Colour comparison of two composite materials to natural tooth structure: an in vitro study

Magister Chirurgiae Dentium (MChD) / This in vitro study compares the ability of two composite materials Filtek Supreme XTE™ (3M, ESPE) and CeramX™ Duo (Dentsply) against extracted tooth structure using the modern two layering concept. Fifty six extracted anterior maxillary central incisor teeth were selected which were caries free, had no surface defects and within the colour range A3, B3, C3 or D2. The teeth were randomly divided into four colour groups (A3, B3, C3 and D2), each containing 14 teeth. A class IV cavity was prepared on all the teeth and the two composite materials were used alternately with no bonding agent to restore the cavity. The colour of both composites was assessed immediately after restoration, 24 hours and after two weeks using the SpectroShade™ (MHT) spectrophotometer. The MHT SpectroShade™ version 3.3 software was used with the CIE L*a*b* colour system to evaluate the colour of the teeth and restorations. The Kruskal-Wallis One- Way analysis of variance was used to compare the differences between the medians of the two composite materials. The significance was set at z>3.1237. The Spearman’s rank correlation coefficient was set at a significance of p<0.01 to evaluate colour change (_E*ab) in the restorations over three time intervals (immediate restoration, 24 hours and two weeks).

O objetivo desse estudo foi avaliar, por meio do método tridimensional dos elementos finitos, a distribuição de tensões na interface osso/implante e implante/pilar em implantes com conexão cone morse, posicionados na região anterior da maxila. A partir de uma reconstrução virtual óssea proveniente de uma tomografia computadorizada, foi simulada uma prótese cantilever (11-21) utilizando componente protético com pescoço de 2,5 mm de altura posicionado sobre implantes de 3,75 mm e 4,0 mm de diâmetro e 8,5 mm e 10 mm de comprimento; e prótese parcial fixa convencional (12-22) com componente protético com pescoço de 1,5 mm posicionado em implantes com 3,75 mm e 3,3 mm de diâmetro e 8,5 mm e 10 mm de comprimento. O ensaio realizado com aplicação de carga de 178 N e inclinação de 45° em relação ao longo eixo do implante na superfície palatina das próteses. A tensão de pico Equivalente Von Mises para ambos os casos clínicos foram localizados em áreas do pescoço dos componentes protéticos, plataforma e pescoço dos implantes e região óssea periimplantar. Na região dos incisivos centrais, o pico mais elevado de tensão Von mises correspondeu ao implante de 3,75 mm x 8,5 mm (370,413 MPa), e o menor ao implante de 4,0 mm x 8,5 mm valor (367, 475 MPa). No osso periimplantar a tensão de pico variou de 53,846 MPa para implante 3,75 mm x 8,5 mm à 47,284 MPa para implante 4,0 mm x 10 mm. Para implantes posicionados na região dos incisivos laterais, o valor mais elevado correspondeu ao implante de 3,3 mm x 8,5 mm (832, 911 MPa), e o menor valor ao implante de 3,75 mm x 8,5 mm (433, 666 MPa). No osso periimplantar a tensão de pico variou de 64,433 MPa, para implante 3,3 mm x 8,5 mm, à 50,277 MPa para implante 3,75 mm x 10 mm. Conclui-se que os valores e áreas de pico de tensão nos componentes protéticos posicionados em implantes com os diferentes comprimentos (8,5 mm e 10 mm) e diâmetros (3,75 mm e 4,0 mm), na região dos incisivos centrais, foram semelhantes. Implantes com mesmo diâmetro, posicionados na região dos incisivos laterais apresentaram valores e áreas de tensão semelhantes no pilar protético e osso periimplantar, independente do comprimento. Implantes com 3,3 mm de diâmetro apresentaram o dobro dos valores que implantes com 3,75 mm. / The aim of this study was to evaluate through the three-dimensional method of the finite elements the stress distribution in the bone / implant interface and implant / abutment on implants with morse cone connection, placed in the anterior maxilla. Klinische Resultate sofort versorgter dentaler Implantate unter Anwendung kortikaler Mikrofixation / Immediate provisionalization of anterior maxillary dental implants stabilized through cortical satellite implants. Clinical results

Absicht: Ziel dieser retrospektiven Arbeit war es den Erfolg des Behandlungskonzeptes der Sofortversorgung von oberen Frontzahnimplantaten unter Anwendung palatinaler kortikaler Satellitenimplantate zu überprüfen. Material und Methode: An der Befragung zur Erfassung der Überlebensrate und der Patientenzufriedenheit nahmen 44 Patienten teil (29 w, 15 m, Durchschnittsalter 58 Jahre), die in der oberen Frontzahnregion mit Implantaten unter Anwendung der palatinalen kortikalen Mikrofixation versorgt wurden. Eine klinische Untersuchung zur Erfassung des Knochenabbaus, der Periotestwerte, der Sondierungstiefen sowie des Papilla Presence Indizes erfolgte bei 35 der befragten Patienten. Ergebnisse: Die Gesamtüberlebensrate der Implantate betrug 98 % (mittlerer Beobachtungszeitraum 46 Monate). Ein mittlerer Knochenverlust von 0,87 mm und ein mittlerer Periotestwert von -0,02 konnten ermittelt werden. Mittlere Sondierungstiefen betrugen 1,72 bis 2,3 mm. Bei der Befragung der Patienten konnte eine hohe Patientenzufriedenheit mit dem Behandlungsergebnis der Implantation festgestellt werden. Fazit: Die Ergebnisse dieser Arbeit zeigen, dass das Konzept der implantologischen Sofortversorgung mit palatinaler subgingivaler kortikaler Mikrofixation in der oberen Frontzahnregion positive Ergebnisse liefert.
