Method verification for homocysteine and a sustainability study on glucose, homocysteine and lactate in different sampling tubes

The pre-analytical phase is known for being the most important step in the laboratory process to reach reliable test results. If handling, transport or preparation of the sample is performed incorrectly the results can deviate from the true value. Today, sampling tubes contains various additives to stabilize concentration levels. The aim of this study was to test a new sampling tube containing fluoride/citrate for glucose, lactate and homocysteine. It was also of interest to evaluate the stability of those three analytes in lithium-heparin, sodium-fluoride/potassium oxalate and fluoride/citrate tubes. To perform the sustainability study, a method verification was done for homocysteine in plasma. The study was performed in a hospital laboratory on the routine instrument Roche Cobas 6000 analyzer. Blood was drawn from 20 patients and was analyzed at the hospital laboratory in Gävle. The blood samples were transported frozen to the laboratory in Hudiksvall and were used in the method verification. For the sustainability study, blood was drawn from 10 healthy volunteers in lithium-heparin, sodium-fluoride/potassium oxalate and fluoride/citrate tubes. The method verification was approved. The results showed that glucose was stable for up to 72 hours in Vacuette Glycaemia tube with fluoride/citrate and this tube also gave more accurate results. Lactate and homocysteine were also stable in fluoride/citrate, but needs further studies. All three analytes were more stable if the sample tubes were centrifuged as soon as possible after blood collection. Fluoride/citrate tubes were stable without centrifugation directly.

Recentemente a terapia com o uso de laser de baixa intensidade foi introduzida como uma nova modalidade de tratamento e tem proporcionado resultados benéficos. Foi realizado um estudo clínico controlado, randômico, duplo-cego, de boca dividida com o objetivo de comparar a efetividade do uso do verniz tópico Fluoreto de sódio 5%, do Laser GaAlAs 780 nm e da combinação do Fluoreto de sódio 5% e o Laser GaAlAs 780 nm para o tratamento de hipersensibilidade dentinária. Os Grupos foram divididos em Grupo Controle (C), Grupo Fluoreto de sódio 5% (F), Grupo Laser GaAlAs 780 nm (LLLT), e o Grupo Fluoreto de sódio 5% + Laser GaAlAs 780 nm (F+LLLT). A aplicação do verniz foi realizada com pinceis do tipo "microbrush", e para cada elemento dental cinco pontos receberam a LLLT, com dose de 52,5 J/cm2 (70mW e 30 segundos) ou 2,1 J por ponto; 6 segundos por ponto. A hipersensibilidade primária foi registrada pela escala analógica visual (VAS: 0-10) para quantificar a dor subjetiva após estímulo com "spray de gelo" no baseline, e após o tratamento nos intervalos de 24 horas e 30 dias. Recently the low level laser therapy has been introduced as a new treatment modality and has provided beneficial results. A split-mout, double-blind, randomized, controlled clinical trial was conducted with the aim of compare the effectiveness of the topical varnish 5% Sodium Fluoride, Laser GaAlAs 780 nm and the combination of 5% Sodium Fluoride and the Laser GaAlAs 780 nm for the treatment of dentin hipersensitivity. The groups were divided in Control Group (C), Sodium Fluoride at 5% Group (F), Laser Group GaAlAs 780 nm (LLLT), and Sodium Fluoride at 5% Group + GaAlAs Laser 780 nm (F + LLLT). The application of the varnish was realized with "microbrush", and for each dental element, five points was received LLLT. The dose was 52.5 J / cm2 (70mW and 30 seconds) or 2.1 J per point, 6 seconds per point. Crystallization and dissolution of electrolyte salts / Κρυστάλλωση και διάλυση αλάτων ηλεκτρολυτών

Η κρυστάλλωση και η διάλυση αλάτων αποτελούν σημαντικές διεργασίες οι οποίες συνεισφέρουν στην φθορά των μνημείων της πολιτιστικής μας κληρονομιάς, τα οποία έχουν κατά κύριο λόγο κατασκευασθεί από δομικά υλικά όπως ο ασβεστόλιθoς και το μάρμαρο. Η κρυστάλλωση ευδιάλυτων αλάτων (π.χ. θειϊκό νάτριο, θειϊκό μαγνήσιο, χλωριούχο νάτριο) σε πορώδη υλικά έχει καταστροφικές επιπτώσεις τόσο στις ιστορικές όσο και στις σύγχρονες κατασκευές από σκυρόδεμα. Το πιο κατεστρεπτικό άλας για την ακεραιότητα των κατασκευών έχει αποδειχθεί, ότι είναι το θειίκό νάτριο. Η κατανόηση του μηχανισμού κρυστάλλωσης του άλατος αυτού είναι απαραίτητη προϋπόθεση για τον περιορισμό ή τον έλεγχο του σχηματισμού του σε ατμοσφαιρικές συνθήκες. Για τον σκοπό αυτό, έγινε συστηματική μελέτη της κρυσταλλικής ανάπτυξης του δεκαένυδρου θειϊκού νατρίου (Μιραμπιλίτης) σε υπέρκορα διαλύματά του. Αναπτύχθηκε μεθοδολογία βασισμένη στον εξώθερμο χαρακτήρα της κρυσταλλικής ανάπτυξης του μιραμπιλίτη. Η πειραματική μελέτη περιορίσθηκε στην ετερογενή κρυσταλλική ανάπτυξη τόσο σε φύτρα Μιραμπιλίτη, όσο και σε ξένα υποστρώματα. Τα υποστρώματα τα οποία μελετήθηκαν περιλάμβαναν ασβεστόλιθο από την Γρανάδα (ασβεστιτικός κυρίως) καθώς και ψαμμόλιθο (Πράγα, Τσεχίας) πυριτικής