Supply for quality care has to continuously catch up to demand. The United States’ primary care system is at a crossroads, as patients rightly demand the best in health care costs, access, and quality. How will the industry continue to deliver high quality of care with our nation’s increasing demands? What technologies can be best utilized when caring for our aging population? This thesis is built around key sections that address our country’s impending physician shortage and how our health care system can combat growing demand with better processes and technologies. First, this thesis will examine physician shortages in America and what opportunities are available to address the problem. Existing health care research has found that advanced practice providers improve the costs, access, and quality of care as a solution to the impending shortage. This review will also assess the effectiveness of advanced care providers and health systems using telehealth, and how technology can alleviate the primary care physician shortage. An analysis was also conducted to document the United States’ current ratio of physician to patients in each state, which shows that adding advanced primary care providers and telehealth services can narrow the gap between supply and demand in most states. Second, this thesis will analyze the impact of telehealth in the U.S. health care system. A literature review of telehealth’s definition and history will give a foundation of how the technology has already changed how Americans access care, especially in rural areas. This thesis is proposing that advance practice providers can deliver primary care services using telehealth. Through the review of studies and literature, in addition to interviews with industry experts, it is suggested that there is a lack of concrete evidence with which to fully assess the economic impact of telehealth. Two of the most common economic evaluation methods are cost-utility analysis (CUA) and cost-effectiveness analysis (CEA). The main x objective of CUA, which is used especially in health technology assessment, is to estimate the ratio between the cost of a health-related intervention and the benefit it produces in terms of the number of years lived in full health by the users. In health technology assessments, the benefits are usually expressed in quality adjusted life years.1

Orientador: Ana Silvia Sartori Barraviera Seabra Ferreira / Resumo: De acordo com a OMS o termo Telemedicina pode ser definido como “a oferta de serviços ligados aos cuidados com a saúde, nos casos em que a distância é um fator crítico”. Ela pode ser utilizada em várias áreas como dermatologia, cardiologia, trauma, emergência, patologia, cirurgia, psiquiatria, radiologia. Já a Telessaúde é um termo mais amplo, envolvendo diferentes profissionais de saúde de áreas como enfermagem, fisioterapia, nutrição, psicologia. Devido à atual crise financeira do país, as características geográficas e dimensões territoriais, contrastes sociais, econômicos e culturais, existência de milhares de pessoas que vivem em locais de difícil acesso e distantes de unidades de saúde especializadas, o uso da tecnologia nas áreas da Telemedicina e Telessaúde mostra-se muito eficaz para a aproximação das populações localizadas em lugares remotos; integração de profissionais e pesquisadores; melhorias na gestão dos serviços de saúde e economia de gastos públicos. It can be used in several areas such as dermatology, cardiology, trauma, emergency, pathology, surgery, psychiatry, radiology. Telehealth is a broader term, involving different health professionals in areas such as nursing, physiotherapy, nutrition, psychology. Due to the country's current financial crisis, geographical characteristics and territorial dimensions, social, economic and cultural contrasts, existence of thousands of people living in difficult to reach places and away from specialized health units, the use of technology in the areas of telemedicine and telehealth are very effective for the approximation of populations located in remote places; integration of professionals and researchers; improvements in the management of health services and savings in public spending. The purpose of this study was to describe, through an ebook, the actions of the telemedicine and telehealth services and activities developed at the Hospital das Clínicas and at the Botucatu Medical School of UNESP between 2017 and 2018. From the 1970s to the present time, the World Health Organization (WHO) explains the importance of health promotion in the world. The increased interest in promoting health and increasing health costs for the economies contributed to the recognition of health care as an important area of research. One of the most important aspects of these research currently is the Telehealth, using Information and Communication Technologies (ICT) to try to provide universal access to health. The use of Interactive Digital TV technology helps in accessing remote locations and creates the opportunity to distribute applications across their infrastructure. Dealing with the problems that arise from the convergence of these concepts is of great importance. The architectural description process helps in the development of applications and systems based on the referenced concepts. However, there are no frameworks to help the description of architectures for this particular domain. This paper proposes the ARCHealth an enterprise architecture framework created to assist the architecture development process in accordance with ISO/IEC/IEEE 42010:2011. The ARCHealth was based on the principles of primary health care, taking into account the main concerns of stakeholders and the specific area. / Desde a década de 1970 até o presente momento, a Organização Mundial da Saúde (OMS) expõe a importância da promoção da saúde no mundo. O aumento do interesse na promoção da saúde e os custos de saúde crescentes para as economias contribuíram para o reconhecimento dos cuidados à saúde como uma importante área de pesquisa. Uma das vertentes mais importantes dessas pesquisas atualmente, é a Telehealth, que utiliza das Tecnologias de Informação e Comunicação (TIC) para tentar prover acesso universal à saúde. O uso da TV Digital Interativa auxilia no acesso a locais remotos e cria a oportunidade de distribuir aplicações através de sua infraestrutura. Lidar com os problemas que surgem a partir da convergência desses conceitos é de grande importância. O processo de descrição de arquitetura auxilia no desenvolvimento de aplicações e sistemas baseados nos conceitos referenciados. Porém, não existem muitos frameworks que auxiliem a descrição de arquiteturas para esse domínio específico. Este trabalho propõe o ARCHealth, um enterprise architecture framework criado para auxiliar o processo de desenvolvimento de arquitetura em conformidade com a norma ISO/IEC/IEEE 42010:2011. O ARCHealth foi elaborado com base nos princípios de Atenção Primária à Saúde, levando em consideração as principais preocupações das partes interessadas e do domínio específico.
