Health Care Utilization Nonuse and High Use of Physician Services Among Older Women, 1969-1979

This research sought to identify the determinants of nonuse and high use of physician services and assess whether or not patterns of nonuse and high use changed over time. The population of interest was a group of elderly unmarried women who participated in the Longitudinal Retirement History Survey from 1969 to 1979. Andersen and Newman's (2) health care services utilization model served as the conceptual framework for this research. Of specific interest was the relationship between age strata and health care behavior. Age proved to be a stratifying variable within the health care delivery system. Over the ten year survey period, the health care behavior of preretirement and postretirement nonusers and high users differed significantly. A decline in nonuse was also associated with the transition years. This finding could be attributed to the "near poor" becoming eligible for Medicare. In any event, these data show that utilization of physician services is likely to increase among some unmarried women in their middle 60's.

Life satisfaction of elderly females who are overweight as compared to elderly females of ideal weight

A descriptive study was done to determine the difference in life satisfaction of elderly females who were overweight as compared to elderly females of ideal weight. The sample consisted of 30 females, aged 60 to 80, who were living in the community. No differences in life satisfaction scores were found between the overweight and ideal weight elderly females. The elderly females who were identified as being overweight had a mean life satisfaction score of 14.8 with a standard deviation of 2.45. Elderly females of ideal weight had a mean life satisfaction score of 15.9 with a standard deviation of 2.12. Major differences in summated results of selected demographic variables were not found for the elderly females who were overweight as compared to the elderly females of ideal weight. Findings from the study and implications for nursing are discussed. Recommendations for further research are included.

Feasibility randomized control trial of physical activity in women aged 55-70 years: a mixed-methods self-determination theory study of dancing and walking

Background: Physical activity (PA) is a health protective behaviour that is critical in the reduction of most major chronic diseases. It also provides the participant with psychological benefits. Despite its well-established health benefits, PA engagement is low in the adult population. Women over the age of 60 are the least physically active segment and thus an important target for behaviour change interventions. Objective: The purpose of this mixed methods feasibility study was to explore the feasibility of six-week Self-Determination Theory (SDT)-based dance and walking programs for older women. Design: This was a parallel, randomized controlled trial with three groups: dance, walking, and wait-list control. This study included the sequential collection of quantitative then qualitative data (sequential explanatory design). Setting: This study was conducted in Victoria, BC. Participants: Participants were community-dwelling, English-speaking women aged 55-70 years who were not meeting PA guidelines. Methods: Data were collected at baseline and two endpoints: post-intervention at six weeks and follow-up at 12 weeks. The primary outcomes were feasibility measures: recruitment, intervention adherence, retention, and satisfaction. Survey data included self-reported PA (Godin Leisure Time Exercise Questionnaire) and measures of behavioural regulations and psychological needs (SDT constructs) using validated tools (Behavioural Regulations in Exercise Questionnaire-2 and Psychological Need Satisfaction in Exercise Scale). Qualitative data were collected in the form of open- and close-ended program evaluation questions and during focus group interviews, both occurring at post-intervention (six weeks). Results: The feasibility measures suggest that it is feasible to recruit and retain participants and that they were generally satisfied with the programs. Thirty-five of 37 randomized participants completed the study (mean age ± SD = 62.8 ± 4.8), representing a 39% recruitment rate and 95% retention rate. Both programs were highly attended. Exploratory effect sizes for the quantitative measures were promising for conducting a larger-scale trial. Emergent themes highlighted the importance of the leadership component of the group-based PA programs. Conclusion: This study had high protocol adherence, promising effect sizes, program evaluation satisfaction, and a recreation centre took on the dance program; these factors provide a foundation for expanding this feasibility trial to a full-scale study. / Graduate / 2017-12-01

Women aged 55 and older have been participating actively in the American work force and recently have been drawing increased attention from researchers in the social sciences (Ashbaugh & Fay, 1987) . This study examines the nature of service work performed by older part-time women workers and their younger women co-workers, and the relationship between the two generations in the workplace. The following research questions guided this exploratory study: Do the older women workers and their younger women co-workers report that there have been changes in the co-workers' work since the older women began working at the job site? What do the older women workers and their younger women co-workers perceive to be the emotional quality of their intergenerational interaction in the workplace? If the co-workers report that the way their work is performed in the setting has changed since the older women began working, is the intergenerational relationship influenced by the perceived change in the distribution of work?

