Vergleich der 24 h Plaque Formation Rate (PFR) auf Keramikkronen von Zähnen und osseointegrierten Implantaten bei Patienten mit chronischer Parodontitis / Comparison of 24-hour plaque formation rate (PFR) on ceramic crowns of teeth and osseointegrated implants in patients with chronic periodontitis

Über die Stärke und Geschwindigkeit der Neubildung bakterieller Biofilme an implantatgetragenem Zahnersatz sind bis lang nur wenige Informationen ver-fügbar. Ziel der durchgeführten Untersuchungen war es daher, die Plaqueneubildungsrate am natürlichen Zahn und Zahnimplantat mit Hilfe der Bestimmung der 24 h Plaque Formation Rate quantitativ zu erfassen und zu vergleichen. 35 Patienten im Alter von 48 - 75 Jahren mit parodontal vorgeschädigtem Ge-biss, welche Keramikkronen auf Zähnen wie auch auf Implantaten aufwiesen, nahmen an der Studie teil. In einem split-mouth Design wurden Metallkeramikkronen, welche einem enossal integrierten Titanimplanat aufsa-ßen, mit Metallkeramikkronen, die natürlichen Zähne aufsaßen und sich im gleichen oder kontrallateralen Kiefersextanten befanden, paarweise zugeordnet. Eine Erfassung der klinischen Parameter Gingiva Index (GI), Papillen-Blutungs-Index (PBI), Probing Attachment Level (PAL) sowie Probeable Pocket Depth (PPD) diente der Dokumentation bekannter, das Wachstum oraler Biofilme modifizierender Faktoren. Nachfolgend wurde das gesamt Gebiss professionell von bakteriellen Biofilmen gereinigt und die Studienteilnehmer angewiesen, für 24 h auf jegliche häusliche Mundhygienemaßnahmen zu verzichten. Nach 24 h Mundhygienekarenz wurde die 24 h Plaqueneubildungsrate (24 h PFR) im gingivanahen Kronendrittel für alle Zahn-Implantat-Paare bestimmt. Die Analyse der Daten offenbarte, dass die 24 h PFR überkronter natürlicher Zähne im Mittel 51,6% betrug und sich signifikant vom 24 h PFR Mittelwert der verpaarten Kronen auf Implantaten mit 32,6% unterschied (p<0,001). Die beobachteten Unterschiede zwischen beiden experimentellen Gruppen bezüglich der klinischen Parameter GI, PBI, PAL sowie PPD waren eher gering und konnten statistisch nicht verifiziert werden. Die Befunde dieser Untersuchung belegen daher, dass bei geringer oder fehlender Entzündung der umgebenden Weichgewebe die Plaqueneubildungsrate im gingivanahen Kronendrittel implantatgetragener Metallkeramikkronen signifikant geringer ausgeprägt ist als auf Metallkeramikkronen, die natürlichen Zähnen aufsitzen. Eine mögliche spekulative Erklärung hierfür könnte in der geringeren Stärke des Austritts nährstoffreichen Sulkusfluids aus dem periimplantären Sulkus bei entzündungsfreiem Zustand der umgebenden Weichgewebe zu suchen sein. Die Daten dieser Studie legen zudem nahe, dass eine übliche, 2 x täglich wiederholte, sorgfältige häusliche Mundhygiene auch für die wirksame Reinigung implatatgetragener Metallkeramikkronen von aufgewachsenen bakteriellen Biofilmen adäquat ist. / Microbial biofilms are closely associated with the etiology of periodontal disease and periimplantitis, thus stressing the necessity for effective plaque control in teeth and implants. The purpose of this study was to compare the 24 h Plaque Formation Rate (PFR) after professional tooth cleaning on the apical third of ceramic crowns at-tached to teeth and osseointegrated implants. 35 patients suffering from mild to moderate periodon-titis and displaying at least 1 osseointegrated implant (in place for > 1 ≤ 5 years; PPD ≤ 4 mm) restored with a single ceramic crown abutment as well as a matching number of teeth restored with a single ceramic crown in the same or the contralateral sextant participated in the study. After complete removal of supragingival plaque on all teeth and implants by air polishing with glycine powder patients were asked to refrain from oral hygiene for the next 24 h. 1 day later teeth and implants were stained with a plaque-disclosing solution (mira 2-tone; Hager & Werken, Germany) and the extent of plaque regrowth was recorded on the apical third of the crown surface divided into 6 segments – 1. mesiobuccal, 2. buccal, 3. distobuccal, 4. mesiooral, 5. oral, 6. distooral. Based on the number of segments with visible new plaque formation the percentage of plaque coverage was calculated: no segment with visible plaque = 0% 6 segments with visible plaque = 100% Data analysis revealed that 24h PFR was significantly higher on ceramic crowns cemented on natural teeth when compared to the plaque formation rate on ceramic crowns on implants (p<0.01). mean SD 24 h PFR Ceramic Crown Teeth 51.6% ±21.5 24 h PFR Ceramic Crown Implants 32.6% ±20.1 Conclusion: In patients suffering from mild to moderate periodontal disease the observed difference in 24 h plaque formation rate between ceramic crowns on teeth and implants suggests a major influence of the sulcular fluid rate on the extent of 24h plaque regrowth on tooth surfaces adjacent to the gingival margin.

Effect of Safflower Oil on the Protective Properties of the in situ Formed Salivary Pellicle

Aim: The prevalence of dental erosion is still increasing. A possible preventive approach might be rinsing with edible oils to improve the protective properties of the pellicle layer. This was tested in the present in situ study using safflower oil. Methods: Pellicle formation was carried out in situ on bovine enamel slabs fixed buccally to individual upper jaw splints (6 subjects). After 1 min of pellicle formation subjects rinsed with safflower oil for 10 min, subsequently the samples were exposed in the oral cavity for another 19 min. Enamel slabs without oral exposure and slabs exposed to the oral cavity for 30 min without any rinse served as controls. After pellicle formation in situ, slabs were incubated in HCl (pH 2; 2.3; 3) for 120 s, and kinetics of calcium and phosphate release were measured photometrically (arsenazo III, malachite green). Furthermore, the ultrastructure of the pellicles was evaluated by transmission electron microscopy (TEM). Results: Pellicle alone reduced erosive calcium and phosphate release significantly at all pH values. Pellicle modification by safflower oil resulted in an enhanced calcium loss at all pH values and caused an enhanced phosphate loss at pH 2.3. TEM indicated scattered accumulation of lipid micelles and irregular vesicle-like structures attached to the oil-treated pellicle layer. Acid etching affected the ultrastructure of the pellicle irrespective of oil rinsing. Conclusion: The protective properties of the pellicle layer against extensive erosive attacks are limited and mainly determined by pH.
