Caso clínico MirellaGranados Pazos, Carolina Antuanett del Carmen 26 June 2023 (has links)
El presente caso clínico es sobre Mirella, una joven de 18 años que llega a consulta
buscando descargar sus emociones sin que la juzguen y la entiendan. El objetivo de este espacio
de consejería, el cual constó de 5 sesiones, fue intentar que ella pueda cuestionarse si realmente
podría confiar en alguien ya que hubo varias experiencias en su vida que la llevaron a un dolor
psíquico asociado a la percepción de mentira y el juicio. Para el desarrollo del análisis se han
utilizado a diversos autores que mencionan sobre aproximaciones psicoanalíticas teóricos
técnicas y se consideró algunas características esenciales en el marco analítico, las cuales se
flexibilizaron en el encuentro con la paciente. El tipo de relación objetal que domina su mundo
interno es la parcial ya que no puede integrar que una persona tiene aspectos positivos y
negativos, sino que percibe a su entorno muy amenazante y la angustia que predomina es la
paranoide porque su Yo tiene miedo a aniquilarse y expulsa hacia afuera lo negativo. Entre sus
mecanismos de defensas está la escisión, la proyección y la represión de sus emociones. Mirella
cuenta con recursos como la capacidad de reflexión, comprensión sobre las opiniones de otros
y una actitud positiva para la sesión; y podría continuar con una terapia psicoanalítica para
seguir elaborando los diversos temas que pueda traer pero debería fortalecer su compromiso
terapéutico, el cual le permitiría ser capaz de involucrarse, confiar y tolerar la intimidad del
proceso. / This clinical case is about Mirella, an 18-year-old girl who comes to the clinic looking to unload
her emotions without being judged and misundertood. The objective of this counseling space,
which consisted of 5 sessions, was to try to make her question if she could really trust someone,
as there were several experiences in her life that led her to psychic pain associated to the
perception of lying and judgment. For the development of the analysis the work of various
authors has been used, work that mentions about technical theoretical psychoanalytic
approaches and some essential characteristics were considered in the analytical framework,
characteristics which were made more flexible in the encounter with the patient. The type of
objectal relationship that dominates her internal world is partial, since she cannot integrate that
a person can have positive and negative aspects, but instead perceives her environment as very
threatening and the distress that prevails is the paranoid one because her Ego is afraid of
annihilating itself and expels the negative outwards. Among her defense mechanisms is
splitting, projection, and repression of her emotions. Mirella has resources such as the capacity
for reflection, understanding of the opinions of others and a positive attitude for the session
and she could continue with psychoanalytic therapy to keep up working on the various subjects
that she may bring up, but she should strengthen her therapeutic commitment, which will allow
her to be capable to engage, trust and tolerate the intimacy of the process.

Una exploración del rol de la mentalización en los episodios de Ruptura y Resolución de una sesión en la mitad de un proceso de psicoterapia psicoanalítica focal: Caso únicoYepez Oblitas, Clis Gabriela 22 July 2022 (has links)
En el campo de investigación en psicoterapia la atención estuvo dirigida a evaluar la eficacia de los tratamientos
poniendo el foco en los resultados (Jiménez, 2005). A través de un giro metodológico, se comenzó a investigar el
proceso terapéutico centrado en los episodios relevantes de cambio; enfocados en la interacción terapéutica, el
proceso de cambio y los factores extra-terapéuticos que producen el cambio (Krause et al., 2006). Se buscó
entender qué ocurre en las sesiones identificando episodios relevantes durante el proceso (Valdés et al., 2005).
Dentro de los mismos se estudian los episodios de ruptura, que suponen la ausencia de colaboración frente a los
objetivos terapéuticos; y los de resolución, mediante los cuales se repara la ruptura y mejoran la situación
terapéutica (Valdés, Gómez y Reinel, 2018). En estudios sobre mentalización en relación a episodios de cambio
y ruptura, se analizan la forma en la que se manifiesta durante el proceso psicoterapéutico (De la Cerda et al.,
2019) y se sugiere profundizar en su estudio por la relevancia de la misma en los campos de investigación
(Katznelson, 2014). Esta investigación, busca describir el papel de la mentalización antes, durante y después de
los Episodios de Ruptura y Resolución en una sesión de la fase media de un proceso de Psicoterapia Psicoanalítica
Focal. Se trabaja a partir del registro literal, auditivo y escrito, de lo acontecido en el proceso con el objetivo de
identificar episodios de ruptura y resolución; y verbalizaciones mentalizadoras a lo largo de la sesión 7. Se
codifican los datos desde un enfoque temático, lo que permite un acercamiento micro a la interacción de terapeuta
y paciente.A nivel macro: se encuentra que hay más momentos de mentalización que de no mentalización en la
sesión. La presencia de mentalización no evita las rupturas, pero la ausencia tiende a anteceder rupturas. Hay
mentalización antes de las resoluciones. Hay más rupturas y no mentalización hacia la mitad. La presencia de
mentalización y resoluciones se da hacia el final de la sesión. A nivel micro: se encuentra que en las rupturas de
retirada hay menor mentalización por parte de ambos. En las rupturas de confrontación el terapeuta puede
conservar la mentalización. Las resoluciones tienden a generar mayor mentalización. Finalmente, se plantea una
discusión que busque profundizar en la exploración de la metalización en el campo de la investigación cualitativa. / In the field of psychotherapy research, attention was focused on evaluating the efficacy of treatments by focusing
on outcomes (Jiménez, 2005). Through a methodological shift, research began to investigate the therapeutic
process focusing on the relevant episodes of change; focusing on the therapeutic interaction, the process of change
and the extra-therapeutic factors that produce change (Krause et al., 2006). The aim was to understand what
happens in the sessions by identifying relevant episodes during the process (Valdés et al., 2005). Within these, we
study the episodes of rupture, which involve the absence of collaboration in the face of therapeutic goals; and
those of resolution, through which the rupture is repaired, and the therapeutic situation is improved (Valdés,
Gómez, & Reinel, 2018). In studies on mentalization in relation to episodes of change and rupture, the way in
which it manifests itself during the psychotherapeutic process is analyzed (De la Cerda et al., 2019) and it is
suggested to deepen its study due to its relevance in the research fields (Katznelson, 2014). This research seeks to
describe the role of mentalization before, during and after the Episodes of Rupture and Resolution in a session of
the middle phase of a Focal Psychoanalytic Psychotherapy process. We work from the literal, auditive and written
record of what happened in the process with the aim of identifying episodes of rupture and resolution; and
mentalizing verbalizations throughout session 7. The data are coded from a thematic approach, which allows a
micro approach to the interaction between therapist and patient. At the macro level: it is found that there are more
moments of mentalization than non-mentalization in the session. The presence of mentalization does not prevent
ruptures, but the absence of mentalization tends to precede ruptures. There is mentalization before resolutions.
There are more breaks and non-mentalization towards the middle. The presence of mentalization and resolutions
occurs towards the end of the session. At the micro level: it is found that in the withdrawal ruptures there is less
mentalization on the part of both. In confrontational breaks, the therapist may retain mentalization. Resolutions
tend to generate greater mentalization. Finally, a discussion is proposed that seeks to deepen the exploration of
metallization in the field of qualitative research.
