The Prevalence of Aspiration Pneumonia in Rest Home Residents with Reduced Cough Reflex Sensitivity

The aim of this study was to determine whether there was an association between a failed test of cough reflex sensitivity and history of chest infection in a general population of rest home residents. One hundred rest home residents from four different levels of care (rest home, hospital, dementia and psycho-geriatric) were recruited and their cough reflex assessed using a solution of 0.6 Mol/L citric acid nebulised and presented via a facemask.Participant’s records were then checked to see if there were any documented episodes of chest infection in the 6 month period prior to cough reflex testing.The results showed that out of 100 participants, 4 failed the cough reflex test. Of the 4 that failed the test, 3 had no documented episodes of chest infections recorded in the 6 month period prior to cough reflex testing. Data was not available for one participant who was deceased by the time of collection of the second data set. As such, there was no direct association demonstrated between a failed cough reflex test and development of chest infection or aspiration pneumonia. The results of the study are unexpected in two ways. Firstly, the relatively low number of participants who failed the cough reflex test is surprising as 72% of the participants for whom a full data set was obtained had neurological conditions that are known predisposing factors for reduced cough reflex sensitivity. Secondly, the finding of no association between a failed cough reflex test and history of recorded chest infection is not consistent with other studies. There is however an established body of research that indicates the causes of aspiration pneumonia are multifactorial and not solely dependent upon aspiration. The characteristics of participants and the implications of the findings are described. The potential use of cough reflex testing as a tool to screen against the risks of silent aspiration in relation to assessment of oro-pharyngeal dysphagia in this frail, elderly population is discussed.

The imagination of care : caregivers’ perspectives on end of life care in rest homes.

This study explores and examines the roles of caregivers within rest home facilities in New Zealand; and specifically, how they unpack and deal with managing the tensions associated with end of life (EOL) care. It adopted a qualitative perspective to enable an exploration of how the experiences of caregivers help guide their practice and understanding of their roles in relation to EOL care. A grounded theory framework was utilised in order to explore how caregivers make meaning and find value in the roles they perform within rest home facilities. This study offers insights into some of the tensions that caregivers face in a working environment where life and death occur simultaneously. It was apparent that caregivers are aware of the many social taboos they must negotiate in order to conduct their work. It was also apparent that they understood that others might perceive their work as dirty but that this did not prevent them from finding job satisfaction and create meaning in their work. Instead, they created meaning by using the conceptual tool the imagination of care. This involved utilising their imagination and past experiences to enable EOL care which represented the dying rites of individual residents but also fitted within their ethic of care.

