Métodos: A amostra foi constituída por 18 indivíduos com idade variando de 19 a 25 anos, com média de 22 ± 3 anos. As sondagens foram realizadas em 504 dentes, em 6 sítios por dente, representou 3024 sítios examinados. Para participar da pesquisa o voluntário deveria preencher os critérios de inclusão preestabelecidos. Resultados: Verificou-se que ao comparar as profundidades médias dos sulcos gengivais nas faces vestibulares e linguais houve diferenças significativas nos grupos dos incisivos, caninos e molares (p<0,05), o mesmo não sendo observado no grupo dos pré-molares. Em relação ao sexo, observou-se que houve diferença significante para todos os grupos dentários (p<0,05), sendo que o sexo feminino apresentou os valores médios menores. A média dos sulcos gengivais foi crescente dos dentes anteriores para os posteriores em ambos os sexos. Conclusão: Os dentes pré-molares não apresentaram diferenças entre as faces vestibulares e linguais, e o sexo feminino apresentou menores médias de profundidade dos sulcos gengivais. / Objective: To evaluate the periodontal conditions in clinically healthy young adults to determine the depth of the gingival sulcus, know and compare the mean values found in the vestibular and lingual faces, intra-groups of teeth and see if there were differences of the values found in relation to sex of the individuals. Methods: The sample consisted of 18 individuals with age ranging from 19 to 25 years, with a mean of 22 ± 3 years. The surveys were conducted in 504 tooth, in 6 sites per teeth, represented 3024 sites examined. To participate in the survey the volunteer should fulfil the inclusion criteria pre-established. Results: It was found that when comparing average depths of gingival sulcus in the vestibular and lingual sides there were significant differences in the groups of incisors, canines and molars (p < 0.05), the same is not being observed in the group of the premolars. In relation to sex, it was observed that there was a statistically significant difference for all dental groups (p < 0.05), whereas the female has smaller average values. The average of the gingival sulcus was growing of anteriors tooth to the posteriors in both sexes. Conclusion: The premolars showed no differences between the vestibular and lingual faces, and the female has less depth averages of gingival sulcus.

Análise comparativa da presença de mediadores inflamatórios no sulco gengival resultante das técnicas de moldagem em prótese fixa

A qualidade de uma reconstrução protética depende, primordialmente, de sua integração biológica e da sua adaptação. O afastamento gengival serve para se obter espaço para o material de moldagem e também para a visualização e acesso aos limites gengivais dos preparos, que são essenciais para a confecção de próteses com uma justeza de adaptação clínica. No entanto, a inserção de um material (fio retrator ou casquete) pode romper o sistema de fibras gengivais que conectam a margem gengival à superfície do cemento causando recessão gengival. Ocasionalmente, ocorre significativa inflamação ou perda de inserção. O objetivo deste trabalho foi quantificar a resposta inflamatória decorrente das diferentes técnicas de moldagem para prótese fixa, em termos da quantidade de IL-2, IL-4, IL-5, IL-10, TNF-&#x3B1;, IFN-&#x3B3;, presentes no fluido gengival dos dentes com necessidade protética. A amostra foi composta por 10 dentes moldados pela técnica de afastamento gengival com fio retrator e um grupo de 10 dentes moldados pela técnica do casquete. Os pacientes foram selecionados considerando-se a necessidade de prótese parcial fixa, higiene bucal satisfatória e não possuírem sinais clínicos de doença periodontal. Previamente à moldagem, uma tira de periopaper foi inserida no sulco gengival do paciente e foi mantida por 30 segundos. A coleta foi repetida 4 vezes para as faces mesiovestibular, distovestibular, mesiolingual e distolingual. Terminado o procedimento da moldagem, após 2 horas, 4 horas e 24 horas, respectivamente, foi repetido o procedimento de inserção do periopaper nos 4 sítios dos dentes. A amostra foi preparada para a citometria de fluxo (CBA - cytometric beads array). Os resultados foram analisados pelo teste de Tukey. Na análise de variância a 3 critérios, por meio da comparação entre técnica de moldagem, paciente e tempo para os valores de TNF-&#x3B1;, a relação tempo e paciente versus tempo foi estatisticamente significante (p?0,05). Na comparação entre a técnica de moldagem com casquete e o fio retrator, não houve superioridade entre uma técnica e outra(p?0,05). O teste Tukey para a comparação intragrupo tempo (4 tempos), independente de técnica e paciente, mostrou relevância estatística entre os períodos antes da moldagem e após 24 horas (p=0,016) e, no período entre 2 horas e 24 horas após a moldagem (p=0,06). Portanto, foi possível concluir que não existe evidência científica que demonstre alguma superioridade das técnicas estudadas. Com isso, a escolha pelo método de afastamento gengival depende da situação clínica e habilidade do profissional. / The quality of a prosthetic reconstruction depends mostly on its integration and its biological adaptation. The gingival retraction serves to provide space for the impression material and also for visualize and give access to the gingival limit of tooth preparations, which are essential for making prostheses with a reliable clinical precision fit. However, the inclusion of a material (retractor cord or acrilic resin coping) can break the system of gingival fibers that connect the gingival margin to cementum´s surface, causing gingival recession. Occasionally, inflammation or significant insertion loss occurs. The aim of this study was to quantify the inflammatory response resulting from differents impression techniques for fixed prosthodontics in terms of the quantity of IL-2, IL-4, IL-5, IL-10, TNF-&#x3B1;, IFN-&#x3B3;, found in gingival fluid of tooths needing prosthetic restorations. The sample consisted of 10 teeth impressed by gingival retraction cord technique and a group of 10 teeth using acrilic resin coping technique. Patients were selected considering the need for fixed partial denture, good oral hygiene and absence of periodontal disease´s clinical signs. Prior taking impressions, a Periopaper strip was inserted into the gingival sulcus of patients and was maintained for 30 seconds. The collect was repeated 4 times to the mesiobuccal, distovestibular, mesiolingual and distolingual sides: After the impression procedure, collecting was repeated after 2 hours, 4 hours and 24 hours respectively. the Periopaper strip was inserted on the teeth´s four sites. The sample was prepared for flow cytometry examination (CBA - cytometric beads array). The results were evalueted by Tukey test. The 3-way analysis of variance criteria,comparing impression technique, patient and time to values of TNF-&#x3B1;, the relationship between time and patient versus time was statistically significant (p ? 0.05). Comparing the acrilic resin coping and retraction cord impression techniques, there was no superiority between each other (p ? 0.05). The Tukey test for intragroup comparison time (4 times) irrespective of technique and patient showed statistical significance between periods before the impression and after 24 hours (p=0,016) and the period between 2 and 24 hours after impressions (p=0,06). Therefore, it was concluded that there is no scientific evidence that exists any superiority between the techniques studied. Thus, the choice of gingival retraction method depends on the clinical situation and professional´s skills.

Materiais e Métodos: 73 dentes maxilares foram tratados com laminados cerâmicos em dissilicato de lítio monolítico. Para todos os elementos foi feita a coleta do GCF previamente à cimentação (baseline) e nos tempos do estudo (t0- 7 dias, t4 180 dias), onde foi calculado o volume do GCF e quantificado os biomarcadores: TNF-α, IL-1β, MMP-8 por ELISA. O dente vizinho ou antagonista foi utilizado como controle. Foram obtidas réplicas da região cervical em resina epóxi, em t0 e t4 para verificação da adaptação marginal em microscópio eletrônico de varredura. A avaliação clínica foi realizada pelos critérios modificados do USPHS nos tempos do estudo e com 1 ano de acompanhamento (t5). Resultados: Os níveis de TNF-α e MMP-8 se mantiveram estáveis na comparação de t4 com o baseline (p>0,05). A concentração de IL-1β aumentou em t4 no grupo tratado em comparação com o baseline (p<0,05), porém sem diferença estatística significante para o grupo controle em t4 e no baseline (p>0,05). (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Objective: To quantify the biomarkers and gingival crevicular fluid (GCF) volume, with additional verification of the adaptation and marginal behavior of thin ceramic veneers cemented on teeth without cervical