Alcohol use during pregnancy is common and its consequences often result in a broad range of
negative, lifelong developmental outcomes. This study describes the effects of prenatal alcohol
exposure and interacting socio-demographic factors on early childhood development. One
hundred and twenty one children from the Northern Cape, South Africa, were clinically
examined using standard diagnostic procedures and assessed using the Griffiths Mental
Development Scales (GMDS/ER) at 7-12 months (Time 1) and 5 years of age (Time 2).
Participants were assigned to either: a Fetal Alcohol Syndrome (FAS/Partial Fetal Alcohol
Syndrome (PFAS); a Prenatal Alcohol Exposed (PAE); or a Control group based on the
diagnosis at 5 years. Mothers/caregivers were interviewed to ascertain socio-demographic
information, including prenatal alcohol exposure. During infancy, the FAS/PFAS group showed
significantly lower gross motor and language abilities, with delays in higher-order executive
functioning becoming more apparent with age. No significant differences were noted during
infancy between the PAE and Control groups over any developmental subscales. However, with
age, higher-order executive function delays were reported in the PAE group. Performance on the
infant and child versions of the GMDS was not significantly correlated, suggesting that the tests
may be measuring different developmental constructs. Lower maternal education, unemployment
and later recognition of pregnancy were associated with reduced social adaptive functioning, and
language and eye hand coordination abilities, irrespective of amount of prenatal alcohol exposure
over both time points. Larger anthropometric birth measurements and longer duration of
breastfeeding were significantly related to increased performance on the GMDS at 5 years within
the groups exposed to prenatal alcohol. Socio–demographic variables are likely to complicate
developmental profiles for all three groups, with prenatal and postnatal nutrition emerging as
possible protective factors for positive developmental outcomes at 5 years of age.

Sabe-se que o PC é relacionado a condições ambientais e de saúde materna, e sua avaliação adquiriu grande importância após a recente epidemia de infecção pelo vírus Zika no Brasil e em outras partes do mundo, que se associou ao aumento da incidência de microcefalia. Objetivo: Avaliar a associação do perímetro cefálico de recém-nascidos com características sociodemográficas e hábitos maternos durante a gestação em duas coortes de nascimentos de Ribeirão Preto, SP, em 1978/79 e 2010. Método: foram analisados dados referentes a dois estudos de coortes de nascimento realizados em Ribeirão Preto, SP, um em 1978/79 e o outro em 2010. Foram incluídos recém-nascidos vivos de parto único hospitalar que tiveram a informação do PC ao nascer. Resultados: Na coorte de 1978/79 participaram 2941 recém-nascidos e em 2010 participaram 7353. A média geral do PC em 1978/79 foi de 34,10 cm, um pouco menor do que a de 2010, de 34,29 cm. Nasceram mais meninos em 1978/79 e mais meninas em 2010. Excetuando a RCIU, as demais variáveis apresentaram mudanças na frequência nesse intervalo de tempo. A escolaridade materna aumentou, a proporção de mães adolescentes ficou estável, em torno de 12% e a de mães >=35 anos aumentou de 8,3% para 12,5%. Tabagismo materno na gestação se reduziu de 28,3% para 11,8% e a taxa de cesáreas mais que dobrou, de 27,5% para 58,6%. Todas as variáveis estudadas se associaram com o PC na análise não ajustada, exceto paridade em 1978/79. Simple and multiple linear regression analyzes were performed to test the association of the response variable \"head circumference\" with the independent variables in each year, using the statistical package SAS 9.3. Results: In the 1978/79 cohort, 2941 newborns participated and 7353 participated in 2010. The general average of the HC in 1978/79 was 34.10 cm, slightly lower than that of 2010, 34.29 cm. More boys were born in 1978/79 and more girls in 2010. Except for IUGR, the other variables showed changes in the frequency in that time interval. Maternal education increased, the proportion of adolescent mothers remained stable at around 12% and that of mothers >=35 years increased from 8.3% to 12.5%. The IUGR was the factor that most impacted, and these children had a HC reduced by 1.30 cm (95% CI -1.43; -1.18) in 1978/79 and 1.74 cm (CI 95% -1.87; -1.61) in 2010 when compared to those born without restriction. Conclusion: Maternal characteristics during gestation, although they have improved over the years, still need attention. Many women still lack information and prenatal care. Efforts should be made for early detection and intervention to reduce the rates and effects of IUGR.

