TITLE: PHYSICAL THERAPISTS' CLINICAL PRACTICES REAGARDING INTRINSIC AND EXTRINSIC FALL RISK FACTORS AND THEIR ATTITUDES TOWARD THE USE OF EVIDENCE-BASED PRACTICE MAJOR PROFESSOR: Dr. Stephen Brown The phenomenon of falls among community-dwelling adults--coupled with an aging baby boomer generation and an increasing life expectancy--presents a significant concern for an increased number of unintentional deaths and injuries and their associated costs. The risk factors associated with falling are often categorized as intrinsic and extrinsic. Physical therapists have a unique opportunity to positively impact issues involving physical dysfunction and to educate their community-dwelling adult patients about the environmental risk factors and interventions that lessen their risk of falling. Abundant evidence-based research exists regarding interventions for the treatment and prevention of falls; however, this research indicates that physical therapists fail to consistently utilize evidence-based practice (EBP) in their daily clinical practices. The diffusion of innovations theory examines how innovations are adopted (Rogers, 2012). However, the innovation of EBP is not always adopted by physical therapists. Lack of time to conduct literature searches was the most common barrier noted by physical therapists for not adopting EBP (Jette et al., 2003; Fruth et al., 2010; Salbach, Jagial, Korner-Bitensky, Rappolt, & Davis, 2007). This study, which utilized a cross-sectional descriptive research design, provided insight into physical therapists' clinical practices regarding intrinsic and extrinsic fall risks in the treatment of community-dwelling adults aged 65 years and older. It examined physical therapists' attitudes and beliefs toward the use of EBP and identified the barriers to their adoption of it. The demographic data provided a descriptive overview of the study respondents. There were 3,523 potential physical therapist respondents, and the study's return rate was 9% (316 respondents. The majority of the respondents held doctoral degrees (49.4%), more than half (55.4%) worked in an outpatient physical therapy clinical setting, and approximately half indicated that they were American Physical Therapy Association (APTA)-certified instructors. The results of the study indicated that physical therapists who had more experience displayed a higher level of attention to clinical practices than those with less experience. The physical therapists who were APTA-certified clinical instructors demonstrated a higher level of attention to the intrinsic and extrinsic risks of falling than those who were not APTA-certified instructors. The physical therapists whose highest level of education was a doctorate placed greater importance on the utilization of EBP than respondents with a baccalaureate or master's degree. Although most physical therapists believe that the utilization of EBP holds significant value, they do not always access or apply it. Insufficient time for using EBP was the major barrier noted by most physical therapists. The results of this study concurred with those of previous studies regarding common barriers to physical therapists' adoption of EBP. Rehabilitation organizations may want to examine methods to promote the use of the most current physical therapy practices based on the evidence revealed in the literature and to explore options for improving staff access to and utilization of EBP research.

Physical Therapy Utilization and Length of Stay among Patients with Low Back Pain in Florida Hospitals

Purpose: The purpose of this work was identify key factors associated with inpatient physical therapy utilization and length of stay for patients with low back pain (LBP) in Florida hospitals. Rationale: Little is known about factors associated with inpatient physical therapy or length of hospitalization for patients with LBP. This group of works identified the key factors associated with inpatient physical therapy and long lengths of hospitalization for this patient population. Since physical therapy and reduced length of stay are known cost-reducers, identifying key factors may represent significant cost savings to the health care system. Methods: Several mixed method procedures were utilized to examine physical therapy utilization and length of hospitalization between the years of 1992 and 2014. Policy, patient and hospital characteristics, as well as, hospital procedures during a patient’s stay were examined as contributors to either physical therapy utilization or length of hospitalization. Conclusion: Many factors are associated with inpatient physical therapy utilization and length of stay for patients with LBP in Florida hospitals.

A relational-phenomenological study of existential counselling psychologists' experience of and working with motivation in the inter-subjective space between them and their clients

This investigation explored the phenomenon of motivation in the inter-relational space between therapists and clients. Its aim was to give voice to a small group of existential therapists who were trained and practise counselling psychology in the UK. Literature review focused mainly on the theoretical conceptualisation of motivation and its therapeutic implications across different modalities. Using a Relational-Phenomenological Approach, the investigation looked at the dialogical and co-creative nature of motivation in therapy. Themes emerged were indicative of motivation existing as multiple differently motivated selves in both therapists and clients. Differently motivated selves are characterised by their inner- and inter- relationality and give rise to kaleidoscopic relational dynamics in the inter-subjective space. Relational features such as openness, ability to stand on the edge with clients and total acceptance of the is-ness of the moment seem to co-create fertile ground for motivational transformations. I also experienced the co-creational, dialogical and motivationally transformational nature of differently motivated selves throughout the research process, which points at the multi-directional nature of motivation. Awareness of motivation is multifaceted and differently motivated selves and is interactional and dialogical, whereby the therapist cannot remain neutral can be useful in Counselling psychology.

Therapists who practice mindfulness meditation : implications for therapy

In the past decade the healing potential of mindfulness and its practice has gained widespread recognition across various health disciplines and institutions, especially mental health. Past and current research on mindfulness interventions have focused almost exclusively on the beneficial effects for clients. However, there is a serious shortage of research on how mindfulness practice influences therapists and their work. The current study looked specifically at how the influence of mindfulness meditation (MM) was experienced by therapists in the context of their work. An interpretive description methodology was used to guide the research process. Semi-structured in-depth interviews were conducted with six therapists who practiced MM regularly. A thematic analysis of interview transcripts highlighted commonalities and differences among participants’ perceptions of the influence of MM on their work. Eleven themes emerged from the data analysis. Thematic findings were considered in relation to key issues in psychotherapy, master therapist traits and other contemporary qualitative research addressing the influence of MM on practitioners. The process of relational play therapy between a trainee therapist and a maltreated child : a case study

Research in the field of attachment theory and object relations theory has indicated that early attachments between a child and his or her primary caregiver have significant implications for the development of that child. Early relationships begin to shape the child’s sense of self and other and healthy relationships lead to secure attachments. However, children who encounter early maltreatment or a disruption in caregivers are particularly vulnerable to developing insecure attachments and a disrupted sense of self and other, which has consequences for their subsequent psychological development. In the South African context, increasing numbers of children are being orphaned or placed in formal foster care with many children at risk for insecure attachments. This has implications for therapeutic work with an increased need to promote secure attachment relationships and a stable sense of self and other. The current case study aimed to describe the relational experience of play therapy that took place between a maltreated five year old female child and a female trainee therapist with this purpose in mind. The therapeutic process was embedded within a relational therapy framework which included object relations and attachment theory. The researcher made use of a qualitative descriptive dialogic research approach to conduct the research. The data were analysed using content analysis, where the play therapy sessions were analysed according to concepts relating to Fairbairn’s (1963) object relations theory as well as Winnicott’s (1965) object relations theory. Prominent themes that emerged included the role of the holding environment, splitting of good and bad objects and the presence of a false self versus a true self. In addition, the conflicting presence of two repressed ego structures, namely the libidinal and antilibidinal ego structure were noted throughout the therapeutic process.