preparation, with the finish line gently positioned within the gingival sulcus. Materials and Met hods: 73 maxillary teeth were treated with monolithic lithium disilicate ceramic veneers . For all elements, the GCF was collected before cementation ( baseline) and at the study times (t0 -7 days, t4 180 days), which was calculated the GCF volume and the quantification of the biomarkers: TNF - α, IL - 1β, MMP -8 by ELISA. The neighboring or antagonist tooth was used as a control. Replicas of the cervical region were obtained in epoxy resin, in t0 and t4 to verify the marginal adaptation in a scanning electron mi croscope. Perfil imunológico e microbiológico de pacientes com periodontites crônica e agressiva / Immunological and microbiological profiles of chronic and agressive periodontitis subjects

O objetivo deste estudo foi determinar os níveis de interleucina-1&#946; (IL-1 &#946;), IL-2, IL-4, IL-8, interferon-&#947; (IFN-&#947;) e a atividade de elastase no fluido gengival (FG) de pacientes com periodontite crônica generalizada (PCG) e periodontite agressiva generalizada (PAgG), e correlacionar com indivíduos de um grupo controle com gengivite apenas. Um objetivo secundário foi analisar o perfil microbiológico subgengival destes indivíduos. Dados clínicos transversais foram obtidos de 20 pacientes com PCG, 17 pacientes com PAgG e 10 indivíduos com gengivite. Amostras de FG foram coletadas com tiras de papel e os níveis de: IL-1&#946;, IL-2, IL-4, IL-8 e IFN-&#947; foram medidos, utilizando um imunoensaio do tipo multiplex (Luminex). Atividade da elastase foi avaliada por um ensaio enzimático. Amostras de placa subgengival foram analisadas através do checkerboard DNA-DNA hybridization. As diferenças de significância entre os grupos para dados imunológicos e microbiológicos foram realizadas utilizando o teste Kruskal-Wallis, ajustando para múltiplas comparações. As médias dos parâmetros clínicos e os volumes de FG foram maiores nos pacientes com PCG e PAgG comparados ao grupo gengivite. Níveis mais elevados de IL-1&#946; e atividade de elastase foram encontrados em sítios profundos quando comparado a sítios rasos em ambos os grupos com periodontite (p <0,05). Os dados microbiológicos apresentaram níveis significativamente mais elevados das espécies do complexo vermelho em pacientes com PCG e PAgG, quando comparados aos indivíduos com gengivite (p <0,05). Não houve diferença estatisticamente significante nos níveis de biomarcadores no FG e nos níveis de espécies bacterianas subgengivais entre pacientes com PCG e pacientes com PAgG. Sendo assim, concluímos que os dados do presente estudo não mostraram diferença estatisticamente significante nos parâmetros imunológicos e microbiológicos medidos entre indivíduos com PCG e PAgG. / The goal of this study was to determine the gingival crevicular fluid (GCF) levels of interleukin-1&#946; (IL-1&#946;), IL-2, IL-4, IL-8 and interferon-&#947; (IFN-&#947;) and elastase activity from patients with generalized chronic (GCP) and generalized aggressive periodontitis (GAgP) and to compare with a control group with gingivitis subjects. A secondary aim was to analyze the microbiological profile of these subjects. Cross-sectional clinical data were obtained from 20 GCP, 17 GAgP and 10 gingivitis subjects. GCF samples were collected with paper strips and the levels of: IL-1&#946;, IL-2, IL-4, IL-8, and IFN-&#947; measured using a multiplexed bead immunoassay (Luminex). Elastase activity was assessed by an enzymatic assay. Subgingival plaque samples were analyzed using checkerboard DNA-DNA hybridization. Significance of differences among groups for immunological and microbiological data was examined using Kruskal-Wallis adjusting for multiple comparisons. Mean clinical parameters and GCF volumes were higher in GCP and GAgP patients compared to controls. Higher levels of IL-1&#946; and higher elastase activity were found in deep sites compared to shallow sites in both periodontitis groups (p<0.05). The microbiological data showed significantly higher levels of the Red complex species in GCP and GAgP patients compared to controls (p<0.05). There were no statistically significant differences in levels of GCF biomarkers and in levels of subgingival bacterial species between GCP and GAgP subjects. In conclusion, there were no statistically significant differences in the measured immunological and microbiological parameters between GCP and GAgP subjects.

