Mindful meditation and mobilization; pulmonary rehabilitation for emphysema patients

BACKGROUND SUMMARY: Pulmonary rehabilitation programs are an important component of the multidisciplinary approach to minimizing the symptomatology of patients with chronic obstructive pulmonary disease. Within the program, patients learn about how to live with their non-curable disease and how to minimize exacerbations. Although patients learn about their disease process, breathing techniques, and exercise, there are no specific components that bridge the mind and body gap to promote mindfulness through the patients’ efforts within the program. LITERATURE REVIEW FINDINGS: This thesis contains a comprehensive literature review composed largely of randomized trials. These trials and studies summarize the framework of pulmonary rehabilitation programs and how yoga is implemented within treatment options for chronic diseases. The literature review highlights that pulmonary rehabilitation programs improve the quality of life in patients with emphysema through patient education on breathing and exercise. However, there is a lack of literature on the use of yoga techniques of breathing and exercise within the framework of pulmonary rehabilitation programs to promote mindfulness when living with a chronic disease. PROPOSED PROJECT: This thesis proposes a randomized controlled study to identify a more mindful approach to a pulmonary rehabilitation program for emphysema patients through the use of timed ujjayi pranayama (mindful breathing) and yoga asanas (poses). CONCLUSIONS: The results will be analyzed to determine if yoga techniques lead to statistically significant improvement in patient outcomes in emphysema patients enrolled in a pulmonary rehabilitation program. SIGNIFICANCE: The compiled data will reveal how yoga breathwork and movement will be beneficial for emphysema patients enrolled within a pulmonary rehabilitation program.

Upplevelser av delaktighet : Patienter med förvärvad hjärnskada som vårdats på en rehabiliteringsavdelning

Inledning: Vid en förvärvad hjärnskada är delaktighet i rehabiliteringen en central del för patientens återanpassning till vardagen och eventuellt tillbakabildande av förlorade funktioner. Delaktighet i vårdförloppet leder till en ökad teoretisk förståelse och praktisk insikt hos patienten. Syfte: Syftet med denna studie var att beskriva hur vuxna personer med förvärvad hjärnskada upplever/har upplevt delaktighet under sin vårdtid på en rehabiliteringsavdelning. Metod: Detta är en kvalitativ studie med beskrivande design med semistrukturerade intervjuer som datainsamlingsmetod. Tio patienter som vårdats minst fem arbetsdagar på en rehabiliteringsavdelning intervjuades gällande frågor om delaktighet under vårdtiden på rehabiliteringsavdelningen. En kvalitativ innehållsanalys inspirerad av Graneheim och Lundman (2012) användes för att analysera insamlad data på latentnivå. Resultat: Utifrån temat ”delaktighet” uppstod tre kategorier: kompetent personal, informerad patient, samt självständighet. Temat ”icke-delaktighet” resulterade i två kategorier: bristfällig information och kontrollförlust. Det var flera av informanternas berättelser som liknade varandra i fråga om delaktighet på vårdavdelningen. Samtliga informanter beskrev att de upplevt sig vara delaktiga någon gång under vårdtiden på rehabiliteringsavdelningen. Det handlade bland annat om att de upplevt att de fått tillräcklig information från personalen angående sjukdom och vårdförlopp, att det fanns kompetent personal som anpassade sig efter patienternas individuella behov och önskemål samt möjlighet till självständighet under exempelvis träning. Ett flertal informanter beskrev även upplevelser av att inte vara delaktiga under vårdtiden. Bristfällig information från personalens sida samt en känsla av kontrollförlust var de huvudsakliga anledningarna till detta. Slutsats: Denna studie kan bidra till en ökad förståelse hos vårdpersonal angående delaktighet under rehabiliteringstiden för patienter med förvärvad hjärnskada. Då vårdtiderna kan vara långa på rehabiliteringsavdelningen är det särskilt viktigt med en god kontakt och bra kommunikation med patienterna för delaktighet och en patientcentrerad vård.

This master´s thesis elaborates a project documentation of sports rehabilitation center in Brno–Bystrc. The building object is four-storeyed and a partial basement. On the basement floor there is a pool with sauna, gym and fitness with support to assist for all activities offered. On the ground floor there is an entrance, reception for accommodation, restaurant and kitchen for the restaurant. The 2nd overground floors is determined by the rehabilitation clinic. On the grand and overground floor are designed sanitary appliances for the restaurant and patient surgeries. There are projected 9 apartments for weekend accommodation. Apartments allow access to the terrace. Foundations are made strip foundations. The building object is bricked and insulated with ventilated facades. Load bearing structure of the roof create roof trusses.

A master thesis „Clinic of holistic medicine“ is processed as a project documentation. The building is projected for the plat no.170/1 in Kladruby u Vlašimy (Benešov district). Is a three-storyed wooden building from large-format wooden panels. Ceilings are ribbed constructions from spruce wood. Stairspaces are made of reinforced concrete. The house is roofed with a saddle roof made by woodentrusses. Stairspaces and machine room of air conditioning are roofed with a one-sheat flat roof. The project is planned as an extension of the Rehabilitation institute complex in Kladruby.
