Psychosociální aspekty akutní fáze léčby dětského onkologického onemocnění. / Psychosocial aspects of acute phase treatment of childhood cancer

The aim of this diploma thesis is to present psychosocial aspects of acute phase treatment of childhood cancer. The literary briefing section is divided into four chapters. The first chapter presents the general medical characteristics of childhood malignancies and the principles of treating pediatric oncology diseases. The second chapter captures key findings of developmental psychology to ontogenesis in childhood and adolescence. The third chapter focuses on the psychosocial aspects of oncological treatment and psychological interventions applied during this treatment. The fourth chapter deals with palliative care in pediatric oncology and the psychological context of the child's death and family bereavement. Qualitative research consists of a set of four descriptive case reports of patients and their parents who are in the acute phase of cancer treatment or are shortly after its completion. We want to focus on the building of the relationship process based on power and authority, whence, verticalized, between medic and patient. How the information dissemination across the media, to empower the patient, put this authority under discussion and which's their effects in this relationship? Let's look at how changes on the speech of mediatized doctors brings the technical language closer to the non-technical language and a translation work made by an NGO to explain medical terms to children undergoing cancer treatment. We will search for an archeology of the enlargement of medicine and biopolitics importance, in addition to making propositions of possible dialogs that are inserted in a complex and horizontalized relationship. This way, we could study the different connections made at the doctor-patient relationship, at the disease experience. For such, we propose a review of Foucalt, Leão and Morin studies, with respect to the semiotics and communication; Rose, Rabinow, Kumar, Illich and Latour, on the sociological research, and Serson, Edler, Castiel and Caprara, for the medical experience and the relationship with the patients. From an archeological method to the birth of medicine as science and the building of medical authority, based on biopolitics; the cartography of the evolution of the relationship between medic-patient-media; and the translation work made by a NGO, we intend to lay the foundation for the creation of a future method wich could be applied by medics in dealing with their patients / Esta pesquisa tem por objetivo estudar a problematização dos discursos e diálogos na comunicação entre paciente e médico. Queremos nos atentar ao processo de construção de uma relação calcada em poder e autoridade, por isso, verticalizada, entre médico e paciente. Como a disseminação da informação por meios de comunicação, ao empoderar os pacientes, põe essa autoridade em discussão e quais os efeitos disso na relação médico-paciente? Vamos observar como a mudança no discurso usada por médicos midiatizados aproxima a linguagem técnica e não-técnica e um trabalho de tradução feito por uma ONG para explicar termos médicos a crianças em tratamento de câncer. Buscaremos fazer uma arqueologia da ampliação da importância da medicina com a biopolítica, além de fazer proposições de possíveis diálogos que estejam inseridos em uma relação complexa e horizontalizada. Dessa forma, podemos estudar os diferentes vínculos criados na relação médico-paciente, na vivência da doença. Para tal, propomos uma revisão dos estudos de Foucault, Leão e Morin no tocante de comunicação e semiótica; de Rose, Rabinow, Kumar, Illich e Latour, na pesquisa sociológica; e de Serson, Edler, Castiel e Caprara, para as vivências médicas e de relação com os pacientes. Relationship between a doctor and a minor patient - criminal aspects. The main aim of this thesis is to analyze the criminal aspects of the relationship between a doctor and a minor patient. I chose this topic because I grew up in a family of doctors and I had listened to the problems that my relatives as doctors had to solve. This thesis is composed of twelve chapters, which are divided into sections and subsections.The first part of my thesis describes and analyzes the sources of law and basic concepts which are used in legal literature and also in legislation. Due to the specific relationship between a doctor and a minor patient lying in the fact that a minor patient must be represented by his parents in some cases, the third chapter analyzes the conduct of the child as one of the vertices of an imaginary triangle (doctor - child - parents). In chapter four, I focused on description of all kinds of legal liability, which are typical for this area of law. The main focus of my thesis consists of chapters five, six and seven. In these chapters I in detail analyzed criminal liability, firstly in general, secondly criminal liability of doctors and in the last mentioned chapter I discusses further about criminal acts in the context of healthcare. Diploma thesis deals with the problems of sleep of the patients in