No. of bitstreams: 1 Hebling_SandraReginaFacciolli_M.pdf: 582549 bytes, checksum: 8dd231548e90b00bf279971ee222a324 (MD5) Previous issue date: 2006 / Resumo: O objetivo geral desse estudo, composto por dois trabalhos, foi avaliar a prevalência de mordida aberta anterior e de mordida cruzada e sua associação com a presença de hábitos bucais deletérios, gênero e condição sócio-econômica, fornecendo subsídios para a elaboração de um programa de Ortodontia Preventiva e Interceptora em Saúde Coletiva. A amostra do primeiro trabalho foi composta por crianças de 5 anos, de ambos os gêneros, matriculadas em pré-escolas públicas (n=428) e privadas (n=300) no município de Piracicaba, Brasil. Os pais ou responsáveis legais das crianças foram submetidos a um questionário com perguntas sobre as condições sócio-econômicas e os hábitos bucais. As anomalias dento-faciais, as características respiratórias e da musculatura peribucal em repouso e durante a deglutição foram avaliadas por meio de exame clínico. Os resultados mostraram uma forte associação da mordida aberta anterior com os hábitos de sucção de chupeta e polegar, deglutição atípica e respiração mista com predominância bucal e, da mordida cruzada com o gênero e os hábitos de sucção de chupeta, respiração mista com predominância bucal e interposição labial. O objetivo do segundo trabalho, baseado na literatura e nos resultados do primeiro trabalho, foi elaborar considerações sobre um protocolo de assistência ortodôntica em saúde coletiva. Os possíveis problemas oclusais foram discutidos e classificados em prioridade primária e secundária de atendimento, enfatizando os desvios oclusais que não deveriam ser tratados precocemente. A idade sugerida para abordagem seria a dentição decídua e início da dentição mista, no período da irrupção dos incisivos superiores e inferiores permanentes. Conclui-se que os hábitos bucais deletérios são fatores predisponentes para a etiologia de más oclusões e um protocolo de assistência ortodôntica apropriado pode ser aplicado para a prevenção e interceptação da má oclusão em Saúde Coletiva / Abstract: The aim of this study, compost of two papers, were to assess the prevalence of anterior open bite and crossbite, and to correlate it with oral habits, gender, and socioeconomic status, given conditions to develop a program for preventive and interceptive orthodontic treatment in public health dentistry. The first paper sample was compound by 5 years children, both genders, enrolled in public (n=300) and private (n=428) schools in Piracicaba, Brazil. Parents or legal guardians of the children were asked to fill out a questionnaire containing items about socioeconomic status and the presence of deleterious oral habits. The dentofacial deformities, respiratory characteristic, peribuccal repose musculature and during deglutition registrations were assessed by clinical trial. The results showed a strong association of the anterior open bite with the pacifier and thumb sucking habits, tongue thrust, mixed breathing, predominantly oral, and lip biting, and an association of crossbite with pacifier sucking habits, mixed breathing, predominantly oral, and lip biting. The aim of the second paper, based on literature review and on first paper results, was to provide help to development of an orthodontic assistance protocol in community health. The management and inclusion of preventive and interceptive orthodontic procedures were based on the complexity of occlusal problems, on physical, human, and financial resources, and on some criteria established to provide this treatment. The possible occlusal problems were discussed and classified as primary or secondary priority, focusing on problems that should not be treated precociously. The orthodontic treatment should primarily be offered to children with deciduous and mixed dentition, especially those with a long eruption of permanent incisors. In conclusion, the oral habits were predispose factor by malocclusion etiology, and an appropriate an orthodontic assistance protocol could be applied in public health / Mestrado / Saude Coletiva / Mestre em Saude Coletiva
