Tourette's Syndrome from a social work perspective : an exploratory study

Tourette's syndrome (TS) is a chronic movement disorder characterized by motor and vocal tics. This report looks at TS through the eyes of three families and two adults diagnosed with the syndrome. The social and psychological reactions and adjustment to the disorder are examined. Three approaches were used to explore the issues: a literature review; field notes and unstructured biographical interviews. Social and psychological reactions include: depression, withdrawal and suicidal ideation. Unresponsiveness on the part of the health care and social service systems contributed to feelings of anger, powerlessness, guilt and shame. The families' and individuals' ability to lobby for services was an important factor in enhancing adjustment. TS is a multi-faceted disorder; it serves as a paradigm for issues and problems that are best served by a multi-disciplinary approach. Services must be coordinated; a case management approach would facilitate efficient coordination and delivery of services.

The etiology of Tourette syndrome has been elusive for researchers ever since its discovery, making treatment especially difficult. After proving the disorder was organic in the second half of the 20th century, researchers have been creating theories of the underlying neural basis for Tourette’s symptomatology. These theories include abnormalities in brain structure, dysregulation of the dopamine system, dysregulation of the serotonin system and overall neurotransmitter system interactions. The etiology is likely a complex combination of all of these. Treatments for this disorder include pharmacological, behavioral and surgical. I believe the best approach for treatment is behavioral first, followed by pharmacological if behavioral does not work, and then surgical as a last resort if the previous two do not show results.

M.Ed. (Educational Psychology) / The purpose of this study was to determine if family therapy has potential in handling the behaviour problems of children with Tourette syndrome. This study was approached within a system theoretical framework and from an educational psychological perspective. The study was presented as follows: Chapter 1: Conceptualization, formulation of the problem and objectives. Chapter 2: A literature study regarding the spectrum of Tourette syndrome. Chapter 3: A literature study regarding the systemic approach on family therapy. Chapter 4: The methodology used in the study. Chapter 5: A qualitative-idiographic evaluation of the study. Chapter 6: Summary, conclusions and recommendations. Findings: Family therapy has potential for the successful treatment of the behavioral problems of children with Tourette syndrome. Conclusions: According to the literature study and the findings of this research, the behavior problems of children with Tourette syndrome can be treated successfully with family therapy. Family interactions as a whole show improvement, which in tum leads to the better handling of behavioral problems. These findings may be attributed to the small number of families involved in the study - thus more research in this field is needed.

GENOMEWIDE GENE EXPRESSION ANALYSIS TO INVESTIGATE SYMPTOM ONSET AND EXACERBATION IN TOURETTE SYNDROME

Tourette Syndrome (TS) is a neurodevelopmental disorder that starts in childhood. It is marked by multiple motor and vocal tics and a fluctuating course with recurrent time periods of symptom exacerbation followed by symptom remission. TS is considered a disorder of complex etiology that involves the interaction of multiple genetic and environmental factors. Previous studies have implicated regulation of the immune system in TS patients, although this remains a matter of debate. <br>Here we present a genome-wide gene expression study comparing data from TS patients at time points of symptom exacerbation and remission as well as upon symptom onset for newly diagnosed patients, with differentially gene expression analysis, co-expression analysis and pathway analysis. Although the fold change of each regulated gene was very mild, it is the first time that changes in the regulation of the immune system have been implicated at different time points in TS patients.<br><br>
