Independence of Mania and Depression across 4 Years in Bipolar Disorder

If mania and depression are part of the same pathological processes, one would predict that episodes of one prospectively increase the odds of episodes of the other. The aim of the present study was to test this hypothesis. For comparison purposes, their relationship was contrasted to the relationship between mania and periods of psychosis. Exploratory analyses also tested the degree to which episodes of each occur with greater frequency over time (i.e., kindling). Participants for the present study came from the Suffolk County Mental Health Project (N = 628), a study of first-admission patients with psychosis. Of these participants, 144 met diagnostic criteria for bipolar I disorder and were analyzed for the current study. Results indicated that mania in a given month predicted depression the following month, even after controlling for other symptoms. The reverse, however, was not the case. Mania and psychosis, in contrast, were found to be robust predictors of one another from month to month. Effects were not due to treatment or demographic differences. These findings provide evidence that mania and depression are weakly related. In contrast, mania and psychosis are more closely linked. Findings are consistent with suggestions that psychiatric nosology regroup mania more closely with thought disorders rather than with internalizing or depressive ones. They also alert clinicians to the strong, longitudinal persistence and comorbidity among these syndromes.

M.A. / Once upon a time in a kingdom that existed along the outskirts of our land, there lived some people who were not too popular in our kingdom because they did not cleave to the way of our world. So, these people were locked away, and sentenced to spend time in our dungeons until they were ready to accept the way the kingdom was run. Delusional Dave believed that he was to marry the princess, even though she was only 11 years old. He was of a lowly nature, not even comparing to the standards of a page in the —courts. It was not acceptable that he should even imagine being with a princess maiden, let alone a princess. Sweet Sandy would rant about ideas that were ahead of the times. She believed that she could run the world through programs in her head, she adhered to the concept of infinity, with millions of people working for her. How could such a lowly subject of the kingdom imagine to have such self imposed importance among so many? Rancid Robby was not an agreeable character in the kingdom, because he admonished others through his belief that they were doubles, impostors to do him harm. He believed that the impostors who paraded as the 'doctors' and 'healers' of the kingdom had planted in his head a microchip so small, whereby damage to his brain would occur daily. These three subjects of the kingdom could be heard screaming, ranting and raving into the small hours of the night. Their cries would fall on deaf ears, as the superiority who ran the kingdom and operated the working of the dungeons, would not listen to the cries of madness, would not hear what the people were trying so desperately to say, and could not find it in their hearts to bring relief and comfort to those with the desperate cries. Oh, not to slander the good of the people, the 'decision-makers', who were in charge of deciding the fate of the madmen. They wanted so badly to help, but all they knew was the 'truth' that ran the soul of the kingdom. That truth being the directive of 'conform to the ways of the world or die in the dungeons'. If only Delusional Dave, Sweet Sandy and Rancid Robby were to conform and be like the others in the kingdom, then they would survive. Alas, this was not to be.... This story does not have a happy ending, as these poor subjects of the kingdom were soon lost in the abyss of the one and only reality that the kingdom was prepared to see and hear. That of normality...

La thérapie cognitive-comportement des psychoses en début d'évolution : étude pilote

