Förutsättningar att vårda patient med trauma nivå-1 : En kvalitativ intervjustudie av operationssjuksköterskans erfarenheter / Qualifications needed to care for trauma level-1 patients : A qualitative interview study of operation theater nurses experiences

Bakgrund: Traumavård är en del av den svenska hälso- och sjukvården. Patienter som råkat ut för ett trauma vårdas utifrån allvarlighetsgraden av skadebilden.  Enligt nationella riktlinjer indelas trauma i två olika nivåer, där trauma nivå-1 är det allvarligaste som ofta kräver livräddande vårdinsatser. Operationssjuksköterskan är en av medarbetarna i teamet och är delaktig i de operativa åtgärderna. Vårdandet ställer krav på yrkeskompetens och stresstålighet i det pre- och intraoperativa arbetet.    Syfte: Syftet med studien var att belysa operationssjuksköterskor erfarenheter av att vårda patienter med trauma nivå-1 på operationssal med fokus på förutsättningar. Metod: I studien användes kvalitativ metod där datamaterialet analyserades med kvalitativ innehållsanalys. Datamaterialet insamlades genom tio intervjuer med operationssjuksköterskor från ett regionalt traumacentrum och ett traumamottagande sjukhus. Resultat: Analysen av datamaterialet gav ett övergripande tema som är operationssjuksköterskans delaktighet i vårdandet och trygghet i professionen. Temat framkom utifrån tre kategorier; personliga och professionella resurser, lärande resurser och organisatoriska resurser med tillhörande underkategorier. Konklusion: I resultatet framkommer att fungerade teamarbete, förtrogenhet i yrkesrollen och olika former av lärande är förutsättningar för vårdandet. Organisatoriska delar som struktur och chefskap är andra förutsättningar som har betydelse för ett fungerande samarbete och för att operationssjuksköterskan ska kunna utföra sitt arbete i samband med traumavård nivå-1. / Background: Trauma care is included in Swedish healthcare services. A patient in need of trauma care is treated by the severity of injuries. According to national guidelines trauma is divided into two different levels, level-1 being the most severe that often require lifesaving efforts. Operating theater nurses is one of the team members that is part of the operative care. Professional competence is required along with stress resistance in the pre- and intraoperative care. The aim: The aim of the study was to illustrate experiences of operating theater nurse´s care of patients with trauma level-1 with focus on qualifications.   Method: A qualitative method was used where the data material was analyzed with qualitative content analysis. The data is based on interviews with ten operating theater nurses from a University hospital and a regional hospital. Results: One main theme with three underlying categories emerged. Operating theater nurses participation in provided care and sense of safety is an overall theme that contains three categories which are personal and professional resources, learning resources and organizational resources, each with associated subcategories. Conclusion: Participation in the trauma care, sense of safety and a variety of learning sources was evident as qualifications for care. Structure and leadership was further conditions that emerged as important parts for collaboration that enhances the possibility for the operation theater nurse to perform trauma care level-1.

Anestesisjuksköterskors upplevelser av att använda ”Checklista för säker kirurgi” i sitt dagliga arbete / Anesthesiology nurses' experiences of using the “Safe Surgery Checklist” in their daily work

