Klinický význam biomarkerů pro posouzení agresivity a prognozu nemalobuněčného karcinomu plic / The clinical relevance of biomarkers for aggression assessment and prognosis in non-small cell lung cancer

Aim: The aim of this thesis was to measure a large spectrum of biomarkers in serum or plasma of patients with operable stage of NSCLC and to evaluate and compare the clinical utility of these biomarkers in the three most important clinical applications for NSCLC: diagnosis, prognosis and postsurgery follow up care. Patients and methods: Total of 22 biomarkers with the most promising profiles were monitored: 8 standard tumor markers (cytokeratines Cyfra 21-1, TPA, TPS, and MonoTotal, CEA, SCC, TK, Chromogranin A) and 14 potential useful biomarkers including pro-inflammatory cytokines IL-6, IL-8, MCP-1, pro-angiogenic cytokine VEGF, matrix metaloproteinases MMP-1, MMP-2, MMP-7, MMP-9 and their inhibitors TIMP-1 and TIMP-2, adhesion molecules ICAM-1, VCAM-1, growth factor IGF-1, and PAI-1 stimulating tumor growth and angiogenesis. With a view of evaluating the clinical relevance of these markers for NSCLC we measured serum or plasma levels of these 22 markers in group of 93 patients with NSCLC undergoing radical surgery and in group of 20 patients with benign lung disease. Vliv vnější motivace na kondici u pacientů před transplantací plic / The influence of extrinsic motivation on physical condition in lung transplant candidates

Rehabilitation takes an important part in the care for lung transplant candidates with chronic lung disease. The theoretical part of this thesis summarizes current knowledge about motivation and adherence to rehabilitation, about telerehabilitation options (telemonitoring physical activity using pedometer), and their use in lung transplant candidates. In the practical part it was investigated whether extrinsic motivation (education, pedometer, communication with physiotherapist via e-mail) leads to an improvement in the six-minute walking test outcome (6MWD) or in adherence to rehabilitation in lung transplant candidates. It was found no statistically significant improvement in 6MWD nor in adherence to rehabilitation in extrinsically motivated patients waiting for a lung transplant. However, data suggested increasing trend of 6MWD after intervention.

Title: Physiotherapy as part of multidisciplinary care for patients following a lung transplant operation Objective: To evaluate the mobility of patients in the first days after lung transplantation. To compare the quality of life according to Karnofsky index and Jones dyspnea index. To relate spirometric values FVC and FEV1 of patients in the period prior to lung transplantation with the values in the first year after lung transplantation. Check the link between the development of quality of life and development of spirometric values. Methods: It is a pilot study, which was attended by 18 probands (13 men and 5 women), aged between 14 and 68 years. The data were collected on regular intervals using questionnaires on quality of life (Karnofsky index and Jones dyspnea index) and spirometric examinations taking place at the Motol University Hospital. The data were statistically analyzed using MS EXCEL, mostly using correlations and pair T - test. Results: Results showed substantial changes in the level of significance of p <0,01 both in the development of the quality of life, and in the development of spirometric values compared during the first year after lung transplantation with values before the transpantion. Title: Effect of a comprehensive rehabilitation program of children with asthma bronchiale Objectives: The main objective of this thesis is to show the usefulness and necessity of physical therapy in children with bronchial asthma and how does it affect them. Another objective was to summarize the possible available methods with demonstrations of examples of individual exercises that can be used for this disease. The aim was also monitoring and evaluation of the patient's subjective feelings. Methods: In the thesis we carried out a single arm experiment, where we used comprehensive kinesiology analysis of individual subjects and the methods of lung function tests. From the lung function test, we chose the spirometry test by method of curve flow / capacity and also we used the peak expiratory flow monitoring using peak-flow-meter. We compared the subjective feelings of clients at the beginning and the end of the treatment program. In the period we watched the changes of value in individual measuring techniques. Results: We found that a comprehensive treatment of children asthma has a significant impact on their posture and also on the respiratory functions and subjective perception of individual symptoms investigated clients. The thesis deals with the possibility to influence the values of the lung function in patiens with chronic obstructive pulmonary disease (COPD) after exercise carried out according to the Methodology of sensorimotor training. The thesis contains a summary of theoretical knowledge about the pathophysiologic expresion of COPD, including their consequences not only for the respiratory system, but musculoskeletal system as well. Furthermore the thesis includes the information about the Methodology of sensorimotor training, that was used to influence the posture correction in our research. Methods: The spirometric examination of flow-volume curves monitor the changes prior to and after the exercise according to sensorimotor training in patiens with COPD (mean age 60, 733 ± 11,817). Results: The using of sensorimotor training can provide correction of the position of the whole body, of the chest, and change a chest mobility and in total to set up conditions for more economical muscle activation. This enable the changes in the values of the lung functions in the patiens with COPD, that we measured. Not all changes showed statistically significant change. Conclusion: Using sensorimotor training have an objective effect on the values of the lung function in patiens with COPD.

