Diseño Interior de un Centro de Rehabilitación Física y Psicológica / Interior Design of a Physical and Psychological Rehabilitation Center for Women with Breast Cancer

Esta tesis fue estructurada con el fin de crear una nueva experiencia vivencial que brinde una mejor calidad de vida a mujeres diagnosticadas con cáncer de mama. Por esta razon, se propone romper con el típico concepto de un Centro de Rehabilitación y aportar con el desarrollo de una nueva estrategia de gestion interna de manera que el sistema sea tan flexible como las necesidades de las pacientes en tiempo real. Otro de los propósitos de este proyecto es que pueda ser ejecutado con el apoyo de una ONG y así, de la mano de psicologos especialistas en transtornos por cáncer de mama, expertos en nutricion y rehabilitacion física unir idea junto a especialistas en paisajismo, infraestructura y diseño. De esta manera poder desarrollar un modelo operativo funcional que rehabilite tanto directa como indirectamente a las pacientes desde su ingreso al centro. Para este proyecto, en primer lugar, se realizó un estudio sobre los efectos y consecuencias del cáncer de mama, así como también los posibles tratamientos para el proceso de rehabilitación independientemente de cada diagnostico u estadio. Debido a que en el país no existe un Centro de Rehabilitación Física y Psicológica para Mujeres con Cáncer de Mama, se realizó el análisis de su arquitectura, experiencia y funcionalidad de las instituciones existentes con el mismo objetivo en países como España, Colombia y Argentina. De esta manera se obtuvo la información necesaria para ejecutar un sistema estructurado por cada especialidad propuesta en esta tesis, fusionando el diseño interior y arquitectura con en el rubro de la medicina. En segundo lugar, se analizó la ubicación y perimetros a la redonda de la vivienda elegida donde se desarrollará el proyecto para posteriormente hacer una intervención estructural y seguidamente el diseño de cada ambiente de manera que puedan satisfacer las necesidades de nuestro público objetivo. Finalmente se plantea las conclusiones en relación con los objetivos propuestos en el inicio del proyecto. / This thesis was structured in order to create a new experience that provides a better quality of life for women diagnosed with breast cancer. For this reason, it is proposed to break with the typical concept of a Rehabilitation Center and contribute to the development of a new internal management strategy so that the system is as flexible as the needs of patients in real time. Another purpose of this project is that it can be executed with the support of an NGO and by the hand of specialists in breast cancer disorders specialists, experts in nutrition and physical rehabilitation join idea together with specialists in landscaping, infrastructure and design. In this way to develop a functional operational model that rehabilitates both directly and indirectly the patients from their admission to the center. For this project, firstly, a study was carried out on the effects and consequences of breast cancer, as well as the possible treatments for the rehabilitation process regardless of each diagnosis or stage. Since there is no Physical and Psychological Rehabilitation Center for Women with Breast Cancer in the country, the analysis of their architecture, experience and functionality of existing institutions with the same objective was carried out in countries such as Spain, Colombia and Argentina. In this way, the necessary information was obtained to execute a structured system for each specialty proposed in this thesis, merging interior design and architecture with that of medicine. Secondly, the location and perimeters around the chosen house where the project will be developed were analyzed to subsequently carry out a structural intervention and then the design of each environment so that they can meet the needs of our target audience. Barriers to reintegration experienced by stroke clients post discharge from a rehabilitation center in Malawi

