Microbiologia do meato médio em crianças com rinossinusite crônica

A rinossinusite crônica é uma doença complexa, com etiologia multifatorial. O seu manejo é controverso, principalmente no paciente pediátrico. O tratamento preconizado é a antibioticoterapia empírica, embora o papel da infecção bacteriana não esteja completamente definido na patogênese da rinossinusite crônica. A técnica de coleta de secreção considerada padrão áureo é a punção do seio maxilar via fossa canina. Entretanto, provoca dor, necessita da colaboração do paciente, pode causar dano ao nervo infraorbitário e em crianças, pode lesionar brotos dentários. Além disso, principalmente em pacientes pediátricos, requer sedação ou anestesia geral e identifica apenas germes restritos ao seio maxilar. A visualização do meato médio, considerado área-chave da doença rinossinusal, através de endoscópios, permite a coleta de amostras com desconforto mínimo e a execução de culturas e testes de sensibilidade. Este estudo visou determinar a microbiologia do meato médio por meio de coleta sob visão endoscópica em crianças com rinossinusite crônica bem como testar a sensibilidade dos germes identificados. Foram incluídos na pesquisa 39 pacientes pediátricos com sinais e sintomas de rinossinusite por mais de 3 meses, cujas endoscopias nasais evidenciaram secreção no meato médio e com tomografia computadorizada demonstrando comprometimento dos seios paranasais. Um grupo de 94 adultos com rinossinusite crônica (RSC) foi estudado paralelamente. Os germes mais freqüentemente encontrados no grupo das crianças foram o Streptococcus pneumoniae (33%), a Moraxella catharralis (23%) e oHaemophilus influenzae (21%). O Streptococcus pneumoniae foi resistente a sulfametoxazol-trimetropim em 23% e à penicilina em 17% das amostras. Cepas produtoras de beta-lactamase foram encontradas em 67% e 12,5% das amostras de Moraxella catharralis e Haemophilus influenzae, respectivamente. Nos adultos, o Staphylococcus aureus e o Staphylococcus coagulasenegativo foram os germes mais freqüentes, e apresentaram 83 % e 89 % de resistência à penicilina, respectivamente. Bactérias Gram-negativas e Streptococcus pneumoniae foram estatisticamente mais freqüentes no grupo das crianças. Haemophilus influenzae e Staphylococcus coagulase negativo foram os germes que apresentaram diferença estatisticamente significativa em relação à resistência antimicrobiana entre crianças e adultos, sendo mais resistentes nestes últimos. / Chronic rhinosinusitis is a complex disease and its etiology multifactorial. Its management is controversal, especially in the pediatric population. Although bacterial infection is not totally defined in its pathogenesis the recomended treatment is still empiric antibiotics. The gold standard technique to obtain secretion for culture is piercing the maxillary sinus through the canine fossa. It needs patient´s colaboration, may damage the infraorbitary nerve and, in children, may hurt the teeth buds. In children it may also require sedation or general anesthesia, and it can only help identify microorganisms restricted to the maxillary sinuses. Visualization through the middle meatus, considered the key area for the rhinosinusal disease, through endoscopy, permits the collection of samples for culture, with minimum discomfort. This study tried to determine the microbiology of the middle meatus through collection of samples for culture and sensitivity testing under direct endoscopic vision in children with chronic rhinusinusitis. Thirty nine children and ninety four adults with signs and symptoms attributable to rhinosinusitis for longer than three months were included in this study. All patients had nasal endoscopy with evidence of secretion in the middle meatus and computed tomography showing envolvment of the paranasal sinuses. The microorganisms most frequently encountered among children were: Streptococcus pneumoniae 33%, Moraxella catharralis 23%, Haemophilus influenzae 21%. Streptococcus pneumoniae was resistant tosulfametoxazol-trimetropim in 23%, and to penicillin in 17% of the samples. Beta-lactamase producing organisms were found in 67% of Moraxella catharralis and 12,5% of the Haemophilus influenzae samples. In adults, Staphylococcus aureus and Staphylococcus coagulase negative were the most frequently found bacteria and were resistant to penicillin in 83% and 89% respectively. Gram-negative bacteria and Streptococcus pneumoniae were statistically most frequent in the children group. Haemophilus influenzae and Staphylococcus coagulase negative showed a statistically significant diference regarding resistance to antimicrobials, between children and adults, being more resistant in the latter.

Objetivo: Comparar a presença de fungos na mucosa dos seios paranasais de pacientes com e sem rinossinusite crônica. Material e Métodos: Através de um estudo Tranversal, onde o desfecho foi a rinossinusite crônica e o fator em estudo a cultura para fungos, avaliou-se um amostra composta de 47 pacientes com rinossinusite crônica e um grupo controle com 28 pacientes. O material removido cirurgicamente dos seios paranasais foi avaliado através de exame direto e cultural. Resultados: Dos 47 pacientes com rinossinusite crônica, 8 (17,1%) apresentaram cultura positiva para fungo. Houve diferença estatisticamente significativa em relação a positividade das culturas entre o grupo com rinossinusite crônica e o grupo controle (p=0,002) e o grupo com rinossinusite crônica sem polipose nasossinusal e o grupo controle (p=0,004). Não houve, entretanto, diferença entre o grupo com rinossinusite crônica com polipose nasossinusal e o grupo controle (p=0,227). O gênero Aspergillus foi o fungo mais encontrado nas culturas positivas. Não houve diferença estatisticamente significativa entre a positividade das culturas e a presença de asma, intolerância a àcido acetilsalicílico, hiposmia e eosinofilia. Conclusões: Apesar de apenas 17,1% das culturas apresentarem positividade para fungo, os mesmos foram mais freqüentes nos pacientes com rinossinusite crônica. Isto evidencia, que, em alguns pacientes, possa existir associação entre fungo e rinossinusite crônica. / Objective: To compare the presence of fungi in the sinus mucosa of patients with and without chronic rhinosinusitis. Study design and setting: Cross-sectional study using conventional culture to detect fungi in the sinus mucosa. Surgical sinus mucosa samples were collected from 47 patients with chronic rhinosinusitis and 28 control subjects. Results: Fungi were detected in 17,1% of subjects with chronic rhinosinusitis and in none of the control subjects using conventional culture (p=0,002). There was statistically significant difference in the prevalence of fungi in the normal volunteers and patients with chronic rhinosinusitis without polyposis (p=0,004). There was no statistically significant difference in the prevalence of fungi in the normal volunteers and patients with chronic rhinosinusitis with polyposis (p=0,227). This study showed a predominance of Aspergillus sp from sinus mucosa sampling.There was no statistically significant difference in the prevalence of asthma, eosinophilia, acetilsalicilic intolerance and hiposmia and positive culture. Conclusion: Although 17,1% of the culture was positive for fungi, the same was more frequent in the patient with chronic rhinosinusitis. Fungi is probably important in the pathogenesis of chronic rhinosinusitis.