Retomando os documentos do asilo desde 1889 com o 1º Estatuto do Asylo de Campinas, as publicações no jornal O Diário de Campinas, o Estatuto do Lar dos Velhinhos de Campinas e o folder sobre o asilo, são analisados processos de legitimação, dissimulação e naturalização de sentidos para o sujeito idoso no discurso social e em instituições. A relação tempo-espaço assume papel importante na configuração dos sujeitos inseridos no asilo. O tempo presente e sem expectativas; o passado, rememoração; o futuro, ausência de expectativas. No contexto asilar, o tempo cronológico deixa de sê-lo, expondo-se a historicidade dos sentidos. A forma de referir e designar mostra a produção de sentidos diferentes para o mesmo referente, seja a instituição ou seus integrantes; a multiplicidade dos sentidos sócio-históricos contrasta com a homogeneidade designada. Se de um lado temos a sociedade burguesa produzindo as normas e construindo seus espaços urbanos de identificação, de outro, o oprimido busca sua sobrevivência submetido as normas e designações. No passado, entendia-se que o "cidadão carente", entre os quais se incluía o idoso, devia receber auxilio e assistência, ao passo que o grupo dominante avaliava sua possibilidade de pertencimento. No momento histórico atual, passa a ser preocupação integrar as diversas gerações, procurando diminuir os preconceitos com relação a idade cronológica e a vida de aposentado. Substitui-se a conotação negativa do envelhecer por uma valorização do estar ativo. Estigmatiza-se a velhice dos que passaram a ser os verdadeiros velhos - os inativos e os dependentes -, como um problema social e, assim, o sujeito asilado ganha notoriedade / Abstract: This thesis deals with the social senses of aging and their repercussion in the rest home Lar dos Velhinhos de Campinas, located in the city of Campinas, State of Sao Paulo, Brazil. Studying documents like the 1st Estatuto do Asylo de Campinas, texts from the newspaper O Diario de Campinas, the Estatuto do Lar dos Velhinhos de Campinas and the folder presenting the rest home, we analyze processes of legitimation, dissimulation and naturalization of the senses of the aged subject in the social discourse. The relation timespace assumes an important role to the subjects living in the rest home. The present time has no expectations; the past is remembering and the future, lack of expectations. In the context of the rest home, the chronological time is affected by the historicity of senses. The ways to refer e to designate show the production of different senses to the same referent, which is the institution and its members; a multiplicity of social historical senses contrasts with the designated homogeneity. If, on the one hand, we have a bourgeois society producing the norms and constructing its urban spaces of identification, on the other hand the oppressed try to survive the norms and designations. In the past, it was understood that "the needy", which included the aged, should receive help and assistance, and the dominant class decided those who could enjoy citizenship. In the present historical moment, it is defended that the generations must be integrated and the prejudice in relation to the chronological age and the retired, banned. The negative senses of aging are changed to a valorization of activity. Assim à necessÃrio que adapte-se a uma nova rotina repleta de normas e regras. à preciso, agora, apÃs passar toda a sua vida convivendo com pessoas conhecidas, com quem mantinha laÃos de amizade e consangÃinidade, ter que aprender a conviver com pessoas totalmente desconhecidas, deixando para trÃs seu estilo de vida pessoal e de viver seu cotidiano. Este estudo tem como objetivo geral: compreender como os idosos (re)constroem o seu cotidiano dentro de uma instituiÃÃo asilar, e especÃficos: 1) Identificar as motivaÃÃes que levaram os idosos ao asilo; 2) Descrever o processo de adaptaÃÃo ao contexto asilar elos idosos; 3) Explicar como os idosos se percebem e organizam as atividades cotidianas.; e 4) Explicar a dinÃmica das relaÃÃes interpessoais dos idosos no asilo. Este estudo teve como referencial teÃrico conceito de processo adaptativo do Modelo de AdaptaÃÃo da teÃrica de enfermagem Callista Roy. O caminho metodolÃgico escolhido foi a estratÃgia de pesquisa Estudo de Caso, jà que esta pesquisa adentrou no universo natural da idosa institucionalizada e teve uma abordagem compreensiva. O estudo foi realizado em um asilo da cidade de Fortaleza-Ce que possui caracterÃsticas filantrÃpicas e à mantida por uma ordem de religiosas. A instituiÃÃo selecionada tem cerca de 60 idosas residentes, todas do sexo feminino. A amostra foi composta por saturaÃÃo dos resultados sendo composta por 9 idosas. A unidade de anÃlise desta pesquisa foi a (re) construÃÃo do cotidiano da idosa na instituiÃÃo asilar selecionada e utilizou um protocolo de estudo de caso com o intuito de aumentar a confiabilidade da pesquisa. Para a coleta de dados utilizou-se a observaÃÃo direta e a entrevista baseada no roteiro de histÃria de vida. As observaÃÃes foram escritas em diÃrio de campo e as entrevistas foram gravadas em gravador digital sendo transcritas na Ãntegra, posteriormente codificadas e, em seguida analisadas. Como tÃcnica para organizaÃÃo e anÃlise dos dados utilizou-se da tÃcnica de AnÃlise de Discurso que visa a compreensÃo de como um objeto simbÃlico produz sentidos, como ele està investindo de significÃncia para e por sujeitos. Este estudo foi submetido e aprovado à ComissÃo de Ãtica em Pesquisa do Complexo Hospitalar da UFC. As categorias, apÃs a anÃlise do sentido das falas foram:1) Vinda para o asilo â principais motivaÃÃes:favorecimento de prÃticas religiosas, evitar a solidÃo, influencia de outras pessoas, acesso a recursos de saÃde e exclusÃo familiar; 2) Processo adaptativo: senti-se produtiva, sentimentos de perda/ enfrentamento da realidade e conhecimento prÃvio do asilo; 3) Auto percepÃÃo do idoso no contexto asilar: pertenÃa e autonomia, e satisfaÃÃo/ insatisfaÃÃo das necessidades humanas; e 5) DinÃmica das relaÃÃes: respeito. Com este estudo percebe-se que a (re)construÃÃo do cotidiano do idoso no contexto asilar à uma atividade complexa que necessita de um esforÃos tanto do idoso como da instituiÃÃo que o acolhe, sendo necessÃrio estar atento ao processo adaptativo e Ãs necessidades humanas do idoso. / When entering a rest home, many elders face one of its main features: the totalitarian nature of these institutions. Thus, it is necessary for them to adapt themselves to a new routine full of rules. Now they need, after spending their whole life living with people to whom theyâve had blood relation and friendship ties, to learn to live with completely unknown people, leaving their own lifestyle behind and not living their everyday lives anymore. This study has as its main aim: to understand how the elderly (re)build their everyday lives in a rest home, and as specific aims: 1) Know the reasons that took the elders to the rest home; 2) Investigate the process of adaptation to this new context; 3) Elucidate how the elders see themselves and how they organize their everyday life; and 4) Analyze the dynamics of the interpersonal relationships among the seniors in the rest home. This study was based on Callista Royâs Adaptation Model theory of the adaptive process. The methodology chosen was the Case Study, since the research involved the natural universe of elder women living in a rest home, and had a comprehensive approach. The study happened in a philanthropic rest home in the city of Fortaleza-Ce, maintained by a religious order. The selected institution houses around 60 elders, all women. The sample was made selected resulting 9 elderly women at the end. The analytical component of this research was the (re)building of elderly womenâs everyday life in the selected rest home. Case study was chosen in order to increase research reliability. Data was collected using both observations and interviews based on their life record. The observation was written in a diary and the interviews were recorded with a digital audio recorder, which were wholly transcribed, encoded, and analysed. For data organization and analysis, Discourse Analysis technique was used, which aims to understand how a symbolic element produces meanings, and how it is full of meanings to and for someone. This study was submitted and approved by the Ethics in Research Committee of the Hospital Complex at UFC. The categories, after the meanings in the speech were analysed, were: 1) Going to the rest home â main reasons: favoring religious practice, avoiding loneliness, other peopleâs influence, access to health care and family exclusion; 2) Adaptive process: feeling productive, feeling of losing/facing reality and previous knowledge of the rest home; 3) Self-awareness of the seniors in the rest home context: taking part and autonomy, and satisfaction and dissatisfaction of human needs; and 5) Relationships dynamics: respect. Through this study, it is realized that the (re)building of the elderlyâs everyday life inside the rest home context is a complex activity which requires efforts from both elderly themselves and from the rest home, thus, it is necessary to be attentive to the adaptive process and the elderlyâs human needs.