Stroke is a worldwide medical emergency, and the trend in its incidence is rising. Stroke leads to activity limitation and participation restriction among stroke clients. However rehabilitation of stroke disability has been found as lacking emphasis on reintegration issues, resulting into stroke clients facing different barriers to reintegration in the community after discharge. There is limited information about experiences of stroke clients in the communities after discharge from rehabilitation centers in Malawi. The current study was conducted to explore the barriers to reintegration experienced by the clients post discharge from a rehabilitation center in Malawi. A qualitative research was adopted to explore the perceived barriers to reintegration at Malawi Against Physical Disabilities (MAP) Kachere rehabilitation centre in Blantyre. In-depth interviews were utilized to capture data from rerspectives of eight stroke clients, eight caregivers and seven service providers. The data was analyzed by means of content analysis. The findings of this study show key barriers that challenge community reintegration at two levels: individual and environmental. At individual level the barriers include impairments mainly depression and anxiety and physical weakness; and personal characteristics such as denial, gender and comorbidity. At the environmental level stroke clients experience attitudinal barriers by people in different settings of the community, poor terrain, long distance to places of interest, inaccessible structures and poverty. From the findings, therefore, stroke client experience wide range of barriers in both the activity and participation domains of community reintegration. Based on the key findings, recommendations have been made along three broad lines. To decrease barriers to reintegration, service providers could use the findings to consider equally addressing the potential negative contextual factors to reintegration when managing stroke clients, a shift from purely medical model approach to management of stroke. To increase rehabilitation services the government of Malawi could address the problem of limited funding to rehabilitation organizations and put measures to increase number of rehabilitation personnel at community, district and national levels. Further research in the area of community reintegration and development of a model of stroke care and rehabilitation in the country has also been recommended with view to improving stroke care and enhanced community reintegration post stroke in the country. / Magister Scientiae (Physiotherapy) - MSc(Physio)

The aim of this study is to investigate professionals’ working experience with elderly peoplewith substance use disorders and how drug or alcohol abuse affects their work with theirclients. The most important areas of focus are professionals’ views on older clients’ substanceand alcohol abuse, moral or ethical dilemmas in relation to work and the elderly’s needs forsupport and help according to professionals. To identify these problems, a qualitative methodwas used with semi-structured interviews, interviewing a total of 8 professionals in twonursing homes specializing in older clients with substance use disorder and a rehabilitationcenter aimed at ages 40 to 70. Interview data was analyzed with thematization and coding.Analysis of the material was carried out using theories such as care perspectives, ethics forsocial workers and ageism. The results of the study indicate that most professionals whowork with this target group lack the knowledge of geriatrics combined with substance abuseknowledge. A comparison is drawn between the two institutions and their organizationalrules regarding alcohol or drug consumption by clients. The study further highlights thestaff's experience of finding themselves between organizational rules, professional rules andtheir own values ​​and beliefs. The results show that professionals are struggling with the lackof rules. In addition to that the results of this study show that stereotypical perceptions ofolder people tend to influence professionals as well as other social institutions that work withthis target group. The study also addresses the issue of inadequate treatment efforts aimed atolder people and a mapping of their alcohol and drug habits.

