Screening of traditionally used South African medicinal plants against Candida albicans.

Candida species were discovered more than a century ago as a causative organism of oral thrush. In HIV patients, the presence of oral candidiasis has been shown to be the earliest opportunistic infection. Candidiasis lesions associated with HIV infections are primarily a reflection of the specific change of the host's immune response caused by the virus. Studies of AIDS all over the world show that 58-81% of all patients contract a fungal infection at some time during the primordial stage or after developing AIDS and 10-20% have died as a direct consequence of fungal infections. Twenty four South African medicinal plants were screened using a modification of the NCCSL broth microdilution antifungal test against Candida albicans standard strain ATCC 10231 and two clinical isolates from a 5-month- old baby and an adult. This assay was performed in order to find a traditional remedy to treat oral candidiasis. Of all the screened plants Allium sativum L., Glycyrrhiza glabra L., Polygala myrtifolia L. and Tulbaghia violacea L. aqueous extracts were found to have the best activity. Allium sativum and Tulbaghia violacea aqueous bulb extracts had MIC values of 0.56 mgml-1 and 3.25 mgml-1 respectively, whilst Polygala myrtifolia leaf extracts and Glycyrrhiza glabra rhizome extracts had MIC values of 1.56 mgml-1 and 3.25 mgml-1 respectively when tested against the isolate from a 5-month-old baby, which was the most susceptible of the isolates used. All the extracts had higher MIC values against the standard strain (ATTC 10231), which was the least susceptible to the extracts used. Stability testing was performed on fresh aqueous extracts of A. sativum, G. glabra, T. violacea and P. myrtifolia stored at 4°C, 23°C and 33°C over a period of one week, to determine the stability of the extracts in solution. All A. sativum extracts maintained stability for three days in solution, whilst T. violacea extracts remained stable for only two days in solution. TLC fingerprinting of A. sativum and T. violacea extracts indicated the presence of the known antibacterial and antifungal compound allicin. The activity of allicin and other active compounds was observed by using the bioautographic assay, which was performed on these extracts. P. myrtifolia and G. glabra extracts lost stability 24 hours after preparation at all tested temperatures. However, it was clear with the four plant extracts tested that storage of solutions at higher temperatures reduced their activity and stability. The unpleasant taste and smell of A. sativum and G. glabra could however not be masked, since the intake of these two extracts would result in HIV patients being recognised. These two plants where therefore not considered for further investigation. G. glabra and P. myrtifolia are both saponin containing plants. These could be the active constituents responsible for the anticandidal action. G. glabra is known for its biological activity as an antibacterial agent, whilst other Polygala species have been reported to possess antifungal saponins. Although P. myrtifolia and G. glabra are not stable for more than 24 hours, they do not have an unpleasant smell or taste. Isolation and characterisation of secondary metabolites of two asteraceae species, Artemisia afra and Elytropappus rhinocerotis.

In this study two medicinal plant species, namely Artemisia afra Jacq. ex. Willd and Elytropappus rhinocerotis Less. (L.f) (= Dicerotamnus rhinocerotis Koekemoer), both belonging to the family Asteraceae, have been investigated and different compounds isolated and characterised. Both species are important plants used in traditional medicine in general in Africa and particularly in South Africa. A. afra, commonly called "African wormwood" is one of 400 species belonging to the genus Artemisia and it is the only one indigenous to Africa. E. rhinocerotis is one of eight Elytropappus species which are all restricted to the Cape floristic region. The aim of this study was to investigate the phytochemistry of these species. In total fifteen compounds were isolated and characterised. From the E. rhinocerotis extract, four known compounds, labdanolic acid, methyl labdanolate, 6, 7- dimethoxycoumarin and a sesquiterpene viridiflorol were isolated. These compounds were not previously reported from E. rhinocerotis. Two different chemotypes of A. afra were studied and eleven compounds were isolated. These compounds include sesquiterpenes such as taurin, artesin, maritimin, artemin, norsantolinifolide, santolinifolide A, and reynosin, a flavonoid, 5- hydroxy-7,4-dimethoxyflavone, a coumarin called scopoletin or 7-hydroxy-6- methoxycoumarin and other aromatic compounds such as p-hydroxyacetophenone, and 2,4-dihydroxy-6-methoxyacetophenone. Except for taurin, scopoletin and 5-hydroxy-7, 4- dimethoxyflavone , none of these other compounds has been reported previously from A. afra. This study has shown that A. afra contains a large number of sesquiterpenoids, mostly from the eudesmane-type. Structural elucidation of different compounds was performed using mainly NMR spectroscopy. Other methods used for identification include LC-MS and infrared spectroscopy. The major compound, labdanolic acid, is known to selectively inhibit cyclooxygenase-2, an enzyme associated with inflammation. A phytochemical investigation of two South African plants with the screening of extractives for biological activity.

