A PILOT INVESTIGATION OF AN INSTRUMENT FOR THE ASSESSMENT OF FEEDING DISORDERS IN CHILDREN WITH SEVERE DISABILITIESNorris, Annaliese E. 01 January 2018 (has links)
The purpose of this pilot investigation was to assess the inter-rater reliability of an expert-validated pediatric feeding assessment, designed for clinician use with children who have severe disabilities. The tool would ideally allow for standardization of the clinical mealtime assessment process.
A comprehensive review of available feeding instruments revealed that there is a need for an assessment tool for children with motor and developmentally based feeding problems.In response to this need, a new clinician administered instrument called the Mealtime Assessment Tool for Children (MATCH) was developed in order to guide the assessment of a child during a clinical meal.
The items on the MATCH were selected based on expert validation of normal and abnormal feeding patterns that remain constant across the feeding assessments found in the literature. The tool includes a rating scale that allows for gradation of the severity of the problem. Child participants were taped while eating, and clips were rated by four SLPs using the MATCH. The resulting data was analyzed quantitatively, and it was found that across items there was a strong percentage agreement.

Vocal Function Exercises (VFEs) is a well-known treatment program that consists of four exercises, in which the first and fourth tasks require maximally sustained phonation. However, the active ingredients responsible for treatment efficacy are still unknown. The primary purpose of this investigation was the explore the effects of maximally sustained phonation on the VFE outcome measure of maximum phonation time (MPT) in individuals between the ages of 18-40 with normal voice. Participants were randomized into three experimental groups that completed VFEs for six weeks. The baseline group sustained tasks one and four for as long as was achieved at the baseline session; The three-week group sustained the exercises for as long as possible the first three weeks, then to their three-week MPT average for the remaining three weeks of the program; The standard group maximally sustained phonation for all six weeks. Results indicated significant improvement in percent to MPT goal attainment in the standard group and three-week group. The baseline group did not result in significant change. Including maximally sustained phonation throughout the course of VFEs is essential to producing substantial improvements in voice production, in terms of percent to MPT goal.

The purpose of this investigation was to determine at which age levels, between four and eight years, children express Praise, Apology, Commiseration, Blame, Challenge, Endearment, and both a Positive and Negative State.
Subjects were thirty children, six from each age level between four and eight years, selected from an elementary and preschool within the Portland area. Sixteen picture cards and stories were designed to elicit the eight different feelings. Each subject responded to questions at the end of the story and was given two chances to express the appropriate feeling. Each response was judged as appropriate or inappropriate and scored accordingly.

Using Parent-Identified Strengths of Autistic Children to Advance Strength-Based InterventionTrigueros, Angelique Francesca 01 January 2018 (has links)
Questions remain about the range of abilities autistic children possess and what constitutes effective treatment. Strength-based intervention contrasts with traditional autistic intervention approaches that focus on children's deficits. Studies on strength-based intervention approaches have not revealed how children's strengths are identified and have not used the insights of parents for this purpose. Neurodiversity serves as the conceptual framework because the tenets of neurodiversity align with those of strength-based approaches and hold that autism is a variation of the human condition rather than a disability. The purpose of this qualitative interpretive phenomenological study was to explore how the parent-identified strengths of autistic children may act as the basis for the advancement of strength-based intervention. The research questions focused on identifying the strengths of autistic children through semistructured interviews with 15 parents of high-functioning autistic children, who were recruited using purposive sampling. Data were analyzed using a three-level method, and six themes emerged: Routine, Caring for Others, Relationship with Parent, Intervention in School, Therapy, and Outlook for the Future. Practical implications for community psychology include development of strength-based approaches based on altruism, parent-child relationships, and positive outlooks for the future. Further research is recommended on caring for others and displaying affection in relation to strength-related constructs, such as resiliency and growth. Effective strength-based interventions may help autistic children develop based on their strengths, leading to positive social change.

