Efecto del masaje funcional en la actividad neuromuscular y el rango de extensión de rodilla

Los propósitos de este estudio fueron determinar los cambios producidos por la aplicación del masaje funcional, en la longitud y en la actividad neuromuscular en el músculo semitendinoso, los que se objetivaron con electrogoniometría y electromiografía respectivamente. El presente estudio es de tipo cuasiexperimental, con un grupo de estudio, en el cual se utilizó una muestra de veintidós sujetos sanos, con un promedio de edad de 20.3 ± 7 años, sin lesiones musculoesqueléticas en la articulación de rodilla, que presentaron acortamiento de la musculatura isquiotibial, la cual se definió con una flexión de rodilla mayor a 15º, evaluada con el test de extensión pasiva de rodilla. Al grupo de estudio se le realizó la intervención de masaje funcional en la extremidad inferior derecha específicamente en la zona del muslo, siendo la extremidad inferior izquierda el parámetro de control. Se midió la actividad neuromuscular en el músculo semitendinoso a través de electromiografía de superficie y el rango articular de extensión de rodilla con un electrogoniómetro, con cadera flectada en los 90º, las mediciones se realizaron antes y después de la aplicación de la técnica de masaje funcional. Para los datos obtenidos se usó un cálculo de variaciones, diferencias de variaciones, promedios y desviación estándar. Los resultados para la electromiografía del músculo semitendinoso, muestran que en promedio hay un aumento de la actividad neuromuscular con una variación antes- después de -170,45 nV para la extremidad inferior derecha y 85,23 nV para la izquierda. El rango de extensión de rodilla aumentó en ambas extremidades inferiores, en la izquierda la variación fue de 2° y en la derecha de 7°. Se utilizó la prueba de Wilcoxon, donde se determinó que tras la aplicación de masaje funcional los cambios producidos en el rango articular de extensión de rodilla de la extremidad inferior izquierda, derecha y la variación entre ellas, fueron estadísticamente significativos, la variación en la amplitud articular se atribuye a la aplicación del test de extensión pasiva de rodilla y a la movilidad pasiva realizada con la técnica respectivamente. Con respecto a la actividad electromiográfica, los cambios no fueron estadísticamente significativos para ambas extremidades y tampoco para la variación entre ellas debido a la una gran dispersión de los datos por lo tanto se concluye que la aplicación del masaje funcional no produce un efecto a nivel neuromuscular. / The purposes of this study were to determine the changes produced by the application of functional massage, in the length and neuromuscular activity of the semitendinosus muscle, which was objectified with electrogoniometry and electromyography, respectively. This type of study it is descriptive and cuasi experimental, with a group of study of twenty two healthy subjects, with a mean age of 20.3 ± 7 years, without muscle esqueletal injuries in the knee joint, who showed a shortening of the hamstring muscle wich was defined with a knee flexion better than 15º evaluated with the passive knee extensión test. To the group of study was performed the intervetion of functional massage in the right leg and the left was the control parameter. Was measured the electromyographic activity in the semitendinosus muscle through superficial electromyography and the range of movement in the knee extension with an electrogoniometer with hip flexion in 90º, the measurements was performed before and after the application of the functional massage technique. From the data obtained, variations, differences in variation, averages and standard deviation were calculated. The results for the electromyography of the semitendinosus muscle, show that in average for the extremity is an increase of the neuromuscular activity with a variation before- after -170,45 nV right inferior and 85.23 nV for the left. The rank of extension of knee increased in both extremities inferiors, in the left the variation was of 2° and in the right of 7°. The test of Wilcoxon was used, where it was determined that after the application of functional massage the changes produced in the rank to articulate of extension of knee of the extremity left, right inferior and the variation among them, were statistically significant, the variation in the amplitude to articulate is attributed respectively to the application of the test of passive extension of knee and to realised passive mobility with the technique. With respect to the electromyography activity, the changes were not statistically significant for both extremities and for the variation among them due to a great dispersion of the data therefore it does not conclude either that the application of the functional massage does not produce an effect at neuromuscular level.

Cambios plásticos de adaptación ultra estructural de las conexiones nerviosas motoras en el adulto en respuesta a un incremento fisiológico de la actividad

Es conocido que las sinapsis cambian en condiciones de normalidad durante el desarrollo que culmina en la maduración. Sin embargo, se han identificado cambios funcionales de corta duración actividad-dependientes en adultos normales. Así en el decremento de uso aparecen fenómenos de crecimiento axonal por elongación de algunos segmentos axonales. Y en el incremento de uso, dominan los fenómenos de retracción de algunas ramas axonales. En este contexto, el presente trabajo tiene como pretensión final conocer de forma clara la capacidad de adaptación plástica de las conexiones neuromusculares en el adulto normal.MATERIAL Y METODOS: Se utilizó el músculo Extensor digitorum longus (EDL) de ratas Sprague-Dawley macho, de 12 semanas de edad. Los animales fueron sometidos a un moderado incremento de actividad locomotora mediante una cinta sin fin. Las bandas de inervación de los músculos EDL fueron procesadas para microscopia electrónica de transmisión convencional. Con las imágenes obtenidas (a 5000x y 10000x) se realizo un estudio morfométrico con el programa semiautomático de análisis de imágenes MOP VIDEOPLAN.CONCLUSIONES MÁS RELEVANTES:1- La retracción axonal en músculos control es compensado por los siguientes cambios en estructuras relacionadas con la neurotransmisión: mas y mayores vesículas sinápticas y también incrementa el numero de vesículas que están en contacto con el axolema sináptico; mas zonas activas axonales; las membranas densificadas de las crestas postsinápticas decrecen en longitud pero esto es compensado por un mayor número.2- Los resultados morfométricos indican claramente que pequeños cambios en el nivel de actividad locomotora inducen a precisas adaptaciones morfológicas de una forma rápida. El incremento en las densificaciones postsinápticas sugiere un mecanismo compensatorio postsináptico a la retracción axonal. / The activity pattern of neuromuscular junctions can influence the structure, growth and remodeling responses and differentiation of the motor nerve terminals. Nerve terminal remodeling involving changes of growth and retraction in the normal adult are affected by the level of the animal's activity. In fact, nerve terminal retraction in the adult can be induced by the level of increased synaptic activity. In the present work we tried to analyze if a small physiologic increase in the locomotor's activity of the normal adult rats during a brief period of time can induce pre- and postsynaptic ultrastructural adaptations.MATERIAL AND METHODS: The Extensor Digitorum Longus (E.D.L.) muscles of 24 three-month-old male Sprague-Dawley rats were studied. The experimental animals spontaneously adapted to the moderately exercise by training a treadmill. Normally housed rats were used as controls. The neural zones from the right and left E.D.L. muscles were fixed in glutaraldehyde and conventionally processed for T.E.M. Cross-sectioned different end plates were directly recorded in magnetic support with a high definition video system. Morphometric analysis of pre- and postsynaptic structural parametres was performed with a semiautomatic image analysis system (MOP VIDEOPLAN).MAIN CONCLUSIONS: 1- Axonal retraction in normal muscles can be compensated by the following changes in the neurotransmission-related structures: Synaptic vesicles increase in diameter and also increase the number of synaptic vesicles in contact with the synaptic axolemma; active zones increase in number; postsynaptic membrane densifications decrease in length and that is compensated by an increase in number of these.2- The morphometric data clearly indicate that small and early physiological changes in the locomotor's activity in the normal adult are followed by precise morphologic adaptations. The increased differentiation of postsynaptic membrane densifications would suggest the existence of the postsynaptic compensatory mechanism for the presynaptic retraction.
