Abstract The unstable policies of both pubic health and payment by National Health Insurance¡]NHI¡^in Taiwan cause great difficulties in the management of clinics since the separation of dispensing practice from medical practice since 1997.In order to survival from the tough circumstance, the managers of clinics must adapt different strategies to increasing income, enlarging size of organization, or gaining the competition advantage. One of the most important strategies is the institution of clinic pharmacies by themselves or cooperating with another pharmacist. Right now the theses about the clinic pharmacies was few. But the name, so called ¡¨ doorway drugstore ¡§, which was considered to against the spirit of the policies of separation of dispensing practice from medical practice and global budget is called by some scholars and the NHI. In order to reducing the payment for the clinic pharmacies, the NIH is planning to set up 3 criteria for further management of the ¡§doorway drugstores¡¨. And if the new rules come true in the future, the managers of clinic pharmacies and the owners of clinics will meet more challenges. In this study, main focus would be the 6 clinic pharmacies at Kaohsiung city and by the means of face to face interview with the managers of these drugstores in order to gathering the primary data about the motivation for selling OTC merchandise, and the situations of merchandise profile, pricing strategy, promotion activity and the situations of human resource etc. The study result revealed that there are two clinic pharmacies have no selling of OTC merchandise because of small space, lack of salesman, or worry about the conflict between the doctor and the pharmacist. The percentage of the OTC income at another four clinic pharmacies is below the level of 20%, and the reasons of difficulties for selling these products were too expensive, not famous, and lack of in job training for salesmen. Further analysis of the needs of the main consumers , whom were the patients of the clinics, of these drugstores were the treatments for their disease and the maintenance of their health. So the health food or function food will play the import roles of OTC merchandise profile. The reasons of success of selling OTC products in the clinic pharmacies were choosing proper merchandies, which were good for these patients` health or some help for control their disease, and were recommoned by the doctor, and with reasonable price. The weakness of the management of clinic pharmacies were lack of salesmen and few investment of I.T. equipments, lack of promotion activity and in job training. According the primary data from the interview with the managers of clinical pharmacies and biotech functional food suppliers, there are some important conclusions from this study¡G¡]1¡^clinical pharmacies now are selling some OTC merchandise and functional food¡]2¡^clinical pharmacies provide the service for the patients whom came from not only one clinic¡]3¡^the owners of the clinical pharmacies would be the physician of clinic or the pharmacist and the ownership status could be sole or partnership¡]4¡^the percentage of income from selling OTC merchandise is still low in these pharmacies¡]5¡^the managers of clinical pharmacies wish to joint chain pharmacy in the future ,but right now they are not urgent ¡]6¡^the major reason of joint chain pharmacy is cost down and the clinical pharmacies managers will share the management data and use the logo of chain pharmacy, but there are tremendous disagreements between them about the franchising fee¡]7¡^clinical pharmacies had the potential of transforming to become retail channels of biotech functional food because of patients may need them to keep their health and they trust their physician and pharmacist, and the successful selling for biotech functional foods need professional explain ,which can provided by the doctor and pharmacist both are available in clinical pharmacies.

Studies On Selective Adsorption Of Aqueous Glucose Or Fructose On Various Cationic Forms Of Zeolite Y

The equilibria of adsorption on calcium and hydrogen forms of zeolite Y by equimolar solutions of 12.5 %, 20%, 25%, 30% and 35% w/v of mixtures of glucose, G and fructose, F / also the non-equimolar mixtures of 20% w/v glucose - 30% w/v fructose, 30% w/v glucose - 20% w/v fructose, 25% w/v glucose &ndash / 35% w/v fructose, and 35% w/v glucose-25% w/v fructose solutions, which were prepared 24 hours in advance at the experimental temperature, have been studied batch wise at 50&ordm / C. Glucose adsorption, in solutions that had adsorption differences, was fast on both zeolites, on the contrary of slow adsorption of fructose with the stable dynamics. Both adsorptions had small amounts of adsorption changes after minute 30. The treatments made under the same conditions with the same mixtures showed Ca-Y zeolite had better separation capacity compared to H-Y zeolite. Some trials were repeated with CaCl2 added to the solutions. The slowed down affection of fructose adsorption in spite of the small change of glucose adsorption led to better separation. Samples were analyzed by classical methods, not HPLC. All the data were considered with various models and their convergence numbers were tested for their closeness to reality. The models were analyzed by response surface methodology and some of those models had correlation factors as high as 88% at the equilibrium points at 30th minutes. Besides, time dependent models have been considering the lag times with a time dependent variable included all the data of all treated solutions with correlation as high as 79.5%.

Development Of Sled Range Test Facility For Store Separation Model

The aim of this thesis is to develop a test facility in order to validate the safe separation of a store model from the aircraft model with the analysis of the acquired data. Different alternatives are considered in the conceptual design and a facility is selected which contains the rail constructed 5 meters above the ground. 300 meter test range is used to accelerate the aircraft model, attached to the carriage with solid rocket engines. The concept is to accelerate the carriage within the first 100 meters, to perform the separation of the model at the desired velocity when the rocket engine is burned out, and in the last 100 meters, to decelerate the carriage in order to recover the models. After the parts of the carriage are designed with respect to the requirements of the separation and completing the similitude study of scaled models with the ratio of 1/8, separation tests are performed. These tests are constrained with 0.9 Mach due to the store and test aircraft limitations. Doppler radar is issued to measure the velocity and photogrammetry method is used to validate and analyze the separation and to obtain the vertical trajectory of the store model. Measured trajectory of the store model is compared with the trajectory of the full scale store.