Acquisition of auxiliary and copula BE in young English-speaking childrenGuo, Ling-Yu 01 December 2009 (has links)
This study tested the unique checking constraint hypothesis and the usage-based account concerning why young children produced tense and agreement morphemes variably via three experiments. Experiment 1 investigated whether subject types influenced the production accuracy of auxiliary 'is' in three-year-olds through an elicited production task. The rate of use of auxiliary 'is' increased as children's tense productivity increased, but the pattern was different for each subject type. The rate of use increased more rapidly with tense productivity for lexical NP subjects than it did for pronominal subjects.
Experiment 2 further examined the role of subject types, predicate types, and predicate word frequency on the use of copula 'is' in three-year-olds via an elicited production task. Overall, the production accuracy of copula 'is' was higher with nominal predicates than with permanent- or temporary-adjectival predicates, followed by locative predicates. Children also produced copula 'is' more accurately with low-frequency predicate words than with high-frequency predicate words. Moreover, the effect of subject types on the use of copula 'is' varied with children's tense productivity. For sentences with nominal, permanent-adjectival, or temporary-adjectival predicates, children with lower tense productivity used copula 'is' more accurately with lexical subjects than with pronominal subjects in. In contrast, children with higher tense productivity produced copula 'is' more accurately with pronominal subjects than with lexical subjects.
Experiment 3 extended Experiment 1 by exploring the degree of abstractness of representations of auxiliary BE via a structural priming task. The production accuracy of auxiliary 'is' in three-year-olds increased above the baseline when the prime-target pair shared the same structure and subject + auxiliary 'is' combinations, but not when the prime-target pair only shared the same structure. However, the production accuracy of auxiliary 'are' did not change with prime types.
These experiments suggest that young children have only lexically-specific representations of auxiliary BE. Frequency, rather than structural properties, of sentence elements influenced the production accuracy of auxiliary and copula 'is' in young children. These findings support the usage-based approach that young children use tense and agreement morphemes variably because they have not yet learned adult-like abstract representations and use highly frequent/ lexically-specific constructions for the production of these morphemes.

The influence of motor production experience on voice perceptionPinkerton, A. Louise 01 August 2016 (has links)
Perceptual speech and voice analysis is an essential skill for all speech-language pathologists, but it is a difficult skill to teach. Even the reliability for experienced experts is variable. Some training literature and practices in speech-language pathology suggest that imitating pathological voices may be useful for developing perceptual judgment. Evidence from other fields suggests that motor experience influences perception. Until now the link between production and perception of voice quality has not been addressed. The purpose of this pilot study is to test the hypothesis that imitating pathological voice samples would improve the perceptual discrimination abilities of naïve, inexperienced listeners.
Three expert listeners rated 25 voice samples using a perceptual voice evaluation scale, the Grade, Instability, Roughness, Breathiness, Asthenia, Strain Scale (GIRBAS) (Dejonckere et al., 1996), and identified anchor samples for the training protocol. These expert ratings were used to develop summary expert ratings that served as a comparison for the naïve listener ratings. Two groups of naïve undergraduate listeners received training in evaluating voice quality and in administering the GIRBAS. They completed a pretest, a training session, a homework session, and a post-test. During each activity, they rated 6 voices and provided a confidence rating for their scores. The experimental group imitated the voice samples during the study, and the control group completed the training without supplemental motor experience.
It was hypothesized that both listener groups would have improved accuracy and confidence levels between the pretest and post-test, with a larger improvement for the experimental group. Data suggested that training improved naïve listener accuracy and confidence levels and that this improvement was maintained for at least seven days after the initial training. Post-test accuracy for both groups was approximately the same. Imitation did not improve the accuracy of ratings, although those subjects had higher confidence levels. The data supported previous research that found that training improved the accuracy of perceptual voice evaluations. However, the hypothesis that imitation could improve perceptual ratings was not supported by this study and bears further investigation due to the small sample size.

