Využití hodnotících a měřících nástrojů pro hodnocení bolesti v chirurgii / The use of evalution and measurement tools for assessment of pain in surgery

Pain can be a warning sign, but in surgery it is associated mainly with medical procedures. The aim of the research survey was to map out the pain rating scales that are used in practice in surgical patients, to determine the opinion of surgical nurses on these scales and patient experience. Furthermore, the paperwas aimed at the creation of a recommended procedure for evaluation of pain in surgical patients. In the empirical part of the thesis, quantitative and qualitative research was used. In the quantitative research, two questionnaires were created - for nurses (a non-standardized questionnaire) and for surgical patients (a combination of a non-standardized and standardized questionnaire). The focus group method with surgical patients and nurses was used as a qualitative method. The research survey shows that the most commonly used scale in practice is the visual analogue scale and verbal evaluation. Although these methods are widely used, our results suggest that they are not entirely sufficient. Respondents from the groups of nurses and patients wanted to add more accurate verbal description to the evaluation and increase the frequency of pain evaluation by a doctor and nurse. The most frequent symptoms of pain from the perspective of patients included: limited mobility and self-sufficiency, emotional lability, and verbal manifestations. And from the perspective of nurses: limited mobility, non-cooperation, limited self-sufficiency and verbal manifestations. Among the methods that are used most often by patientsto relieve painare medicinesprescribed by the doctor, relief positions, sleep and rest. More than 90% of patients are satisfied with the cooperation with the nurse and the doctor in the evaluation and treatment of pain. Based on the data analysis, a procedure for assessing pain in surgical patients has been proposed. Its clarity and usability were verified using the focus group method with surgical nurses and patients. Both the nurses and the patients assessed the recommended procedure quite positively. Possible pitfalls were perceived in the necessity of sufficient nurse-patient cooperation and enough time to implement the proposed assessment. The research survey was carried out in the territory of the South Bohemian region and is processed as part of thesolution of the GAJU team grant project - Use of Measuring Instruments in Nursing Practice (059/2018/S).

Efeito da escala e do modelo de análise na avaliação genética de escores visuais : estudo de simulação /

Resumo: Várias características são avaliadas por meio de escores visuais nos programas de avaliação genética no Brasil. Os sistemas de atribuição dos escores variam quanto ao número de classes de notas, ao referencial adotado para atribuição das notas e à frequência das notas nas diferentes classes e ao modelo empregado na análise dos dados. O objetivo neste estudo foi avaliar o impacto de diferentes escalas de atribuição dos escores, modelos de análise dos dados e sua interação na identificação dos genótipos superiores para essas características, por meio de dados simulados. Foram simulados dados de uma característica contínua na escala subjacente, transformados em valores (1 a 6) na escala discreta observada, com diferentes escalas (relativa uniforme, relativa normal, relativa assimétrica e fixa), dois valores de herdabilidade (0,25 e 0,49) e duas variâncias de grupos de contemporâneos (0,25 e 1,00). Estimaram-se os valores genéticos utilizando modelos de limiar, linear e linear após transformação dos dados. Análises com modelo de limiar e notas com escala relativa normal apresentaram melhores acurácias para a predição dos valores genéticos. A utilização do modelo de limiar incrementa a acurácia das estimativas, mas em razão da demanda computacional e de problemas de implementação, o modelo linear pode ser empregado na análise de dados categóricos avaliados sob escala relativa normal com seis classes. A transformação de dados proposta foi eficiente em algumas situações, podendo ser considerada como alternativa ao modelo linear, dependendo da estrutura dos dados / Abstract: Genetic improvement programs in Brazil involve the monitoring of many traits that are evaluated by means of visual scores. The systems used to evaluate animals vary in terms of the number of classes, the referential from which the scores are given, the distribution of observations in score classes and the model used to analyze the data. The objective of this study was to evaluate the impact of different scales of score evaluation, models of analysis and their interactions on the identification of superior genotypes for visual scores, by means of a simulation study. Data of a continuous trait were simulated in a subjacent scale and then transformed into a discrete trait (1 to 6) in the observable scale, with different scale distributions (uniform relative, normal relative, asymmetric relative and fixed), two heritabilities (0.25 and 0.49) and two contemporary group variances (0.25 and 1.00). Breeding values were estimated using threshold model, linear model and linear model after data transformation. The threshold model and the relative normal scale showed better accuracy for the prediction of breeding values. The use of a threshold model can lead to an increase in the estimates' accuracy, but despite the computational demand and implementation issues, the linear model can be used to analyze discrete data assigned under a normal relative scale with six score classes. Reducing the Search Space of Ontology Alignment Using Clustering Techniques

With the emerging amount of information available in the internet, how to make full use of this information becomes an urgent issue. One of the solutions is using ontology alignment to aggregate different sources of information in order to get comprehensive and complete information. Scalability is a problem regarding the ontology alignment and it can be settled down by reducing the search space of mapping suggestions. In this paper we propose an automated procedure mainly using different clustering techniques to prune the search space. The main focus of this paper is to evaluate different clustering related techniques to be applied in our system. K-means, Chameleon and Birch have been studied and evaluated, every parameter in these clustering algorithms is studied by doing experiments separately, in order to find the best clustering setting to the ontology clustering problem. Four different similarity assignment methods are researched and analyzed as well. Tfidf vectors and cosine similarity are used to identify the similar clusters in the two ontologies, experiments about threshold of cosine similarity are made to get the most suitable value. Our system successfully builds an automated procedure to generate reduced search space for ontology alignment, on one hand, the result shows that it reduces twenty to ninety times of comparisons that the ontology alignment was supposed to make, the precision goes up as well. On the other hand, it only needs one to two minutes of execution time, meanwhile the recall and f-score only drop down a little bit. The trade- off is acceptable for the ontology alignment system which will take tens of minutes to generate the ontology alignment of the same ontology set. As a result, the large scale ontology alignment becomes more computable and feasible.

Title: Possibilities of functional Evaluation of Patients after acute cerebral Palsy in Czech Republic Objectives: The main objective of this study was to describe the problems of functional assessment of patiens after stroke. Another objektive was to determine and compare the properties ofthe most commonly used functional measures designed for stroke,in our country. Methods: This work is theoretical - research work, which is the theoretical part of the period from September 2012 to June 2013 when retrieving data from literature sources using advanced targeted keywords and thein hotkey in databases based on EBM. The practical part of the study consisted of a questionnaire, which was using the questionnaire survey gathered data needed to evaluace the clinical utility, in the period from May 2013 to July 2013. After obtaining the necessary data from both theoretical and practical part of this data was evaluated and analyzed according to the standard metodology for the evaluation of functional measures for spinal patiens (Chapman, J.R. et al, 2007) and the results of the individual measures were mutually compared.