Background: Breast and Gynecological cancers are a major public health problem. Smoking is associated with several chronic diseases including cancer. Other lifestyles of public health predispose many people to dyslipidemia, hypertension and obesity; risk factors for metabolic syndrome, and are associated with cancer.
Objectives: The purpose of this study is to find if those who smoke, and have the metabolic syndrome, are more likely to have breast or gynecological cancers, and to find the distribution by education, having health insurance, race/ethnicity and socio-economic status.
Methods: A case-control study of females aged 20 years and above who participated in the United States National Health and Nutrition Examination Survey (NHANES) 2001-2010.
Results: Females who have smoked more than hundred cigarettes in life and still smoke; a) have a 42 percent less chance of having a breast cancer diagnosis (OR 0.58; 95% CI 0.36 – 0.93, p-value 0.025), and b) are 2.67 times as likely to report a cervical cancer diagnosis as females who have smoked less than hundred cigarettes in life (OR 2.67; 95% CI 1.72 – 4.13, p-value
Conclusion: Smoking and metabolic syndrome are very important indicators of reproductive health and needs further study. Smoking cessation interventions should be an integral part of cervical cancer prevention programs especially targeted at younger females and females who live below the federal poverty level.

Community participation for people living with spinal cord injury in the Tshwane Metropolitan areaMothabeng, Joyce Diphale 21 July 2011 (has links)
Background and Purpose: People living with SCI (PLWSCI) have to cope with various challenges when they return home after institutionalized rehabilitation, especially with integrating back into and participating in their communities. To date no study has been conducted in South Africa to empirically measure community integration or to evaluate factors affecting the community integration of PLWSCI. The purpose of the study was to investigate factors influencing the community participation of PLWSCI after rehabilitation. Methodology: A cross-sectional, analytical research design employing both qualitative and quantitative approaches was used. Data collection: Phase one: Participants were identified from the databases of two rehabilitation centers, and the snow balling technique. Data were collected by implementing: the socio-demographic and injury profile (SDIP), the Return to Normal Living Index (RNLI), the Spinal Cord Injury Measure – version II (SCIM II) and the Craig Hospital Inventory of Environmental Factors – short form (CHIEF-SF). Phase two: In-depth face to face interviews were conducted with a purposely selected sample group from participants of phase one of the study to determine how participants perceive their community participation. Data Analysis: Phase one: Data were analyzed using version 17 of the Statistical Package for the Social Sciences (SPSS 17). Descriptive statistics, T- tests, Pearson productmoment correlation coefficients and one way analysis of variance (ANOVA), with Bonferroni adjustments for multiple comparisons, was done to examine demographic characteristics and participants’ community participation. Data from Phase 2 was subjected to data-reducing procedures using qualitative techniques. Results – Phase One One hundred and sixty PLWSCI (134 males and 26 females) from the Tshwane metropolitan area participated in this phase of the study. The participants were predominantly young, male, unemployed and single and their major cause of SCI was road traffic accidents, which accounted for 71% of the injuries. The participants’ satisfaction with their community participation was generally low, only 20% expressed satisfaction with their community participation. Satisfaction with community participation was significantly associated with the participants’ race, level of education, employment, educational qualifications, years of living with SCI, level of SCI, health complications, perceived health status, functional ability and perceived environmental factors such as physical (structural and geographic) barriers and lack of transport. Results – Phase 2 Fifteen PLWSCI participated in interviews. Two themes influencing participation were identified from the interview transcripts: Personal factors (coping skills, rehabilitation experience, future aspirations, personal needs, psycho-emotional issues and meaningful use of time) and Environmental factors (attitudes of others, social support and accessibility issues). Conclusion The results of the two phases revealed that community participation of PLWSCI was mainly related to three major categories of factors: personal factors, disability-related factors and environmental factors. Satisfaction with community participation was greater in