Spontaneous Adverse Drug Reaction Reporting by Health Consumers in Canada: A Multi-Methods Study

Monitoring adverse drug reactions (ADRs) through pharmacovigilance is vital to health consumer safety. Health Canada accepts ADR reports from physicians, health consumers, lawyers and manufacturer. This study uses a multi-methods approach to evaluate health consumers ADR reporting to pharmacovigilance programs. Methods: Guided by Risk Perception Theory, this study involved three phases. 1) A systematic review to identify factors influencing health consumer ADR reporting 2) An observational study to compare health consumer and physician ADR reports received by Health Canada from 2000-2014 to determine differences in seriousness, system organ class, and anatomical therapeutic class. 3) A qualitative study to explore health consumers’ experiences with ADR reporting and usability of the Canadian Vigilance ADR system. Results: 1) Of 1435 citations identified, 22 studies were eligible. Common barriers for health consumers to report ADRs were poor awareness and confusion on who should report. Common motives were preventing ADRs in others and wanting feedback. 2) Of 198,781 ADR reports, 57,078 (29.0 %) were from health consumers and 52,843 (27.0%) from physicians. Compared to physicians, health consumers reported significantly more ADRs (serious and non-serious) (p<0.0001) and reported differently for organ systems affected and by types of medication. 3) Interviews from 15 adult health consumers revealed barriers (e.g., poor awareness about the available reporting systems) and motives for reporting ADRs (e.g., intolerable side effect impacting daily activities). Few were aware of the Canadian Vigilance reporting system and usability was limited by the number and complexity of questions. Conclusion: Health consumer role in directly reporting ADRs is influenced by several factors. In Canada, most were unaware of the Canadian Vigilance System and unclear about their role in reporting ADRs. Quarters of Canadian ADRs are reported by health consumers and their ADRs had different characteristics from physicians. These findings are relevant to policymakers, public health officials, and regulatory agencies for improving medication safety in Canada and worldwide.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A pesquisa analisa a criação de adversidade através da materialidade crua e rústica utilizada nos Bólides de Hélio Oiticica. O papel da materialidade tornou-se evidente a partir da percepção de que os ditos objetos realizam um convite àquilo que chamamos de participação adversa. Ao mesmo tempo em que eram objetos capazes de despertar a curiosidade do espectador e incitá-lo à manipulação, o faziam através do toque de substâncias ásperas e pouco suaves, como por exemplo, pedaços de madeira mal pintada, telas de nylon, conchas e pigmento em pó. O despertar de sensações contrárias ao prazer sensorial sugeriu que não apenas a materialidade estaria no cerne da questão da criação de adversidade como um dos fios condutores da obra de Oiticica, como também que a captura de objetos descartados oriundos do cotidiano seria a maneira do artista estruturar a sua noção de adversidade (da vida e da arte). Para compreender o papel estrutural dos objetos despejados do dia-a-dia, realizamos a aproximação entre o adverso e o abjeto (KRAUSS, 2000), bem como o pensamento do informe como elemento operacional da obra (YVE-ALAIN BOIS, 2000). A materialidade rude foi vista ainda como criadora de uma temporalidade de instantes para os Bólides e aqueles que os manipulavam, como organizadora de fricção com o espaço e a cultura e ainda como elemento de rebaixamento da condição matérica e do espectador / The research investigates the adversity that is created through the rough and crude materiality of Hélio Oiticicas bólides series. The role played by the materiality becomes evident with the perception that such objects are invitations to what we call adverse participation. These are curious and manipulative objects, but at the same time, they propose the spectator to touch non-smooth and coarse substances and surfaces, such as pieces of badly painted plywood, wire mesh, shells and pigment. The discovery of these sensations opposed to the sensorial pleasure suggested not only that the materiality was the core of the adversity creation but also that the use of disposed objects from everyday was the way Oiticica found to structure his notion of adversity of life and of art. To understand how everyday objects could form adversity, we made the approximation between the adverse and the abject (KRAUSS, 2000) and employed the concept of the informe as the operational element of the work of art (YVE-ALAIN BOIS, 2000). The rough materiality was also seen as the creator of moment temporalities for the bólides and for those who wanted to manipulate them, and also as the organizer of the friction between these objects and the space and culture, being hold responsible for the debasement of the spectator