Efeito do movimento dentário nos níveis de metaloproteinases da matriz no fluido gengival de pacientes com periodontite controlada / Levels of GCF matrix metalloproteinases in periodontally compromised teeth under orthodontic forces

Fundação de Amparo à Pesquisa do Estado do Rio de Janeiro / O objetivo deste trabalho foi examinar o impacto da movimentação dentária de dentes periodontalmente comprometidos no volume do fluido gengival crevicular (FGC) e nos níveis de expressão das metaloproteinases de matriz (MMPs) -1, -2, -3, -7, -8, -12 e -13 no FGC. Dez pacientes periodontalmente controlados (8 do sexo feminino e 2 do sexo masculino, média de idade de 46,2 10,4 anos) com incisivos projetados labialmente foram submetidos a tratamento ortodôntico. Uma arcada dentária foi submetida a movimentação ortodôntica e a arcada oposta foi usada como controle. Amostras de FGC foram coletadas da face lingual de dois incisivos do lado movimento e dois do lado controle uma semana antes da ativação ortodôntica (-7d), imediatamente após a ativação ortodôntica, e após 1 h, 24 h, e 7, 14 e 21 dias. A coleta do FGC foi feita utilizando-se tiras de papel absorvente e o volume foi calculado através do uso do Periotron. Todos os pacientes receberam orientações de higiene bucal e um kit contendo escova de dente, pasta de dente e bochecho de Chlorexidina 0,12% para ser usado durante todo o experimento. A técnica da multianálise imunoenzimática com microesferas foi usada para medir as MMPs no FGC. Os dados foram analisados utilizando-se os testes estatísticos Friedman e Mann-Whitney. Não foram encontradas diferenças estatisticamente significativas no volume do FGC. Em relação aos níveis de MMPs, a única diferença estatisticamente significativa encontrada no decorrer do tempo foi nos níveis de MMP-1 no grupo movimento (p<0,05). Quando os dois grupos foram comparados após a ativação, a única diferença estatisticamente significativa encontrada foi nos níveis de MMP-12 24 horas após a ativação (p<0,05). Estes achados sugerem que o volume de FGC não sofre alteração relacionada ao movimento dentário ortodôntico e que o movimento ortodôntico de dentes periodontalmente comprometidos não resultou em mudanças significativas nos níveis de MMPs no FGC. / The aim of this study was to evaluate whether orthodontic tooth movement of periodontally compromised teeth would result in detectable changes in gingival crevicular fluid (GCF) volume and in the levels of matrix metalloproteinases (MMP) -1, -2, -3, -7, -8, -12 and -13 in the GCF. Ten controlled periodontitis subjects (8 females and 2 males, mean age of 46.2 10.4 years) with flared incisors were submitted to orthodontic treatment. One dental arch was subjected to orthodontic movement, and teeth in the opposite arch were used as controls. GCF samples were collected from the lingual sites of two test and two control incisors one week before orthodontic activation (-7d), immediately after orthodontic activation, and after 1 h, 24 h, and 7, 14 and 21 days. Filter paper strips were used in combination with a Periotron for GCF volume measurements. All patients received hygiene instructions and a kit containing toothbrush, fluoride dentifrice and gluconate chlorexidine 0.12% mouthwash to be used during all experiment. Multiplexed bead immunoassay was used to measure MMPs in GCF. Data were analyzed using Friedman and Mann-Whitney statistical tests. There were no statistically significant differences over time in the GCF volume for any of the two groups. Regarding the MMPs levels, the only significant change found over time was in the levels of MMP-1 in the movement group (p<0.05). When the two groups were compared after activation, the only statistically significant difference found was in levels of MMP-12 24 h after activation (p<0.05). This findings suggested that the GCF volume could not be associated to orthodontic movement and that the orthodontic movement of periodontally compromised teeth did not result in significant changes in the GCF levels of MMPs.

