess the effects of intervention on the consumption of fruits and vegetables. The intervention was based on the ecological model of health promotion, with 4 different stages for 6 months. Data collection occurred in two stages, before the intervention and after intervention. The effect of the intervention was investigated using two indicators: the availability in grams of fruit and vegetables in meals served in companies and consumption in grams of food by employees. Secondarily, the impact of intervention on the availability of energy, macronutrients and fiber in meals was assessed. It was made a comparison between the intervention and control groups and intra-group comparison before and after intervention to evaluate the effect of intervention on the availability of food and nutrients in meals. Efeito da Lei Federal 11.947/09 na qualidade nutricional dos cardápios propostos pelo Programa de Alimentação Escolar do estado de São Paulo / Effect of the Brazilian Federal Law 11,947/09 in the nutritional quality of the menus proposed by the School Feeding Program of São Paulo

Em 2009, foi promulgada a Lei nº 11.947 do Programa Nacional de Alimentação Escolar (PNAE), que estipula o mínimo de 30 por cento de compra de gêneros alimentícios seja proveniente da agricultura familiar (AF), fazendo necessário conhecer seu efeito sobre a qualidade da alimentação escolar. O objetivo do estudo foi avaliar o efeito da implementação da Lei Federal 11.947/09 sob a qualidade nutricional dos cardápios propostos pelo PNAE do estado de São Paulo. O estudo teve o objetivo de avaliar o efeito da implentação da Lei Federal 11.947/09 na qualidade nutricional dos cardápios propostos pelo PNAE em municípios do estado de São Paulo. Trata-se de um estudo observacional analítico de cunho avaliativo baseado na tríade Estrutura Processo Resultado, com elaboração de matriz avaliativa. Foi realizado em 2013 com 38 municípios do estado de São Paulo divididos em 2 grupos: os que adquirem gêneros da AF e os que ainda não o fizeram. A coleta de dados primários foi através de questionário estruturado aplicado com nutricionistas e cardápios das escolas. Os alimentos dos cardápios foram analisados segundo a NOVA. Foram calculadas frequência absoluta, média e o teste exato de Fischer foi utilizado para a análise de associação. Um total de 25 municípios adquiriam algum gênero alimentício da AF e 18 não implementaram as compras. Os municípios que adquirem gêneros alimentícios da AF têm prevalência de 76 por cento para cozinheiros escolares em número suficiente e de 60 por cento para realização de ações de apoio aos agricultores familiares da região (p >0,05). Quase todas as cidades atentem a legislação quanto a oferta mínima de 3 porções de frutas ou hortaliças e máximo de 2 porções doces. Evidenciou-se que 77,8 por cento dos municípios que adquirem gêneros da AF, atendem a exigência mínima da lei de 70 por cento de alimentos básicos nos cardápios e, 83,3 por cento deste grupo de cidades estão em acordo limite de 30 por cento de alimentos restritos. Houve maior participação média dos alimentos ultraprocessados no lanche em relação à refeição em ambos os grupos de municípios. Os achados permitem refletir que a aquisição de alimentos da AF não foi suficiente para promover cardápios de melhor qualidade nutricional, sugerindo que o marco legal apresenta lacunas quanto a definição dos alimentos que devem compor os cardápios, sendo permissivo quanto ao uso de alimentos ultraprocessados / The Brazilian Federal Law no. 11.947 of the National School Feeding Program (PNAE) was published in 2009 sets a minimum of 30 per cent of purchase of food comes from the family farming (AF) and its effect on food quality of the school must be known. We evaluate the effect of the Brazilian Federal Law 11,947/09 in the nutritional quality of the menus proposed by PNAEs São Paulo. This is an observational analytical evaluative study based on structure - process - results with drafting evaluative matrix. Thirty-eight cities of São Paulo State were analyzed in 2013 and divided into 2 groups: cities who acquire food of AF and cities who have not yet done. Primary data were collected through school menus and interviews with nutritionist by previous structured questionnaires. Foods of the menus were analyzed according NOVA. Absolute frequencies and mean, measures of central tendency and Exact Fisher test analysis were calculated. 