Water ingestion effects on gas turbine engine performance

Although gas turbine engines are designed to use dry air as the working fluid, the great demand over the last decades for air travel at several altitudes and speeds has increased aircraft’s exposure to inclement weather conditions. Although, they are required to perform safely under the effect of various meteorological phenomena, in which air entering the engine contains water, several incidents have been reported to the aviation authorities about power loss during flight at inclement weather. It was understood that the rain ingestion into a gas turbine engine influences the performance of the engine and particular the compressor and the combustor. The effects of water ingestion on gas turbine engines are aerodynamic, thermodynamic and mechanical. These effects occur simultaneously and affect each other. Considering the above effects and the fact that they are timedependent, there are few gas turbine performance simulation tools, which take into account the water ingestion phenomenon. This study is a new research of investigating theoretically the water ingestion effects on a gas turbine performance. It focuses on the aerodynamic and mechanical effects of the phenomenon on the compressor and the combustor. The application of Computational Fluid Dynamics (CFD) is the basic methodology to examine the details of the flow in an axial compressor and how it is affected by the presence of water. The calculations of water film thickness, which is formed on the rotor blade, its motion (direction and speed) and the extra torque demand, are provided by a code created by the author using FORTRAN programming language. Considering the change in blade’s profile and the wavy characteristics of the liquid film, the compressor’s performance deterioration is calculated. The compressor and combustor’s deterioration data are imported to a gas turbine simulation code, which is upgraded to calculate overall engine’s performance deterioration. The results show a considerable alteration in engine’s performance parameters and arrive at the same conclusions with the relevant experimental observations.

O estudo foi realizado em 35 pacientes sob terapia com nifedipina (grupo teste ) e em um grupo controle de 35 pacientes. Foram feitos os registros das variáveis demográficas (idade e sexo do paciente), farmacológicas (dose e tempo de uso da nifedipina) e das variáveis periodontais (índice de placa, índice gengival, profundidade de sondagem, nível de inserção clínico, sangramento à sondagem) e do crescimento gengival. O teste Z (nível de significância a 5% e p<0.05) e a correlação de Spearman foram usados para a comparação dos resultados entre os grupos teste e controle. Os resultados mostraram que o crescimento gengival foi observado em 68% dos pacientes e que não houve associação entre o crescimento gengival e as variáveis demográficas e as variáveis farmacológicas. No entanto, houve associação entre o crescimento gengival e as variáveis periodontais, exceto para o índice de placa. Podemos concluir que a inflamação gengival apresentou influência no crescimento gengival associado à nifedipina. / The aim of this study was to evaluate the occurrence of nifedipine induced GO in patients and the risk factors associated using a New Clinical Index for Drug Induced Gingival Overgrowth (DIGO) and the relation between The study was carried out with 35 patients under treatment with nifedipine (test group) and 35 patients without treatment (control group). There was assessed the characteristics of demographic (age, gender), pharmacological (dose, time of use), periodontal variables (plaque index, gingival index, probing depth, attachment level, bleeding on probing) and gingival overgrowth of the sample. The test Z (significance level at 5% - p< 0.05) and the Spearman correlation were used to compare data in test and control groups. Gingival overgrowth was noticed in 68% of patients. The statistical analyses showed no association between the gingival overgrowth and demographic and pharmacological variables, except for the plaque index. However, there was an association between the gingival overgrowth and periodontal variables. Further, it is possible to conclude that the presence of gingival inflammation was the main factor of risk to promote nifedipine-induced gingival overgrowth.