Les résultats aux méta-analyses sont concluants quant à l'efficacité de la thérapie cognitive-comportementale des psychoses (TCCp) sur la symptomatologie positive auprès de populations chroniques. Néanmoins, seulement 50% des participants répondent à la psychothérapie et peu de données sont disponibles concernant les facteurs associés à son efficacité. Aussi, l'accès à la formation est difficile pour les cliniciens. Conséquemment, l'accès au service est difficile pour les patients. L'application des programmes de TCCp à des milieux cliniques autres que les milieux de recherche les ayant développés est nécessaire afin d'évaluer la validité externe de la TCCp. La présente thèse se propose d'étudier l'efficience d'un programme de TCCp australien auprès d'une population de psychoses débutantes de Québec. Dans cet objectif, une étude ouverte d'implantation en milieu clinique a été réalisée. Le programme offre 25 sessions sur 6 mois à vingt patients âgés de 14 ans et plus. Dans un premier temps, l'étude vise à évaluer l'application clinique de la TCCp sur le terrain (Chapitre II). Les résultats suggèrent que la TCCp est applicable dans les milieux cliniques. Le taux d'acception est de 75 %, le taux moyen d'observance aux sessions est de 84 % et les participants s'en disent généralement satisfaits. Les analyses rapportent des améliorations statistiquement significatives de la symptomatologie psychotique et du fonctionnement, lesquelles sont maintenues 6 mois plus tard. L'autocritique, l'observance à la médication et l'anxiété sont également améliorées de façon statistiquement significative au post-traitement. Dans un deuxième temps, l'étude vise à identifier des prédicteurs d'évolution chez les participants (Chapitre III). Les résultats montrent que le nombre d'erreurs persévératives au Wisconsin Card Sorting Test (Heaton et al., 1999) est associé à une moins bonne évolution et que la présence d'un diagnostic d'abus de substance en pré-traitement est associée à une meilleure évolution. Aussi, l'amélioration de l'adhérence à la médication entre le pré et le post-traitement est associée à une amélioration plus importante des symptômes psychotiques chez les participants, ce qui suggère que la TCCp ait une action synergique avec la pharmacothérapie. Enfin, l'analyse de l'ensemble des données permet d'émettre des recommandations concernant l'application de la TCCp dans les cliniques.

Postnatal depression: exploring adolescent women's experiences and perceptions of being depressed.

Adolescent pregnancy has been of longstanding societal concern primarily because of the inability of most young mothers to provide adequately for their infants. Depression often results in disengagement from mother-child interaction. Adolescent mothers identified as depressed are at increased risk of future psychopathology, with additional deleterious effects on their infants&rsquo / lives. The purpose of this study was to explore adolescent mothers&rsquo / experiences of motherhood and memories of feeling depressed during or after the birth of their babies. The first aim was to explore the young women&rsquo / s experiences of mothering, by focusing upon the practice of being a mother. The second aim was to explore the young women&rsquo / s experiences of depression, by focusing on their physical behaviour and emotional experiences. The third aim was to explore their perceptions of the causes of their depression. The rationale for this study was that these issues will further enhance the body of knowledge available to practitioners working with adolescent mothers. It will also provide a source of insights and hypotheses for preventive intervention research. The study was located within a feminist standpoint framework that begins from the perspective of women with the aim to explore women&rsquo / s accounts of their experiences in relation to depression as an important source of knowledge. A qualitative research design and methodology was employed in the region of the Western Cape. Eight adolescent mothers between the ages of 16 and 19 were recruited and interviewed. The interviews were semi-structured and consisted out of open-ended questions. Interviews were recorded, transcribed verbatim and thematic analysis of data was carried out. The findings of this research yielded some interesting areas for future research and implications for treatment and intervention with first-time adolescent mothers. The adolescent mothers in this study experienced similar depressive symptoms to adult mothers in previous research. All the participants revealed that they feared their parents&rsquo / disappointment in them for being pregnant. This factor contributed to their depression, because they received very little (if any) support from their parents or the father of their baby. The participants discussed that their pregnancy or giving birth was linked to various aspects of themselves that they had lost as an adolescent. Before they received counselling, none of the participants understood why they experienced depressive symptoms or what was happening to them at the time after their pregnancies.

Distorted Time Perception as an Underlying Factor of Psychosis Proneness and Dissociation

Distortions in the perception of time historically have been associated with dissociation and psychosis in clinical populations. However, the relations among dissociation, psychosis, and time perception in sub-clinical populations have not been investigated. In the present study, college undergraduates scoring either normally or deviantly high on the Per-Mag were given a Dissociative Experiences Scale (DES) and a computerized time-estimation/production task. Participants scoring high on the Per-Mag obtained higher scores on the DES than participants scoring low on the Per- Mag. Per-Mag scores also correlated positively with DES scores across 608 total participants screened. The relation between dissociative and psychotic symptomatology is discussed considering dichotomous versus continuous conceptualizations of psychopathology. The effects of intelligence, social desirability, malingering, gender, and post-traumatic stress on the measures used are also discussed.