Two South African medicinal plants, Strophanthus speciosus and Eucomis montana, were investigated phytochemically. From Strophanthus speciosus a cardenolide, neritaloside, was isolated, whilst Eucomis montana yielded three homoisoflavanones, 3,9- dihydroeucomin, 4'-demethyl-3,9-dihydroeucomin, and 4'-demethyl-5-0-methyl-3,9- dihydroeucomin. The structures were elucidated on the basis of spectroscopic data. The homoisoflavanones were screened for anti-inflammatory activity using a chemiluminescent luminol assay, modified for microplate usage. All of the homoisoflavanones exhibited good inhibition of chemiluminescence, with IC50 values for 3,9-dihydroeucomin, 4'-demethyl-3,9-dihydroeucomin, and 4'-demethyl-5-0-methyl-3,9- dihydroeucomin being 14mg/mL, 7mg/mL, and 13mg/mL respectively. The IC50 value of 4'-demethyl-3,9-dihydroeucomin compared favourably with the NSAID control (meloxicam), which had an IC50 of 6mg/mL. Neritaloside was not screened for biological activity as the yield of 14.4mg was insufficient for the muscle-relaxant screen for which it was intended. An assay for antioxidant/free radical scavenging activity was also performed. All the compounds had excellent antioxidant/free radical scavenging activity, with percentage inhibition of the reaction being 92%, 96%, and 94% for 3,9-dihydroeucomin, 4'-demethyl- 3,9-dihydroeucomin, and 4'-demethyl-5-0-methyl-3,9-dihydroeucomin respectively at a concentration of 10mg/mL. However, the control compounds, diclofenac and meloxicam, also exhibited strong activity, with the result that the precise mode of anti-inflammatory activity could not be unequivocally determined. The results from the biological screenings thus provided a rational scientific basis for the indigenous ethnomedicinal use of Eucomis species in the treatment of rheumatism, inflammation and pain. / Thesis (M.Sc.)-University of KwaZulu-Natal, 2006.

A homoeopathic drug proving of Acacia xanthophloea 30CH with a subsequent comparison to its use in African medical tradition

