Understanding nurse practitioner-patient communication : reconceptualizing power and relationships through music metaphor

In nursing literature, power is conceptualized as an object transferred, distributed, controlled or conquered by empowerment. In this management care paradigm, the service of care provides power to achieve the product of health. The socio-philosophical framework proposes power as intra-interpersonal set of relationships. Interdisciplinary collaboration allowed discovering power-and-relationships as inseparable mind-body subunits constituting micro and macro health interactions, through a mixed methods instrumental case study. Control and power mechanisms were revealed analyzing body movements and conversations in Case A-15min- and Case B-16.10min- nurse practitioner-patient videotaped encounters. Catalyzed by a hermeneutical music metaphor this thesis proposes relational healing care. Despite interruptions and disruptive postures, nurse practitioners-patients reverse differentials by sharing potentials in simultaneous connections. Power balance is developed by equitable-inequitable communication. Like diverse related tones, nurse practitioner-patient is an Intermelody solving tension continuum in concordance. Power and identity in theory-practice relationships : an exploration of teachers' work through qualitative research.

This thesis provides two interwoven sets of detailed descriptions with narrative lines. The first relates to five case studies involving secondary school teachers in schools in and around Durban during 1993 and 1994. This account focuses on the relationships between the teachers' thinking about knowledge and learning and their classroom practice. The second account describes the processes and difficulties involved in qualitative research incorporating case study and participant observation methodologies - from gaining access to schools and developing a task to access teachers' thinking about knowledge to acquiring skills for observation, writing lesson descriptions, conducting interviews and completing different levels of analysis. In essence, this account traces the development of the researcher during the course of this project and also highlights both the strengths and the weaknesses of qualitative research as a mode of social inquiry. Analysis of theory/practice relationships in each of these descriptions is centred around issues of power and identity, the data collected during the course of the fieldwork being used to develop grounded theory. The work of George H. Mead, Michel Foucault and Thomas Popkewitz provide the basis for the concept of power identity. The relational and shifting nature of power and its role in identity and theory/practice relationships - both in the work of the five teachers work and in qualitative research - is explored. In the former, seven interrelated components of power are identified and the ways in which these strengthen and limit teachers' power identities are described. In the latter, the connections between epistemology and research methodology and the similarities between qualitative research and local criticism are highlighted together with the critical roles played by contradiction, language and reflexivity. Finally, the insights gained about theory/practice relationships and power identity are extended to provide possibilities for conceptualising rationality and teacher education. An epistemological study of the power of women as nurses : a phenomenological approach.

Aim: The aim of the study was to do an epistemological analysis of the power of women as nurses working in one of the larger and more deprived regions of South Africa, namely KwaZulu-Natal. This research was based on the premises of the Standpoint theory. Concurrent to the collection of data, a literature review and a concept analysis of power, powerfulness, powerlessness and empowerment were done and incorporated where applicable in the final theoretical framework. Methodology: A phenomenological approach was used. This incorporated two to three in depth interviews with each participant, lasting an average of thirty minutes each, was used. Women, relating to the gender factor, as nurses were also marginalised in terms of class, as they belonged to the enrolled category of nurses, and race, being African. A fourth selection criterium was added to analyse the reality of locality, called rurality. The researcher applied the principle of theoretical saturation and a total number of nine women, who belonged to the enrolled category of nurses and who worked in a distinct rural health care setting, were interviewed. A second group consisting of five women enrolled nurses and working in an urban setting were interviewed, as were a third relatively contrasting group of four women registered nurses. All forty four interviews were audio taped and transcribed, and a qualitative software package called NUD*IST was used to identify and refine experiential themes. Findings: The relationship between power and rights was often layered in contradiction during the interviews and the participants portrayed a picture of being oppressed or marginalised and powerless. The women as nurses belonging to the enrolled category were alienated as women and as nurses in terms of being severed from the nursing profession and from the ruling gender of men, of being lost in an ever present routinization of activities, of being misused, maternalised and domesticated at home and at work. These phenomena were quantitatively more voiced by the rural group of women and these participants strongly emphasized the limiting influence of their prescribed scope of practice, the approach of the senior category of nurses and they conveyed a traditionalist and altruistic view of nursing and nurses. The women as registered nurses created their own freedom often away from their men as in divorce. They also sought for solutions concerning powerlessness in more global and distant terms, for example in relation to cultural practices. They communicated a sense of empowerment in terms of for example education, personal qualities and increased job satisfaction. Culture rather than race was emphasized as an essence of womens' oppression. An epistemological framework of the power of women as nurses developed by the researcher constructed the totality of empowerment in terms of fifteen transformational and hierarchial actions incorporating and adapting Dooyeweerd's theory of modalities and the surfaces of class (categorial divide), gender (the eternal carer), race (culture) and locality (rurality). These actions and reformulated concepts could be used for the development of specific strategies to facilitate the empowerment of individuals, groups and communities of women as nurses and nurses as women. Further collaborative research into the phenomenon of power, a reconceptualization of nursing education and levels of expertise and hierarchies within nursing were some of the suggestions for the way forward. / Thesis (Ph.D.)-University of Natal, Durban, 1996.

Discourse, dogma, and domination: knowledge work as art and politics.

Faculty of Business. / The thesis critically analyses the gaps among management literatures as discourses of ambition and evaluates them against the realities that constitute praxis. The work provides a different insight into organisational and management theory that encourages critical thinking about the normalising effects of discourse, and points to the possibilities that can emerge from engaging with alternative perspectives, such as those emanating from practitioners. The analytic framework that is used to identify and explicate this hiatus is drawn from Foucault’s genealogy, which is used as a method for conceptualising and explaining relationships between and among discourses. Genealogy is also used to show that there is not merely one way of perceiving an object of discourse and thus creating meaning, but many. The topic of the thesis is knowledge work. The assumption that there is a clear and abiding descriptor of knowledge work supports an erroneous perception that there is consensus in interpretation and that its meanings are fixed and uncontested. Rather, the concept of knowledge work is ambiguous and highly contested. It is inconsistently conceptualised in the literature and scholars frequently omit any definition or clarification of what knowledge work is, perhaps assuming that their readers will have an inherent and automatic understanding of it. The thesis navigates the many discourses of knowledge work. It shows that in practical terms, inferences of neutrality and normality are instead prescriptions, through which different interpretations pit those who prescribe against those who do. Knowledge work has emerged as a significant domain of practice and discourse that resonates within the fields of organisational and management theory, and within the circuits of business, consulting, education, and policy formation. Knowledge has become the business of business, such that the discourse of knowledge work has become significant within the discursive knowledge fields of organisation studies, management studies, economics, technology, intellectual property, globalisation, and finance. The importance of knowledge work is such, that in contemporary discourses it is seen as facilitating a new golden age of a knowledge society. The dissertation tackles this hypothesis through two historical illustrations. The first shows that the modern concept of knowledge work emerged as a response to particular historical conditions to refract social, economic and political circumstances. The second illuminates an antecedent of the contemporary ‘knowledge society’ to show that it is neither new nor unique.