The effect that design of the Nucleus Intracochlear Electrode Array and age of onset of hearing loss have on electrically evoked compound action potential growth and spread of excitation functionsChiou, Li-Kuei 01 May 2016 (has links)
The purpose of this study was to investigate how design changes in Cochlear Nucleus cochlear implants (CIs) (CI24M, CI24R, CI24RE and CI422) affected electrode impedance and ECAP measures, and to determine if these design changes affected post-lingually deafened adults and children with congenital hearing loss in a similar way.
Results of this study showed that electrode impedance was inversely related to the area of the electrode contacts in the array: lowest for the full-banded CI24M CI and highest for adults who used the CI422 device which has the smallest electrode contacts of all four devices. The noise floor of the NRT system likely plays a significant role in the finding that CI users with older devices (the CI24M, and CI24R CIs) had higher ECAP thresholds than individuals with the CI24RE electrode array. The position of the electrode array in the cochlea was also found to have a significant effect on ECAP measures. CI users with modiolar hugging (the CI24R and CI24RE CIs) electrode arrays were found to have lower ECAP thresholds than CI users whose electrode arrays were seated more laterally in the cochlear duct (e.g. the CI24M and CI422 implants). The position of the electrode contacts relative to the modiolus of the cochlea was found to be related to slope of the ECAP growth functions. The lowest slopes were found in CI24RE users. It also had a significant impact on the width of the channel interaction function. Electrode arrays seated further from the modiolus have significantly more channel interaction than electrode arrays that hug the modiolus of the cochlea.
Differences between results recorded from post-lingually deafened adults and children with congenital hearing loss were minimal. The difference only reflected on the ECAP slopes. Slopes in children with congenital hearing loss were significantly steeper than those recorded from adults. This may indicate that children with congenital hearing loss may have better neural survival than adults with acquired hearing loss.
In conclusion, the results of the current study show evidence of the effects of variations in design and function of the implanted components of the Nucleus CI. Perhaps the most significant finding from the current data set is that electrode arrays located closer to the modiolus of the cochlea have lower thresholds and exhibit less channel interaction than electrode arrays that are positioned more laterally. An argument could be made that lower stimulation levels and less channel interaction may result in better outcomes and/or longer battery life. For CI candidates who do not have significant residual acoustic hearing, the CI24RE implant might be a better choice than the more recently introduced CI422 electrode array.

Thyroarytenoid and cricothyroid muscular activity in vocal register controlHull, Darcey M. 01 December 2013 (has links)
Register and pitch are two distinct perceptual entities of the human voice. Without clear evidence for the use of the terminology, sources have begun to refer to lighter, or "falsetto", register as being "cricothyroid dominant" and heavier, or "chest", register as being "thyroarytenoid dominant" (Hirano, 1987; Miller, 1996; McCoy, 2004; Henrich, 2006; Spivey, 2008; Edwin, 2008; Phillips, Williams, & Edwin, 2012). The same intrinsic laryngeal musculature (i.e. the cricothyroid, CT, and thyroarytenoid, TA, muscles) play a role in the control of both register and pitch. Higher-pitched phonation, typically used to produce falsetto register, is mediated primarily by the cricothyroid (CT) muscle. The thyroarytenoid (TA) muscle plays a larger role in controlling lower-pitched voicing, the pitch range in which chest register tends to be used (Titze, 1989b; Shipp and McGlone, 1971). Despite their frequent co-occurrence, high and low pitched phonation are not controlled in the same way as light and heavy register productions.
The purpose of this study was to examine the ratio of CT and TA muscular activity in the control of chest and falsetto registers. Data were collected from untrained voice users: four females and one male. Hooked-wire electrodes were inserted into both the CT and TA muscles of each participant in order to collect electromyographic (EMG) data during glissando from low to high pitch on the vowel /i/ twice per subject, and tasks eliciting maximal activation of CT and TA muscles. A trained singing instructor with 17 years of experience determined and recorded the occurrence of register transition during each glissando. CT and TA EMG activity data from the glissando were normalized relative to maximum elicited CT and TA EMG activity, and were then retrospectively analyzed. CT muscular dominance was defined as a ratio of percentage of maximum CT EMG activity to percentage of maximum TA EMG activity greater than 1 (i.e. CT:TA greater than 1). TA dominance is a ratio of CT:TA activity less than 1 (i.e. CT:TA less than 1).
During glissando, all subjects experienced register transition from chest to falsetto register. In all subjects, the majority of chest register, and all of falsetto register, was produced with CT muscular dominance. Only the 3-4 lowest semitones, on average, in chest register were TA dominant. The transition from chest to falsetto register consistently did occur when the CT muscle was dominant, however, register transition did not occur as CT muscle activity began to dominate TA muscle activity. Results of the study showed that CT muscular dominance did not define falsetto register, nor was chest register defined by the TA muscular dominance.