intensive care. Sleep is the very basic of human needs and its correct pattern is crucial for any activity of an organism. A person who is sick has an increased need for sleep and are among the most frustrated needs. The theoretical part defines sleep as a basic human need to deal with physiology and sleep disorders. I include a chapter dealing with intensive care and aspects of the environment that affect sleep of the patients. The main objective is to determine whether the need for sleep of the patients is saturated or not. I also set milestones as to gain information regarding patients sleeping pattern at home.To find out, what influence nurses in approaching sleep as a matter and to look for factors that have an influence of some sort from the nurses point of view regarding the sleeping pattern same as from the patients view of the matter. In the empirical part, I reached the set goals through interviews and questionnaires. Thanks to interviews with patients, I discovered that need of sleep of patients in intensive care is not saturated.Out of the 30 respondents I interviewed, 17 of them therefore deemed their sleep as poor and insufficient. Experience shows that a prerequisite for the rational use of both instruments is sufficient awareness among both health professionals and patients. Objective: The aim of this study was to analyze the opinions, knowledge and experience of patients regarding generic drugs and generic substitution. Methods: Data were collected between 1 March 2015 and 31 March 2015 in a workplace practitioner in Prague. It was an observational survey. We contacted all patients who visited a doctor during that period. Patients had to agree to their inclusion in the investigation and also meet "inclusion criteria" (e.g., at least 18 years old, literate in Czech, long-term use at least one medicine). The aim of this thesis was to carry out an research which was focused on attitudes, opinions and experiences of patients (general public) regarding generic drugs and generic substitution and to analyze extracted results. Metodology: Data were collected since February to June 2016 in a workplace practicioner in Jičín. We asked all patients who came to the doctor's office during that period. Data were collected through printing anonymous questionnaire. The questionnaire was piloted. Patients got a cover letter with questionnaire together. Respondents were categorized based on the cover letter and the questionnaire was filled out by the respondents who the survey was focused on. AJ Health care is very important part of our lives, therefore legal regulation of providing health care deserves more attention. Specialist publications mostly engaged legislation of providing health services, without contradiction, who is recipient of health care and regulation of status minor doesn't get attention like should have. Minor has special legal status, that is why legal aspects of providing health care is specific too. Related with development of private law many changes happened, which also showed in legal status of underage patient, therefore goal of my thesis is to tender comprehensive view of legislation about minor patient status at provision health care. Thesis is divided into the five main chapters. First chapter provides a comprehensive overview about sources of regulation in selected topic and describe mutual connection some of law regulation. 1 Abstract: The diploma thesis applies sociological perspective on interrelationships of pharmaceutical industry, physicians and patients. Its main focus is on the role of pharmaceutical industry in context of relationships with the two other actors. The first part of the work summarizes available Czech and mainly foreign social science literature referring to the broad network of relations. The transformation of medical profession in current society and the development of international pharmaceutical industry in the Czech Republic are also discussed. The empirical part of the thesis reports the findings of a qualitative interview study undertaken with ambulatory physicians concerning their relationships with pharmaceutical companies. The goal of the research was to explore how and why Czech physicians interact with the pharmaceutical companies, gain insight into their ethical evaluation and assessment of consequences and detect the strategies and rationalizations they use to cope with these relationships. The topic of this thesis is "Evaluation of patient satisfaction with perioperative anaesthesia care." The work is divided into theoretical and practical parts. The theoretical section describes the definition of satisfaction from multiple points of view and analyses factors affected satisfaction of patients and psychosocial needs that have a significant impact on the experience of the hospital care. The practical part deals with the questionnaire survey. The questionnaire is focused on the evaluation of patient satisfaction with perioperative anaesthesia care. Questionnaire results are statistically analysed. The aim of this work is to determine the level of satisfaction of patients in the preoperative and postoperative period.