Amostras de FG foram coletadas com tiras de papel e os níveis de: IL-1&#946;, IL-2, IL-4, IL-8 e IFN-&#947; foram medidos, utilizando um imunoensaio do tipo multiplex (Luminex). Atividade da elastase foi avaliada por um ensaio enzimático. Amostras de placa subgengival foram analisadas através do checkerboard DNA-DNA hybridization. As diferenças de significância entre os grupos para dados imunológicos e microbiológicos foram realizadas utilizando o teste Kruskal-Wallis, ajustando para múltiplas comparações. As médias dos parâmetros clínicos e os volumes de FG foram maiores nos pacientes com PCG e PAgG comparados ao grupo gengivite. Níveis mais elevados de IL-1&#946; e atividade de elastase foram encontrados em sítios profundos quando comparado a sítios rasos em ambos os grupos com periodontite (p <0,05). Os dados microbiológicos apresentaram níveis significativamente mais elevados das espécies do complexo vermelho em pacientes com PCG e PAgG, quando comparados aos indivíduos com gengivite (p <0,05). Não houve diferença estatisticamente significante nos níveis de biomarcadores no FG e nos níveis de espécies bacterianas subgengivais entre pacientes com PCG e pacientes com PAgG. Sendo assim, concluímos que os dados do presente estudo não mostraram diferença estatisticamente significante nos parâmetros imunológicos e microbiológicos medidos entre indivíduos com PCG e PAgG. / The goal of this study was to determine the gingival crevicular fluid (GCF) levels of interleukin-1&#946; (IL-1&#946;), IL-2, IL-4, IL-8 and interferon-&#947; (IFN-&#947;) and elastase activity from patients with generalized chronic (GCP) and generalized aggressive periodontitis (GAgP) and to compare with a control group with gingivitis subjects. A secondary aim was to analyze the microbiological profile of these subjects. Cross-sectional clinical data were obtained from 20 GCP, 17 GAgP and 10 gingivitis subjects. GCF samples were collected with paper strips and the levels of: IL-1&#946;, IL-2, IL-4, IL-8, and IFN-&#947; measured using a multiplexed bead immunoassay (Luminex). Elastase activity was assessed by an enzymatic assay. Subgingival plaque samples were analyzed using checkerboard DNA-DNA hybridization. Significance of differences among groups for immunological and microbiological data was examined using Kruskal-Wallis adjusting for multiple comparisons. Mean clinical parameters and GCF volumes were higher in GCP and GAgP patients compared to controls. Higher levels of IL-1&#946; and higher elastase activity were found in deep sites compared to shallow sites in both periodontitis groups (p<0.05). The microbiological data showed significantly higher levels of the Red complex species in GCP and GAgP patients compared to controls (p<0.05). There were no statistically significant differences in levels of GCF biomarkers and in levels of subgingival bacterial species between GCP and GAgP subjects. In conclusion, there were no statistically significant differences in the measured immunological and microbiological parameters between GCP and GAgP subjects.

Análise comparativa da presença de mediadores inflamatórios no sulco gengival resultante das técnicas de moldagem em prótese fixa

A qualidade de uma reconstrução protética depende, primordialmente, de sua integração biológica e da sua adaptação. O afastamento gengival serve para se obter espaço para o material de moldagem e também para a visualização e acesso aos limites gengivais dos preparos, que são essenciais para a confecção de próteses com uma justeza de adaptação clínica. No entanto, a inserção de um material (fio retrator ou casquete) pode romper o sistema de fibras gengivais que conectam a margem gengival à superfície do cemento causando recessão gengival. Ocasionalmente, ocorre significativa inflamação ou perda de inserção. O objetivo deste trabalho foi quantificar a resposta inflamatória decorrente das diferentes técnicas de moldagem para prótese fixa, em termos da quantidade de IL-2, IL-4, IL-5, IL-10, TNF-&#x3B1;, IFN-&#x3B3;, presentes no fluido gengival dos dentes com necessidade protética. A amostra foi composta por 10 dentes moldados pela técnica de afastamento gengival