The present work aims to answer questions regarding lung function among students of two types of grammar schools and describe selected indicators of differences between different groups of students within a reference population. The starting point for this work was speculation that the obligations defined by seconadary education in the ordinary type of school may significantly affect the students of the same age group and same Framework educational program for intensive regime of movement, breathing function. Is it possible that the secondary school educational program could affect the degree of respiratory function of students, for example by having less time and not having time for regular exercise? Are students of common type of grammar school physically less active than their peers from sports-oriented type of school? What are lung volumes of today's teens and how much do they differ from the current standards? The selection of respondents was elected from the students of sport-oriented grammar school and the common type of secondary school students. A total of 196 students participated in the study. Title: Comparison of respiratory function in school age children Objectives: The main aims of this study were to estimate influence of age, anthropometric measures, and a type of physical activity on forced vital capacity (FVC), and additionally to create reference values of FVC for children aged 5 to 10 years. Methods: 281 girls and 345 boys were enrolled in this cross-sectional study. Data were obtained during the sport propagation event called Sportacek 2016. Descriptive statistics and regression models were carried out in data processing. Results: Statistically significant influence of body height as well as influence of physical activity in boys' gymnastics on FVC was found. Multiuser Interference Cancellation in Multicarrier CDMA System with Constrained Adaptive Inverse QRD-RLS Algorithm

In this thesis, the multi-carrier (MC) code division multiple access (CDMA) system is considered in Rayleigh fading channel. The main concern of this thesis is to devise a new direct linearly constrained constant modulus (LCCM) inverse QRD-RLS algorithm for multiple access interference (MAI) cancellation and the problem due to the mismatch of the channel estimator. In the conventional approach, two significant detectors are applied to the system for multiuser interference suppression, one is the blind adaptation algorithm and the other is adaptive linearly constrained PLIC approach. However, the mirror effect may occur when the blind adaptation algorithm is employed. It might affect the performance in terms of bit error rate (BER), although the desired signal to interference (due to other users) improvement is still acceptable. Moreover, in case that the channel coefficients could not be estimated perfectly, the mismatch problem may occur to degrade the performance of the adaptive linearly constrained PLIC approach with the LMS or RLS algorithm. To overcome the mismatch problem, the conventional approach is to use the LCCM criterion with gradient algorithm. However, the convergence rate of the gradient algorithm is too slow to be implemented in real-time wireless communication system. In this thesis, to have fast convergence rate and to circumvent the mismatch problem, the robust LCCM-IQRD algorithm is devised and applied to the MC-CDMA system in Rayleigh fading channel. The proposed robust LCCM-IQRD algorithm has shown to be more effective in terms of MAI cancellation and the mismatch due to imperfect channel estimator. The performance, in terms of BER, of the proposed algorithm is superior to that of the conventional PLIC based algorithms, the blind adaptation algorithm, and the conventional LCCM gradient algorithm.

Wavelet-Based Multiuser MC-CDMA Receiver with Linearly Constrained Constant Modulus Inverse QRD-RLS Algorithm

In this thesis, the problem of multiple access interference (MAI) suppression for the multi-carrier (MC) code division multiple access (CDMA) system, based on the wavelet-based (WB) multi-carrier modulation, associated with the combining process is investigated for Rayleigh fading channel. The main concern of this thesis is to derive a new scheme, based on the linearly constrained constant modulus (LCCM) criterion with the robust inverse QR decomposition (IQRD) recursive least squares (RLS) algorithm to improve the performance of the conventional MC-CDMA system with combining process. To verify the merits of the new algorithm, the effect due to imperfect channel parameters estimation and frequency offset are investigated. We show that the proposed robust LCCM IQRD-RLS algorithm outperforms the conventional LCCM-gradient algorithm [6], in terms of output SINR, improvement percentage index (IPI), and bit error rate (BER) for MAI suppression under channel mismatch environment. Also, the performance of the WB MC-CDMA system is superior to the one with conventional MC-CDMA system. It is more robust to the channel mismatch and frequency offset. Moreover, the WB MC-CDMA system with robust LCCM IQRD-RLS algorithm does have better performance over other conventional approaches, such as the LCCM-gradient algorithm, maximum ratio combining (MRC), blind adaptation algorithm and partitioned linear interference canceller (PLIC) approach with LMS algorithm, in terms of the capability of MAI suppression and bit error rate (BER).

Lung cancer is the leading cause of cancer mortality worldwide. Understanding biological processes of specific antitumor immune response remains of an eminent interest and represents necessity for designing successful antitumor immunotherapeutic strategies. The theoretical part of the thesis describes components of the immune system that are involved in antitumor response and discusses their role in the hitherto known and used lung cancer immunotherapy. In the practical part of the thesis, two studies studying different aspects of anticancer immune response are described. Both studies were conducted in cooperation with 3rd Surgical Department 1st Faculty of Medicine, Charles University and University Hospital Motol and with the biotechnology company Sotio a.s. The first study is focused on the humoral component of the specific antitumor response and prospectively analyses serum frequency of antitumor antibodies against NY-ESO-1, Her2/neu and MAGE-A4 antigens in 121 patients with NSCLC. Here it was shown for the first time that tobacco smoking significantly increases the frequency of NY- ESO-1 antibodies in sera of smokers in comparison to ex-smokers and non-smokers.
