Investigation of the Pt-Al-Cr system as part of the development of the Pt-Al-Cr-Ru thermodynamic database

The ternary Pt-Al-Cr system was investigated as part of the continued development of a thermodynamic database for the Pt-Al-Cr-Ru system. Scanning electron microscopy with energy dispersive X-ray spectroscopy and X-ray diffraction analyses were used to obtain phase equilibria data. The alloys were studied in the as-cast condition, as well as after annealing at 600°C and 1000°C respectively. A solidification projection was constructed and a liquidus surface derived. Isothermal sections at 600°C and 1000°C were also determined. It was concluded that all phase regions were identified correctly since the results were selfconsistent. Three ternary phases were found and 19 ternary invariant reactions identified. A thermodynamic database was developed for the Pt-Al-Cr system using Thermo-Calc. Phase relations could be reasonably accurately predicted between 600°C and 1000°C, and even up to temperatures close to the melting point. However, the match between the calculated and experimental diagrams could be improved. As with the Pt-Cr-Ru system, problems with the constituting binary systems seemed to be the major cause for problems encountered in the modelling. Only once the Al-Pt and especially the Cr-Pt and Cr-Ru binary phase diagrams are confirmed more rigorously, the calculated ternary phase diagrams could be worked on with more confidence. More than half of the alloys investigated had hardnesses in excess of 600 HV10 regardless of their state of heat treatment. Based on the examination of hardness indentations, alloys in the Pt-Al-Cr system were also often brittle due to the presence of hard intermetallic compounds. Alloys containing ~Pt3Al showed better behaviour with regard to toughness which was encouraging for the Pt-based alloys that are being developed by Mintek.

Effect of microstructure on properties of selected Pt-based alloys

A thesis submitted to the Faculty of Engineering and the Built Environment, University of the Witwatersrand, in fulfilment of the requirements for the degree of Doctor of Philosophy in Engineering / This study investigated the effect of microstructure on properties of selected Pt-based alloys. Six alloys of different compositions were analysed after heat treatment at 1500°C for 18 hours, followed by quenching in water; then annealed at 1100°C for 120 hours and air cooled, equivalent to a potential industrial specification. Microstructural characterisation utilised OM, SEM, AFM, TEM and EDX. Further characterisation was carried out using a nanoindentation hardness tester for nanohardness and elastic modulus measurements. The research focus was to characterize the different morphologies of γ′ ~Pt3Al precipitates during a single heat treatment, and to understand the nano-mechanical properties of the γ′ precipitates and γ (Pt) matrix, taking their proportions into account. In the present work, the samples were successfully etched, (which was not possible before) allowing optical microscopy and SEM to give much clearer microstructures than previously. The precipitate volume fractions were measured from SEM and AFM images, and agreed well. The γ′ volume fraction (expressed as percent) of nominal Pt78:Al11:Cr6:Ru5 (at.%) alloy was 51 ± 6% (SEM) and 57 ± 10% (AFM), while for nominal Pt85:Al7:Cr5:Ru3 (at.%) it was 45 ± 6% (SEM) and 48 ± 8% (AFM). A comparison of the γ′ volume fractions obtained from TEM showed that, compared to SEM, as the γ′ volume fraction observed with SEM increased, the γ′ volume fraction measured in TEM increased, although the TEM volume fraction results are believed to have considerable error due to TEM only revealing the microstructure of relatively small regions compared to SEM. Comparing with Pt-Al-Cr-Ni alternatives with γ′ volume fractions of 51-57%, the nominal Pt78:Al11:Cr6:Ru5 and nominal Pt85:Al7:Cr5:Ru3 (at.%) alloys have comparable γ′ volume fractions within, experimental error, and are considered as promising. From a microstructural viewpoint, these alloys were identified as the most promising. TEM revealed that at the specific heat treatment there were multiple size ranges of γ′ precipitates. The ~Pt3Al precipitate structure was found to be cubic L12, rather than tetragonal. The orientation relationship between the γ matrix and γ′ precipitates was found to be [114]M||[114]P; [001]M||[001]P; [103]M||[103]P. The nano-mechanical properties of the γ matrix and γ′ precipitates of Pt-Al-Cr-Ru alloys were investigated for the first time. At 2.5mN, it was possible to measure mechanical properties inside the individual γ′ precipitates and γ matrix channels, and in all six alloys the γ′ precipitates were the harder phase. The hardness of γ´, γ and the overall alloy was a function of the Pt content, and the hardness of the overall alloy was also a function of the Al content. The overall alloy hardness for nominal Pt85:Al7:Cr5:Ru3 (at.%) was 9.0 ± 0.3GPa and 9.2 ± 0.3GPa for nominal Pt78:Al11:Cr6:Ru5 (at.%). The new findings on image analysis showed that the precipitate volume fractions of nominal Pt78:Al11:Cr6:Ru5, nominal Pt85:Al7:Cr5:Ru3 and nominal Pt78:Al11:Cr8:Ru3 (at.%) were comparable to commercial nickel-based superalloys (NBSAs). TEM has shown that the precipitate morphology was similar to that of NBSAs, while nanoindentation studies indicated that the Pt-Al-Cr-Ru alloys’ overall, γ and γ phase nanohardnesses and elastic moduli were also similar to NBSAs. These results were encouraging, since the NBSAs already have commercial applications. Thus, more research efforts are encouraged on the Pt-Al-Cr-Ru alloys in order to further improve the properties of these alloys.