Factors regulating resting energy expenditure and thermic effect of food in elderly women

Hypothesis. We hypothesized that contrary to "normal aging," frail elderly women would have a heightened resting energy expenditure per kg fat free mass (REE/kg FFM) and thermic effect of food (TEF) response. / Methods. 13 healthy (H) [X +/- SEM: 81.4 +/- 1.1 yr] and 9 frail elderly women (F) [84.7 +/- 1.6 yr], free from acute conditions underwent REE and TEF measurements (liquid standard mixed meal: 720 kcal, 58% carbohydrate, 14% protein, 28% fat) using ventilated hood indirect calorimetry. Anthropometric and body composition measurements, thyroid hormones, cortisol, cytokines and catecholamine concentrations, and physical activity (PASE) and mobility ("time up and go" TUG) scores were related to the results obtained. / Results. H had a faster TUG: 10.2 +/- 1.6 vs. 21.0 +/- 9.3 s (p=0.001), greater PASE score: 179.2 +/- 116.1 vs. 47.1 +/- 26.2 (p=0.003), greater triiodothyronine [T3]: 5.1 +/- 0.6 vs. 4.3 +/- 0.4 rhomol/L (p=0.003) and lower cortisol concentrations: 351.1 +/- 19.9 vs. 474.5 +/- 43.6 nmol/L (p=0.011). In H vs. F: REE/kg FFM was 1044 +/- 20.4, 1021 +/- 23.7 kcal/day (NS) and TEF response as expressed as % of REE was 18.4 +/- 5.6, 19.5 +/- 4.2 (NS). The time course of glucose (p=0.043) and insulin from 90 minutes onwards (P=0.013) suggested insulin resistance in F. REE was positively correlated with FFM, percent body fat and [T3] (r>0.499, p<0.021). / Conclusion. REE/kg FFM and TEF response was not different between H and F and thus the greater prevalence of malnutrition in F, cannot be explained by these parameters.

THE PLACE OF DIETARY PRACTICES IN THE LIVES OF OLDER WOMEN

Studies have supported the benefits of positive dietary behaviors in preventing or reducing morbidity and extending longevity, as well as the psychosocial function of dietary practices for quality of life. Research is needed though on the dietary behaviors of elders in general and older women in particular, for whom gender affects lifelong dietary patterns. Health behavior theory has typically focused on psychological factors, to the neglect of sociocultural processes. This investigation utilized a life course perspective, enhanced by social interactionist elements, to address aspects of development and change in behavior neglected by health behavior theories, such as temporal dimensions and social contexts. Using primarily an ethnographic approach centered on in-depth narrative interviews of 18 older women residing in a retirement community, this study explored how social milieu influences the development and progression of dietary behavior throughout life, the potential of life course transitions to modify dietary behavior, and how the retirement community environment shapes current dietary behavior. The interviews probed current dietary experiences and practices, as well as constructed histories of dietary behavior. Through the use of coding techniques and thematic analysis, themes and concepts that emerged from the data were organized for further analysis. Four levels of influence on dietary behavior were identified: 1) person factors, including psychological and physiological processes; 2) interpersonal relationships and social interaction; 3) social roles and statuses; and 4) contexts, particularly environmental, community policy and political economic contexts. Analysis additionally revealed four major food-related themes in the lives of the women: dietary morality, dietary wellness, dietary sociability and dietary duty. Interpretation of the findings, in terms of lifelong social experiences, the impact of relationships, roles and transitions, and structural characteristics of the retirement community that constrain or facilitate dietary practices, contributed to the development of a theoretical model. The research findings and model of life course influences on the nature of dietary behaviors of older women provide a more holistic understanding of dietary practices of older women and have implications for future research and practice, particularly as related to quality of life issues.
