Gates, GAVI and Giving: Philanthropic Foundations, Public-Private Partnerships and the Governing of Government

International development has become an increasingly fragmented and complex undertaking, with private wealth assuming an increasingly important role. At the forefront of this group sits the Bill and Melinda Gates Foundation, which has put significant resources behind Public-Private Partnerships such as the Global Alliance for Vaccinations and Immunizations (GAVI). Utilizing Foucault’s concept of governmentality, this thesis argues that foundations are key catalysts in the formation of such globally oriented partnerships, a trend not indicative of a shift in power from multilateral organizations to non-state actors, but representative of changing rationalities and practices of the government of populations at a global scale. This position is contextualized through a case study of the GAVI Alliance, which demonstrates that in the process of governing specific populations, such conglomerations of public and private actors seek to modify the governmental practices of states, in what Dean (1999) refers to as the “government of government”.

In this sense, it is essential we understand that the historical production on health policies, aimed at fighting the disease, can contribute to the understanding of care models that have been adopted, the political relations that have been established, as has occurred the dialogue between the different political spheres. We consider also that the historical knowledge has much to contribute to the fight against cancer, as well as allowing us to ensure the registration and the memory of what has been lived and experienced, enables us also to understand the actions that have already integrated policies control. Thus, we can see possible ways to achieve more positive results in relation to prevention and control. From this perspective, this dissertation will talk on "Cancer Prevention and Control Policy in Amazonas, 1974 - 2011”, taking as object of study the Oncology Control Center – CECON”. To do this, we base our analysis on laws, decrees and reports on health policy in the region; and oral history research, where we use an instrument with a set of questions, which was applied with actors identified with the history of the institution. The work is divided into four chapters, of which three of them refer specifically to the proposed objectives. A chapter that looks at how public policies related to cancer have been applied in the state of Amazonas, from this renowned institution, another chapter, where we analyze the process of establishing public policies on the prevention and control of cervical cancer, in the state of Amazonas and the last chapter with the appreciation and analysis of the performance of two philanthropic foundations: Amazonense League Against Cancer / AM and Women's Network Against Cancer / AM, which perform support activities to the services provided by FCECON. Finally, the study of the Amazon cancer policy from the history of FCECON led us to more comprehensive analyzes bringing us confirmation that the history of local health policy is closely linked to the history of the national health policy, and which presents itself as a result of the reality at the time of its formulation, but also as a result of past actions that interact and give the dynamics of the system. / No estado do Amazonas, o câncer tem alcançado índices preocupantes tanto em relação à incidência quanto à mortalidade, favorecendo o surgimento de políticas eficazes de prevenção e controle da doença. Nesse sentido, torna-se fundamental compreendermos, que a produção histórica sobre as políticas de saúde, dirigidas ao enfrentamento da doença, podem contribuir para o entendimento dos modelos assistenciais que têm sido adotados, das relações políticas que têm sido estabelecidas e como tem se dado a interlocução entre as diferentes esferas políticas. Consideramos, ainda, que o conhecimento histórico em muito tem a contribuir na luta contra o câncer, pois além de nos permitir garantir o registro e a memória daquilo que já foi vivido e experimentado, nos possibilita também, compreender as ações que já integraram as políticas de controle. Desse modo, podemos perceber possíveis caminhos para o alcance de resultados mais positivos em relação a prevenção e controle. Dentro desta perspectiva, esta dissertação discorrerá sobre a “Política de Prevenção e Controle do Câncer no Amazonas, 1974 – 2011, tomando como objeto de estudo o Centro de Controle de Oncologia – CECON”. Para tal, embasamos nossa análise em leis, decretos e relatórios relativos a políticas de saúde da região; e na pesquisa de história oral, onde utilizamos um instrumento com um roteiro de perguntas, que foi aplicado com atores identificados com a história da Instituição. O trabalho se divide em quatro capítulos, dos quais três deles se referem especificamente aos objetivos propostos. Um capítulo que analisa como as políticas públicas referentes ao câncer foram aplicadas no estado do Amazonas, a partir desta instituição de referência, outro capítulo, onde analisamos o processo de estabelecimento das políticas públicas referentes à prevenção e ao controle do câncer do colo do útero, no Estado do Amazonas e no último capítulo com a apreciação e análise da atuação de duas instituições filantrópicas: Liga Amazonense Contra o Câncer/AM e Rede Feminina de Combate ao Câncer/AM, que realizam atividades de suporte aos serviços prestados pela FCECON. Por fim, o estudo da política do câncer no Amazonas a partir da história da FCECON nos conduziu a análises mais abrangentes trazendo-nos a confirmação de que a história da política de saúde local tem estreita ligação com a história da política de saúde nacional, e que se apresenta como resultado da realidade vivida no momento de sua formulação, mas também como resultado das ações do passado que interagem e dão a dinâmica do Sistema.

