Esses grupos, normalmente, têm um índice alto de entradas em hospitais, bem como grandes períodos de internação, o que acarreta um impacto negativo na qualidade de vida do paciente, bem como um aumento no custo do tratamento. Esse cenário é responsável pela tendência mundial em se buscar alternativas para diminuir a pressão sofrida pelos sistemas de saúde. Com a expansão do acesso aos meios de comunicações por uma fração cada vez maior da população mundial, a alternativa que se mostra cada vez mais viável é o Telehomecare. O Telehomecare é uma aplicação da Telemedicina que promove serviços de saúde aos pacientes em suas casas. Por sua vez, Telemedicina é o uso de Tecnologia da Informação e Tecnologia de Telecomunicações para promover serviços de saúde à distância. Baseado nesses conceitos, este trabalho propõe um sistema Telehomecare para monitoramento de pacientes a partir de suas residências. Esse sistema consiste em uma unidade de monitoramento que se conecta a sensores de sinais vitais presentes na residência do paciente, capta os dados desses sensores e os transmitem de forma segura para uma central de monitoramento, onde esses dados podem ser armazenados, analisados e disponibilizados aos profissionais de saúde envolvidos no tratamento através de unidades de consulta. Este trabalho priorizou a implementação da unidade de monitoramento e implementou minimamente as outras partes para fazer a validação do sistema

Solución de Telehomecare que brinda atención médica para adultos mayores diagnosticados con Diabetes Mellitus tipo 2 / Telehomecare solution that provides medical attention to older adults diagnosed with Type 2 Diabetes Mellitus

El presente proyecto nace de la búsqueda de una solución que brinde atención médica remota a los adultos mayores diagnosticados con Diabetes tipo 2, con el fin de mantener un control periódico de su enfermedad y poder reducir las complicaciones presentadas por el incumplimiento del tratamiento médico y control glucémico. La solución consiste en una plataforma tecnológica que soporta una aplicación móvil para el paciente que controla de forma autónoma su enfermedad a través de información sobre la Diabetes, medicamentos, detalle del tratamiento recomendado por el médico y el control periódico del nivel de glucosa. Y una página web permite al endocrinólogo realizar el seguimiento del paciente y videollamadas periódicas. El proyecto se validó en una casa de reposo para adultos mayores y con un médico endocrinólogo en Lima, Perú. Para medir la efectividad del proyecto se evaluó lo siguiente: Constancia del registro del nivel de glucosa Tiempo promedio de respuesta ante emergencias e indicadores fuera de rango, por parte del médico Porcentaje de reducción de las complicaciones de la enfermedad Nivel de satisfacción, de los adultos mayores, sobre la solución de Telehomecare Los resultados demuestran que la frecuencia del uso de la aplicación se incrementó conforme adoptaron la solución como parte de su rutina. El tiempo de respuesta promedio del endocrinólogo fue 4,13 minutos aproximadamente desde su registro, el porcentaje de complicaciones se redujo en un 14% y el nivel de satisfacción de la solución se concentró en las dimensiones de tiempo de respuesta y comprensión de la necesidad del usuario / This project begins to seek a solution that provides remote medical care to older adults diagnosed with Type 2 Diabetes Mellitus, in order to maintain periodic control of their disease and to reduce the complications presented by non-compliance medical treatment and glycemic control. The solution consists in a mobile application for the patient that controls his disease through information related to diabetes, medications, details of the treatment recommended by the doctor and the periodic monitoring of glucose level. Another application allows the endocrinologist to monitor the patient and make periodic video calls. The project was validated in a nursing home for the elderly and with an endocrinologist in Lima, Peru. To measure the effectiveness of the project, the following was evaluated: Record of glucose level record Average response time to emergencies and indicators out of range, by the doctor Percentage reduction in complications of the disease Level of satisfaction, of the elderly, about the Telehomecare solution The results show that the frequency of use of the application increased as they adopted the solution as part of their routine. Un système de télésanté contextuel avec support de qualité de service pour le maintien à domicile / A context-aware and QoS-aware telehomecare system

Cette thèse est une thèse CIFRE entre le LORIA et la société MEDETIC et porte sur la conception des systèmes de télésurveillance pour le maintien à domicile des personnes âgées.Le système est conçu aux personnes âgées qui veulent passer leur vieillesse dans leur propre maison, à cause de son potentiel pour augmenter l'indépendance et la qualité de la vie. Cela profiterait non seulement aux personnes âgées qui veulent vivre dans leur propre maison, mais aussi le système de santé publique en coupant des prix de façon significative.Mis à part la conception d'une architecture de télésurveillance « Vill’Âge® »basée sur des réseaux de capteurs hétérogènes (Domotique, IEEE802.15.4/Zigbee, Wifi, Bluetooth), la thèse a contribué essentiellement sur la proposition d'un protocole de clustering et de routage dans le réseau de capteurs sans fil avec une approche de la logique floue, et d'un middleware pour la collecte et le traitement des données des capteurs avec la gestion de la qualité de service comme particularité.Une première plateforme de test à été développée à Colmar (MEDETIC) et une seconde, plus complète et fait suite de cette thèse, est en cours de développement au LORIA (http://infositu.loria.fr/).Nous avons participé dans le concours de ministère de l'Enseignement supérieur et de la recherche avec un projet intitulé MPIGate: « Multi Protocol Interface and Gateway for telecare, environment Monitoring and Control » et nous avons gagné le prix de ce concours au niveau d’émergence. / This thesis is a thesis CIFRE between LORIA and the MEDETIC Company and focuses on the design of telehomecare system for the elderly.In addition to the design of a remote surveillance architecture “Vill’Âge ®” based on networks of heterogeneous sensor (home automation, IEEE802.15.4/Zigbee, Wifi, Bluetooth), thesis has contributed essentially on the proposal of a clustering and routing protocol in the network of wireless sensors with an approach to fuzzy logic, and of a middleware for the collection and processing of data from sensors with the management of the quality of service as a special feature.A first platform was developed at Colmar (MEDETIC) and a second who is more complete is under development at LORIA (http://infositu.loria.fr/).By using this system, MEDeTIC, offers a new concept of smart homes for the senior citizens, named in French “Maisons Vill’Âge”. The first housing schemes are being built in 2 departments of France. A flat is entirely equipped to act as a demonstrator and as laboratory of research and development.The system is designed for the elderly who wish to spend their old age in their own home, because of its potential to increase independence and quality of life. This would not only benefit the elderly who want to live in their own home, but also the national health care system by cutting costs significantly. Based on this PhD thesis, MPIGate, a “Multiprotocol Interface and Gateway for for telecare, environment”, has been developed.
