Healthcare practitioners' and patients' perspectives of a weight management service and the place of psychological support within this

Both the NHS and Public Health are keen to identify how best to manage long term health condition’s as a result from obesity and vice-versa. There is evidence to support the efficacy of psychological support in weight management programmes. This study explored the perceived importance of psychological support within weight management services; perspectives of both client and healthcare practitioners, in view of considering the implications for the role of a counselling psychologist. There were nine interviews conducted with five healthcare practitioners and four clients. The professionals’ disciplines included: physiologist, dietician, health psychologist, programme manager, and a medical consultant. Of the four patients, two had accessed psychology services as part of their weight management programme and two had not. Data was analysed using thematic analysis. Five overarching themes were identified. Tension (pivotal central theme) this connected to: Lifestyle; Quality of Life (QoL); Service Delivery Model; and Professional and Personal beliefs and values. The findings highlighted that perceived importance of psychological support was influenced by an individuals’ background and experiences by both groups. The implications for the role of a counselling psychologist was to provide training to health professionals as well as raising clients’ awareness of the role of counselling and psychological support within such programmes. Further research is needed to understand better the potential of psychological support within weight management services to help contain UK obesity.

Social facilitation maintenance treatment for adults with obesity: study protocol for a randomised-controlled feasibility study (SFM study)

Introduction: The long-term success of non-surgical weight loss treatment in adults with obesity is limite by substantial relapse, and only a few evidence-based weight loss maintenance treatments exist. This clinical trial investigates the feasibility and efficacy of a social facilitation maintenance programme for weight loss maintenance, tailored to meet the needs of obese adults who have undergone a lifestyle weight loss intervention. Methods and analysis: In a single-centre, open feasibility trial, 72 adults currently or previously obese or overweight who have undergone a lifestyle weight loss intervention are centrally randomised to 4 months of social facilitation maintenance treatment or treatment as a usual control condition. In 16 outpatient group sessions, the social facilitation maintenance treatment, based on a socioecological model and on evidence supporting social facilitation as a key process in maintaining weight loss, focuses on promoting interpersonal relationships to build up a healthy lifestyle for long-term weight loss maintenance. Primary outcome is the amount of weight regain at 6-month follow-up, compared with pre-treatment weight, derived from measured body weight. Secondary outcomes address feasibility, including recruitment, attrition, assessment non-completion, compliance and patients’ programme evaluation; and in comparison with pre-weight loss maintenance, social and interpersonal functioning, eating behaviour and physical activity, psychological and physical symptoms, body composition and risk of comorbidity, and quality of life at post-treatment and follow-up assessments. Ethics and dissemination: The study was approved by the Ethical Committee at the University of Leipzig (165-13-15072013). The study results will be disseminated through peer-reviewed publications. Its treatment must be initiated with clinical measures and if there is therapeutic failure, there is the option of surgical treatment. Post-operative rapid weight loss causes significant body changes that can compromise patients' body image and have a significant impact on the health and quality of life of individuals. The objective of this research was to evaluate the quality of life and the perception of the body image of patients submitted to bariatric surgery. An observational longitudinal study was carried out from December 2016 to October 2017 in a private clinic in the South of Brazil and the sample was composed of 104 obese individuals. Quality of life was assessed through the SF-36 questionnaire (The Medical Outcomes Study Short Form Health Survey) and the body image by the Brazilian scale of silhouettes for adults. Follow-up occurred in the pre and postoperative periods of 1, 2 and 6 months. Patients' quality of life improved significantly (p <0.001) during the pre and postoperative period in mental and physical health components. The body image of the patients evaluated presented distortion throughout the follow-up period, where the overestimation of the body image in the preoperative period was replaced by a 6-month underestimation in the postoperative period. Patients undergoing bariatric surgery had significant improvements in quality of life and altered body image distortion in the 6-month postoperative period. / A obesidade é uma doença crônica associada ao excesso de gordura corporal que pode desencadear diversas patologias. Seu tratamento deve ser iniciado com medidas clínicas e se houver insucesso terapêutico, existe a opção de tratamento cirúrgico. A perda rápida de peso pós-operatório provoca mudanças corporais significativas, que podem comprometer a imagem corporal dos pacientes e apresentar um impacto significativo na saúde e na qualidade de vida dos indivíduos. O objetivo dessa pesquisa foi avaliar a qualidade de vida e a percepção da imagem corporal de pacientes submetidos à cirurgia bariátrica. Foi realizado um estudo observacional de delineamento longitudinal no período de dezembro de 2016 a outubro de 2017 em uma clínica privada no Sul do Brasil e a amostra foi composta por 104 indivíduos obesos. A qualidade de vida foi avaliada através do questionário SF-36 (The Medical Outcomes Study Short Form Health Survey) e a imagem corporal pela Escala brasileira de silhuetas para adultos. O acompanhamento ocorreu nos períodos pré e pós-operatório de 1, 2 e 6 meses. A qualidade de vida dos pacientes obteve melhora significativa (p<0,001) durante o período pré e pós-operatório nos componentes de saúde mental e físico. A imagem corporal dos pacientes avaliados apresentou distorção durante todo o período de acompanhamento, onde a superestimação da imagem corporal no pré-operatório foi substituída por subestimação no pós-operatório de 6 meses. Os pacientes submetidos à cirurgia bariátrica apresentaram melhorias significativas na qualidade de vida e modificação na distorção da imagem corporal no pós-operatório de 6 meses.