Comparison of direct and indirect susceptibility test methods for detection of bacteraemic methicillin-resistant Staphylococcus aureus

MMed (Clinical Microbiology), Faculty of Health Sciences, University of the Witwatersrand, 2009 / Introduction: The clinical laboratory is required to rapidly identify Staphylococcus aureus as a cause of bacteraemia, and in particular, to detect methicillin resistance amongst bacteraemic isolates, to facilitate prompt initiation of appropriate therapy which may directly impact on patient survival, and to allow for implementation of appropriate infection control measures. Hence, the laboratory needs to choose tests to detect methicillin-resistant S. aureus (MRSA) bacteraemia which are rapid, accurate, simple, cost-effective and appropriate for the setting. Primary study objective: To determine the accuracy of four phenotypic susceptibility tests to directly detect MRSA from blood culture specimens (BC) compared with detection of the mecA gene by the polymerase chain reaction (PCR) from S. aureus cultured from the same BC. Materials and Methods: BCs were selected from patients with incident, S. aureus bacteraemic episodes at two hospitals, during January and February 2006. S. aureus was identified by standard phenotypic tests, including the presence of a deoxyribonuclease (DNAse). Direct susceptibility tests (DST) were performed (oxacillin (1μg) and cefoxitin (30μg) disk diffusion (DD), oxacillin Etest® (AB bioMérieux) and CHROMagar®-MRSA (CHROMagar® Microbiology)), and repeated on stored cultures. Detection of nuc and mecA genes by PCR confirmed S. aureus and methicillin resistance respectively. The sensitivity and specificity of the DST were calculated with reference to the mecA PCR result, to fulfil the primary study objective. Results: During the two-month study period, 9,400 BC were submitted to the clinical laboratories at the 2 hospitals; S. aureus was isolated from 156 specimens. Of these, 89 BC from 89 incident cases were included in the study, and 65 were subjected to all tests, including PCR. Of the 65 nuc-positive S. aureus isolates from 65 BC, all were positive with the direct DNAse test, and 25 (38%) were mecA positive. Compared to PCR, sensitivity and specificity for the direct oxacillin DD, cefoxitin DD, oxacillin Etest® and CHROMagar®- MRSA was 100% and 90%, 98% and 100%, 100% and 100%, and 96% and 42% respectively. Discussion: In this study, we found that, compared to PCR for the nuc and mecA genes, the combination of a direct DNAse test and oxacillin Etest®, facilitated accurate detection of MRSA bacteraemia. The direct oxacillin Etest® result did not appear to be influenced by a non-standardised inoculum, in contrast to the other direct tests, and provided an oxacillin minimum inhibitory concentration. The direct cefoxitin DD test produced more accurate results than the direct oxacillin DD test, was easier to read and distinguished MRSA from MSSA with zone diameters clustering into more clearly defined susceptibility categories. Although the chromogenic agar performed well when used to identify methicillin resistance amongst cultured S. aureus isolates, it was apparent that this test, read at 24 hours, could not be used reliably as a DST. Since the Etest® is more costly than the DD test; its use should be reserved for BC from patients in “high-risk” hospital areas, e.g. intensive care units. The direct cefoxitin DD could be used for all BC positive for GPCC, and could be used without a direct identification test because of its lower cost; it is further recommended that the direct cefoxitin DD test replace the direct oxacillin test.