com fio retrator e um grupo de 10 dentes moldados pela técnica do casquete. Os pacientes foram selecionados considerando-se a necessidade de prótese parcial fixa, higiene bucal satisfatória e não possuírem sinais clínicos de doença periodontal. Previamente à moldagem, uma tira de periopaper foi inserida no sulco gengival do paciente e foi mantida por 30 segundos. A coleta foi repetida 4 vezes para as faces mesiovestibular, distovestibular, mesiolingual e distolingual. Terminado o procedimento da moldagem, após 2 horas, 4 horas e 24 horas, respectivamente, foi repetido o procedimento de inserção do periopaper nos 4 sítios dos dentes. A amostra foi preparada para a citometria de fluxo (CBA - cytometric beads array). Os resultados foram analisados pelo teste de Tukey. Na análise de variância a 3 critérios, por meio da comparação entre técnica de moldagem, paciente e tempo para os valores de TNF-&#x3B1;, a relação tempo e paciente versus tempo foi estatisticamente significante (p?0,05). Na comparação entre a técnica de moldagem com casquete e o fio retrator, não houve superioridade entre uma técnica e outra(p?0,05). O teste Tukey para a comparação intragrupo tempo (4 tempos), independente de técnica e paciente, mostrou relevância estatística entre os períodos antes da moldagem e após 24 horas (p=0,016) e, no período entre 2 horas e 24 horas após a moldagem (p=0,06). Portanto, foi possível concluir que não existe evidência científica que demonstre alguma superioridade das técnicas estudadas. Com isso, a escolha pelo método de afastamento gengival depende da situação clínica e habilidade do profissional. / The quality of a prosthetic reconstruction depends mostly on its integration and its biological adaptation. The gingival retraction serves to provide space for the impression material and also for visualize and give access to the gingival limit of tooth preparations, which are essential for making prostheses with a reliable clinical precision fit. However, the inclusion of a material (retractor cord or acrilic resin coping) can break the system of gingival fibers that connect the gingival margin to cementum´s surface, causing gingival recession. Occasionally, inflammation or significant insertion loss occurs. The aim of this study was to quantify the inflammatory response resulting from differents impression techniques for fixed prosthodontics in terms of the quantity of IL-2, IL-4, IL-5, IL-10, TNF-&#x3B1;, IFN-&#x3B3;, found in gingival fluid of tooths needing prosthetic restorations. The sample consisted of 10 teeth impressed by gingival retraction cord technique and a group of 10 teeth using acrilic resin coping technique. Patients were selected considering the need for fixed partial denture, good oral hygiene and absence of periodontal disease´s clinical signs. Prior taking impressions, a Periopaper strip was inserted into the gingival sulcus of patients and was maintained for 30 seconds. The collect was repeated 4 times to the mesiobuccal, distovestibular, mesiolingual and distolingual sides: After the impression procedure, collecting was repeated after 2 hours, 4 hours and 24 hours respectively. the Periopaper strip was inserted on the teeth´s four sites. The sample was prepared for flow cytometry examination (CBA - cytometric beads array). The results were evalueted by Tukey test. The 3-way analysis of variance criteria,comparing impression technique, patient and time to values of TNF-&#x3B1;, the relationship between time and patient versus time was statistically significant (p ? 0.05). Comparing the acrilic resin coping and retraction cord impression techniques, there was no superiority between each other (p ? 0.05). The Tukey test for intragroup comparison time (4 times) irrespective of technique and patient showed statistical significance between periods before the impression and after 24 hours (p=0,016) and the period between 2 and 24 hours after impressions (p=0,06). Therefore, it was concluded that there is no scientific evidence that exists any superiority between the techniques studied. Thus, the choice of gingival retraction method depends on the clinical situation and professional´s skills.