Submitted in fulfillment of the Master’s Degree in Homoeopathy, Department of Homoeopathy, Durban University of Technology, Durban, South Africa, 2016. / Aim The aim of this homoeopathic proving study was to determine and document the arising symptomatology of Acacia xanthophloea (Fever tree) in the potentised homoeopathic form, 30CH, and to provide this data for inclusion to the homoeopathic materia medica. The results of this proving study and comparative analysis to African traditional medicinal uses of this substance confirms the potential therapeutic value of the remedy. Methodology The homoeopathic drug proving of Acacia xanthophloea 30CH was conducted in the form of a double blinded placebo controlled study. The investigation consisted of a total of 30 provers divided equally between two researchers ((A. Gobind and G. Zondi). The sample was randomly divided into two groups in which 24 provers(80%) were assigned into the verum group and the remaining six provers (20%) were allocated to the placebo group. All provers were requested to record their daily symptoms on the physical, mental and emotional planes in their journals for one week prior to administration of the proving substance. This formed as a mode of control for the comparison of symptomatology for the pre-proving and post proving period. A thorough case history was taken and physical examination performed on each prover before the commencement of the proving and after the duration of the proving period. Each prover received a total of nine powders. Starting on day 8 of the study the provers consumed one powder three times a day for three days and documented their daily symptoms in a journal. The duration of the proving term was six weeks in total. During this interval the researcher maintained consistent contact with the provers. Upon completion of the proving period all journals were collected and the information contained within these journals was translated into the materia medica and repertory format. This facilitated the establishment of the remedy portrait of Acacia xanthophloea 30CH. A subsequent comparison between the symptomatology that materialised in the provers and the African traditional medicinal uses was duly conducted. Results An extensive range of symptoms was reported by the provers. The outstanding themes that emanated from this proving on the mental plane include anger, anxiety, aversion to company, cheerful, depression, irritability, mood swings, restlessness, tranquillity and stress. A broad range of headaches were described with some headaches being associated with the eye. There were many eye symptoms displayed by the provers which include itching, redness, burning sensation and pain. The stomach symptoms revealed marked increased thirst, changeable appetite, bloating, constipation and diarrhoea with watery stools.The female genitalia / sex indicated several symptoms ranging from painful menstruation, bleeding and copious blood flow.The greatest number of symptoms in a system was associated with extremities, producing the greatest number of rubrics in the repertory section. Dream themes depicted by the provers were especially visionary, about family and friends in addition to other themes. The correlation process between the homoeopathic drug proving of Acacia xanthophloea 30CH and the African traditional medicinal use of the substance brought several resemblances to light.There were clear similarities with the eye symptoms, gastrointestinal symptoms and headaches. Conclusion As hypothesised Acacia xanthophloea 30CH did produce distinctly observable signs and symptoms when administered to healthy provers. The symptoms that emerged during the proving provide evidence that an overlay exists between the remedy Acacia xanthphloea 30CH and the traditional use of the crude substance Acacia xanthophloea. The researcher proposes that further research should be conducted to determine the symptomatology of various homoeopathic potencies so that a complete image of the remedy Acacia xanthophloea 30CH can be established and the clinical applications can be broadened. / M

The aim of this diploma thesis was to define the basic features of traditional Mongolian treatment guidelines, with an emphasis on traditions of non-buddhist treatment. The main attention was paid to the healing method "dom", which stands on the border between folk medicine and magic, and is part of Mongolian cultural traditions. The first chapter "Classification of Mongolian traditional medicine" is dedicated to arranging the division of medical techniques in Mongolian territory and tries to set the techniques into historical context. The second chapter "Healing Tradition in the territory of Mongolia" focuses on the description of each treatment technique. The third chapter is devoted to the treatment of the Mongolian tradition called "dom". Since the Western literature refers to "dom" as magic, attention is paid first to magic. The second part consists of interviews on the issue and the definition of "dom" recorded in Mongolia and the final part follows with concrete examples of the "dom" set within the wider context of traditional methods of medical treatment. Primary mental health care in South Africa

M.A. / The research describes the different methods followed by traditional healers and western professionals in dealing with mental illness. The study is intended to provide an in-depth analysis of the process involved when the mentally ill patient consult a traditional healer in the community and a western professional at the local clinic in Emzinoni. The study also intends to examine whether or not the two approaches can complement each other. In this research, it was found that the mentally ill patient would consult either a traditional healer or a western professional worker. In some cases, when the patient has consulted the traditional healer and his/her condition does not improve, he/she resorts to western medicine. The same applies to the patients, who have consulted a western professional. If his/her condition does not improve, he/she consults the traditional healer. The study showed that patients who are mentally ill do not use medicines from both traditional healers and western professional at the same time. The patient chooses the treatment that makes him/her become better. It was recommended in the study that traditional healers and western professionals should develop a referral system.