La presente tesis realiza el análisis de inseguridad en el Perú ocasionado por jóvenes que cometen el crimen de homicidio, debido a que es un tema de gran relevancia que afecta a la sociedad y la sensación de inseguridad que sienten ellos al transitar por las calles. El objetivo de este estudio es analizar el estado actual de los centros de rehabilitación, y referentes mundiales que han tenido éxito en la reinserción, así como el usuario, para luego diseñar un centro de rehabilitación juvenil que cumpla con las estrategias de diseño necesarias para el desempeño del usuario. Con este fin, la pregunta de investigación es la siguiente: ¿Cómo se plantea el diseño interior de un centro de rehabilitación de medio cerrado (CJDR) para jóvenes homicidas? En este contexto, el diseño es un factor importante, ya que será el factor principal que influenciará en la rehabilitación y posterior reinserción a la sociedad. La pregunta de investigación se responde luego de aplicar estrategias de diseño y formular la programación, organigrama y zonificación, teniendo como resultado una mejor ubicación y distribución de los espacios interiores y de esparcimiento. Teniendo esto en cuenta, se recomienda que las investigaciones futuras tomen en cuenta los puntos mencionados y sirva para mejorar los actuales centros de rehabilitación. Se podrían realizar investigaciones adicionales para identificar otros factores a tener en cuenta a la hora de diseñar para jóvenes que no hayan cometido este crimen, sino delitos menores. / This thesis carries out the analysis of insecurity in Peru caused by young people who commit the crime of homicide, also the current state of juvenile detention centers, what systems they use and what are the methods to achieve reintegration into society, the result obtained from this research will serve to better approach of a juvenile detention center. The objective is to design a juvenile detention center with open spaces and vegetation in Ancon district. Below, the content of the chapters will be briefly explained. In Chapter I, the research problem is described in such a way that such an analysis serves as the basis for subsequently proposing a solution. Chapter II, emphasizes the analysis of research frameworks, for a better analysis of the problem. Chapter III, explains the design strategies that will be taken into account for the design of this new juvenile detention center located in Ancon district. In working with the disabled and/or handicapped person it is necessary to consider all aspects of social functioning if realistic goals are to be reached in the rehabilitation process. It is recognized that if the family can give interest and support to that member of the family who is disabled and/or handicapped, and agrees to participate in rehabilitation planning that the effect will be helpful. The dichotomy of this study sample is made between those patients living alone and those patients living with some member of their family. With the development of the concept of disability and consequently rehabilitation concept, today&amp / #8217 / s achievements of rehabilitation centers are perceived different from the traditional ones. Conventional approach of rehabilitation was aiming at improving functional deficiencies of people with disabilities with the help of medical treatment. Within the last two decades depending upon the increasing in the awareness of idea of inclusivity in society there has been developed a shift from traditional medical based approach to more social based ones where rehabilitation has been perceived as a process to enhance the &amp / #8216 / quality of life&amp / #8217 / rather than a process of a medical curation of people with diverse disabilities. The new social-based rehabilitation approach refers community based rehabilitation strategy that aims to integrate various sectors of social life for the achievement of an effective rehabilitation while promoting inclusion of people with less or severe disabilities in social life. It is stated in this thesis that the architectural program of a community-based rehabilitation centers can be elaborated with the parameters of Universal Design (UD) which not only responds to the shift in community-based rehabilitation approach while promoting inclusion in the society, but also has a potential to advance spacial formative characteristics of related centers in a more descriptive way. The ideas of equity and participation are the significant parameters of UD that are referred in the thesis in order to elaborate the supportive social services of an architectural program and to investigate spatial characteristics of community-based rehabilitation centers.

Predicting the Clinical Outcome in Patients with Traumatic Brain Injury using Clinical Pathway Scores

The Polytrauma/TBI Rehabilitation Center (PRC) of the Veterans Affairs Hospital (VAH) treats patients with Traumatic Brain Injury (TBI). These patients have major motor and cognitive disabilities. Most of the patients stay in the hospital for many months without major improvements. This suggests that patients, family and the VAH could benefit if healthcare provider had a way to better assess or "predict" patients' progression. The individual progress of patients over time is assessed using a pre-defined multi-component performance measure Functional Independence Measures (FIM) at admission and discharge, and a semi-quantitative documentation parameter Clinical Pathway (CP) at weekly intervals. This work uses already de-identified and transformed data to explore developing a clinical outcome predictive model for patients with TBI, as early as possible. The clinical outcome is measured as percentage of recovery using CP scores. The results of this research will allow healthcare providers to improve the current resource management (e.g. staff, equipment, space) through setting goals for each patient, as well as to provide the family more accurate and timely information about the status and needs of the patient.

The subject of the diploma thesis was the elaboration of an architectural study of the renovation and completion of the Secondary school Gemini in Brno on Vaculíkova street. The assignment was the completion of the school and the emergence of new accommodation, restaurant and services. This object is situated in Brno - Lesná. It is a relatively sloping land. The basis of the design was the completion of one school floor and the completion of a new w connected to the existing building. The original school was built parallel to the contour lines, while the completion of the new part of the object is perpendicular to the contour lines. This will create an optically enclosed garden space from Vaculíkova Street. The school's garden remains open to the park towards the Devil's Gorge. On the west side a new building with a restaurant and services is built, which is connected with the original school by a connecting neck.The basis of the design was the completion of one school floor and the completion of a new wing connected to the existing building. The original school was built parallel to the contour lines, while the completion of the new part of the object is perpendicular to the contour lines. This will create an optically enclosed garden space from Vaculíkova Street. The garden of this school is open to the park towards the Čertova rokle. On the west side was built a new objects with a restaurant and services, which is connected with the original school by a connecting neck. The design consists of three buildings with separate functions. There are classrooms, cabinets, sanitary facilities, etc. in the school. A rehabilitation center with facilities was situated in the southern part and restaurant, a dining room with a kitchen, an administrative department of the school and spaces for rent were built in the western building.
