A literatura médica corrente evidencia o baixo seguimento da prescrição. O Autor se propõe a analisar esses achados através do estudo do cotidiano em uma organização médica utilizando a metodologia de Análise Institucional. Os dados coletados sao provenientes de observações do local de trabalho, de reuniões da organização, de entrevistas com pessoal médico e administrativo, de documentos sobre a organização - nos quais se busca captar o discurso verbal, gestual, material e psicossocial - numa tentativa de evidenciar os fatores que influenciam o seguimento da prescrição. Mostra, nos vários níveis organizacionais e, em particular, na relação médico/paciente no momento da prescrição - como o projeto organizacional de atenção pediátrica global com equipe multidisciplinar - tem em sua consistência o modo de produção de relação social próprio da sociedade em que ela está inserida. O Autor aponta o baixo grau de transversalidade que na Instituição tem organizado a relação médico/paciente numa tendência a produzir prescrição articulada à dimensão biológica dos clientes em detrimento de outras dimensões de sua existência, limitando conseqüentemente as possibilidades do seguimento dessa prescrição. O Autor conclui que a solução para melhoria de seguimento das prescrições produzidas, dependem de uma radical transformação da qualidade de relação entre os membros da equipe de saúde e desta com a população. / The current rnedical literature documentss low follow through of the prescriptions dispensed. The Author proposes to analyze pertinent research findings through a field study of the daily practice in a medical organization utilizing the methodology of Institutional Analysis. The field study consisted of observations of organizational meetings, interviews with administrative and medical personnel, and examination of admninistrative and medical documents. These areas were examined through verbal, behavioral, environmental and psychosocial perspectives to assess those factors that were influential in determining prescription follow through. The study shows at various organizational levels, how the doctor-patient relationship at the moment of the prescription and the organizational plan of multidisciplinary pediatric general pratice is influenced by the social realities in which the organization exists. The Author shows that the existing organizational and medical attitudes often lead to prescriptions based upon the patients biological complains and not upon the biopsychosocial reality of the patient population. As such, the prescription often does not address the real needs of the patient thus limiting prescription follow through. The Author concludes that significant prescription follow through gains would be attained through enlarging the whole medical organization\'s attitude and rearranging the organizations plan to conform to the biopsychosocial realities of the patient population.