κατά κύριο λόγο σύστασης. Η μελέτη της κινητικής της κρυσταλλικής ανάπτυξης του Μιραμπιλίτη, έδειξε ότι το καθορίζον την ταχύτητα στάδιο είναι η διάχυση των δομικών μονάδων στην επιφάνεια των κρυσταλλικών φύτρων του Μιραμπιλίτη. Το συμπέρασμα αυτό οδήγησε στην δοκιμή οργανοφωσφορικών ενώσεων, ως προς την επίδρασή τους στην κινητική της κρυσταλλικής ανάπτυξης του μιραμπιλίτη. Οι ενώσεις αυτές ιονίζονται και αλληλεπιδρούν αποτελεσματικά με την κρυσταλλική επιφάνεια δηλητηριάζοντας τα ενεργά κέντρα κρυστάλλωσης. Ο βαθμός ιονισμού βρέθηκε ότι είναι καθοριστικός για την ανασταλτική τους δράση. Πειράματα ταχείας καταβύθισης ευδιάλυτων αλάτων ηλεκτρολυτών σε ασβεστολιθικά και ψαμμιτικά δοκίμια τα οποία έγιναν τόσο με εμβάπτιση, όσο και με έκθεση σε θάλαμο αλατονέφωσης επιβεβαίωσαν τα αποτελέσματα της μελέτης της κινητικής της κρυστάλλωσης του Μιραμπιλίτη και της σχετικής αποτελεσματικότητος των αναστολέων που χρησιμοποιήθηκαν. Πλην της κρυσταλλικής ανάπτυξης ευδιάλυτων αλάτων, σημαντική συνεισφορά στην αποδόμηση των δομικών υλικών παίζει και η διάλυση του ανθρακικού ασβεστίου, το οποίο και αποτελεί τη βασική συστατική τους ένωση. Για τη μελέτη της διεργασίας της κρυσταλλικής ανάπτυξης και διάλυσης του ανθρακικού ασβεστίου τόσο απουσία όσο και παρουσία ανιόντων όπως τα θειϊκά και τα ανιόντα φθορίου χρησιμοποιήθηκε η μικροσκοπία ατομικής δύναμης,η οποία έδωσε την δυνατότητα in situ μέτρησης του ρυθμού κρυσταλλικής ανάπτυξης και διάλυσης σε συνθήκες σταθερού κορεσμού. Η παρουσία θειϊκών ανιόντων έδειξε ότι η κρυσταλλική ανάπτυξη του ανθρακικού ασβεστίου αναστέλλεται ενώ η παρουσία φθορίου επιταχύνει τη διάλυση. Τα αποτελέσματα των μετρήσεων της κινητικής των διεργασιών έδειξαν ότι οι επιμολύνσεις στα υπέρκορα διαλύματα δρούν στην κινητική λόγω προσρόφησης και δέσμευσης των ενεργών κέντρων κρυσταλλικής ανάπτυξης διάλυσης. / The research made in the present PhD Thesis entitled “Crystallization and dissolution study of electrolyte salts”. The objective of the dissertation concerned both crystallization and dissolution processes of soluble sodium sulfate and insoluble calcium carbonate electrolyte salts approached by different experimental aspects but directly connected to environmental and scientific issues. Salt crystallization is an important cause for the weathering and damage of historical and cultural heritage artifacts and ornamental stones. Soluble salts (i.e. sodium sulfate, magnesium sulfate, sodium chloride) have also a damaging and fateful impact on civil engineering structures of roads and building foundations. Most damaging salt for such built frameworks is proven to be sodium sulfate salt. Only when we have a better understanding of sodium sulfate crystallization in an unconstrained fluid medium can we progress to a study of sodium sulfate crystallization in porous building materials. At the beginning of this study batch crystallization experiments of sodium sulfate salt were conducted. Additionally potential organic inhibitors (i.e. organophosphonate, polyacrylates) were tested in the same batch experimental system. Popular and applicable limestone building material is of uniform composition consisting mainly calcium (calcitic material). Granada’s calcarenite has been selected as one of the target material to study. This natural stone is representative of the building material utilized in construction material monuments and susceptible to salt crystallization. In situ AFM experiments of both calcite crystal growth and dissolution were performed in the presence of different electrolyte solutions (i.e. sodium sulfate and sodium fluoride). Nanoscale phenomena during the growth of solid solutions on calcite surfaces were performed in the presence of sodium sulfate electrolyte solutions. Further molecular-scale surface processes during both growth and dissolution of calcite in the presence of sodium fluoride electrolyte solutions were carried out. In all cases significant kinetic data and reaction mechanisms were extracted. Accelerated degradation of Granada’s limestone and Czech sandstone experiments were executed. Tested materials were exposed and impregnated in concentrated solutions of soluble salts (i.e. sodium sulfate, magnesium sulfate and sodium chloride). Immersed limestone type material showed susceptibility to sodium sulfate and sandstone type material to sodium chloride salt solution. Different pre-treatments of limestone specimens with organophosphonate compounds resulted in limiting material damage from sodium sulfate influence. The use of such organic compounds may direct towards a potential implication of conserving building frameworks. Finally suspended limestone rods were subjected to sodium sulfate spray chamber. Pre-treatment of limestone rods with organophosphonate compounds were completed. Again applications of organophosphonate compounds to exposure of limestone material in salt spray chamber may work towards a case of preventing porous material from salt damage and protect building stones.

