The organisational and communication implications of electronic ordering systems for hospital pathology services

Doctor of Philosophy / Computerised Provider Order Entry (CPOE) systems provide clinicians with the ability to electronically enter hospital orders for laboratory tests and services. CPOE is able to integrate with hospital information systems and provide point of care decision support to users thereby making a potentially significant contribution to the efficiency and effectiveness of care delivery. The evidence of the impact of CPOE systems on pathology services is not extensive and insufficient attention has been paid to their effect on organisational and communication processes. This thesis aimed to investigate the implications of CPOE systems for pathology laboratories, their work processes and relationships with other hospital departments, using comparative examinations to identify the tasks they are involved in and the particular needs the laboratories expect to be filled by the new system. This longitudinal study of a CPOE system was carried out over three years using multiple cases from a hospital pathology service based at a large Sydney teaching hospital. Multi-methods using quantitative and qualitative data were employed to achieve triangulation of data, theory and methods. The findings provide evidence of a significant 14.3% reduction of laboratory turnaround times from 42 to 36 minutes when laboratory data for two months were compared before and after CPOE implementation. The findings also reveal changes in the pattern and organisation of information communication, highlighting transformations in the way that work is planned, negotiated and synchronised. These findings are drawn together in a comprehensive organisational communication framework that is highly relevant for developing a contingent and situational understanding of the impact of CPOE on pathology services.

The aim is to verify the interference and effectiveness of this system on the activities of the Production Planning and Control of a thermoplastics process factory. Its theory s reference comes from the Bata Manufacturing Strategy (BMS), the Ordering Systems (OS) and the Performance Measurement Systems (PMS). The Action- Research method was used to verify the relation between the ideal model and the one obtained when put on a practical experience. Firstly, a model system is proposed on what this work was developed, its main features and details that connect the BSM, the OS and the PMS. Then, an application example of the system in a thermoplastics process factory is showed using the blow molding process of High Density Polyethylene and Polyvinyl Chloride bottles for cosmetics industries and drugstores. Furthermore, the main results obtained when implementing this system are also presented as well as an evaluation of the procedures created to control the material and human resources and how these controls influence the Production Planning and Control activities of this factory. Another important point of this work is also to present a new approach on how to implement the Bata Manufacturing Strategy / O presente trabalho apresenta uma proposta de implantação de um Sistema de Coordenação de Ordens (SCO) baseado na Estratégia Bata de Manufatura (EBM). O intuito deste trabalho é verificar a interferência e eficácia deste sistema nas atividades de PCP de uma fábrica de processamento de termoplásticos. O referencial teórico deste trabalho é fundamentado na EBM, nos SCO e nos Sistemas de Medição de Desempenho (SMD). Foi utilizado o método de pesquisa-ação como recurso para se determinar a relação entre o modelo idealizado e o verificado na prática no objeto de estudo analisado. Inicialmente é proposto um modelo de sistema sobre o qual foi realizado o estudo, suas principais características e detalhes que interligam a EBM, os SCO e os SMD. Em seguida, é apresentada a aplicação do sistema numa fábrica de processamento de termoplásticos, cujo processo é o de sopro de embalagens de PEAD (Polietileno de Alta Densidade) e PVC (Policloreto de Vinila) para indústrias de cosméticos e farmácias de manipulação. São apontados, portanto, os principais resultados obtidos com a implantação deste sistema no processo definido. São também avaliados neste estudo os procedimentos criados para controle de recursos materiais e humanos, bem como a influência que estes controles exercem sobre as atividades de PCP desta fábrica. Outro ponto chave deste estudo é apresentar uma nova abordagem de aplicação da Estratégia Bata de Manufatura

Coordenação do fluxo de produção por meio do uso combinado de práticas utilizadas na gestão da cadeia de suprimentos e de sistemas de coordenação de ordens puxados