25 municipalities acquired some food from the AF and 18 did not implement the purchases. Cities that purchase foodstuff of the AF obtained prevalence of 76 per cent for sufficient numbers of the cook school and 60 per cent performing actions to support family farmers who sold products for school feeding in the area (p> 0.05). Almost all cities comply with the legislation regarding the minimum offer of 3 servings of fruits or vegetables and maximum of 2 sweet portions. 77.8 per cent of the cities that purchase foodstuff of the AF meet the minimum requirement of 70 per cent of staple food on the menus, and 83.3 per cent of this group of cities are in a 30 per cent restriction of restricted food agreement. A large quantity of ultra- processed foods mainly in the snack in both groups of municipalities. Our results show that purchase of food from AF is not enough to promote better nutritional quality of the school menus. suggesting that the legal framework has gaps regarding the definition of foods that should be included in the menus, and is permissive regarding to purchase ultra-processed foods

Considering the importance of school feeding for students, this project aim to analyze the management and enforcement of The Nacional School Feeding Programme in the Federal Institutes of Education, Cience and Tecnology. This Project is a transversal study, the data collection happened in the second semestre of 2014 by phone call interviews in 203 Campi of the Institutes, distributed proportionaly in brasilian macro-regions.The descriptive variables were expressed in relative and absolute frequency. For variables comparision (Campi with and without nutritionist) Pearson x2 tests were held with Yates correction or Fisher Exato. In order to measure the variables association magnitude with p<0,05, the prevalence reason were calculated. In order to investigate the numeric variable simetry the Shapiro – Wilk normality test was used and then the median was calculated. The statistic significance level of 5% was considered. The results indicated that 22,16% of the Campi received the NSFP resources and among these 27 had nutritionist. Among the Campi that received the resources 46,7% bought familiar agriculture supriment , 60% had food and nutritional education activities, 40% have an especific menu for the programme, 71,1% show the menu to the students, 46,7% offer diferente food for students with special nutritional needs and 51,1% make acceptability tests. It was concluded that few Campi received financial resources of the Scholar Food Nacional Program and that the presence of the nutricionist, even without contributing efectivelly in the resource reception, favored to make the food and nutrition educational activities, one of the main axis of the program. / O Programa Nacional de Alimentação Escolar, uma das políticas públicas de alimentação e nutrição mais importantes do Brasil, busca promover a Segurança Alimentar e Nutricional e garantir o direito aos estudantes da educação básica a uma alimentação gratuita, nutritiva, saudável e de qualidade higiênicossanitário durante o ano letivo. Diante da importância da alimentação escolar aos estudantes, este trabalho objetivou analisar a gestão e execução do Programa Nacional de Alimentação Escolar nos Institutos Federais de Educação, Ciência e Tecnologia. Trata- se de um estudo transversal, com coleta de dados realizada no segundo semestre de 2014 por entrevista telefônica em 203 Campi dos Institutos, distribuídos proporcionalmente nas macrorregiões brasileiras. As variáveis descritivas foram expressas em frequência absoluta e relativa. Para comparação das variáveis (Campi com e sem nutricionista) realizou os testes do χ2 de Pearson com correção de Yates ou Exato de Fisher. Para mensurar a magnitude de associação das variáveis com p<0,05, calculou-se razão de prevalência. Para averiguar a simetria da variável numérica utilizou o teste Shapiro – Wilk normality e em seguida calculou-se a mediana. Considerou-se nível de significância estatística de 5%. Os resultados indicaram que 22,16% dos Campi recebiam o recurso do PNAE e destes, 27 possuíam nutricionista. Dos Campi que recebiam o recurso 46,7% compravam gêneros da agricultura familiar, 60% realizavam atividades de educação alimentar e nutricional, 40% possuíam cardápio específico para o Programa, 71,1% disponibilizavam o cardápio ao aluno, 46,7% forneciam alimentação diferenciada para estudantes com necessidades nutricionais especiais e 51,1% realizavam teste de aceitabilidade. Concluiu-se que poucos Campi recebiam o recurso financeiro do Programa Nacional de Alimentação Escolar e que a presença do nutricionista, mesmo não contribuindo de forma efetiva para o recebimento deste recurso, favoreceu a realização das atividades de educação alimentar e nutricional, um dos principais de eixos de atuação do Programa.
