January 2008 (has links) <p>The aim of the study was to measure patient waiting and service times, describe the causes of high waiting time as well as to determine staff and patient derived norms on waiting times at the Manzini Regional Hospital HIV Clinic. Literature reviewed show that complaints regarding long waiting time have been effectively dealt with through patient flow analysis studies, which measure patient waiting and service times as well as other characteristics. A descriptive cross sectional quantitative survey methodology was undertaken using a time-delimited sample of all patients attending the HIV clinic over one week and all the staff who were in contact with the patients. Waiting time tracking instruments and short questionnaires were used to collect data from both patients and staff. Roman&quot / ,&quot / serif&quot / ">Background: The care and treatment for people with HIV/AIDS (PLWHA) at the Kagera regional hospital began in February 2005. It is important to evaluate quality of life (QOL) of highly active anti-retroviral therapy (HAART) recipients periodical to assess HAART&rsquo / s effectiveness. This study aimed to assess the health-related QOL (HRQOL) of HAART recipients and determine factors associated with it, two years after the programme had started at the kagera regional hospital in Tanzania. Results: The HRQOL of HAART recipients was generally lower than that of the general population. However, the general health perceptions (p=0.191) and mental health (p=0.161) means score of HAART recipients were not different from those of the Tanzanian general population. About 89% of the HAART recipients reported better health than in the previous years. Chronic diseases were associated with lower HRQOL of HAART recipients independently of other factors. HAART recipients with chronic diseases co-morbidity were more likely to score below the general population&rsquo / s mean score for mental health (p=0.007). While the effect of chronic diseases on HRQOL among those who recorded CD4+ cell increase was negative (OR=13.6,95%CI=3.7 / 49.9), there was no effect on those who did not have such an increase. Roman&quot / ,&quot / serif&quot / ">Background: The care and treatment for people with HIV/AIDS (PLWHA) at the Kagera regional hospital began in February 2005. It is important to evaluate quality of life (QOL) of highly active anti-retroviral therapy (HAART) recipients periodical to assess HAART&rsquo / s effectiveness. This study aimed to assess the health-related QOL (HRQOL) of HAART recipients and determine factors associated with it, two years after the programme had started at the kagera regional hospital in Tanzania. Results: The HRQOL of HAART recipients was generally lower than that of the general population. However, the general health perceptions (p=0.191) and mental health (p=0.161) means score of HAART recipients were not different from those of the Tanzanian general population. About 89% of the HAART recipients reported better health than in the previous years. Chronic diseases were associated with lower HRQOL of HAART recipients independently of other factors. HAART recipients with chronic diseases co-morbidity were more likely to score below the general population&rsquo / s mean score for mental health (p=0.007). While the effect of chronic diseases on HRQOL among those who recorded CD4+ cell increase was negative (OR=13.6,95%CI=3.7 / 49.9), there was no effect on those who did not have such an increase. Denna uppsats undersöker detta energibesparingsinitiativ genom att studera vad som händer med luftrörelser och temperaturer i ett rum på sjukhuset vid varierande ventilationsflöden. Metoden är rökförsök med observation och analys av mätdata som jämförs med krav på inomhusklimat och beräkningar av temperatureffektiviteten, ett kvantitativt mått på hur väl tillförd värme kommer rummets vistelsezon tillgodo. Resultaten indikerar bland annat att det kan vara till nackdel för luftkvalitén med alltför drastiska ventilationsflödessänkningar i det observerade rummet, detta fastän mängden tillförd luft är mer än tillräcklig enligt gällande krav. Bäst temperatureffektivitet uppnås då frånluft går via både frånluftsdon och frånluftsfönster vilket pekar mot att bäst förutsättningar för att sänka ventilationsflöden eller tilluftstemperatur finns med denna frånluftsinställning. / With the purpose of lowering energy consumption at Sundsvall Hospital, focus is now being directed towards the ventilation system and the possibility to reduce air flow. The thesis examines this energy-saving initiative by studying what happens to air movements and temperatures in one room at the hospital when ventilation air flow is varied. Air movements were made visible with smoke and observed and temperatures were logged. Observations and data were then analyzed with regard to indoor climate requirements and temperature efficiency, a quantitative measure of how well supplied heat will benefit a room’s occupied zone. The results indicate, among other things, that a too great reduction in air flow rates can have a negative impact on the air quality in the observed room, even though the amount of air supplied is more than sufficient according to current requirements. Best temperature efficiency is achieved when exhaust air passes through both the exhaust air device and the ventilated windows, indicating that this is the most favorable exhaust air setting for lowering ventilation air flow rates or room supply air temperature. 8	Enhancing the support systems of nurses in accident and emergency units of regional urban hospitals at KwaZulu-Natal Mbokazi, Chinisile Albertina 08 1900 (has links) Introduction: Working in the hospital accident and emergency unit is perceived to be very stressful. However, minimal or no organizational support measures are made available to enable the nurses working in these hospital units to cope effectively with stressful incidents. The purpose of the study was to develop strategies to enhance the support systems of nurses in the accident and emergency unit. Method: The study used a qualitative approach to explore the experiences of and describe the coping strategies used by nurses working in the accident and emergency units of regional urban hospitals at KwaZulu-Natal, in South Africa. Fourteen participants were interviewed in the accident and emergency units of four (4) hospitals. Data were collected through in-depth individual interviews. The data were analysed manually following the steps explained by Creswell (2009:186). Findings: The interviews led to the description of the nurses’ experiences in the accident and emergency units. Four themes emerged from the findings of the interviews, and included working under pressure and stressful conditions, the need for safety and security, limited resources and psychological and emotional problems. Among the problems discussed within the themes, there was anger that was directed by the patients and their relatives to the nurses; absenteeism that contributed to an increase in work pressure; emotional disturbance; lack of physical security; and lack of continuous support from the hospital management. Conclusion: Five strategies were developed to generate a support systems for the nurses whose hospital units formed part of the study and were