Att genomgå ett kirurgiskt ingrepp är förenat med risk för komplikationer. WHO har utarbetat en checklista för säker kirurgi som används av operationspersonal för att minska risken för komplikationer. Patientsäkerhet finns med som en av grundpelarna i kompetensbeskrivningen för anestesisjuksköterskor, och i enlighet med denna ska anestesisjuksköterskan arbeta på ett patientsäkert sätt och uppmärksamma arbetsrelaterade risker. Syftet med studien var att undersöka anestesisjuksköterskors upplevelser av att använda ”Checklista för säker kirurgi” i sitt dagliga arbete. En kvalitativ studie genomfördes där datainsamlingen bestod av semistrukturerade intervjuer med sju anestesisjuksköterskor. Analys av data gjordes genom en kvalitativ innehållsanalys som resulterade i fyra kategorier: Att vården blir tryggare genom en heltäckande bild, Att praktiska skäl medför att anestesisjuksköterskor tar stort ansvar för genomförandet av checklistan, Att göra avsteg men med checklistan i huvudet och Att alla i teamet måste vara delaktiga och ta ansvar. Slutsatser som kan dras är att mer fokus på utbildning av checklistan för samtliga berörda professioner skulle kunna generera ökad följsamhet vid användning av checklistan, ökad förståelse för varandras yrkesområden samt ökad sammanhållningen i teamet. Det bör läggas mer fokus på såväl modifiering av checklistan som kan användas vid akuta situationer som på åtgärder som främjar kommunikationen och kulturen på operationssalen. Vidare studier bör göras om varför avsteg görs och hur detta kan motverkas så att patientsäkerheten kan upprätthållas. / Having a surgical procedure is associated with risk of complications. WHO has prepared a checklist for safe surgery used by surgical staff to reduce the risk of complications. Patient safety is included as one of the cornerstones in the competence description for anesthesia nurses, and in accordance with this, the anesthesia nurse should work in a patient-safe manner and pay attention to work-related risks. The aim of the study was to examine anesthesia nurses' experiences of using the "Safe Surgery Checklist" in their daily work.A qualitative study was conducted in which seven anesthesia nurses participated. Data was analyzed using qualitative content analysis, which resulted in four categories: Care becomes safer through a comprehensive picture; Anesthesia nurses take great responsibility for the implementation of the checklist due to practical reasons; Making deviations but with the checklist in mind; and Everyone in the team must be involved and take responsibility. Conclusions that can be drawn are that more focus are needed on the education of all professions involved, which could generate increased compliance when using the checklist, increased understanding of each other’s occupations and increase cohesion in the team. Also, there should be more focus on modification of the checklist that can be used in emergency situations and on measures that promote communication and culture in the operating room. Further studies should be done on why deviations are made and how this can be counteracted so that patient safety can be maintained.

Specifika ošetřovatelské péče u pacienta s MRSA na operačním sále z pohledu anesteziologické sestry / Specifics of nurse care at the patient with MRSA on surgery from nurses' point of view.

Title of the thesis: Specifics of the nursing care of patients with MRSA in the operating room from an anesthesiology nurse perspective. The aim of this thesis was to identify the specifics of nursing care of a patient with the MRSA in the operating room from the perspective of nurses working in the operating room. In the theoretical part focus is given to nursing care and its specifics for a patient with MRSA. Chapters relating to the regime measures, barrier nursing procedures and the use of specific aids, instruments and tools are treated in detail. The nursing care provided by anesthesia nurses and perioperative nurses in the operating room is defined. To reach the goals the quantitative research through questionnaires was chosen. The research sample consisted of anesthesia nurses and perioperative care nurses. Two types of standardized questionnaires were used, consisted of closed questions where respondents chose an appropriate answer, and from semi-open questions where respondents could specify their answers, and in one case open questions. Both questionnaires were completed by 12 test questions, where respondents had the choice of three possible answers, where one answer was correct. The results of the investigation are summarized in two types of graphs. Bar charts were used in questions where more answers were possible to mark, and for a better clarity the results were supplemented by tables. Results to other questions, where options were limited by one answer, are shown in percentages in pie graphs. Our thesis is very narrowly focused, because we concentrated on specifics of nursing care in a patient with MRSA in the operating room. We have determined the specifics of barrier nursing, nursing problems in a patient with MRSA in the operating room. We have discovered barrier nursing procedures in a patient with MRSA in the operating room. We have also succeeded in assessing the knowledge of nurses working in the operating room. The goal was met. An interesting finding was that most of anesthetic nurses and perioperative nurses had responded that the structural and technical arrangement in the operating room was not satisfactory in terms of barrier nursing of a patient with MRSA. This problem was expected, owing to the author´s experience in operating theaters. Thus Hypothesis 1 is confirmed. In total five hypotheses were stated, four of which were confirmed and one was disproved. The aim of the thesis was to draw attention to problems in the operating room during nursing a patient with MRSA. In the course of the research work there were no problems, however, it is necessary to mention the lack of literature on MRSA with regard to nursing care in operating theaters. The theoretical section of this thesis could be recommended as a resource for the education plan created on the basis of information collected and the results of this research. The results of the research survey may serve for general public, anesthesia and perioperative nurses who could benefit from the educational plan in practice while providing nursing care for a patient with MRSA.