Influência do ângulo entre a crista óssea e a superfície radicular na profundidade do sulco gengival clínico / Influence of the angle between the bone crest and root surfaces in the depth of the clinical gingival sulcus

Foi desenvolvido projeto de pesquisa clínica e radiográfica para avaliar a influência do ângulo formado entre a superfície dental e a crista óssea na determinação da profundidade de sondagem do sulco gengival e complementarmente no comportamento da margem gengival por vestibular de dentes molares inferiores inclinados para mesial. Para tanto foram incluídos 30 sítios mesiais e 30 distais no grupo teste, com igual número de sítios controles de molares inferiores com inclinação normal. Nos dentes que forneceram esses sítios também foi feita a determinação da profundidade do sulco gengival na região vestibular central do dente, identificando-se a qualidade e quantidade de gengiva ceratinizada, respectivamente pela metodologia de Kan et al. (2010) e pela mensuração com sonda periodontal da distância da margem gengival à junção mucogengival. Foram incluídos pacientes periodontal e sistemicamente saudáveis, excluindo-se pacientes que tivessem sido submetidos a procedimentos ósseos regenerativos prévios nas áreas de interesse, diabéticos relutantes ao controle médico, usuários de drogas e/ou álcool, portadores de alterações sistêmicas que interfiram no metabolismo ósseo (como por exemplo, osteoporose e hiperparatireoidismo). Os exames foram realizados por examinador competente, devidamente calibrado. Para análise radiográfica as imagens foram transferidas para o computador, realizando-se as mensurações dos ângulos interessados com o programa de computador MB-Ruler Pro (MB-Software solutions). A análise estatística foi realizada no programa GraphPad Prism versão 5.03 para Windows (GraphPad, Usa). Os resultados obtidos em linhas gerais mostraram que houve influência significativa do ângulo entre a superfície dental e a crista óssea (p > 0,0001) na determinação do sulco gengival proximal em áreas de dentes inclinados, porém não há essa influência na profundidade de sondagem do sulco gengival por vestibular para áreas com qualidade e quantidade de gengiva comparáveis ao controle (p=0,08). A despeito desses resultados, não se encontrou correlação definida entre nenhum dos parâmetros de interesse analisados. Dentro dos limites do estudo os resultados também evidenciaram que, embora possam ocorrer variações desses parâmetros, a saúde periodontal pode ser mantida pelo indivíduo nas condições analisadas. / A clinical and radiographic research project was developed to assess the influence of the angle formed between the tooth surface and the alveolar bone crest in determining the probing depth of the gingival sulcus and complementary also to evaluate the behavior of the buccal gingival margin of mesially inclined molars. Therefore, 30 mesial and 30 distal sites of inclined lower molars were included in the test group, with an equal number of sites of lower molars with normal inclination in the control group. In addition in all the teeth of both test and control groups the depth of the clinical gingival sulcus at the central buccal region of the tooth was assessed by measuring with a periodontal probe, identifying the quality of the keratinized gingiva through the methodology of Kan et al. (2010) and determining the width of keratinized gingiva by measuring the distance from the free gingival margin to the mucogingival junction. The study was done in systemic and periodontally healthy individuals, excluding patients who had undergone bone-regenerative procedures in the areas of interest, diabetics reluctant to medical control, alcohol and / or drug users, and individuals suffering from systemic conditions that might interfere with bone metabolism (like osteoporosis and hyperparathyroidism). The examinations were performed by a competent and properly calibrated examiner. The radiographic images were then transferred to a computer in order to analyze the measurements of the involved angles with the computer program MB-Ruler Pro (MB-Software solutions). Statistical analysis was performed using GraphPad Prism version 5.03 for Windows (GraphPad, Usa). The results in general showed that there was significant influence of the angle between the tooth surface and the alveolar bone crest on the depth of the gingival sulcus in proximal areas of inclined teeth (p > 0,0001), but there was no such influence in the probing depth of the buccal gingival sulcus of these inclined molars provided that the quality and quantity of gingiva were similar to controls comprised by molars with normal inclination (p=0,08). Despite this significance, there was no definite correlation between any of the analyzed parameters of interest. The results also showed that, although there may be variations of these parameters, within the limits of this study the conditions of periodontal health can be maintained by the individual.

