Absorption, toxicity and deposition of transition metal based pharmaceuticals following oral administrationBinks, Stephen Peter January 1988 (has links)
The aim of this dissertation was to study the absorption, and subsequent toxic side effects of transition metal based pharmaceuticals following oral administration to rats. Administration of cisplatin (30mg/kg), carboplatin (37mg/kg) and iproplatin (42mg/kg) by oral gavage resulted in their rapid absorption so that respective peak blood levels of 2.63mug platinum/ml, 1.48mug platinum/ml and 3.13mug platinum/ml were achieved within 2-4 hours. Approximately 3-4% of the dose was excreted in the urine, but the major route of elimination was the faeces (>75%). This indicated that although rapid, absorption was relatively poor. Absorption was enhanced by employing a period of starvation prior to administration. A series of novel platinum (IV) mixed amines were absorbed to a greater extent than cisplatin and its congeners, carboplatin and iproplatin. However, absorption was somewhat slower with peak blood levels being attained some 24 hours after administration. Of the other transition metal complexes studied, auranofin and ruthenium acetylacetonate were particularly well absorbed so that peak blood levels of 12.53mug gold/ml and 6.4mug ruthenium/ml were achieved respectively. Urinary clearance of the ruthenium complex was especially significant with up to 45% of the administered dose being eliminated by this pathway within 48 hours. In vitro everted gut sac and in situ perfusion techniques confirmed the in vivo finding that cisplatin is absorbed from the small intestine more readily than carboplatin. No evidence for active or carrier-mediated transport was found and kinetic studies confirmed that absorption was by passive diffusion. Toxicology studies after oral administration of cisplatin (57 or 30mg/kg) or carboplatin (282mg/kg) indicated that the toxicities associated with the perenteral use of the complexes would also apply to the oral route. This was exemplified by the fact that oral cisplatin was profoundly nephrotoxic, whereas carboplatin was not. In addition, gastro-intestinal toxicity manifested as acute necrotizing enteritis and ulcerogenicity of the stomach was potentiated by the oral route. Studies in the ferret indicated that cisplatin is significantly more emetogenic than carboplatin. Examination of liver morphology indicated changes, such as mitochondrial swelling and vesiculation of the endoplasmic reticulum, that might indicate a higher incidence of hepatotoxic responses associated with administration of platinum complexes by the oral route. Both cisplatin and carboplatin induced a degree of myelosuppression but the most pronounced haematological lesion associated with oral administration was severe erythrocytosis which occurred as a result of a dehydration related decrease in plasma volume. Electrothermal atomic absorption analysis of tissues excised from cisplatin and carboplatin rats indicated that platinum deposition was highest in the kidney. In an attempt to explain the prolonged retention of cisplatin in this organ, the intracellular location of the compound was studied using electron microprobe analysis and subcellular fractionation. The various lysosome populations of the proximal tubule were identified as sites of concentration of platinum and it was hypothesised that sequestration within these organelles might be an important mechanism of detoxification.

Bilateral Upper Extremity Thrombophlebitis Related to Intravenous Amiodarone: A Case ReportAljitawi, Omar, Shabaneh, Baha, Whitaker, Jack 01 August 2005 (has links)
A 47-year-old male had bilateral upper extremity thrombophlebitis after use of intravenous amiodarone for sustained ventricular tachycardia complicating myocardial infarction. Intravenous amiodarone has been widely used since it was introduced 20 years ago for severe intractable arrhythmias. Superficial thrombophlebitis was frequently noted in the early case reports when high-dose intravenous amiodarone was used. Superficial thrombophlebitis could extend hospitalization and become a significant source of distress to our patients. Some authors recommend insertion of a central line to administer intravenous amiodarone especially with expected extended use of therapy. The treating physician should be vigilant and switch from intravenous therapy to oral therapy as soon as the patient's condition stabilizes and oral therapy can be started.

Enhancing Network Data Obliviousness in Trusted Execution Environment-based Stream Processing SystemsAlsibyani, Hassan 15 May 2018 (has links)
Cloud computing usage is increasing and a common concern is the privacy and security of the data and computation. Third party cloud environments are not considered fit for processing private information because the data will be revealed to the cloud provider. However, Trusted Execution Environments (TEEs), such as Intel SGX, provide a way for applications to run privately and securely on untrusted platforms. Nonetheless, using a TEE by itself for stream processing systems is not sufficient since network communication patterns may leak properties of the data under processing. This work addresses leaky topology structures and suggests mitigation techniques for each of these. We create specific metrics to evaluate leaks occurring from the network patterns; the metrics measure information leaked when the stream processing system is running. We consider routing techniques for inter-stage communication in a streaming application to mitigate this data leakage. We consider a dynamic policy to change the mitigation technique depending on how much information is currently leaking. Additionally, we consider techniques to hide irregularities resulting from a filtering stage in a topology. We also consider leakages resulting from applications containing cycles. For each of the techniques, we explore their effectiveness in terms of the advantage they provide in overcoming the network leakage. The techniques are tested partly using simulations and some were implemented in a prototype SGX-based stream processing system.

Determinants and Impacts of Demand-side Management Program Investment of Electric UtilitiesDegens, Philipp 01 January 1996 (has links)
From the late seventies through the early 1990's electric utilities were facing many different forces that caused them to invest into demand-side management programs (DSM). Roots of the growth of DSM can be found in the high inflation and energy price shocks of the late seventies and early eighties, spiraling building costs of generation, safety and environmental concerns, increased costs of new capacity with possible exhaustion of scale economies, unexpected high elasticity in the demand for electricity, and public utility commissions that sought alternatives to the resulting high rate increases. This study develops and estimates four equations that look at the more aggregate utility level impacts of DSM. The goal of two equations is to determine what factors influence utility investments in DSM and if stock market investment in utilities is affected by DSM. Two additional equations are developed to determine system level impacts of DSM on cost of and quantity demanded of electricity. To estimate these models four years of annual data were collected for 81 utilities spanning 1990-1993. These utilities have sold over 60% of all the electricity in the US and were responsible for over 80% the national spending in DSM. The DSM investment model indicated that of the major variance in DSM investment is due to the utility's regulatory environment. Both an above average regulatory climate and least-cost planning requirements had major impacts on the level of DSM investment. The cost of equity capital equation revealed that DSM expenditures had a positive impact on the valuation of utility's stock. Cost and quantity equations were estimated both individually and simultaneously. DSM expenditures seemed to have a negative impact on both average cost and quantity demanded. Although these relationships were statistically significant, the impacts were quite small. To summarize; the regulatory environment seems to have the strongest impact on the level of DSM investment; DSM spending was associated with an increased stock valuation; as expected DSM investments were found to have a negative relationship with quantity demanded; and finally DSM investment appeared to reduce the average cost.