My graduation thesis focuses on training memory with senior citizens. The main aim of this work is to define methods, techniques and benefits of memory training for senior citizens. The first aim was to investigate if there was some memory training (i.e. which forms were applied, how often, what kind of education the persons conducting this form of activation had achieved) in rest homes in Strakonice District. The second aim was to find out how beneficial the memory training was from the point of view of senior people as well as from the point of view of workers carrying out memory training in old people's homes. The research was done in the Rest Home for Seniors in Blatná, in the Home of Restful Old Age of St. Anna in Sousedovice, in the Home for Seniors in Strakonice - Jezárky, in the Home for Seniors in Strakonice - Lidická and In the Home for Seniors in Vodňany. To get the necessary information I have chosen a semi-controlled interview, i.e. qualitative research. The research group consisted of 32 seniors participating in memory training courses and the other one of 7 memory trainers conducting memory training sessions in the homes for seniors in Strakonice District. The results of the research have been processed in the form of a case study.

The chapter titled Present state focuses on the problem of gerontology and geriatrics. Other chapters include description of the most outstanding neuropsychical and somatic changes in the old-age. Next chapters deal with functional examine and geriatric rehabilitation problem.The objective of the thesis was assessing the extent of self-reliance and psychical changes affecting all the occupants of using Crichton Geriatric Rating Scale.146 seniors were tested according to the Crichton Geriatric Rating Scale.Three hypotheses were stated. It was expected that the results of the Crichton tests would correlate with the results of MiniMental Test. Another hypothesis was that there would be better results of the test done to the people whose family care for them actively. The last hypothesis expected worse results of the people who have been in the retirement home for more than three years.The research has confirmed that the results of the Chrichton test and MiniMental test correlate. Correlation of the Crichton test with the active care and interest of the family for the senior and the length of their stay was not proved.