as follows: develop or strengthen an existing employee assistant program (EAP) specifically for counselling and support of the nurses within the unit; address staff absenteeism through the provision of extra staff through overtime or agency work; enforce attendance of monthly debriefing sessions; strengthen the security system through increasing and evenly distributing the security staff; and give attention to all the problems and provide a quick response. KRATOCHVÍLOVÁ, Martina January 2013 (has links) This work focuses on finding state of preparedness of hospitals with acute inpatient care in the Pardubice region in the case of a direct threat to the Pardubice Regional Hospital, Inc. (hereinafter "PRH") by chemical, biological, radiological or nuclear agents (hereinafter referred to as "CBRN"). The thesis is based on a comparison between the trauma plans that have been established as an indicator of a preparedness of hospitals in case of mass disaster and the legal standard, i.e. Decree No. 101 of 2012 Sb. dealing with the details of the content of trauma plan of the one-day or inpatient medical care and process for its preparation and negotiation, as amended (hereinafter the "Decree 101 of 2012"). The theoretical work describes the applicable laws governing the conditions for the provision of health care, trauma plan content and procedures of the Emergency Service (ES) components in preparation for dealing with emergencies. The next part discusses the characteristics of CBRN agents. Furthermore, the work focuses on defining terrorism and CBRN terrorism. The chapter also includes a brief historical summary of the use of CBRN agents and their threats. The following section describes the Security System of the Czech Republic and its elements. It is crucial to mention its existence in relation to the prevention of emergencies and crisis situations and elimination of their consequences. This part also introduces a selection of documents, which have a direct relationship to the security policy of the country and to this thesis. It may be noted that most of these documents have been prepared, inter alia, on the basis of current CBRN threats. The penultimate section characterizes individual hospitals with acute inpatient care in the Pardubice region and includes also a risk analysis of this region. The analysis is based on the Emergency Plan of Pardubice region. I believe that the knowledge of the possible threats pointed out in the risk analysis is important in a preparation for an emergency response in the relation to an emergency medical care. Analysis of risks and threats, based on the Decree 101 of 2012, is also a part of the trauma hospital plans. The last part of the thesis describes the different types of trauma plans and certain procedures in the process of an activation of these plans. Even though, the theme of the work is focused more on trauma plans of hospitals, in cooperation with ambulance services and regional authorities in addressing the consequences of emergencies, I find it important to pay attention to a trauma plan for emergency medical services, and also the one, which is part of the emergency plan of the region. Based on the practical experience it was decided the readiness of hospitals with acute inpatient care in the Pardubice region in case of a threat PRH is sufficient. Subsequently, a relative comparison of interdependence trauma plans was performed in selected hospitals to evaluate their index of readiness by means of comparative analysis. The overall index IPi scales the state of preparedness of the hospitals. The input parameter?s values for the index evaluation were determined based on the contents of the trauma plans. The concluding chapter presents the results evaluated by the described method. This uncovered that the degree of readiness of the examined hospitals, based on the mentioned parameters, was insufficient in three cases. Only Chrudimska Hospital, Inc. achieved satisfactory results. Based on these results, the specific measures were recommended to be taken the level of preparedness of the hospitals. The findings and the results of this thesis are mainly practical. The most important benefit of this work is the suggested measures for the activation of trauma plans for hospitals with acute inpatient care in the Pardubice region.
10	Aplikace výsledků ošetřovatelského výzkumu v praxi - role vedoucích sester / Application of the Nursing Care Research Results into Practice {--} the Role of Head Nurses NETOLICKÁ, Jana January 2010 (has links) One of the key factors influencing the quality of nursing care practice is the research applied within nursing processes. Therefore it is vital for theory to be closely linked with practice. To perform research and apply its results in practice is one of the conditions which lead to the development of evidence-based practice. Research applied in nursing care is a key strategy resulting into the unification of the human educational and scientific aspects of nursing care. This thesis focuses on the nursing care research and its application in practice together with the role of senior nurses within this process. The aim was to investigate whether and how the nurses are actually involved in nursing care research plus to examine whether ordinary nurses are willing to accept any changes associated with the introduction of the results of nursing care research into nursing practice. Besides this, also to determine whether nurses are encouraged to implement the results of nursing research into nursing practice, to observe nursing care deputies and leading nurses experience in respect to the implementation of the nursing care research results into practice in different types of hospitals plus to determine whether and how nurses introduce the results of nursing care research into practice. The final objective of this thesis was to determine whether the head nurses manage to introduce the results of the nursing care research into their nursing practice. In quantitative survey performed, the research sample consisted of nurses who work in inpatient departments at clinics and regional hospitals - public limited companies of the Central, South, West Bohemia, and Prague. 350 questionnaires were distributed. The research involved 186 questionnaires. Qualitative research data collection involved 12 respondents, out of which there were 3 head sisters employed at clinics and 3 nursing care deputies plus 3 head nurses from regional hospitals - public limited companies. All interviews were entirely anonymous. According to the survey findings, most of the nurses replied that they are not actively involved in the nursing care research. Nurses generally follow published results of nursing care research, but on the other hand, it is obvious, that they follow nursing care research outcomes randomly and without particular focus. Nurses not only that they refuse any changes, but they do not see any benefits of the implementation of research findings into the nursing care practice. The qualitative survey performed met objectives and all questions were answered. Any of the responses can serve for further quantitative research, especially when the results of the implementation of the nursing care research into practice at clinics and regional hospitals are compared.