participants who had been living with SCI for longer periods, had more years of basic education, were not black Africans, lived in suburbs, and were employed. A positive outlook on life and engagement in creative activities during free time enhanced community participation. Disability-related factors included level of SCI, functional ability and perceived general health influenced satisfaction with community participation. Community participation was greater in participants who experienced fewer environmental barriers. “Attitudes of members of society”, “accessibility of the environment” and “social support” influenced the participants’ satisfaction with community participation. A framework for facilitating community participation of PLWSCI was developed. This study examined the association between different socio-demographic factors and food insecurity in the Central Florida Communities of Maitland, Winter Park, and Eatonville. Data from the Institute for Social and Behavioral Sciences at the University of Central Florida were utilized to analyze 3 main questions: In which community is food insecurity more prevalent? To what extent are food consumption, transportation, poverty, and unemployment associated with food insecurity? Does the association between food consumption, transportation, poverty, unemployment, and food insecurity remain when controlling for self-reported overall health, education, marital status, and race. The results revealed differences in predictors of food insecurities. Particularly, there was a positive relationship between food consumption and the knowledge of recommended number of servings of fruits and vegetables suggesting that those who have nutritional knowledge practice healthy dietary behaviors. Furthermore, structural dysfunctions and affordability pose food consumption limitations on the communities studied (mainly Eatonville).

Relations of Depression, Social Support, and Socio-Demographic Factors on Health Behaviors of Mothers with Premature Infants Hospitalized in a Neonatal Intensive Care Unit (NICU)Kanotra, Surbhi 10 December 2010 (has links)
The present study examined the relationships of depression, social support, and socio-demographic factors on health behaviors of mothers with preterm infants hospitalized in the neonatal intensive care unit (NICU). In addition, the study also assessed the moderation effect of social support on the relationship between depression and health behaviors. Eighty-nine mothers with hospitalized infants in the central Richmond area participated in the study. Analyses found that mother’s education level and her marital status to be significantly associated with her health behaviors. Mothers with a higher level of education and those who were married, were less likely to smoke and more likely to incorporate high fiber foods in their diet. In addition, the more support a mother perceived from family and friends, the less likely she was to smoke cigarettes. Neither depression nor social support from the father was significantly related to health behaviors. This study did not find social support to be a moderator between depression and health behaviors. However, the study found direct effects of socio-demographic factors and certain types of support on health behaviors. Assessing a mother’s personal and interpersonal factors will inform clinicians of possible areas of interventions for mothers during the postpartum period.

There is increasing recognition of the relationship between depression and obesity in the pediatric population and recently, there has been a focus on inflammation as a potential link. Both conditions are considered to be pro-inflammatory states, and certain inflammatory markers are linked to depression in obese adults and vice versa. Leptin has also been implicated in depression as a potential mediator between inflammation and depression. Brain derived neurotrophic factor (BDNF), which is associated with depression and obesity, is influenced by inflammation and leptin in animal models as well.
Few studies have examined the interactions between depression, adiposity, and biological markers in obese youth and therefore, our objective was to explore the determinants of depression in obese youth in a clinical setting. We studied 244 youth aged 8-17 years (125 girls, 119 boys) at the time of entry to a weight management program, as part of a prospective, longitudinal study. The CES-DC depression-screening tool was used to assess depressive symptoms, and a participant was classified as having high depressive symptoms if the CES-DC score ≥15 or taking antidepressants. Questionnaires assessed socio-demographic factors and puberty while adiposity was measured using dual-energy X-ray absorptiometry (DXA). Inflammatory markers (IL-6, TNFα, CRP, IL-10), leptin, and BDNF were quantified by immunoassays.