O objetivo geral deste trabalho foi avaliar géis com flúor para remineralização dentária de uso tópico profissional quanto às características físico-químicas especificadas nos registros concedidos pela ANVISA. Foi verificada a existência de quinze (15) registros válidos na ANVISA, referentes a dezesseis (16) produtos de gel com flúor de uso tópico odontológico. Quanto a estes produtos, são oito (8) géis de flúor neutro e oito (8) géis de flúor acidulado. Todos os produtos possuem em sua composição fluoreto de sódio e o polímero mais utilizado para formação do gel é hidroxietil celulose, conforme declarado nos processos de registro na ANVISA. Em 60% dos processos de registro válidos há informação sobre o pH dos produtos. Com o levantamento realizado em março de 2014 nas dentais do Distrito Federal (DF), verificou-se a comercialização de dois produtos de um mesmo fabricante, sendo um gel de flúor neutro e um gel de flúor acidulado. Para a caracterização físico-química, foram adquiridos nove frascos de 200 mL de gel de flúor comercializados no DF: três de gel de flúor acidulado dentro da validade, três de gel flúor acidulado fora da validade e três de gel de flúor neutro. Estes produtos foram avaliados pelas seguintes técnicas: Microscopia eletrônica de varredura (MEV), Espectroscopia de energia dispersiva de raios X (EDS), espectroscopia na região de infravermelho com transformada de Fourier (FTIR), calorimetria exploratória diferencial (DSC), análise termogravimétrica (TGA) e avaliação do potencial hidrogeniônico (pH). Os elementos químicos identificados na análise por EDS estão condizentes com a composição química dos produtos declarados pelo fabricante à ANVISA. Os resultados de FTIR não demonstraram alterações significativas entre as amostras avaliadas. Foram verificados resultados diferentes entre as amostras, nos ensaios de TGA e DSC, isto provavelmente está relacionado com a desidratação das amostras e a formação de aglomerados. Os produtos avaliados a 25°C apresentaram resultados satisfatórios quanto ao pH. Mais pesquisas sobre a caracterização físico-química para o gel de flúor tópico de uso profissional odontológico são necessárias para embasar o desenvolvimento de normas técnicas específicas. / Clinical use of fluoride gels for topical application by professionals is indicated for prevention and treatment of dental caries and dental erosion due to the ability to interact with the process of demineralization - remineralization of tooth mineral matrix. The general objective of this work was to evaluate the physicochemical characteristics of topical gels for professional dental remineralization specified in the records granted by ANVISA. There were fifteen (15) valid records, relating to sixteen (16) products of fluoride gel use dental topic has been verified. As for these products, there were eight (8) neutral fluoride gels and eight (8) acidulated fluoride gels. All products contain in their composition sodium fluoride and the most used polymer for gel formation is hydroxyethyl cellulose. All acidulated fluoride gels have in their composition 1.23 % fluorine ions and all neutral fluoride gels contain 2 % NaF in its composition, as stated in the registration process at ANVISA. In 60 % of valid registration process there was information about the pH of the product. In the survey conducted in March 2014 in dental stores at Federal District (DF), it was observed the marketing of two products from the same manufacturer, being one neutral fluoride gel and one gel of acidulated fluoride. For physicochemical characterization, nine bottles of 200 ml of fluoride gel were acquired in DF: three of acidulated fluoride gel within the validity, three of acidulated fluoride gel out of date and three of neutral fluoride gel. These products were evaluated by the following techniques: Scanning Electron Microscopy (SEM), Energy Dispersive X-ray Spectrometry (EDS), Fourier Transform Infrared Spectrometer (FTIR), Differential Scanning Calorimetry (DSC), Thermogravimetry (TG) and evaluation of the hydrogen potential (pH). The chemical elements identified by EDS analysis, in regions without clusters and with clusters of dehydrated samples, are consistent with the chemical composition of the products declared by the manufacturer to ANVISA. The FTIR outcomes showed no significant changes among analyzed samples. Different results among samples were observed during DSC and TGA tests, this is probably related to dehydration of the samples and the formation of agglomerates. More studies about the physicochemical characteristics of topical gels for professional dental remineralization are needed to support the development of specific technical standards.
