Já na década de setenta, detecta-se a distorção de seu modelo, baseado no psicofármaco e no psiquiatra, sendo apontado como um serviço responsável pela cronificação e pelas re-internações dos pacientes psiquiátricos. Com a implantação da Política Nacional de Saúde (PNSM) em 2001, que preconiza a implantação dos Centros de Atenção Psicossocial, como serviço substitutivo ao modelo asilar, toma-se as categorias chaves que norteiam esse novo modelo de atenção em Saúde Mental para se detectar o impacto que essa política teve no modelo assistencial do ambulatório psiquiátrico. Como metodologia, foi aplicado um questionário semiestruturado a 41 técnicos de nível superior para se identificar o perfil dos técnicos que trabalham no Ambulatório do Hospital Juliano Moreira (HJM). Destes, foram escolhidos 11 profissionais, representando as 8 categorias que atuam naquele serviço e mais três gestores da unidade, os quais representavam os informantes-chaves desta pesquisa, para uma entrevista em profundidade, com um roteiro aberto. As entrevistas foram gravadas, transcritas, codificadas e analisadas, utilizando-se a técnica da análise do discurso. Como resultado, encontrou-se que esses técnicos, na sua grande maioria, são formados há mais de 15 anos, com 15 a 30 anos de experiência em Saúde Mental, principalmente em hospital e ambulatórios psiquiátricos; poucos são os que têm experiência com CAPS. Estão confortáveis e satisfeitos pela escolha profissional e por estarem atuando no HJM. Em relação à humanização, acolhimento, territorialização e integralidade, como eixos do novo modelo de atenção, citaram algumas atividades que ocorrem no complexo hospitalar, com tais características, sem implicar essas práticas com a com a PNSM. Alguns desses técnicos tinham dificuldade de definir os 4 eixos orientadores do novo modelo de atenção e a maioria deles informou nunca ter lido o documento da PNSM. De um modo geral, foi detectado que a PNSM é pouco conhecida, em profundidade, pelos técnicos responsáveis por sua implantação na capilaridade da rede; que o ambulatório psiquiátrico continua com o modelo farmacológico e centrado no médico, com tratamentos com mais de 20 anos de duração, sem intervenções que apontem para a integralidade das ações esteadas na interdisciplinaridade e nas discussões dos casos; que a maioria das queixas, quanto à qualidade de atenção, foram direcionadas para a prática dos psiquiatras, tendo em vista que boa parte das vezes não encaminham os pacientes para outros serviços a fim de ampliar o leque terapêutico, reproduzindo as receitas dos mesmos psicofármacos por vários anos. Finalmente, esta pesquisa apresenta duas propostas, uma que propicia o enxugamento do excesso de pacientes compensados, fazendo uma articulação com a Atenção Básica à Saúde e outra proposta que incorpora os 4 elementos do eixo norteador da PNSM no atual modelo de atenção do ambulatório psiquiátrico, re-orientando-o para o novo modelo de atenção, ou seja, o modelo de atenção psicossocial. / Salvador

The thesis focuses on the use of Evidence Based Medicine (EBM) methods and principles at the Psychiatric Clinic of the First Faculty of Medicine of Charles University and the General University Hospital in Prague. There is a theoretical introduction to EBM - its origin and development, EBM information resources as well as general basic principles of this approach to work with clinical evidence. Part of this work introduces the history and current activities of the Psychiatric Clinic and the related Psychiatric Association of the Czech Medical Association of J. E. Purkyně. The practical part of the thesis describes the preparation of research and its results. Based on semi-structured interviews with physicians of the Psychiatric Clinic, the results of this research represents their current attitude towards EBM and use of EBM methods in their daily work. The aim of the thesis is also a proposal to raise awareness of EBM information sources in the form of information materials and educational seminars.

The subject of my diploma thesis are selected parts of the construction technology project of the psychiatric clinic of the University Hospital Brno based on the provided project documentation. It is an extension of a new six-storey building to the existing buildings. The thesis contains a technical report to the construction technology project, the coordination situation of the construction, the situation of wider transport routes, time and financial plan by objects, study of the realization of the main technological stages, a site equipment design, a design of a machine set, technological regulation for monolithic structures, quality control and testing plan of monolithic structures, noise study and selected documents for LEED certification.