DISCUSSIONS ON PRESCRIPTION TRANSPARENCY AND PATIENTS' SATISFACTION--BASED ON STUDY OF OUTPATIENTS IN KAOHSIUNG METROPOLOIS

Medicine cures and kills people. In some advanced nations, prescription transparency is achieved through the separation of prescribing and dispensing, which further leads to public examination of prescriptions. Such an operation results in a restriction mechanism to prevent misuse and abuse of drugs and to guarantee the safety of consumers. Taiwan initiated the separation of prescribing and dispensing on March 1, 1997; however, most people do not recognize the significance of the system and conclude the issue to be an interest dispute between doctors and pharmacists. In the separation of prescribing and dispensing, disputes over the right of dispensation between doctors and pharmacists are commonly seen, but literatures are rarely found to review from consumers' position. This study plans to investigate consumers' viewpoints in respect of prescription transparency and understand its effect on satisfaction. Consequently, this study would be the benchmark for the government and health care sectors in promoting the separation of prescribing and dispensing. The study found the followings: 1. The discrepancy between satisfaction and attention to drug information is 0.88. Most people are used to nontransparent prescriptions, so such non-transparency does not cause significant dissatisfaction. 2. Among the people interviewed, the prescription transparency service they receive is 100% in academic medical centers, 66% in hospitals, and 23% in clinics. 3. 52.3% of the interviewees pays strong attention to drug information, 31.8% pays attention, 11.4% pays little attention, 2.8% pays very little attention and 1.7% do not pay attention at all. 4. The results show the diversity of demand to prescription transparency. Public displays different recognition to the importance of the list of medications: 82.5% thinks listing medication titles for easy reference. 81.9% considers the list represents a sense of responsibility from doctors and their prescriptions. 65.0% keeps the list for later use or for resolving medical disputes. 35.6% uses the list to enrich knowledge in medication. And 1.1% considers the list insignificant in all aspects. 5. Under the condition of non-transparent prescriptions, the educational level will influence the newly-developed factor in satisfaction. In other words, highly educated people tend to care more about prescription transparency

Hingeless flow control over an airfoil via distributed actuation

An experimental investigation was undertaken to test the effectiveness of a novel design for controlling the aerodynamics of an airfoil. A synthetic jet actuator (SJA) was placed inside a NACA 0015 airfoil with its jet at 12.5% of the chord length, hereby referred to as the leading edge actuator. Four centrifugal fans across the span were mounted at 70% of the chord and the jet formed by them was located at 99% of the chord, hereby referred to as the trailing edge actuator. The effects of these actuators on the aerodynamic properties were studied, separately and then in conjunction, with varying angles of attack. The leading edge actuator delays the onset of stall up to 24 degrees, the maximum angle of attack that could be attained. The control of the aerodynamics was achieved by controlling the amount of separated region. There was no effect of the actuation at lower angles of attack. The trailing edge actuator provides aerodynamic control at both low and high angles of attack. The study investigated the effect of jet momentum coefficient on the aerodynamic properties for various angles of attack. The data obtained shows that lift control (in both positive and negative direction) was achieved even at low angles. The actuator enhances the aerodynamic properties by changing the pressure distribution as well as by delaying flow separation. Study of the combined actuation shows that the synthetic jet actuator was very effective in delaying stall when the trailing edge jet was ejected from the upper surface. For the case when the jet is ejected from the lower surface, there is less control. This can be accounted for by the difference in aerodynamic loading for both cases.

Microfabricated continuous flow separation and manipulation systems for human whole blood

The objective of the research in this dissertation is to develop microsystem based separation technologies for whole cell cancer analysis using human whole blood as the input sample. This research work is carried out with two different approaches; one based on a miniaturized cascade magnetophoresis system and a second based on dielectrophoresis. The miniaturized systems can be fabricated using MEMS technologies combined with plastic fabrication techniques. The design, fabrication, packaging, and characterization of several versions of the magnetophoresis and dielectrophoresis microsystems for whole cell cancer analysis in human whole blood sample are presented. The developed magnetophoresis systems have demonstrated improved throughput in the removal of RBC from a human whole blood sample and its application to the separation of tagged cancer cells based on their surface expression level of a specific protein. The dielectrophoresis microsystem has successfully shown the ability to steer a blood stream between two outlets and to separate WBCs or cancer cells from a human whole blood sample. The developed microsystem based separation technologies can be further applied to the development of integrated system for cancer detection and treatments.

Removal of the fermentation inhibitor, furfural, using activated carbon in cellulosic -ethanol production

Commercial activated carbon and newly polymer-derived carbon were utilized to selectively remove the model fermentation inhibitor, furfural, from water solution during bio-ethanol production. Morphology, pore structure and surface chemistry of the sorbents were characterized. The oxygen groups on the carbon surface were believed to have contributed to the decrease on the selectivity of activated carbon between furfural and sugars (Sugars are the valuable source of bio-ethanol production and should not be separated from solution). Oxidization of activated carbon by nitric acid generated more information which supports the above assumption. Different adsorption isotherm models and kinetic models were studied to fit commercial activated carbon and polymer-derived carbon individually. Bacterial cell growth, sugar consumption, and ethanol yield during the fermentation were investigated after inhibitors were selectively removed from the broth. The fermentation time was reduced from one week to one day after inhibitor removal. Different methods of sorbent regeneration were investigated, including thermal regeneration, pH adjustment and organic solvent stripping. Low ethanol-containing water solution appears to be the most cost-effective way to regenerate the spent sorbent in the industrial application. A sorption/desorption cycle was designed and the sorbents were regenerated in a fixed-bed column system using ethanol-containing liquid from fermentation. The results were stable after running 20 times of sorption/desorption cycle.
