THE EFFECT OF BEHAVIORAL SKILLS TRAINING FOR DENTAL HYGIENE STUDENTS ON TREATMENT OF SPECIAL NEEDS PATIENTS DURING DENTAL CARE PROCEDURES

This study assessed the effect of Behavioral Skills Training (instructions, modeling, rehearsal, and feedback) using a multiple baseline design to train 3 dental hygiene students to implement basic behavioral procedures (picture schedules, differential reinforcement, contingent escape, escape extinction, and least-to-most prompting) to manage and prevent challenging behavior during dental care procedures on special needs patients. The study took place in a mobile school-based dental clinic set up within 4 special schools. Training consisted of one group training session and several in-vivo training sessions. After training, participants' performance in the absence of feedback was assessed. Data show that participants performed less than 35% of steps correctly before receiving the training package and quickly reached criteria during training sessions. Results suggest that Behavioral Skills Training could be used to increase skills during one 3-hr class period, with further increases in skills to over 90% accuracy after some follow-up in-vivo practice and feedback.

O Prontuário Eletrônico do Paciente (PEP) é atualmente um dos principais temas de pesquisa e desenvolvimento no âmbito da Informática em Saúde. No entanto, no Brasil e em odontologia, poucos são os estudos publicados. Frente às deficiências apresentadas no fluxo das informações clínicas nos prontuários baseados em papel e visando o aperfeiçoamento deste fluxo para benefício do paciente e da própria instituição, nos propusemos a desenvolver um prontuário eletrônico direcionado ao atendimento odontológico de pacientes portadores de necessidades especiais do Centro de Atendimento a Pacientes Especiais da Faculdade de Odontologia da USP. Para tanto foi utilizada a metodologia de desenvolvimento denominada ?modelagem orientada ao objeto?. O software foi desenvolvido em conjunto com o Centro de Tecnologia XML do Instituto de Pesquisas Tecnológicas de São Paulo. Uma vez terminado, o software foi submetido ao processo de obtenção de registro na Agência USP de Inovações. / The Electronic Patient Record (EPR) is currently one of the main subjects of research and development in the scope of Health Informatics. In Dentistry, however, few are the published studies in Brazil. Facing deficiencies presented in the clinical information flow in paper based patient record and aiming to improve this flow for both patient and institution benefit, we proposed to develop a electronic patient record system suited to Dentistry attendance of patients with special needs of the Special Care Dentistry Center of University of São Paulo. For such task, it was utilized the ?Object-oriented modeling? software development methodology. The software was developed with the Center of Technology XML of the Institute of Technological Research of São Paulo. Once finished, it was submitted to patent attainment at the University of São Paulo Innovations Agency.

Em odontologia, pacientes com necessidades especiais são aqueles com doença ou situação clínica, que implique em um atendimento diferenciado. O Centro de Especialidades Odontológicas é a referência municipal da Estância Hidromineral de Poá - SP, para atendimento odontológico especializado a todos os grupos desta população, nos âmbitos ambulatorial e domiciliar. O objetivo deste estudo foi comparar as necessidades de tratamento periodontal, de restaurações dentárias, exodontias e do uso de próteses removíveis destes indivíduos, atendidos no período de janeiro de 2011 a dezembro de 2016, em relação aos diferentes grupos de diagnóstico e com a população em geral. A amostra total foi composta pelos Pacientes com Necessidades Especiais, também denominados PNE, do CEO-Poá (n=497). Foram avaliados gênero, idade, diagnóstico principal, história médica, âmbito de atendimento, tabagismo, etilismo, acometimentos sistêmicos, e necessidades odontológicas. Os dados foram compilados para o formulário eletrônico epi info 7.0, tabulados, e analisados estatisticamente. No perfil deste PNE, idade média de 37,9 anos, sendo 50,75% do gênero masculino, principalmente com doenças sistêmicas (32,59%) seguida de deficiências físicas (16,70%). As principais necessidades dos PNE foram os tratamentos periodontais (60,70%), seguido das exodontias (59,55%), e restaurações dentárias (57,95%), sendo todos estes percentuais maiores que da população em geral. A reabilitação protética foi possível em 17,11% dos indivíduos, principalmente no grupo denominado condições sistêmicas. O uso de próteses removíveis destes PNE foi menor que da população em geral, e associada ao tipo de diagnóstico e faixa etária. Esses achados reforçam a importância da prevenção odontológica para PNE em CEOs. / In dentistry, patients with special needs are those with a disease or a clinical situation that require a differentiated care. The Center of Dental Specialties of Poá Spa Town is the municipal reference for specialized dental care, therefore have a large number of patients encompassing all categories of diagnosis in this diverse group of people. Treatments are carried out in the outpatient setting, at home, or patient may also be referred to a hospital for general anesthesia. The present study aims to analyze the impact patients diagnosis has on their dental needs. Dental needs for each diagnosis will be considered for attended patients from 2011 to 2016. The physical records will be reviewed, and the demographic data compiled to an electronic form that was specially developed for this study (Annex C). After statistical analysis, it will be possible to verify if there is a relation between main diagnosis, age, gender, scope of care, medical history and dental needs of this group of patients. The compilation of this information will allow a better understanding the characteristics of population attended in a particular municipality. Protocols to meet the dental needs of each group will be suggested. This knowledge may favor safe dental practice, implantation of new services or their improvement. Also may help developing new strategies to promote oral health and better quality of life for these patients.

The information in the medical records serve many purposes: they can be used for diagnosis and therapy, medical documentation contains information that can be the basis for financial authorities for treatment or for pumping reimbursement from health insurance. The data can be used in statistics and other scientific purposes. The aim of study is to verify the simplicity of data process implementation and time of data storing for modification of classical paper WHO dental card, lifetime dental EHR controlled by keyboard and lifetime dental EHR controlled by voice. All three methods were applied on 126 patients. At first the patients were inspected by a standard technique (communication between dentist and nurse) and the data recorded into the paper WHO dental card. The same person recorded all data to lifetime dental EHR using keyboard and using voice. Then we compared the time, which was needed for recording the data using these three methods. Using Friedman test we found very significant differences in time of recording among three methods (p<0.001). We can see that the paper WHO dental card was recorded quickly, but its rise due to missing electronic form is difficult. The Relationship Between The Educational Experiences Of Senior Dental Students And Their Attitudes Towards Intellectual And Developmental Disabilities

This research explores final year dental students’ knowledge and attitudes towards the treatment and management of patients with Intellectual and Developmental Disabilities (I/DD) as related to variations in the dental practices in Special Health Care Needs (SHCN) training they received. A quantitative, non-experimental study was conducted, employing survey data collection methods, to address descriptive, comparative, and correlational questions. One cohort, Class 2020, attended extramural rotations in specialized clinics for patients with I/DD and in-person lecture concerning this topic. However, due to the COVID-19 pandemic and its health-related restrictions, another cohort, Class 2021, did not have the same opportunity as rotations were cancelled and the lecture was prerecorded and viewed online. Responses from a 35-item survey, distributed through an online platform, were obtained from both cohorts (n= 14 for Class 2020; n= 19 for Class 2021), addressing descriptive, comparative, and correlational research questions. Results revealed higher levels of knowledge and more positive perceptions of the program from Class 2020, the group that received clinical guidance, than Class of 2021. Analysis revealed positive relationships between students’ confidence levels and whether they had examined, assisted, or treated patients with I/DD. Associations were also found between students’ confidence and willingness to treat patients with I/DD. Although only correlational, this study’s findings suggest ways to foster students’ confidence and induce positive attitudes in dental school graduates that are likely to improve equitable access to healthcare for those with I/DD.
