Guidelines for the training of ancillary healthcare workers in home-based care: patients' perspective

M.Cur. / The objectives of the research were to explore and describe the healthcare needs of long-term, home-based physically disabled persons and then to develop guidelines for the training of Ancillary Health Care Workers (AHCWs) to meet these needs. The guidelines were developed specifically from the patients‟ perspective and were derived from the two research questions: “What are your healthcare needs?” and “How can these be met?”. A contextual, qualitative, exploratory and descriptive research design was utilised. The population consisted of physically disabled people, aged 18 years or older, living in the northern suburbs of Johannesburg who employed an AHCW to assist with the longterm, home-based care. Purposive sampling was used with subsequent snowballing to identify further participants for the study. Interviews with open-ended questions were conducted with all the participants during which they could express their views freely. Tesch‟s method of data analysis was used to identify categories, sub-categories and themes. The model of Lincoln and Guba (1985:301f) was used to ensure trustworthiness and ethical considerations were maintained throughout the process. Informed consent was given by the patients. Three categories and sub-categories of healthcare needs were identified: physical healthcare needs, interpersonal relationship needs and social needs. Twelve themes were derived from these categories, notably i. Awareness of the unique physical environment needs of the physically disabled person, including injury prevention ii. Management of hygiene and elimination iii. Assistance with nutrition and hydration iv. Assistance with mobilisation v. Administration of medication vi. Required training of the AHCW iv vii. Personality traits of the AHCW viii. Cultural tolerance ix. Maintenance of privacy and dignity x. Communication skills xi. Companionship xii. Promotion of independence of the patient . From the themes twelve guidelines were developed for the training of AHCWs in home-based care looking after physically disabled patients. The research‟s limitations were described and recommendations relating to practice, nursing education and further research were made.

Trata o presente trabalho de um estudo sociológico que tem por objeto a Saúde Pública abordada através de um de seus principais pontos de estrangulamento: o poliqueixoso. São levadas em consideração as condições de saúde/doenca desses doentes na situação de atores sociais culturalmente envolvidos pelas regras determinadas tanto pelas Políticas de saúde, quanto pela rotinização dos seus hábitos. A compreensão ampla desse processo visa evitar o reducionismo dos fatos, buscando compreender a relação existente entre os sujeitos adoecidos e a realidade sócio-histórica que os cerca, configurando seu cotidiano, imiginário e sistema de representação sociais. Na condição de usuários dos Serviços de saúde, os poliqueixosos aqui referidos são identificados a partir da especlalidade Cardiologla, ocasião em que apontam sintomas não clinicamente diagnosticados ou organicamente confirmados, o que, no entanto, não os afasta do continuado processo de busca pela saúde. Na medida em que rotinizam suas ações em nome do alcance de tratamentos eficazes e de cura, ritualizam seus corpos e as doenças que neles se instalam, edificando imaginários e identidades, enquanto constroem \"metáforas\" típicas ao seu estado adoecido, numa narrativa também adoecida, tomadas neste trabalho como fundamento do discurso sobre saúdc, doença, tratamento, cura, articuladas segundo as regras da contradição, da desesperança e da alienação. The ample comprehension of this process aims to avoid the minimization of the facts, seeking to understand the existing relation between sickened people (or individuais) and the social and historical sorrounding reality, confiauring their everyday and imaginary life and all systems of social presentations. As users of all healthy services the \"poliqueixoso\" here described are identified, beginning from heart specialists, when and where they point out the symptoms not clinically contihuous process of seeking for health. Physical illness and depression: changes over time in middle aged and elderly persons

Recent research in gerontology and geriatrics has identified that one factor repeatedly associated with depression in the elderly is the presence of physical illness. The increasing numbers of elderly persons in the population and their high rate of chronic physical illness make it important to identify critical disease and individual characteristics that play a role in the association of depression and physical illness. In order to investigate and clarify these relationships and concerns, a panel survey of 133 middle aged and elderly persons with recent exacerbations of various physical illnesses was completed. Subjects were referred by medical offices and agencies in the Portland metropolitan area. Two in-depth interviews were completed approximately three months apart. The data were analyzed using descriptive statistics, correlational analysis, analysis of variance, multiple regression, and dynamic (change focused) correlational analysis. Results indicated a slight but consistent decrease in level of depression with increasing age. Increased levels of income, social support, religiosity, subjective health, internal locus of control for health, and life expectancy were associated with decreased levels of depression. Conversely, increased levels of pain, physical dependency, progressiveness of the disease, death anxiety, external locus of control for health, and worry about medical resources were associated with increased levels of depression. While the residential setting of urban, suburban, or rural had a significant effect on income and a slight effect on size of support system, it had no significant impact on level of depression. The four best predictors of level of depression at Time 1 were subjective health, pain, death anxiety, and income. Analysis of change over time revealed moderate stability in levels of depression. The best predictor of level of depression at Time 2 was level at Time 1. Only change in pain added significantly to the prediction of depression at Time 2. The results of this study will help to identify physically ill middle aged and elderly persons at risk for the development of depression. Analysis of change over time suggests causal relationships for further investigation.