The relative importance of human and animal sources of vancomycin-resistant Enterococcus faecium in immunocompromised patients in hospital

Enterococcus faecium is a leading cause of hospital-acquired infection, disproportionally affecting immunocompromised and critically ill patients. Despite infection control measures, rates of vancomycin-resistant E. faecium (VREfm) bacteraemias have failed to decline in the United Kingdom, and Cambridge University Hospitals (CUH) report the highest numbers nationally. The aims of my PhD were to use epidemiological and genomic surveillance data to establish risk factors for acquisition and infection with E. faecium in patients at CUH, and to use a One Health approach to consider possible sources for hospital patients by relating bloodstream-associated isolates with those cultured from livestock and the environment in the same geographic region. A retrospective matched nested case control study was performed to determine risk factors for VRE bacteraemia relating to antibiotic exposure. 235 cases were matched to 220 controls for length of admission, year, specialty and ward type. Multivariable analysis demonstrated that duration of exposure to parenteral vancomycin, fluoroquinolones and meropenem were independently associated with VRE bacteraemia. This provides evidence for the importance of antimicrobial stewardship targeting high-risk antibiotics in patients at risk of VRE bacteraemia. VREfm bacteraemia may be complicated by disease recurrence. Whole genome sequencing was used to distinguish between relapse and reinfection in 14 episodes of recurrent VREfm bacteraemia. This demonstrated that 10 (71%) episodes were due to reinfection with a new strain, with reinfection being more likely with increasing time between two positive cultures. This study also evaluated 9 patients with blood cultures positive for both VREfm and vancomycin-susceptible E. faecium (VSEfm), the majority (78%) of which were found to be unrelated strains. More than half of all study isolates from these two patient groups were closely related to another isolate causing bacteraemia at CUH, suggesting that hospital acquisition of VREfm is a driver for infection and recurrence. A cross-sectional study of E. faecium in raw and treated wastewater from 20 municipal water treatment plants across the East of England revealed widespread dissemination of healthcare-associated lineages of VREfm in all sampled locations including rural areas, and environmental release in treated wastewater in 17/20 locations. Wastewater isolates were genetically intermixed with isolates causing bacteraemia at CUH, including highly related isolates indicating recent transmission between the two reservoirs. These findings are consistent with widespread distribution of healthcare-associated VREfm in community populations. A One Health approach incorporating sampling from livestock (10 pork, 10 cattle, 9 poultry farms) detected no VREfm in animals whilst 2 independent meat surveys demonstrated VREfm in 1-2% of uncooked products. Genomic comparison of >1400 E. faecium isolates from livestock, meat, wastewater and almost 800 people with bloodstream infection demonstrated that livestock and human isolates were genetically distinct. Analysis of the accessory genome added further evidence for distinct gene content associated with niche adaptation. An analysis of mobile genes encoding antibiotic resistance revealed limited evidence of sharing between human and animal populations. A prospective longitudinal study in haematology patients at CUH over 6 months revealed high rates of VREfm carriage (63% of cases) and environmental contamination (49% of samples). Genomic analysis elucidated complex colonisation dynamics with frequent loss and acquisition of subtypes, including unsuspected acquisition of new VREfm subtypes in patients already colonised with VREfm, and multiple transmission chains involving patients and the environment, including some leading to bacteraemia. These findings highlight the shortcomings of infection control and environmental cleaning and provide the basis for revised interventions.

Weight variation over time and its relevance among multidrug-resistant tuberculosis patients

Objectives: We aimed to assess the variation in patient body weight over time according to the treatment outcome among multidrug-resistant tuberculosis (MDR-TB) cases. Methods: This was a retrospective cohort study. The data of patients commencing MDR-TB therapy were analyzed. Data were collected from different public TB treatment facilities located in peri-urban areas to the south of Lima, Peru. The outcome was patient body weight (kilograms) from treatment commencement, measured monthly. A random effects model was fitted using robust standard errors to calculate 95% confidence intervals. Results: Of a total of 1242 TB cases, 243 (19.6%) were MDR-TB. Only 201 cases were included in the analysis; 127 (63.2%) were males and the mean patient age was 33.6 (standard deviation 16.2) years. Weight changes over time among the patients who were cured differed from changes in those who died during therapy (p < 0.001). Weight curve divergence was important at the end of the third, fourth, and fifth treatment months: on average, the weight difference was 2.18 kg (p < 0.001), 3.27 kg (p = 0.007), and 3.58 kg (p = 0.03), respectively, when cured patients were compared to those who died. Conclusions: Our results show that weight variation during treatment can be a useful surrogate for the treatment outcome, specifically death during therapy. MDR-TB patients with weight loss should be followed more closely, as they are at greater risk of death. / Revisión por pares
