D.Cur. / A qualitative approach was followed to conduct a case study. Data was collected through participative observation, document analysis and interviewing of team members representing seven different disciplines. The practice model of Dickoff, James and Wiedenbach (1986:415-435) was utilized as theoretical framework to explore and describe the role players involved in team functioning within the context of primary health care. Team objectives and processes relevant to team functioning as well as the dynamics of team work were described and compared to a guide for team functioning, the elements which were derived from a literature study. The credibility of the research was enhanced through long term involvement in the activities of the study case as participative observer, through triangulation of methods to collect and verify data and through peer evaluation and auditing. It was determined that within the context of a home care service at primary health care level, it was mainly the nurse, being the team member with whom the patient and family have continued personal contact, who acts as team leader and initiates involvement of members from other disciplines. The decisions regarding whom to involve and how and when to involve them, are based on the knowledge and skills of the nurse in relation to the situation he/she has to manage, the acceptability of the team approach and compliance by the patient and family, the attitude, commitment and perception of other team members as well as the availability of facilities and infrastructure to enable team functioning. The organizational and philosophical framework within which service is delivered have a direct impact on team functioning. Community involvement and its acceptance of responsibility for health, enhances quality team functioning, but is dependant on effective empowerment strategies, a sound trust relationship and a reliable support system. Clear and agreed upon goals and objectives for team functioning are essential for effective team work. Innovative and creative strategies are necessary to enable team members representing different disciplines, who function in the context of primary health care, to have sufficient contact with each other to discuss objectives, give feedback and communicate effectively. Processes for and dynamics in team functioning are complex in nature and need to be identified, evaluated and, if necessary, adapted on a regular basis to promote effective team functioning. The objectives of the research, namely to explore and describe team functioning at primary health care level and to formulate guidelines for effective team functioning, were realized. A structure for working from the basis of a core team was proposed, as well as interdisciplinary training of team members, commencing at undergraduate level and continuing throughout professional life. The researcher recommends that the guidelines formulated with regard to the role players, context, objectives, processes and dynamics of team functioning at primary health care level be implemented. Aspects related to team functioning to be further researched have been identified, namely: assessing the quality of team functioning at primary health care level; the effect of interdisciplinary training on team functioning; the relationship between team functioning and the health status of communities; cost-efficiency of team functioning; utilizing latest technological developments for communication between and support for team members at primary health care level; the relationship between community empowerment, community involvement and interdisciplinary team functioning and the design of a model for team functioning at primary health care level. The research report is written in Afrikaans, but the conclusions reached in each of the nine chapters have also been translated into English to enhance the accessibility of research findings.

M.Cur. / The aim with this study is to describe guidelines for curriculation, with primary health care as departure point. This study is based on the Judeo-Christian human and world premise and philosophy. The Nursing Theory for the Whole Person serves as meta-theoretic departure point, while the theoretic assumptions are based on: The Nursing Theory for the Whole Person (Oral Roberts University, Anna Vaughn School of Nursing, 1990). Van Niekerk's, Accompaniment Guidelines for Theory - Practice Correlation (1993). Curriculation Models of Malan & Jorrisen (1990) and Uys (1983). Klopper's Model for Accompanied selfstudy in Nursing Science (1994). The Botes's Model for Research in Nursing Science serves as methodological departure point (1991). Emphasis is placed on participative decision making regarding basic needs, including health care, within the ideology of the new Government of National unity (ANC, 1994:45) The Reconstruction and Development Programme, (RDP) accepted within the new political dispensation of South Africa, supports primary health care as total departure point, such as the National health Plan (ANC, 1994: 45). This, however, is not a new concept in health care services. The South African Nursing Council (SANC) has been supporting this principle for a considerable time. However there is little or no existing literature regarding the curriculation of the four year diploma programme, with primary health care as the departure point. This Qualitative study is executed in four phases. The first and second phase explore and describe the perceptions and interpretations of the share holders of the curriculum, in respect of primary health care as departure point, within curriculums, in terms of focus group interviews. The third phase describes a conceptual framework, according to a comprehensive literature study. Phase four describes guidelines for the facilitation of meso and micro curriculation, with primary health care as departure point, in respect of inferential data obtained from focus group interviews, the literature study and conceptual framework. The study's unique contribution, within the context of the Mpumalanga region is embodied in the manner in which the researcher succeeds to describe guidelines for curriculation for the four year diploma programme, that leads to registration as Professional Nurse (General-, Psychiatric-, and Community-) and Midwife, with primary health care as the departure point. This was achieved through inductive deliberation and cross triangulation in all four phases, which increases the trustworthiness of the study.

M.Cur. / With the announcement of the devolution of primary health care services to the local authority by the Cabinet in 1991, the role fulfilment of the community health nurse becomes more complex and greater demands are continuously made on her. The question, to what extent will the Implementation of primary health care (with the critical elements as framework) make greater demands on her role and function, led to this study. An exploratory, descriptive study, within a contextual framework was carried out. The purpose of the research was to analyse the task of the nursing staff working at local government, to determine which critical elements In primary health care are seen as part of the tasks of the community health nurse and to give guidelines with regard to primary health care and community health nursing. Content analyses done of job descriptions Indicated that some of the critical elements of primary health care are not expected to be performed by the nursing staff. According to the information gained through the questionnaires, nurses do more than is expected of them as Indicated In their job descriptions. " According to the conclusions resulting from this study, most of the critical elements of primary health care is being performed by nurses as part of their duty. The most Important recommendations Include the training of those nurses who feel that they need refresher courses with regard to the examination of patients and the making of diagnoses. Nurses need to know about the changes and what Is expected of them In future as a result of these changes.