Impacto da terapia mecânica não cirúrgica em pacientes com periodontite crônica e agressiva generalizadas / Impact of non-surgical periodontal therapy on generalized chronic and aggressive periodontitis

O objetivo deste estudo foi avaliar o impacto do tratamento periodontal não cirúrgico sobre a atividade de elastase e o volume de fluido gengival nos pacientes portadores de periodontite crônica e agressiva generalizadas. Foram avaliados 18 pacientes com periodontite crônica (idade média 48,6 DP 7,5 anos), e 11 com periodontite agressiva (idade média 27,9 DP 6,54 anos). Foram utilizados os parâmetros clínicos de avaliação de profundidade de bolsa à sondagem (PB) (mm), nível de inserção (NI) (mm) e sangramento à sondagem (SS). As medidas clínicas e as amostras de fluido gengival foram colhidas a partir dos cinco sítios mais profundos (P) e de cinco sítios rasos com gengivite (G) de cada paciente, antes e 90 dias após o término do tratamento. As etapas clínicas obedeceram ao seguinte cronograma: seleção e exame periodontal; coleta de fluido gengival; tratamento periodontal; reavaliação, compreendendo o exame periodontal e coleta de fluido gengival. O tratamento levou, em média, 4 sessões de 40 minutos cada por paciente, com intervalo de 1 semana entre elas. As consultas para reavaliação foram feitas 90 dias após o término do tratamento. O teste de Wilcoxon foi utilizado para comparar os dados antes e depois do tratamento e o teste não pareado de Mann-Whitney U-test foi utilizado para comparar os grupos de periodontite crônica e agressiva. A amostra analisada antes e após o tratamento não apresentou diferenças significativas entre o grupo com periodontite crônica e agressiva, que responderam de forma similar a todos os indicadores avaliados, exceto para a profundidade de bolsa à sondagem nos sítios rasos com gengivite (p = 0,039) e para o sangramento à sondagem (p = 0,021) nos sítios profundos, ambos mais reduzidos na periodontite crônica após o tratamento. A elastase apresentou, após o tratamento, redução significativa nos sítios profundos, para a periodontite crônica (p = 0,012) e agressiva (p = 0,02). Em relação ao volume de fluido gengival, houve significativa redução após o tratamento nos pacientes com periodontite crônica e agressiva, tanto nos sítios rasos (p = 0,03 e p = 0,03) como nos profundos (p &#706; 0,001 e p = 0,003), respectivamente. Concluindo, os grupos com periodontite crônica e agressiva generalizadas comportaram- se de maneira semelhante frente à terapia mecânica não cirúrgica. Ainda, a terapia periodontal mecânica não cirúrgica mostrou redução significativa do volume de fluido gengival em todos os sítios analisados, e da atividade neutrofílica, nos sítios profundos, associada a reduções significativas em todos os indicadores clínicos analisados após o tratamento. / The aim of this study was to investigate the impact of non-surgical mechanical treatment on elastase activity and gingival crevicular fluids (GCF) volume of patients with untreated generalized chronic and aggressive periodontitis. Eighteen patients with generalized chronic periodontitis (mean age 48,6 SD 7,5 years old) and eleven with generalized aggressive periodontitis (mean age 27,9 SD 6,54 years old) were evaluated. The clinical parameters adopted were pocket probing depth (PPD) (mm); attachment level (AL) (mm) and bleeding on probing (PB). Clinical measures and GCF were collected from the 5 deepest sites (P) and from 5 shallow sites with gingivitis (G) from each patient of each group, before and 90 days after non- surgical periodontal treatment. There were adopted the following clinical steps: patients selection and periodontal exam; GCFs collection; periodontal treatment; re- evaluations, comprising periodontal exams and GCFs collection. The periodontal treatment lasted about about 4 sections with 40 minutes each, 1 section per week. Re-evaluations were performed 90 days after completion of the treatment. Wilcoxons statistic test compared the data before and after treatment and Mann-Whitneys non parametric U-test was used to compared chronic and aggressive periodontitis groups. There were no significant differences for all clinical parameters analysed before and after treatment,except for pocket probing depth on shallow sites (p = 0,039) and for bleeding on probing (p = 0,021) on deep sites, both in lower levels in chronic periodontitis after treatment. The elastases activity showed, after treatment, a significant reduction on deep sites for chronic (p = 0,012) and aggressive (p = 0,02) periodontitis. In concern to GCFs volume, there was significant reduction on both chronic and aggressive periodontitis, in shallow (p = 0,03 e p = 0,03) and in deep pockets (p &#706; 0,001 e p = 0,003), respectively, after treatment. In conclusion, both groups responded simmilarly to the mechanical non- surgical treatment. Moreover, non- surgical periodontal treatment showed significant reductions on GCFs volume on all analysed sites and reduction on neutrophilics activity, on deep sites followed by the significant reduction on clinical indicators after treatment.