O atual modelo de atenção à saúde do Brasil tem suas origens no modelo biomédico flexneriano, idealizado e implantado nos Estados Unidos por meio da ação combinada do corporativismo médico local e do grande capital. Sua importação e implantação se insere nas relações gerais de dependência econômica e subordinação política do nosso País aos interesses norte-americanos, desde as últimas décadas do século XIX até as primeiras décadas do século XX. O objetivo desta tese é identificar e analisar como as grandes fundações norte-americanas, financiadoras do modelo de atenção médica originado a partir da publicação do Relatório Flexner em 1910, mais os interesses capitalistas, tanto do grande capital internacional quanto nacionais, e o corporativismo médico brasileiro, construíram o modelo de atenção médico-hospitalar no Brasil e o moldaram à sua conveniência, de sorte a torná-lo hegemônico, preparando o terreno para a implantação da fase seguinte, a do complexo médico-industrial. Para tanto, apropriando-se do discurso dominante da ciência, deverão introduzir-se no Estado ou com ele manter estreito relacionamento, de forma a controlá-lo ou dirigí-lo ainda que parcialmente, privilegiando seus interesses, enquanto vestidos de um discurso que se pretende assistencialista, promovendo a expansão da base de assistência médica individual para cristalizar um padrão hospitalocêntrico e crescentemente tecnificado / The current model of health care assistance in Brazil stems from the biomedical Flexneriam medicine model once imagined and implemented in the United States through the combined action of the local medical corporativism and corporate class, mediated by the great philanthropies, and ultimately exported in the wake of financial capitalist expansionism, resulting in the Brazilian economical and political subordination to American interests between the late XIX and mid XX centuries. The main theme of this dissertation is to analyze how the great American philanthropic foundations, who financed the medical reform after the Flexner Report was published in 1910, combined their interests with the corporate class ones, both international and local, and, adding the support of the local medical corporativism, conformed the Brazilian health care model to its convenience, just preparing the ground for the introduction of the next phase, i.e., the medical-industrial complex. In order to succeed, and under the banner of the medical science, they either penetrated the structure of the State or maintained with it a close relationship, in order to control or at least partially steer it, always meaning its own interests while maintaining a betterment discourse, but promoting individual medical care in order to foster a hospital and technology centered pattern

Indiana University-Purdue University Indianapolis (IUPUI) / This dissertation analyzes the efforts to develop a pluralistic political culture and democratic practices of governance through the training of democratic leaders in Germany's first school of public affairs, the German School of Politics. The investigation of the thought-leaders that formed this school illustrates two main points. First, through the prism of the School, I detail the efforts to develop a conception of civil society that, by being grounded in civility, could retie social bonds and counter the brutalization of politics characteristic of the post-World War One years. By providing practical knowledge, courses in public affairs could not only free Germans from the blinders of ideologies, but also instill in them an ethos that would help viewing the political enemy as an opponent with an equal right to participate in the political process. Secondly, I point to the limits of trans-national philanthropy in supporting the development of civil society in young democracies. By analyzing the relationship between U.S. foundations and the School, I focus on the asymmetry that existed between American ideals of democracy and the realities of the German political system. This study thus focuses on the dynamics between the actions of institutions and organizations, and the broader social behaviors that constitute public life.