Of the 244 participants, 8 were on antidepressants and 88 (36.4%) met the criteria for high depressive symptoms. We confirmed previous findings that household income and body fat were important determinants of depressive symptoms. However for the first time, it was identified that leptin levels predicted CES-DC score independent of body fat. Neither inflammatory markers nor BDNF were significantly related to depression scores. Our findings suggest that leptin may mediate the relationship of adiposity and depression but it is uncertain if this is related to direct action or to the phenomenon of leptin resistance. / Thesis / Master of Science (MSc) / Obesity has a significant impact on depression in children and adolescents. Inflammation – the body’s response to injury – is measured through markers in the blood and leptin – the marker of body fat – have shown to be related to depression. Research indicates that depression influences these factors to act on obesity. However, research on the interactions of biological and socio-demographic factors with depression in youth with obesity is lacking. Therefore, our objective was to explore the impact of these factors on depression in obese youth entering into a weight management program. Using a depression-screening tool, we studied 244 youth under 18 years and confirmed that household income and body fat were important factors of depression. However for the first time, we found leptin influenced depression regardless of the amount of fat present suggesting that depression acts on obesity through leptin but it is uncertain how this occurs and further research is warranted.

Objetivo: determinar la relación entre los factores sociodemográficos con el nivel de conocimientos de dengue en personas que asisten al centro de salud de Chongoyape, 2022. Método: Se realizó una investigación cuantitativa de diseño correlacional, transversal. La población, fueron 500 sujetos atendidos el mes de Mayo en el centro de salud de Chongoyape, la muestra se estableció en 194 personas, muestreadas por conveniencia. Los datos se recolectaron mediante el cuestionario denominado “conocimientos acerca del dengue: documento que fue adaptado y validado con (V de Aiken =1; p= 0.004); y la confiabilidad con Kuder-Richadrson (KR20 (0.70). Los datos fueron procesados y analizados con el programa SPSS versión 27. Se consideró el principio de anonimato y confiabilidad, trato justo y de participación voluntaria e informada. Resultados: Los entrevistados mayormente fueron mujeres adultas, con nivel de instrucción de secundaria, cuentan con trabajos independientes, reportaron almacenar agua y haber observado criaderos de mosquitos
en la vivienda; el 73,7% se calificaron con nivel de conocimiento alto y se encontró relación significativa entre el nivel de conocimientos y el nivel educativo (<0,001).
Conclusión: La investigación realizada demuestra que la población estudiada obtuvo un alto nivel de conocimiento sobre el tema de dengue, el cual nos permite aprovechar el beneficio al capacitar agentes comunitarios, los cuales guíen a la población para identificación del vector y sus múltiples síntomas durante la epidemia actual sobre dengue que pone en riesgo la salud de la población del norte. / Introduction: Dengue is a metaxenic disease, which affects all people without distinction, causing symptoms of fever, headache, retro-ocular pain and joint muscle. In the Chongoyape District, a large number of people became infected with this virus, probably due to poor preventive practices and ignorance of the disease. Objective: to determine the relationship between sociodemographic factors with the level of knowledge of dengue in people who attend the Chongoyape health center, 2022. Results: The interviewees were mostly Adult women, with a high school level of education, have independent jobs, reported storing water and having observed mosquito breeding sites in their homes; 73.7% qualified themselves with a high level of knowledge and a significant relationship was found between the level of knowledge and the educational level (<0.001).Conclusion: The research carried out shows that the population studied obtained a high level of knowledge on the subject of dengue, which allows us to take advantage of the benefit by training community agents, who guide the
population to Vector identification and its multiple symptoms during the current dengue epidemic that puts the health of the northern population at risk.