Thus, practices have been developed both in academic and business environments that are used in supply chain management (SCM) since the late 1980's that allows such coordination. For manufacturing flow management process within the plant of a single company, the knowledge about usage of ordering systems can be found in current literature. Thus, the purpose of this thesis is to propose the use of the combined practices in SCM and pull ordering systems in SC. However, these ordering systems are seldom used for coordinating operations in SC, thus, can be assumed with a theoretical gap being exploited in terms of coordination mechanism of production flow in CS. The theoretical and empirical approach was used as methodological procedures. In terms of theory a bibliographic review was applied and was built a conceptual model. In terms empirical, was applied the conceptual model in a real case and was analyzed their consistence and result by simulation. A literature review was conducted to ensure the meaning of production flow coordination and SC and a detailed view of activities that theoretically ensures such coordination through the implementation of the Manufacturing Flow Management Process (MFMP). Based on this set of activities, a survey of the practices used in SCM and a conceptual analysis of contribution of with practice to the MFMP was performed. Meanwhile, another theoretical survey about pull ordering systems was performed with report analysis on the literature about the use of SC. Based on the elements that condone the use of each used practice in SCM and each pull ordering systems, a conceptual model of decision making for the combined use of both was designed. This model can help managers on decision making regarding the best practices and systems for each situation of pull systems. In order to validate the model a case study was performed in an automobile SC. The conceptual model was applied in two cases of the relationship with their suppliers and the impact on production flow coordination was analyzed through simulation. Based on the selected analysis variables there was an increase in the level of production flow management in all cases from the combined use of the practices compared to the previous case. To conclude it can be stated that through appropriate choices of combined use of practices used in SCM and pull ordering systems it is possible to achieve greater production flow coordination in SC. / Um dos maiores desafios enfrentados pelas empresas é a coordenação do fluxo de produção (informações e materiais) ao longo da cadeia de suprimentos (CS) em que estão inseridas. Deste modo, tanto no meio acadêmico como no empresarial, a partir do final da década de 1980, têm sido desenvolvidas práticas que são utilizadas na gestão da cadeia de suprimentos (GCS) que, entre outras funções, contribuem para tal coordenação. Para a coordenação do fluxo de produção das operações no interior da planta de uma única empresa, encontra-se na literatura e na prática a utilização de sistemas de coordenação de ordens (SCOs). No entanto, estes SCOs são raramente utilizados na coordenação de operações na CS, podendo assim admitir uma lacuna teórica a ser explorada em termos de mecanismos de coordenação de fluxo de produção na CS. Desta forma, o objetivo desta tese de doutorado é propor a utilização do uso combinado de práticas utilizadas na GCS e SCO com características puxadas para coordenação fluxo de produção no âmbito da CS. Como procedimento metodológico, o método empregado foi do tipo teórico-empírico. Em termos teóricos utilizou-se como delineamento a pesquisa bibliográfica e o desenvolvimento de um modelo conceitual. Quanto aos aspectos empíricos, a partir de dados de um caso real de CS, foi ilustrada a aplicação do modelo conceitual e analisados a consistência e os resultados da proposta por meio de simulação. Para tanto, foi realizada uma revisão bibliográfica sobre o significado de coordenação de fluxo de produção na CS e detalhadas as atividades que teoricamente garantem tal coordenação por meio da implantação do Processo de Gestão de Fluxo de Manufatura. Baseado neste conjunto de atividades foi realizado um levantamento e uma análise conceitual da contribuição para a coordenação do fluxo de produção de práticas utilizadas na GCS. Paralelamente, foi realizado um levantamento teórico de SCOs que atuam com lógica puxada e verificados relatos na literatura sobre o uso na CS. A partir dos elementos que recomendam o uso de cada prática utilizada na GCS e cada SCO, das especificidades dos sistemas de produção puxados e dos critérios do portfólio de gestão de fornecedores, foi elaborado um modelo conceitual de tomada de decisão para o uso combinado de práticas utilizadas na GCS e SCOs. Tal modelo auxilia os gestores para a tomada de decisão quanto às práticas e os sistemas mais adequados para cada situação de sistemas de produção puxados. Com o intuito de ilustrar o uso do modelo, fez-se a aplicação do mesmo em uma montadora de automóveis em sua relação com alguns fornecedores. Aplicou-se o modelo conceitual para cada caso e analisou-se via simulação o impacto na coordenação no fluxo de produção nos mesmos. Baseado nas variáveis de análise selecionadas, em todas as situações verificou-se o aumento no nível de coordenação do fluxo de produção a partir do uso combinado das práticas em relação à situação anterior. A partir do estudo empírico, pode-se presumir que por meio de escolhas adequadas de uso combinado de práticas utilizadas na GCS e SCOs puxados é possível obter maior coordenação de fluxo de produção na CS.

In this context, the Engineering to Order Capital Goods Industry, exposed to a very adverse market conditions such as high cyclical instability of demand for its products, which depends exclusively on the economy expansion moments, causing, therefore, longer cycles of fluctuation demand with long periods of stagnation followed by periods of high demand, have sought for modern production management strategies, already consolidated in more dynamic segments of the economy, such as lean production and the theory of constraints, to improve production flow and minimize the impact inherent in these production systems with high variety and low volume production, high levels of work-in-process, functional production layouts, high varied workloads, excess of materials handling, etc. This paper studies the Engineering to Order Capital Goods Industry, especially large capital goods, also known as Large Assemblies production systems, aiming to propose a method of joint implementation of lean principles and theory of constraints to simplify the material and information flow, propose the use of suitable SCO (ordering systems) and reduce production lead time in this environment. / Ao longo do tempo, a tarefa de desenvolver e gerir sistemas de produção tem se tornado progressivamente mais complexa e, atualmente, para enfrentar as mudanças de demandas dos clientes e a competição global, os gestores têm a difícil tarefa de combinar a eficiência da produção em massa com a customização da era produção artesanal pré-industrial. Neste contexto, a Indústria de Bens de Capital Sob Encomenda, expostas a condições de mercado muita adversas, como a alta instabilidade cíclica de demanda por seus produtos, que depende exclusivamente dos momentos de expansão da economia causando, portanto, ciclos mais longos de flutuação da demanda com longos períodos de estagnação seguidos por períodos de alta demanda, têm buscado estratégias modernas de gestão da produção, já consolidadas em segmentos mais dinâmicos da economia, como a produção enxuta e a teoria das restrições, para melhorar o fluxo de produção e minimizar o impacto inerente desses sistemas de produção com alta variedade e baixos volumes de produção, altos níveis de estoque em processo, produção em layouts funcionais, cargas de trabalho muito variadas, excesso de movimentação de materiais, etc O presente trabalho estuda o setor de bens de capital sob encomenda, em especial os bens de capital de grande porte, também conhecidas como sistemas de produção de grandes montagens, com objetivo de propor um método de implantação conjunta dos princípios da produção enxuta e da teoria das restrições para simplificar o fluxo de materiais e informações, propor a utilização dos Sistemas de Coordenação de Ordens adequados e reduzir o lead time de produção nesse ambiente.
